Effect of short- and long-time exposure to ambient air pollution on blood pressure
It has been known for a number of years that air pollution significantly increases the morbidity and mortality of people exposed. More and more research and the last two meta-analyses also showed that even a short-time increase in air pollution can raise blood pressure and long-term air pollution leads to an increase in the incidence and prevalence of arterial hypertension. The elevation of blood pressure caused by air pollution may contribute to the increase in cardiovascular diseases observed in areas affected by air pollution. This increase in blood pressure caused by air pollution can be particularly dangerous for women, the elderly, obese people and those already burdened with cardiovascular diseases
Cardiac troponin measured by a high-sensitivity assay in hypertensive patients
Thanks to the rapid advances in laboratory techniques sensitive assays for measurement of cardiac troponin level have been recently introduced, and now is possible to determine ten-fold lower concentration of troponin in blood serum than the standard method used until now. The blood level of highly sensitive cardiac troponin in hypertensive patients is higher than in healthy persons, which may indicate increased necrosis or apoptosis of cardiac cells. Elevated high-sensitivity cardiac troponin concentration is an important risk factor for future cardiovascular complications independent of many co-existing contributing factors. In particular, elevated concentration of this troponin indicates the left ventricular hypertrophy and a tendency to develop heart failure. It has also been shown that elevated concentrations of high-sensitivity troponin are more common in non-dipper patients than in those with a normal circadian pattern of blood pressure. Few studies so far indicate that currently used antihypertensive therapy only slightly lower the increase concentration of those compounds in serum

The most common metric used is standard deviation of mean blood pressure, variation independent of the mean and average real variability. Blood pressure variability is higher in female, older age and patients with hypertension, with cardiovascular diseases, with diabetes mellitus and chronic kidney disease. Therefore, we are not sure if higher blood pressure variability is the only marker of arterial stiffness and consequence of end organ damage or prognostic indicator of cardiovascular events. We also do not know whether reducing blood pressure variability would reduce the risk of cardiovascular outcomes. Sacubitril/valsartan in therapy of hypertension
In recent years a new very promising drug was introduced for the treatment of chronic heart failure. This drug, sacubitril/valsartan, prevents the clinical progression of surviving patients with heart failure more effectively than enalapril and it reduces the need for hospitalization by more than 20%. At the same time it has been shown that it reduces blood pressure particularly in patients with essential hypertension. This drug is an inhibitor of neprilysin, an enzyme that degrades natriuretic factors. Natriuretic factors (ANP, BNP, CNP) increase diuresis and natriuresis, reduce the activity of the sympathetic system, relax the blood vessels and inhibit fibrosis. Several studies have shown that sacubitril/valsartan therapy more significantly reduces blood pressure in patients with salt-sensitive or refractory to treatment hypertension than is observed with valsartan or amlodipine. Furthermore, beneficial effects of this drug on central blood pressure and renal function have been demonstrated in patients with chronic renal failure. More rarely than enalapril it leads to hyperkalaemia in patients treated simultaneously with mineralocorticoid receptor inhibitors. Sacubitril/valsartan is the first representative of a new class of drugs that inhibit neprilysin, which probably in the future will be frequently used in hypertension

Zalecenia dietetyczne rekomendowane przez Towarzystwa Nadciśnienia Tętniczego dotyczące umiarkowanego ograniczenia soli kuchennej w diecie chorych na nadciśnienie tętnicze zostały ostatnio podważone. W piśmiennictwie podkreśla się nikły wpływ diety z ograniczeniem sodu na wartości ciśnienia tętniczego. Przeciwnicy stosowania rygorów dietetycznych zwracają uwagę na bezcelowe ich stosowanie u chorych sodoopornych. Ponadto notowano wzrost insulinooporności oraz wzrost stężenia lipidów w surowicy krwi w następstwie przestrzegania diety ze znacznym ograniczeniem sodu. Przede wszystkim jednak zdaniem Aldermana i wsp. dieta niskosodowa zwiększa prawdopodobieństwo
wystąpienia zawału serca. Wyniki obserwacji klinicznych publikowane w ostatnich latach nie potwierdzają jednak ujemnych następstw stosowania diety niskosodowej. Według tych danych dieta wysokosodowa sprzyja chorobie niedokrwiennej serca, a zwłaszcza udarom mózgu, a w przypadku osób otyłych zwiększa ogólną śmiertelność.
