    AbstractComplications related to coronary sinus lead are not infrequent in recipients of cardiac resynchronization devices. We describe the case of a patient with a biventricular implantable cardioverter defibrillator with persistent phrenic nerve stimulation, previous coronary sinus lead fracture, and severe left subclavian vein stenosis. The reimplantation of a new coronary sinus lead on the left side, ipsilateral to the original implant, was unsuccessful. In order to avoid more complex and risky procedures, we performed the repair of the fractured abandoned lead with the reconstruction of the unipolar lead terminal. Effective biventricular pacing was obtained with satisfactory electrical parameters and it was maintained at twelve months follow-up

    Climate change impact on cultivated and wild cacao in Peru and the search of climate change-tolerant genotypes

    Aim: Cacao (Theobroma cacao L.) is expected to be vulnerable to climate change. The objectives of this study were to (a) assess the future impact of climate change on cacao in Peru and (b) identify areas where climate change-tolerant genotypes are potentially present. Location: Peru Methods: Drawing on 19,700 and 1,200 presence points of cultivated and wild cacao, respectively, we modelled their suitability distributions using multiple en semble models constructed based on both random and target group selection of pseudo-absence points and different resolutions of spatial filtering. To estimate the uncertainty of future predictions, we generated future projections for all the ensem ble models. We investigated the potential emergence of novel climates, determined expected changes in ecogeographical zones (zones representative for particular sets of growth conditions) and carried out an outlier analysis based on the environmental variables most relevant for climate change adaptation to identify areas where climate change-tolerant genotypes are potentially present. Results: We found that the best modelling approaches differed between cultivated and wild cacao and that the resolution of spatial filtering had a strong impact on future suitability predictions, calling for careful evaluation of the effect of model selection on modelling results. Overall, our models foresee a contraction of suitable area for cultivated cacao while predicting a more positive future for wild cacao in Peru. Ecogeographical zones are expected to change in 8%–16% of the distribution of cultivated and wild cacao. We identified several areas where climate change-tolerant genotypes may be present in Peru. Main conclusions: Our results indicate that tolerant genotypes will be required to facilitate the adaptation of cacao cultivation under climate change. The identified cacao populations will be target of collection missions

    Determinants of Treatment Benefit and Post-Treatment Survival for Patients with Hepatocellular Carcinoma Enrolled in Second-Line Trials after the Failure of Sorafenib Treatment

    Second-line treatments are standard care for advanced hepatocellular carcinoma (HCC) patients with preserved liver function who are intolerant of or progress on first-line therapy. However, determinants of treatment benefit and post-treatment survival (PTS) remain unknown. HCC patients previously treated with sorafenib and enrolled in second-line clinical trials were pooled according to the investigational treatment received and the subsequent regulatory approval: approved targeted agents and immune checkpoint inhibitors (AT) or other agents (OT) not subsequently approved. Univariate and multivariate analyses using Cox proportional hazards models established relationships among treatments received, clinical variables, and overall survival (OS) or PTS. For 174 patients (80 AT; 94 OT) analyzed, baseline factors for longer OS in multivariate analysis were second-line AT, absence of both portal vein thrombosis and extrahepatic spread (EHS). Treatment with AT (versus OT) was associated with significantly longer OS among patients with EHS (pinteraction = 0.005) and patients with low neutrophil-to-lymphocyte ratio (NLR; pinteraction = 0.032). Median PTS was 4.0 months (95% CI 2.8–5.3). At second-line treatment discontinuation, alpha-fetoprotein (AFP) levels <400 ng/dl, albumin-bilirubin (ALBI) grade 1, and enrolment onto subsequent trials independently predicted longer PTS. Treatment with AT, PVT, and EHS were prognostic factors for OS, while AFP, ALBI grade and enrolment onto a third-line trial were prognostic for PTS.     Background: In Italy, no long-term studies regarding the natural history of acute pancreatitis have been carried out. Aim: To report the results of a follow-up on a large series of patients hospitalised for pancreatitis. Methods: Data of 631 patients admitted to 35 Italian hospitals were retrospectively evaluated 51.7 \ub1 8.4 months after discharge. Results: The average recovery time after mild or severe pancreatitis was 28.2 and 53.4 days respectively. Fourteen sequelae were not resolved and 9 cases required late surgical intervention. Eighty patients (12.7%) had a second hospital admission. Of the patients with mild biliary pancreatitis, 67.9% underwent a cholecystectomy. The overall incidence of relapse was 12.7%. Mortality was 9.8% and no death was related to pancreatitis. Three patients died from carcinoma of the pancreas. Conclusion: Reported recovery time after an attack of pancreatitis was longer than expected in the mild forms. The treatment of sequelae was delayed beyond one year after discharge. The incidence of relapse of biliary pancreatitis in patients not undergoing a cholecystectomy was low, due to endoscopic treatment. Mortality from pancreatic-related causes is low, but there is an association with malignant pancreatic or ampullary tumours not diagnosed during the acute phase of the illness. \ua9 2013 Editrice Gastroenterologica Italiana S.r.l
