    Temporal evolution of carbon stocks, fluxes and carbon balance in pedunculate oak chronosequence under close-to-nature forest management

    Under current environmental changes, forest management is challenged to foster contrasting benefits from forests, such as continuous wood supply while preserving biomass production, biodiversity conservation, and contribution to climate change mitigation through atmospheric carbon sequestration. Although being found as globally important, estimates of long-term forest C balance are still highly uncertain. In this context, the chronosequence experiments (space-for-time substitution) might fill this gap in even-aged forests, as they represent an approach that enables the assessment of forest net C balance in the long term. In this research, we explored the dynamics of C stocks and fluxes in different forest pools throughout the rotation period (140 years) of a Pedunculate oak (Quercus robur L.) forest in Croatia. For this purpose, we selected a chronosequence that was made up of seven forest stands with different age (5, 13, 38, 53, 68, 108, and 138 years). To address the issues of uncertainty in C balance estimates, we compared net ecosystem carbon balance (NECB) estimated while using two different approaches, which we name pool-change (from C stocks) approach and component-flux (from C fluxes) approach. Overall, the pool-change approach showed higher NECB estimate, with the greatest difference being observed in younger stands (<50 years). Component-flux approach showed significantly higher uncertainty. Throughout the rotation period, managed pedunculate oak stands become a C sink early in their development phase, between the age of 13 and 35 years according to pool-change and component-flux approach, respectively. During the 140 years, oak forest provided 187.2 Mg C ha−1 (604 m3 ha−1) through thinnings and 147.9 Mg C ha−1 (477 m3 ha−1) in the final cut, while preserving, on average, 88.9 Mg C ha−1 in mineral soil down to 40 cm, 18.2 Mg C ha−1 in dead wood, and 6.0 Mg C ha−1 in the forest floor. Soil C stocks in our chronosequence did not show any age-related trend, indicating that current management practice has no negative effect on soil C stocks. Finally, under current close-to-nature forest management, Pedunculate oak forest showed to be sustainable in providing both economic and ecological ecosystem services

    Positrons in gas filled traps and their transport in molecular gases

    In this paper we give a review of two recent developments in positron transport, calculation of transport coefficients for a relatively complete set of collision cross sections for water vapour and for application of they Monte Carlo technique to model gas filled subexcitation positron traps such as Penning Malmberg Surko (Surko) trap. Calculated transport coefficients, very much like those for argon and other molecular gases show several new kinetic phenomena. The most important is the negative differential conductivity (NDC) for the bulk drift velocity when the flux drift velocity shows no sign of NDC. These results in water vapour are similar to the results in argon or hydrogen. The same technique that has been used for positron (and previously electron) transport may be applied to model development of particles in a Surko trap. We have provided calculation of the ensemble of positrons in the trap from an initial beam like distribution to the fully thermalised distribution. This model, however, does not include plasma effects (interaction between charged particles) and may be applied for lower positron densities

    Undergraduate medical textbooks do not provide adequate information on intravenous fluid therapy: a systematic survey and suggestions for improvement

    <b>Background</b><p></p> Inappropriate prescribing of intravenous (IV) fluid, particularly 0.9% sodium chloride, causes post-operative complications. Fluid prescription is often left to junior medical staff and is frequently poorly managed. One reason for poor intravenous fluid prescribing practices could be inadequate coverage of this topic in the textbooks that are used.<p></p> <b>Methods</b><p></p> We formulated a comprehensive set of topics, related to important common clinical situations involving IV fluid therapy, (routine fluid replacement, fluid loss, fluids overload) to assess the adequacy of textbooks in common use. We assessed 29 medical textbooks widely available to students in the UK, scoring the presence of information provided by each book on each of the topics. The scores indicated how fully the topics were considered: not at all, partly, and adequately. No attempt was made to judge the quality of the information, because there is no consensus on these topics.<p></p> <b>Results</b><p></p> The maximum score that a book could achieve was 52. Three of the topics we chose were not considered by any of the books. Discounting these topics as “too esoteric”, the maximum possible score became 46. One textbook gained a score of 45, but the general score was poor (median 11, quartiles 4, 21). In particular, coverage of routine postoperative management was inadequate.<p></p> <b>Conclusions</b><p></p> Textbooks for undergraduates cover the topic of intravenous therapy badly, which may partly explain the poor knowledge and performance of junior doctors in this important field. Systematic revision of current textbooks might improve knowledge and practice by junior doctors. Careful definition of the remit and content of textbooks should be applied more widely to ensure quality and “fitness for purpose”, and avoid omission of vital knowledge

    Challenges and improvement needs in the care of patients with central diabetes insipidus

