    An Investigation Into the Electrical Activity of Tender, Resting Paraspinal Muscles Using Surface Electromyography: A Pilot Study

    Abnormal resting paraspinal muscle activity has been claimed to be responsible for changes in spinal tissue texture which are detectible by manual palpation. This pilot study investigated whether there was significant electrical activity in paraspinal musculature that was tender and that appeared to have altered tissue texture on palpation. Sixteen healthy volunteers between 18 and 35 years of age had their thoracic erector spinae mass palpated bilaterally from spinal levels T3 to T10 to identify paraspinal regions exhibiting altered tissue texture relative to the contralateral muscle mass. Surface electromyography (sEMG) was used to measure electrical activity in the muscle mass at the selected levels. No significant differences in electrical activity were observed between the tender and non-tender muscle masses, although a large difference existed in the one symptomatic subject. All muscle sites displayed EMG activity at rest, although the source of activity is not clear. A number of methodological problems with the EMG recording were encountered and are discussed. Future research is recommended using symptomatic participants

    Teachers learning to use the iPad in Scotland and Wales: a new model of professional development

    In learning to use a new technology like the iPad, primary teachers adopt a diverse range of experiential, informal and playful strategies contrasting sharply with traditional models underpinning professional development which emphasise formal courses and events led by ‘experts’ conducted in formal settings such as the school. Since post-PC devices like the iPad have been linked with transformational educational learning, there is an imperative to better understand how teachers can be encouraged to use them more effectively. Despite their growing popularity in schools, there is little research to indicate how and under what circumstances teachers learn to integrate these technologies into their daily practices. This paper uses data collected from two national studies of iPad use in Scotland and Wales to propose a new model of professional development. This model reflects findings that the teachers reject traditional models of sequential, or staged, professional development (often led by external providers or ‘experts’), in favour of a more nuanced and fluid model where they learn at their own pace, in a largely experiential fashion, alongside their pupils in a relationship which reverses the traditional power nexus. The model has the potential to inform professional development for both trainee and serving teachers in learning to use the iPad in the primary classroom

    The Value of Life Near its End and Terminal Care

    Medical care at the end of life, which is often is estimated to contribute up to a quarter of US health care spending, often encounters skepticism from payers and policy makers who question its high cost and often minimal health benefits. It seems generally agreed upon that medical resources are being wasted on excessive care for end-of-life treatments that often only prolong minimally an already frail life. However, though many observers have claimed that such spending is often irrational and wasteful, little explicit and systematic analysis exists on the incentives that determine end of life health care spending. There exists no positive theory that attempts to explain the high degree of end-of life spending and why differences across individuals, populations, or time occur in such spending. This paper attempts to provide the first rational and systematic analysis of the incentives behind end of life care. The main argument we make is that existing estimates of the value of a life year do not apply to the valuation of life at the end of life. We stress the low opportunity cost of medical spending near ones death, the importance of keeping hope alive in a terminal care setting, the larger social value of a life than estimated in private demand settings, as well as the insignificance in quality of life in lowering its value. We derive how an ex-ante perspective in terms of insurance and R&D alters some of these conclusions.
