Estudio de las características histológicas y anatómicas de la mucosa oral humana. Un paso hacia la fabricación de mucosa oral artificial hecha a medida
Several pathologies may affect the histological structure and physiology of the human oral mucosa,
being necessary to use oral mucosa grafts. In this context, the scarce number of donors has increased
the need to develop artificial substitutes of human oral mucosa, most of them, without attending to
the histological differences found in the different anatomical areas of the oral cavity. The objective
of the present bibliographic review is to carry out a histological description and a morphological
classification of the human oral mucosa and to determine the translational potentiality of the new
artificial models of oral mucosa.Diversas patologías afectan sustancialmente la estructura histológica y la fisiología de la mucosa
oral humana haciendo necesario el uso de trasplantes de mucosa oral. En este contexto, el escaso
número de donantes ha incrementado la necesidad de producir sustitutos artificiales de mucosa oral
humana, la mayoría de ellos sin atender a las diferencias histológicas encontradas en las diferentes
áreas anatómicas de la cavidad oral. El objetivo de la presente revisión bibliográfica es realizar una
descripción histológica y una clasificación morfológica de la mucosa oral humana y determinar la
potencialidad traslacional de los nuevos modelos artificiales de mucosa oral
BMP-2 in Traumatology. Advances in Tissue Engineering
Las proteínas morfogenéticas óseas - en inglés, Bone Morphogenetic Proteins (BMPs) - son proteínas producidas
localmente por los osteoblastos y que intervienen en multitud de funciones celulares como son la regulación de
la hematopoyesis, la estimulación de la síntesis de matriz extracelular, el mantenimiento celular (incluida la apoptosis),
el crecimiento y diferenciación de osteoblastos y condroblastos, así como en la morfogénesis de diferentes
tejidos y órganos, como el renal y el nervioso. Las BMPs son los únicos factores morfógenos existentes, capaces
de estimular la multiplicación de las células conectivas y transformarlas en células osteoprogenitoras. Dentro
de esta familia de proteínas, destaca la BMP-2 como un factor de crecimiento capaz de inducir la formación de
hueso y cartílago mediante la estimulación de la diferenciación de los osteoblastos y condrocitos en diversos tipos
celulares. En la actualidad, son numerosos los campos donde se estudian los efectos de la BMP-2 o, incluso, se usa
como complemento a otras técnicas quirúrgicas existentes en el ámbito hospitalario. La proteína BMP-2 ha mostrado
ser capaz de estimular la producción de hueso, ayudando a la curación de defectos en la unión de huesos
largos o la fusión espinal, así como al tratamiento de fracturas abiertas o aplicaciones odontológicas, entre otras.
En este artículo se realiza una revisión de la literatura sobre los avances recientes del uso de la BMP-2, prestando
especial atención a los ámbitos de la Traumatología (en particular al tratamiento de fracturas abiertas, pseudoartrosis
y fusión lumbar) y la Ingeniería Tisular. Aunque parece que, en general, la mayoría de trabajos muestran que
la BMP-2 proporciona resultados positivos e incluso puede reducir los costes de intervención en comparación con
otras técnicas, otros estudios indican los posibles eventos adversos que su uso puede producir.Bone Morphogenetic Proteins (BMPs) are proteins locally produced by osteoblasts that are involved in many
cellular functions such as the regulation of the hematopoiesis and the stimulation of the extracellular matrix
synthesis; they also influence the cellular maintenance (including the apoptosis) and are involved in the growth
and differentiation of osteoblasts and chondroblasts. BMPs are the unique existing morphogens that are capable
of stimulating the proliferation of connective cells and transform them into osteoprogenitor cells. Within this protein
family, BMP-2 is presented as a key growth factor that induces the bone and cartilage formation, stimulating
the differentiation of osteoblasts and chondrocytes in several cell types. The effects of BMP-2 have been studied
in many fields. Moreover, BMP-2 has been even used as a supplement to other existing surgical techniques in
clinic. In addition, the BMP-2 protein is able to stimulate the bone production, aiding the healing of defects in
the long bone union or spine fusión, and the treatment of open fractures or dental applications, among others.