    Central diabetes insipidus (CDI) is a rare condition, with significant impact on patient health and well-being. It is a chronic condition which usually requires meticulous long-term care. It can affect both children and adults. There is limited literature considering the needs and challenges inherent in providing high quality care to patients with CDI, across the care pathway. This paper seeks to address this gap by providing a unique and well-rounded understanding of clinical and healthcare systems-related challenges. It draws on insights from the literature, from direct clinical experience contributed by five clinicians as co-authors (providing insights from France, Ireland, Italy, Spain and the United Kingdom), and from patient perspectives provided through interviews with patient representatives from three patient organisations. We identify clinical challenges related to the diagnosis of CDI, including differentiating between other similar conditions and determining the underlying aetiology. Treatment is challenging, given the need to tailor medication to each patient’s needs and ongoing management is required to ensure that patients continue to respond adequately to treatment. Ongoing support is required when patients switch between formulations. We also identify healthcare systems challenges related to limited awareness of CDI amongst primary care physicians and general paediatricians, and the need for highly skilled specialist care and appropriate workforce capacity. There is also a significant need for raising awareness and for the education of both healthcare professionals and patients about different aspects of CDI, with the aim of supporting improved care and effective patient engagement with healthcare professionals. We reflect on this information and highlight improvement opportunities. These relate to developing guidance to support patients, carers, primary care physicians and general paediatricians to identify clinical features earlier, and to consider CDI as a possible diagnosis when a patient presents with suggestive symptom

    On new developments in the physics of positron swarms

    Recently a new wave of swarm studies of positrons was initiated based on more complete scattering cross section sets. Initially some interesting and new physics was discovered, most importantly negative differential conductivity (NDC) that occurs only for the bulk drift velocity while it does not exist for the flux property. However the ultimate goal was to develop tools to model positron transport in realistic applications and the work that is progressing along these lines is reviewed here. It includes studies of positron transport in molecular gases, thermalization in generic swarm situations and in realistic gas filled traps and transport of positrons in crossed electric and magnetic fields. Finally we have extended the same technique of simulation (Monte Carlo) to studies of thermalization of positronium molecule. In addition, recently published first steps towards including effects of dense media on positron transport are summarized here

    Evaluating a complex research capacity-building intervention: reflections on an evaluation of the African Institutions Initiative

    Increasing policy demand for realist evaluations of research and capacity-building programmes reflects a recognition of the management, governance and impact gains that can result from evaluation. However, the evidence base on how to successfully implement realist evaluations of complex interventions in international development efforts is scarce. We know little about the associated merits, limitations and ways to mitigate challenges. There is a need for reflective work which considers the methodology in context. This paper shares learning from the experience of conducting a realist, theory-of-change driven evaluation of the African Institutions Initiative, a Wellcome Trust funded programme which aimed to build sustainable health research capacity in Africa at institutional and network levels, across seven research consortia. We reflect on the key challenges experienced throughout the evaluation and recommend ways of managing them, highlight opportunities and critical success factors associated with this evaluation approach, as well as elaborate on alternative evaluation approaches

    Influence of Sleeve Gastrectomy on NASH and Type 2 Diabetes Mellitus

    Background. Nonalcoholic fatty liver disease is present in up to 85% of adipose patients and may proceed to nonalcoholic steatohepatitis (NASH). With insulin resistance and obesity being the main risk factors for NASH, the effect of isolated sleeve gastrectomy (ISG) on these parameters was examined. Methods. 236 patients underwent ISG with intraoperative liver biopsy from December 2002 to September 2009. Besides demographic data, pre-operative weight/BMI, HbA1c, AST, ALT, triglycerides, HDL and LDL levels were determined. Results. A significant correlation of NASH with higher HbA1c, AST and ALT and lower levels for HDL was observed (P < .05, <.0001, <.0001, <.01, resp.). Overall BMI decreased from 45.0 ± 6.8 to 29.7 ± 6.5 and 31.6 ± 4.4 kg/m2 at 1 and 3 years. An impaired weight loss was demonstrated for patients with NASH and patients with elevated HbA1c (plateau 28.08 kg/m2 versus 29.79 kg/m2 and 32.30 kg/m2 versus 28.79 kg/m2, resp.). Regarding NASH, a significant improvement of AST, ALT, triglyceride and HDL levels was shown (P < .0001 for all). A resolution of elevated HbA1c was observed in 21 of 23 patients. Summary. NASH patients showed a significant loss of body weight and amelioration of NASH status. ISG can be successfully performed in these patients and should be recommended for this subgroup

    Measurements of top quark spin observables in tt events using dilepton final states in √s=8 TeV pp collisions with the ATLAS detector

    Measurements of top quark spin observables in tt events are presented based on 20.2 fb(-1) of root s = 8TeV proton-proton collisions recorded with the ATLAS detector at the LHC. The analysis is performed in the dilepton final state, characterised by the presence of two isolated leptons ( electrons or muons). There are 15 observables, each sensitive to a different coefficient of the spin density matrix of tt production, which are measured independently. Ten of these observables are measured for the first time. All of them are corrected for detector resolution and acceptance effects back to the parton and stable-particle levels. The measured values of the observables at parton level are compared to Standard Model predictions at next-to-leading order in QCD. The corrected distributions at stable-particle level are presented and the means of the distributions are compared to Monte Carlo predictions. No significant deviation from the Standard Model is observed for any observable