Pojedyncze prace sugerują również niekorzystny wpływ diety wysokosodowej na przerost lewej komory serca, ciśnienie wewnątrzkłębuszkowe i białkomocz, a także częstość
kamicy moczowej i raka żołądka. Celowe są długotrwałe badania dotyczące wpływu umiarkowanego ograniczenia sodu w diecie chorych z nadciśnieniem
tętniczym na układ krążenia, niemniej jednak rekomendacje Towarzystw Nadciśnienia Tętniczego przedstawione na początku są nadal aktualne.Sodium-reduced diet widely recommended by various societies of hypertension has been recently questioned. Low impact
of sodium reduced diet on blood pressure and its lack of effect in salt resistant patients is stressed in literature. Severe sodium reduced diet increases insulin resistance and cholesterol level in blood. Alderman has described increased risk of myocardial infarction in subject on sodium reduced diet. However, results of last studies do not confirm these observations. Analysis of several studies indicates that high sodium
intake is strongly and independently associated with increased risk of cardiovascular disease and with all-cause mortality in overweight subjects. Some papers suggest adverse effect of high sodium intake on left ventricular mass, glomerular capillary pressure, risk of proteinuria, kidney stone formation and cancer of the stomach. Long term studies on efficacy and safety of sodium reduced diet are advisable, but this diet should continue to be recommended for
patients with hypertension
Ischaemic heart disease and hypertension in patients with chronic obstructive pulmonary disease and obstructive sleep apnoea
Przewlekła obturacyjna choroba płuc (POChP) dotyka prawie 10% dorosłej populacji w Polsce, obturacyjny bezdech senny jest także coraz częściej rozpoznawany i dotyczy w zależności od przyjętych kryteriów od 2 do 9% kobiet i od 4 do 24% mężczyzn. Największa śmiertelność w POChP nie jest spowodowana niewydolnością oddechową, lecz powikłaniami sercowo-naczyniowymi, w tym niedokrwieniem serca. Obturacyjny bezdech senny w połowie przypadków powikłany jest nadciśnieniem tętniczym, bardzo często opornym na terapię hipotensyjną. W pracy omówiono patogenezę choroby niedokrwiennej serca u chorych na POChP zwracając szczególną uwagę na stan zapalny występujący w obu tych jednostkach chorobowych. Przedstawiono także patogenezę nadciśnienia tętniczego w przebiegu obturacyjnego bezdechu sennego ze szczególnym uwzględnieniem hipoksji i pobudzenia układu sympatycznego. Zapobieganie chorobie niedokrwiennej serca powinno stanowić jeden z priorytetów postępowania w POChP. W pracy omówiono także terapię choroby niedokrwiennej serca, zwracając uwagę na modyfikację tego leczenia u chorych na POChP oraz przedyskutowano wpływ leków stosowanych w POChP na progresję niedokrwienia serca. Nadciśnienie tętnicze w przebiegu obturacyjnego bezdechu sennego jest bardzo często oporne na terapię pomimo zastosowania w godzinach nocnych ciągłego dodatniego ciśnienia powietrza w drogach oddechowych, natomiast często zmniejsza się po zastosowaniu antagonistów aldosteronu. Zwrócono także uwagę na przeciwzapalne działanie statyn i próby ich stosowania w zapobieganiu zaostrzeń POChP.Chronic obstructive pulmonary disease (COPD) affects almost 10% of the adult population of our country; obstructive sleep apnoea is increasingly being recognized and concerns, according to accepted criteria, 2–9% of females and 4–24% of men. The greatest mortality in chronic obstructive pulmonary disease is not caused by respiratory failure, but cardiovascular complications, including ischaemic heart disease. Obstructive sleep apnoea in half the cases is complicated by hypertension, often refractory to antihypertensive therapy. The paper discusses the pathogenesis of ischaemic heart disease in patients with COPD with particular attention to the inflammation that occurs in these two diseases. The pathogenesis of hypertension in the course of obstructive sleep apnoea is also presented with particular emphasis on hypoxia and sympathetic stimulation. Prevention of coronary heart disease should be a priority of the procedure in chronic obstructive pulmonary disease. The paper also discusses the treatment of ischaemic heart disease, paying attention to the modification of treatment in patients with chronic obstructive pulmonary disease, and discussing the influence of drugs used in COPD on the progression of ischaemic heart disease. Hypertension in the course of obstructive sleep apnoea is often resistant to therapy despite the use of continuous positive airway pressure devices, and often decrease after the use of aldosterone antagonists. Attention is drawn to the anti-inflammatory action of statins and trials of their use in the prevention of exacerbations of chronic obstructive pulmonary disease
Działania przeciwnadciśnieniowe
Wielokrotnie zdarzało się, że leki stosowane od lat
w terapii jakiegoś schorzenia nagle uzyskiwały nowe,
odmienne wskazania. Nitroglicerynę od dawna stosuje
się w łagodzeniu dolegliwości bólowych związanych
z niedokrwieniem serca. Dożylne wlewy nitrogliceryny
są stosowane w doraźnym obniżaniu wysokiego
nadciśnienia tętniczego. Jednak stosunkowo
od niedawna ponawia się próby doustnego stosowania
tego leku w izolowanym nadciśnieniu skurczowym.