In this paper, recent advances on the usage of BMP-2 are reviewed in the literature, paying attention to the areas
of Traumatology (particularly the treatment of open fractures, nonunions and spinal fusion) and Tissue Engineering.
Even though, most of the studies show that there are applications of BMP-2 that provide positive results and
may reduce the costs of intervention compared to other techniques, other studies indicate the potential adverse
events that their use can produce
Mesenchymal stem cells differenciate to human corneal endothelium. A literature review of published data
Antecedentes y Objetivo: En la presente revisión se expondrán las principales fuentes de células madre mesenquimales utilizadas para la generación de endotelio corneal humano artificial o en sus usos como terapia regenerativa. Así mismo, los distintos modelos de diferenciación sugeridos por los autores a lo largo de la historia; los marcadores celulares o fisiológicos de los que se han servido para caracterizar las células diferenciadas y se describirá la potencialidad traslacional de dichas células a patologías del endotelio corneal mediante las técnicas de las que se han servido para su implantación o trasplante.
Material y métodos: Revisión narrativa de publicaciones de diferenciación de células madre mesenquimales a endotelio corneal humano o de otras aplicaciones de estas células como terapia regenerativa en modelos de daño endotelial corneal.
Resultados: Un total de seis trabajos utilizaron células madre mesenquimales en diferenciación o regeneración de endotelio corneal, de los cuales cuatro utilizaron células provenientes de cordón umbilical. En cuatro de estos estudios se diseñó un protocolo de diferenciación distinto. Los autores coincidieron en los marcadores de diferenciación ATP1A1, COL8A2, PITX2 y ZO-1.
Conclusiones: Si bien son pocos los ensayos clínicos publicados en materia de endotelio corneal humano que utilizan dichas células; el potencial de regeneración y de diferenciación de las MSC junto con el desarrollo que ha experimentado la Ingeniería Tisular, hace que se postulen como el futuro de la terapia sustitutiva.Background and purpose: The main sources of mesenchymal stem cells used for the creation of artificial corneal endothelium or their uses such as regenerative therapy will be exposed in this review. Moreover, the different models of differentiation suggested by authors throughout the history just as cellular or physiological markers which have been used to typify defined cells will be described. Finally, it will also be described the translational potential of these cells to corneal endothelium diseases through the techniques which have been used to its implantation or transplant.
Material and methods: Narrative review of publications on the differentiation of mesenchymal stem cells to human corneal endothelium or other applications of these cells as regenerative therapy in models of corneal endothelial damage.
Results: A total of six studies used mesenchymal stem cells in differentiation or regeneration of corneal endothelium, of which four used cells from the umbilical cord. In four of these studies a different differentiation protocol was designed. The authors agreed on the differentiation markers ATP1A1, COL8A2, PITX2 and ZO-1.
Conclusions: Although there are few published clinical trials on human corneal endothelium that use these cells; The regeneration and differentiation potential of MSCs and the development that Tissue Engineering has undergone, makes them postulate as the future of replacement therapy
Successful restoration of corneal surface integrity with a tissue-engineered allogeneic implant in severe keratitis patients
Objectives: Corneal diseases are among the main causes of blindness, with approximately 4.6 and 23 million
patients worldwide suffering from bilateral and unilateral corneal blindness, respectively. The standard treatment
for severe corneal diseases is corneal transplantation. However, relevant disadvantages, particularly in
high-risk conditions, have focused the attention on the search for alternatives.
Methods: We report interim findings of a phase I-II clinical study evaluating the safety and preliminary efficacy of
a tissue-engineered corneal substitute composed of a nanostructured fibrin-agarose biocompatible scaffold
combined with allogeneic corneal epithelial and stromal cells (NANOULCOR). 5 subjects (5 eyes) suffering from
trophic corneal ulcers refractory to conventional treatments, who combined stromal degradation or fibrosis and
limbal stem cell deficiency, were included and treated with this allogeneic anterior corneal substitute.
