Médicaments immunosuppresseurs et antirétroviraux : une association riche en interactions
Le développement des médicaments antirétroviraux a permis
d'améliorer considérablement l'espérance de vie des patients
infectés par le VIH (virus de l'immunodéficience humaine).
Cependant, l'allongement de cette espĂ©rance de vie, combinĂ© Ă
une meilleure prise charge des infections opportunistes, peut favoriser
l'évolution vers une insuffisance organique sévère dont la phase
ultime nécessite une transplantation organique. Dans ce contexte,
l'utilisation nécessaire d'immunosuppresseurs va engendrer des
interactions pharmacodynamiques et pharmacocinétiques avec les
antirétroviraux. En effet, médicaments immunosuppresseurs et
antirétroviraux partagent des voies communes de biotransformation. Le
suivi thérapeutique pharmacologique occupe donc une importance
particulière pour une prise en charge thérapeutique individuelle
adéquate
Polymorphisme génétique et traitement des maladies inflammatoires chroniques de l'intestin : exemple de l'azathioprine
L'azathioprine est un médicament largement utilisé dans la prévention et le traitement des maladies inflammatoires cryptogénétiques de l'intestin. Sa biotransformation est soumise à un polymorphisme génétique qui impose un suivi pharmacothérapeutique du traitement. La méconnaissance de ces éléments peut se traduire par la survenue d'accidents hématologiques sévères mais réversibles en l'absence de complications infectieuses. Ce polymorphisme pourrait expliquer la survenue de cytolyses hépatiques. Les modalités de prise en compte de ce polymorphisme dans la conduite du traitement sont controversées et font l'objet de recommandations divergentes. Une analyse des bases pharmacologiques sur lesquelles reposent les recommandations proposées dans la littérature est présentée
Urinary leukotriene E4 excretion: a biomarker of inflammatory bowel disease activity.
International audienceBACKGROUND: Crohn's disease (CD) and ulcerative colitis (UC) are chronic inflammatory disorders collectively referred to as inflammatory bowel diseases (IBD). Cysteinyl leukotrienes are proinflammatory 5-lipoxygenase-derived products that play a major role in the immune and inflammatory response. Consequently, they may be involved in the pathogenesis of IBD. The aim of this study was therefore to evaluate 1) the urinary excretion of leukotriene E(4) (LTE(4)) in IBD patients and healthy volunteers, and 2) the association between LTE(4) production and the activity (relapse/remission) of the disease. METHODS: IBD patients and healthy volunteers were prospectively recruited. CD and UC activity was determined on inclusion with the Crohn's Disease Activity Index and Clinical Activity Index, respectively. Urine was collected and the urinary excretion of LTE(4) was measured by liquid chromatography tandem mass spectrometry. RESULTS: 32 CD patients, 28 UC patients, and 30 controls were enrolled in the study. LTE(4) urinary excretion was significantly increased (P < 0.01) in CD [52.0 pg/mg creatinine (10th-90th percentiles: 26.2-148.0)] and UC [64.1 pg/mg creatinine (10th-90th percentiles: 26.7-178.0)] patients compared to controls [32.3 pg/mg creatinine (10th-90th percentiles: 21.8-58.8)]. LTE(4) levels were higher (P < 0.001) in patients with active disease than in patients in remission, for whom the levels of LTE(4) were similar to the levels of controls. CONCLUSIONS: Cysteinyl leukotriene pathway activation could contribute to the inflammation associated with IBD. The quantification of urinary LTE(4) could be an interesting noninvasive biomarker for the assessment of IBD activity
Intérêt d'un logiciel de déconvolution (AMDIS) et d'une détection SIM/SCAN pour le screening toxicologique par CPG-SM
Objectif : Cette Ă©tude propose une optimisation du screening
toxicologique classique en chromatographie gazeuse couplée à une
détection en spectrométrie de masse (CPG-SM), en utilisant un
logiciel de déconvolution : Automated Mass Spectral Deconvolution and
Identification System (AMDIS) et une détection simultanée en mode
SIM et en mode SCAN. MĂ©thodes : Les analyses sont
effectuées avec un appareil CPG-SM Agilent équipé d'un passeur
d'échantillons liquides et associé aux logiciels Retention Time
Locking Software (RTL) et AMDIS. Deux bibliothèques commerciales (PMW et
NIST) sont utilisées ainsi qu'une bibliothèque propre au
laboratoire. Un plasma A surchargé avec 16 molécules et un autre
plasma B surchargé avec de la buprénorphine et son métabolite la
norbuprénorphine ont été utilisés pour tester respectivement
le logiciel AMDIS et la sensibilité de la détection SIM/SCAN.