W badaniach wykazano, że nitrogliceryna,
w przeciwieństwie do hydralazyny, znacznie zmniejsza
indeks wzmocnienia (augmentation index), obniżając
ciśnienie centralne, jest więc szczególnie wskazana
w leczeniu nadciśnienia u osób w podeszłym
wieku. Pierwsze kontrolowane badania kliniczne
wskazują, że, podając nitroglicerynę, można obniżyć
skurczowe ciśnienie tętnicze o 10-14 mm Hg.
Magen i wsp. wykazali, że podawanie kwasu acetylosalicylowego
w dawce kardiologicznej (100 mg/d.)
chorym z nadciśnieniem tętniczym otrzymującym statyny
nieznacznie, lecz znamiennie obniża ciśnienie
tętnicze. Jednocześnie cytowani autorzy oceniali
czynność śródbłonka i wykazali, że podanie kwasu acetylosalicylowego poprawiało jego czynność, lecz
tylko u chorych leczonych statynami.
Analiza badania PROGRESS ujawniła, że osoby w podeszłym
wieku leczone inhibitorem konwertazy
(perindoprilem) rzadziej zapadają na zapalenie płuc.
Wśród Azjatów to prewencyjne działanie inhibitora
konwertazy osiągnęło istotność statystyczną. Takashi
i wsp. wśród ponad 500 chorych leczonych inhibitorem
konwertazy angiotensyny również obserwowali
istotnie rzadsze występowanie zapaleń płuc.
Osoby w starszym wieku często mają upośledzony
odruch kaszlowy, co prowadzi do zalegania wydzieliny
w drzewie oskrzelowym i sprzyja stanom zapalnym.
Podwyższone stężenia bradykininy i substancji
P w trakcie stosowania inhibitorów konwertazy zwiększają
odruch kaszlowy, ułatwiając odksztuszanie i zapobiegając
w ten sposób rozwojowi zapalenia.
Nowe wskazania dla omawianych leków są kontrowersyjne,
lecz wymagając uważnego sprawdzenia

Pathogenesis and therapy resistant hypertension in patients with obstructive sleep apnea. The role of aldosterone
Nadciśnienie tętnicze oporne na terapię rozpoznaje się u tych chorych, u których podawanie trzech leków hipotensyjnych, w tym diuretyku, w optymalnych dawkach nie powoduje normalizacji ciśnienia tętniczego. Ostatnie badania wykazały, że nadciśnienie tętnicze oporne na terapię nie jest schorzeniem sporadycznym, lecz występuje u około 12–15% wszystkich chorych z nadciśnieniem tętniczym. Najczęstszą, bo w 60–80%, przyczyną oporności na terapię nadciśnienia tętniczego jest obturacyjny bezdech senny (OSA, obstructive sleep apnea). Patogeneza nadciśnienia tętniczego u chorych z OSA jest złożona. Bardzo ważną przyczyną jest wzmożone napięcie układu sympatycznego wywołane powtarzającymi się bezdechami. Kolejnymi czynnikami są stres oksydacyjny i przewlekły stan zapalny, jaki towarzyszy OSA. Przewlekły stan zapalny, jak wykazano w wielu pracach, może prowadzić do rozwoju nadciśnienia. W przebiegu OSA wykazano wzmożoną aktywność układu renina–angiotensyna–aldosteron (RAA) oraz upośledzenie czynności śródbłonka. Zmniejszone przesączenie kłębuszkowe prowadzi do postępującej niewydolności nerek, a to z kolei powoduje wzrost ciśnienia tętniczego. Ostatnio zwrócono uwagę na stosunkowo częste (w 20–30%) występowanie podwyższonego stężenia aldosteronu u chorych z OSA. Kilku autorów wykazało, że wzrost stężenia aldosteronu w surowicy krwi koreluje z nasileniem objawów OSA. Nadmiar aldosteronu w połączeniu z dietą wysokosodową prowadzi do zatrzymania wody w ustroju, obrzęku śluzówek gardła i upośledzenia drożności dróg oddechowych. W terapii nadciśnienia tętniczego w przebiegu OSA należy zwrócić uwagę na częstą u tych chorych redukcję nadwagi oraz stosowanie diety z ograniczeniem soli kuchennej. Stosowanie protezy powietrznej najczęściej nieznacznie tylko obniża dobowe ciśnienie tętnicze. W terapii farmakologicznej stosuje się inhibitory konwertazy angiotensyny lub sartany, leki z grupy antagonistów wapnia oraz diuretyki (tiazydy, indapamid lub furosemid). Niektórzy autorzy z powodzeniem stosowali jako kolejne leki beta- lub alfa-adrenolityki. Najbardziej jednak skuteczne jest dodanie antagonistów aldosteronu. Leki te bardziej obniżają ciśnienie tętnicze niż inne grupy leków hipotensyjnych i są skuteczne w obniżaniu ciśnienia również u chorych z prawidłowym stężeniem aldosteronu w surowicy krwi. Gaddam i wsp. wykazali ponadto, że istotnie zmniejszają liczbę bezdechów sennych.Resistant