Results: The implant completely covered the corneal surface, and ocular surface inflammation decreased
following surgery. Only four adverse reactions were registered, and none of them were severe. No detachment,
ulcer relapse nor surgical re-interventions were registered after 2 years of follow-up. No signs of graft rejection,
local infection or corneal neovascularization were observed either. Efficacy was measured as a significant
postoperative improvement in terms of the eye complication grading scales. Anterior segment optical coherence
tomography images revealed a more homogeneous and stable ocular surface, with complete scaffold degradation
occurring within 3–12 weeks after surgery.
Conclusions: Our findings suggest that the surgical application of this allogeneic anterior human corneal substitute
is feasible and safe, showing partial efficacy in the restoration of the corneal surface.Biobank of the Andalusian Public Health

Cell viability evaluation in tissue constructs. A preliminary study

de microscopía electrónica analítica por energía dispersiva de rayos-X.Objetivo: Aplicar la microscopia electrónica analítica por energía dispersiva de rayos X a la evaluación de la viabilidad
celular en constructos tisulares de fibrina agarosa y determinar la viabilidad celular en el seno del constructo
Material y Métodos: Se evalúan fibroblastos aislados y fibroblastos en el seno del constructo a los 7 y 21 días con
la técnica de Live/Dead y de microscopia electrónica analítica por energía dispersiva de rayos X.
Resultados: El estudio demuestra que la aplicación de la técnica es factible y que la viabilidad es elevada en los
constructos a los 21 días sin que existan signo de apoptosis en la población celular.
Conclusión: La microscopia electrónica analítica por energía dispersiva de rayos X permite evaluar la viabilidad
celular en los constructos tisulares y establecer un control de calidad más exigente para su aplicación terapéutica.Objective: Apply X-ray microanalytical electron microscopy to evaluate cell viability in fibrin agarose tissue
constructs and determine cell viability within the construct.
Material and Methods: Isolated fibroblasts and fibroblasts within the construct are evaluated at 7 and 21 days
with the technique of Live/Dead and with X-ray microanalytical electron microscopy.
Results: The study shows that the application of the technique is feasible that viability is high in the constructs at
21 days without sign of apoptosis in the cell population.
Conclusion: X-ray microanalytical electron microscopy allows the evaluation of cell viability in tissue constructs
and establish a more demanding quality control for therapeutic application
Aislamiento de células epiteliales corneales a partir del limbo esclerocorneal humano
Introducción: La obtención de cultivos celulares epiteliales corneales humanos podría ser de gran utilidad
tanto para la investigación básica como para su aplicación clínica en el tratamiento de diversas
enfermedades corneales.
Material y Métodos: Muestras de limbo esclerocorneal procedentes de donante humano cadáver se
pusieron en cultivo con medios específicos para células epiteliales con distintos factores de crecimiento. En
la mitad de los cultivos se utilizaron células alimentadoras 3T3.
Resultados: Se obtuvieron cultivos primarios de células epiteliales corneales con un alto grado de
proliferación, sin importar si el crecimiento se dio sobre células 3T3 o no. La tasa de contaminación de los
cultivos fue menor en aquellos donde sí se encontraban estas células alimentadoras.
Discusión: La obtención de células epiteliales corneales es posible en el laboratorio a partir de pequeñas
biopsias del limbo esclerocorneal tras su expansión en cultivo, utilizando una técnica de cultivo estándar,
fácilmente reproducible.Introduction: The achievement of human corneal epithelial cultures could be useful not only for research
purposes, but also for clinical applications in the treatment of several corneal diseases.
Material and Methods: Sclerocorneal limbi were obtained from human donors and cultured in specific
medium containing several hormones and growth factors. Half of the cultures were established using a
layer of 3T3 feeder cells.
Results: we obtained primary cultures of corneal epithelial cells with a high proliferation rate in culture. No
relationship with the use of a 3T3 feeder cell layer was found, but the likelihood of fibroblast contamination
in the epithelial cultures was lower when these cells were used.
