    Poursuite de l’allaitement maternel lors d’une nouvelle grossesse : le vécu des mères sur l’instauration d’un co-allaitement

    During the period of breastfeeding, pregnancy can occur and the mother has to make a decision about whether or not the elder child is weaned. Breastfeeding can continue and after the birth of the youngest child. In this situation, the mother is tandem nursing. This situation is little known to health professionals. This can be a source of taboo and stigmatization, which led us to wonder about the experiences of mothers. We chose to examine their experiences from semi-structured interviews. Our population consisted of mothers who have experienced tandem nursing, past or present. Our study was based on the thematic analysis. Mothers who breastfeed have the same feelings about breastfeeding and weaning, valuing the pursuit of a mother and child bond through breastfeeding. Also, when a mother is breastfeeding during pregnancy, mental and physical changes may occur. This is the sign of a psychic phenomenon which appears during pregnancy, which has an impact on the mother’s lactation. Finally, we have shown the importance of peer groups for these mothers and the positioning of the father, in connection with a loss of professional’s legitimacy. This work has allowed us to better understand the experiences of mothers and to have a better understanding of psychic phenomena.Pendant la période d'allaitement, une grossesse peut survenir alors que l'aîné n'est pas sevré. L'allaitement maternel peut se poursuivre après la naissance du cadet : il s'agit alors d’un co-allaitement. Cette situation est peu connue des professionnels de santé. Cela est source de tabou et de stigmatisation, ce qui nous a amené à nous interroger sur le vécu des mères. Nous avons choisi d’interroger leur vécu à partir d’entretiens semi-dirigés. Notre population était composée de mères ayant l’expérience d’un co-allaitement actuel ou passé. Notre étude reposait sur une analyse thématique. Les mères qui co-allaitent ont des représentations communes vis-à-vis de l’allaitement et du sevrage, valorisant la poursuite d’une relation mère-enfant à travers l’allaitement. De plus, lors de l’allaitement pendant la grossesse, des modifications psychiques et physiques peuvent survenir. Elles sont la marque d’un remaniement psychique à l’œuvre et ont un impact sur l’allaitement en cours. Enfin, nous avons mis en évidence l’importance des groupes de mères et de la place du père, en lien avec une perte de légitimité des professionnels. Ceci nous a donc permis de mieux appréhender le vécu des mères et d’avoir une meilleure compréhension des phénomènes psychiques

    Bone graft substitutes such as calcium phosphates are subject to the innate inflammatory reaction, which may bear important consequences for bone regeneration. We speculate that the surface architecture of osteoinductive β-tricalcium phosphate (TCP) stimulates the differentiation of invading monocyte/macrophages into osteoclasts, and that these cells may be essential to ectopic bone formation. To test this, porous TCP cubes with either submicron-scale surface architecture known to induce ectopic bone formation (TCPs, positive control) or micron-scale, non-osteoinductive surface architecture (TCPb, negative control) were subcutaneously implanted on the backs of FVB strain mice for 12 weeks. Additional TCPs samples received local, weekly injections of liposome-encapsulated clodronate (TCPs + LipClod) to deplete invading monocyte/macrophages. TCPs induced osteoclast formation, evident by positive tartrate resistant acid phosphatase (TRAP) cytochemical staining and negative macrophage membrane marker F4/80 immunostaining. No TRAP positive cells were found in TCPb or TCPs + LipClod, only F4/80 positive macrophages and foreign body giant cells. TCPs stimulated subcutaneous bone formation in all implants, while no bone could be found in TCPb or TCPs + LipClod. In agreement, expression of bone and osteoclast gene markers was upregulated in TCPs versus both TCPb and TCPs + LipClod, which were equivalent. In summary, submicron-scale surface structure of TCP induced osteoclastogenesis and ectopic bone formation in a process that is blocked by monocyte/macrophage depletion

    Parallel block structured adaptive mesh refinement on graphics processing units.

    Block-structured adaptive mesh refinement is a technique that can be used when solving partial differential equations to reduce the number of zones necessary to achieve the required accuracy in areas of interest. These areas (shock fronts, material interfaces, etc.) are recursively covered with finer mesh patches that are grouped into a hierarchy of refinement levels. Despite the potential for large savings in computational requirements and memory usage without a corresponding reduction in accuracy, AMR adds overhead in managing the mesh hierarchy, adding complex communication and data movement requirements to a simulation. In this paper, we describe the design and implementation of a native GPU-based AMR library, including: the classes used to manage data on a mesh patch, the routines used for transferring data between GPUs on different nodes, and the data-parallel operators developed to coarsen and refine mesh data. We validate the performance and accuracy of our implementation using three test problems and two architectures: an eight-node cluster, and over four thousand nodes of Oak Ridge National Laboratory’s Titan supercomputer. Our GPU-based AMR hydrodynamics code performs up to 4.87x faster than the CPU-based implementation, and has been scaled to over four thousand GPUs using a combination of MPI and CUDA

    The structure of the RbBP5 β-propeller domain reveals a surface with potential nucleic acid binding sites

    The multi-protein complex WRAD, formed by WDR5, RbBP5, Ash2L and Dpy30, binds to the MLL SET domain to stabilize the catalytically active conformation required for histone H3K4 methylation. In addition, the WRAD complex contributes to the targeting of the activated complex to specific sites on chromatin. RbBP5 is central to MLL catalytic activation, by making critical contacts with the other members of the complex. Interestingly its only major structural domain, a canonical WD40 repeat -propeller, is not implicated in this function. Here, we present the structure of the RbBP5 -propeller domain revealing a distinct, feature rich surface, dominated by clusters of Arginine residues. Our nuclear magnetic resonance binding data supports the hypothesis that in addition to the role of RbBP5 in catalytic activation, its -propeller domain is a platform for the recruitment of the MLL complexes to chromatin targets through its direct interaction with nucleic acids

    PSYCHOLOGICAL WELL-BEING DURING RETIREMENT TRANSITION AND ADJUSTMENT FOR SOUTHERN BAPTIST PASTORS: A PHENOMENOLOGICAL MULTI-CASE STUDY

    The purpose of this phenomenological multi-case study was to understand the psychological well-being experience of Southern Baptist pastors during transition and adjustment to retirement. Questions centered around their upbringing, their calling experience, their education, career progression in churches, reasons for retiring, preparation for retirement, experience after retiring, and how identity and purpose may or may not have been related to their role as pastor. The goal of the study was to investigate these experiences to provide rich data and analysis that is missing in current literature. The research was based on two theories: continuity theory and role theory. The study design included a purposeful sampling of 10 retired Southern Baptist pastors and qualitative interviews with each participant. The interviews were transcribed, coded, and analyzed to reveal themes in the data. Findings were discussed including a conceptual model and implications for research and practice. In the conclusion, limitations of the study were disclosed and future research opportunities were uncovered