Résultats : Parmi les 16 molécules du plasma A, 11 ont
été mises en évidence par la déconvolution tandis que
seulement 5 ont été identifiées par une recherche en librairie
classique. Cinq composés n'ont pas été repérés pour des
problèmes chromatographiques ou de sensibilité en mode SCAN. La
buprénorphine et son métabolite n'ont été mis en
évidence que grâce au mode SIM/SCAN. Conclusion : La
déconvolution permet de détecter des molécules même
lorsqu'aucun pic n'est repéré sur le chromatogramme, ceci se
vérifie au quotidien sur des cas réels. Le mode SIM/SCAN permet de
diminuer le nombre d'injections, tout en conservant la mĂŞme
sensibilité
5-lipoxygenase expression and activity in aorta from streptozotocin-induced diabetic rats.
International audienceWe previously reported an activation of the 5-lipoxygenase pathway in aorta from streptozotocin-induced diabetic rats. The aim of this study was to investigate whether this activation was associated with an increased expression of 5-lipoxygenase, an increased cysteinyl leukotriene (CysLT) production in response to arachidonic acid or calcium ionophore A23187 and/or a hypersensitivity of the aorta to CysLTs in streptozotocin-induced diabetic rats. In aorta from diabetic and control rats, reverse transcriptase-PCR and western blot analysis with a specific 5-lipoxygenase antibody provided evidence for the presence of 5-lipoxygenase in aorta. However, the expression of 5-lipoxygenase was not significantly different between diabetic and control rats. Challenge by A23187 (10 microM) and arachidonic acid (10 microM and 0.1 mM) with or without A23187 (10 micromol/l) induced a significant increase of CysLT release (measured by enzyme immunoassay) that was in the same range in aorta from control and diabetic rats. In contrast, aortas from diabetic rats showed a greater sensitivity to LTC4 and LTD4 contractile effects. These data suggested that the activation of the 5-lipoxygenase pathway previously reported in streptozotocin-induced diabetic rats could be explained by an augmented sensitivity to CysLTs of the diabetic aorta
Ribavirin Quantification in Combination Treatment of Chronic Hepatitis C
Ribavirin in combination with alpha 2 interferon is the consensus treatment for chronic hepatitis C. However, recent preliminary pharmacological studies have suggested that the bioavailability of ribavirin displays great interindividual variability. In order to monitor serum ribavirin levels during combination treatment, we developed and validated a quantitative assay using an approach adaptable for routine hospital laboratories. The method involved solid-phase extraction on phenyl boronic acid cartridges followed by high-performance liquid chromatography with a C(18)-bonded silica column and a mobile phase containing 10 mM ammonium phosphate buffer (with the pH adjusted to 2.5) and UV detection (207 nm). The sensitivity, recovery, linearity of the calibration curve, intra- and interassay accuracies, precision, and stability at 4°C were consistent with its use in the laboratory routine. In addition, other nucleoside analogues sometimes used with ribavirin in patients coinfected with hepatitis C virus (HCV) and human immunodeficiency virus did not interfere with the quantification of ribavirin levels. The ribavirin concentration was quantified in 24 serum samples from patients with chronic hepatitis C treated with a combination of ribavirin and alpha 2 interferon. The mean serum ribavirin concentration was 2.67 ± 1.06 μg/ml (n = 24) at week 12 of treatment (W12) and 3.24 ± 1.35 μg/ml (n = 24) at week 24 of treatment (W24). In addition, ribavirin concentrations displayed high interindividual variabilities: the coefficients of variation of the serum ribavirin concentrations adjusted to the administered dose were 44 and 48% at W12 and W24, respectively. Moreover, the ribavirin concentration was higher in patients with a sustained virological response (n = 11) than in patients with treatment failure (n = 13), with significant intergroup differences at W12 (P = 0.030) and W24 (P = 0.049). The present study describes a simple analytical method for the quantification of ribavirin in human serum that could be a useful tool for the monitoring of ribavirin concentrations in HCV-infected patients in order to improve the efficacy and safety of therapy with ribavirin plus interferon