Discussion: Establishing primary cultures of corneal epithelial cells from small explants of limbal tissue is a
technique that can be carried out in the laboratory using standard culture methods.Financiado

A cell viability study on an experimental model of human gingival fibroblasts cultured without foetal bovine serum
El objetivo del estudio es aportar más informaciones metodológicas y conceptuales a cerca del efecto del SBF 10% en
ensayos de viabilidad celular sobre fibroblastos gingivales humanos, con la finalidad de optimizar las condiciones
experimentales para la obtención de resultados más predictivos. Se utilizarán el ensayo colorimétrico WST-1 para la
evaluación de proliferación celular, el ensayo de LDH para evaluación de la integridad de membrana celular, el ensayo
de cuantificación de ADN mediante espectrometría y la microscopía óptica de contraste de fases para observar las
alteraciones morfológicas. Entre 6 y 24 h, las células sufren poco con la ausencia del SBF 10% y no resulta
imprescindible su utilización para evaluar proliferación celular y la integridad de membranas plasmática y nuclear. Sin
embargo, la morfología de las células se altera, de forma intensa, a partir de 6 h de ausencia de SBF 10%, por lo que
es conveniente utilizarlo para dichos estudios morfológicos.The aim of this study is to improve the methodological and conceptual knowledges about the effect of 10% foetal
bovine serum in cell viability assays on human gingival fibroblasts, in order to optimize the experimental conditions to
obtain more predictable results. In this study, different methods were used to determine cell proliferation, cell and
nuclear membrane integrity and morphological study by using WST-1 assay, LDH assay, DNA quantification and
phase contrast microscopy. Our results demonstrated that 10% foetal bovine serum deprivation for 6h up to 24h did
not show changes in terms of cell proliferation, cell and nuclear membrane integrity. However, important
morphological changes were observed under foetal bovine serum deprivation conditions after 6h. We conclude that,
10% foetal bovine serum does not result essential its utilization to evaluate cell proliferation and their deprivation do
not compromise cell survival range and could be suppressed in cell proliferation, cell and nuclear membrane assays
without affecting the assays results. In contrast, the utilization of 10% foetal bovine serum becomes crucial in
morphological studies and we strongly recommend 10% foetal bovine serum supplement in morphological studies
Carotid atherosclerosis: clinical-histological correlation in vulnerable plaques
Objetivos: Caracterizar histológicamente placas humanas carotídeas vulnerables para plantear nuevas opciones diagnósticas
y terapéuticas mediante la aplicación de nanomateriales en pacientes asintomáticos.
Métodos: Se incluyeron tres pacientes varones, con enfermedad carotidea e indicación quirúrgica (endarterectomía).
Las placas fueron teñidas con hematoxilina-eosina, tricrómico de Masson, picrosirius y orceína. Se analizaron histológicamente,
determinando su remodelamiento, núcleo lipídico, infiltración inflamatoria, presencia de células espumosas,
calcificación y neovascularización con hemorragia. Se realizo un análisis estadístico comparativo del porcentaje de fibrosis,
así como de su intensidad por tercios entre las muestras e intramuestra.
Resultados: A nivel clínico, la placa 1 era sintomática, correspondía a un paciente con antecedente de accidente isquémico
transitorio, el paciente 2 presentaba un aneurisma carotideo con gran trombo mural y la placa 3 era asintomática.
A nivel histológico, las placas 1 y 2 se asociaban con un estadio más evolucionado que la placa 3. Las placas 1 y 2 presentaron
ruptura de su pared, intenso infiltrado de macrófagos, abundante calcificación y neovascularización con hemorragia
intraplaca. Las placas carotídeas diferían principalmente en el grado de fibrosis. En concreto, la placa 3 destacó por
la presencia de células espumosas migrando hacia el núcleo lipídico y la formación de líneas de calcificación. El estudio
estadístico mostró una notable fibrosis de la placa 1 (5,4%), siendo inferior en la placa 3 (2,51%) y en la 2 (1,84%). Así
mismo, se observaron diferencias estadísticamente significativas al comparar el tercio inferior de las placas 1 y 3, y en
el análisis intramuestra de la placa 2.
Conclusiones: El presente estudio permitió determinar las características histológicas de placas vulnerables que se
pueden asociar con la manifestación de síntomas clínicos. Estos hallazgos, sugieren un conocimiento potencial para
el desarrollo de nuevas opciones diagnósticas y terapéuticas que mejoren las herramientas actualmente disponibles.Objectives: Histological characterization of vulnerable human carotid plaques to propose new diagnostic and
therapeutic options through the application of tissue engineering strategies in asymptomatic patients.
Methods: In this study three male patients were included who presented carotid disease with surgical criteria
(classic endarterectomy). The plaques were stained with hematoxylin-eosin, Masson’s trichrome, picrosirius and
orcein. All samples were histologically analyzed to study remodeling capability, lipid nucleus, inflammatory infiltration,
presence of foam cells, calcification, neovascularization and intraplaque hemorrhage. In addition, we
performed a comparative statistical analysis of the fibrosis percentage, as well as its intensity by thirds between
samples and intra-sample.
Results: Clinical analysis revealed that Plaque 1 was syntomathic, trigging a stroke. Plaque 2 set up a carotid
aneurism with a large mural thrombus. Plaque 3 was asynthomatic. Histologycal analysis from Plaques 1 and 2 determined
they had developed a more advanced stage than Plaque 3. Plaques 1 and 2 were rupture plaques with
a severe macrophages infiltrated and overall calcification and neovascularization with hemorrage. All plaques
differed in the degree of fibrosis. At Plaque 3 foams cells were standing out, migrating to lipidic nucleous, as well
as calcification lines. Statistic analysis presented a notorious Plaque 1 fibrosis (5,4%), below Plaque 3 (2,51%), and
Plaque 2 (1,84%). They were statistically significant differences between the lower third of Plaques 1 and, 3, and
at intra-sample analysis of Plaque 2.
Conclusions: The present study allowed us to determine the histological characteristics of vulnerable plaques that
can be associated with the manifestation of clinical symptoms. These findings suggest a potential knowledge for
the development of new diagnostic and therapeutic options that improve clinical currently available tools
Evaluation of cell viability and apoptotic patterns in stem cells isolated from human dental pulp
Introducción: El desarrollo de sustitutos biológicos mediante Ingeniería Tisular requiere de la utilización de una
fuente de células madre que, además de ser capaz de autorenovarse y diferenciarse, mantengan una
funcionalidad y viabilidad óptima justo en el momento de su uso. El estudio de la viabilidad celular constituye un
importante control de calidad, especialmente en aquellas poblaciones celulares con un alto potencial para su
utilización en Ingeniería Tisular, como son las células madre de la pulpa dental (DPSC). El objetivo de este
trabajo es la evaluación de la viabilidad durante los tres primeros subcultivos de células madre de la pulpa
dental humana (hDPSC).
Material y métodos: Se obtuvieron 3 subcultivos consecutivos de hDPSC de terceros molares humanos
sanos (N = 3) mediante un proceso de digestión enzimática. La concentración intracelular de los iones sodio
(Na), potasio (K), y cloro (Cl) fue determinada mediante microscopía analítica por energía dispersiva de
rayos X (EPXMA). El porcentaje de células en apoptosis (fragmentación de DNA) fue determinado mediante
el ensayo de fluorescencia TUNEL en el primer y tercer subcultivo.
Resultados: En el segundo subcultivo se detectó un descenso significativo del potasio (p = 0,011) indicando
un descenso en la viabilidad celular. En el tercer subcultivo, los niveles de cloro y sodio aumentaron de forma
significativa (p = 0,010 y p = 0,002), generando un perfil iónico compatible con células en estado apoptótico.
La fluorescencia a partir del ensayo TUNEL reveló 2,76 ± 1,80 % de células apoptóticas en el tercer
subcultivo, mientras que en el primer subcultivo, dicha proporción fue de 0,55 ± 0,27%.
Discusión: Las hDPSC se encuentran de forma nativa en la pulpa dental y estas condiciones nativas se
pueden alterar cuando pasan a las condiciones de un medio de cultivo in vitro. Estas alteraciones pueden ser
la respuesta a un proceso de adaptación. Dicha adaptación, junto con el estrés adicional generado por el
tratamiento enzimático realizado para digerir la matriz extracelular, tiene como consecuencia una pérdida
transitoria y leve de la viabilidad producida por un mecanismo de apoptosis. En resumen, los 3 primeros
subcultivos de hDPSC deberán ser descartados para su uso en ingeniería tisular, por ser células en un estado
de apoptosis activa.Introduction: The development of biological substitutes by Tissue Engineering needs the use of a stem cell
source able not only to self-replicate and differentiate, but also to maintain optimal functionality and cell
viability when used. The evaluation of cell viability is considered an important quality control, especially in
cell lineages with high capabilities for being used in tissue engineering, as dental pulp stem cells (DPSC). The
aim of this research is to evaluate the cell viability through three human dental pulp stem cells (hDPSC)
subcultures. Materials and methodology:Three consecutive hDPSC subcultures were obtained from human sound third
molars (N = 3) by enzymatic digestion. Intracellular ionic concentration of sodium (Na), potassium (K) and
chlorine (Cl) was determined by electron probe X-ray microanalysis (EPXMA). Apoptotic cells percentage
(DNA fragmentation) was determined by TUNEL fluorescence assay at first and third subculture
Results: A significant decrease of potassium was detected at second subculture (p = 0,011), suggesting a
loss of cell viability. At third subculture, chlorine and sodium statistically increased (p = 0,010 y p = 0,002),
inducing an apoptotic-like ionic profile. Fluorescence from TUNEL assay revealed 2,76 ± 1,80 % of apoptotic
cells at third subculture, while that ratio was 0,55 ± 0,27% at first subculture.
Discussion: hDPSC are natively stored in the dental pulp and these native conditions may be altered by the in
vitro cell culture conditions. In this regard, these alterations could be in response to an adaptative process.
This adaptative process, besides cell stress as a consequence of an enzymatic treatment for the extracellular
matrix digestion, produces a mild and temporary loss of cell viability by apoptosis. In summary, the first,
second and third subculture of hDPSC should be discarded for the use in tissue engineering as they are
apoptotic

However, current bidimensional (2D) culture methods are associated to several limitations, including low efficiency and the loss of key cell differentiation markers on cultured cells. Methods: In the present work, we have designed a novel biofabrication method based on a three-dimensional (3D) culture system (FIBRIAGAR-3D). Human Wharton’s jelly mesenchymal stromal cells (HWJSC) were cultured in 3D using 100%, 75%, 50%, and 25% concentrations of fibrin-agarose biomaterials (FA100, FA75, FA50 and FA25 group) and compared with control cells cultured using classical 2D systems (CTR-2D). Results: Our results showed a significant increase in the number of cells generated after 7 days of culture, with cells displaying numerous expansions towards the biomaterial, and a significant overexpression of the cell proliferation marker KI67 was found for the FA75 and FA100 groups. TUNEL and qRT-PCR analyses demonstrated that the use of FIBRIAGAR-3D was not associated with an induction of apoptosis by cultured cells. Instead, the 3D system retained the expression of typical phenotypic markers of HWJSC, including CD73, CD90, CD105, NANOG and OCT4, and biosynthesis markers such as types-I and IV collagens, with significant increase of some of these markers, especially in the FA100 group. Finally, our analysis of 8 cell signaling molecules revealed a significant decrease of GM-CSF, IFN-g, IL2, IL4, IL6, IL8, and TNFα, suggesting that the 3D culture system did not induce the expression of pro-inflammatory molecules.