hypertension is defined as blood pressure remaining above goal (< 140/90 mm Hg) despite the concurrent use 3 antihypertensive agents of different classes including diuretics in optimal doses. Recent studies have shown that hypertension resistant do therapy is not a rare diseases but occurs in approximately 12–15% of all patients with hypertension. The most common because 60–80% causing resistant hypertension is obstructive sleep apnea (OSA). The pathogenesis of hypertension in patients with OSA is complicated. A very important reason is increased sympathetic activity caused by repetitive apnea. Other factors are oxidative stress and chronic inflammation that accompanies OSA. Chronic inflammation has been shown in many works can lead to the development of hypertension. In the course of OSA have shown increased activity of the renin-angiotensin-aldosterone system and endothelial dysfunction. Reduced glomerular filtration lead to progressive renal failure, which in turn causes an increase in blood pressure. Recently drew attention to the relatively high prevalence (20–30%) of elevated levels of aldosterone in blood in patients with OSA. Several authors have shown that the increase in aldosterone levels in serum correlates with the severity of OSA. Aldosterone excess in combination with high sodium diet leading to water retention, swelling of the mucous membranes of the throat and obstructive airways. In lifestyle changes patients with hypertension and OSA should pay attention to weight reduction and dietary salt restriction. The use of CPAP mostly only slightly lowers blood pressure during day. ACE inhibitors and sartans, calcium antagonists and diuretics (thiazides, indapamide or furosemide) are used in pharmacological treatment. Some authors have successfully used beta and alpha blockers. However, the most effective is to add aldosterone blockers. These drugs reduce blood pressure more than other groups of antihypertensive agents and are also effective in lowering the blood pressure in patients with normal serum aldosterone levels. Gaddam et al. also demonstrated that spironolactone significantly reduce the severity of OSA
How low is recommended to decrease the blood pressure in patients with hypertension, diabetes mellitus and glaucoma?

Hypertension/European Society of Cardiology (ESC/ESH) u pacjentów z cukrzycą i nadciśnieniem tętniczym
powinno się dążyć do uzyskania docelowych wartości ciśnienia tętniczego poniżej 130/80 mm Hg. Jeszcze
niższe wartości są zalecane w nocy. Istotnym, ale
niedocenianym problemem związanym z leczeniem
hipotensyjnym u osób z cukrzycą są polekowe nocne
spadki ciśnienia tętniczego. Są one związane z ryzykiem
rozwoju lub progresji jaskry, zwłaszcza u pacjentów
z dysfunkcją autoregulacji krążenia ocznego.
Szczególnym ryzykiem obarczone są osoby z cukrzycą.
Brak dotychczas jednoznacznych zaleceń co do doboru
leków w tym zakresie, ale przesłanki wskazują na większe
bezpieczeństwo terapii opartej na lekach beta-adrenolitycznych.
Konieczne są dalsze randomizowane badania,
które ostatecznie wyjaśnią rolę nocnej hipotonii
w progresji jaskry u chorych na cukrzycę. Badania te
powinny ustalić bezpieczne wartości ciśnienia, zwłaszcza
rozkurczowego w godzinach nocnych, do jakich
można obniżać ciśnienie tętnicze u chorych z jaskrą,
cukrzycą i nadciśnieniem tętniczym

z nadwagą i nadciśnieniem tętniczym oraz u prawie
połowy szczupłych chorych z nadciśnieniem tętniczym pierwotnym. W pracy omówiono mechanizmy
prowadzące do insulinooporności przedreceptorowej, receptorowej i poreceptorowej, ze szczególnym uwzględnieniem tych zmian, które prawdopodobnie występują w pierwotnym nadciśnieniu
tętniczym. Metody służące rozpoznawaniu insulinooporności oparte są na równoczesnych pomiarach
stężeń insuliny i glukozy w surowicy krwi. Najdokładniejsze metody oceny insulinooporności polegają
na pomiarach stężeń insuliny i glukozy w surowicy
w warunkach stałego, określonego ilościowo wlewu
glukozy i insuliny lub też stałego dożylnego wlewu
insuliny i glukozy z tak dobraną szybkością, aby
utrzymać fizjologiczne stężenie tego cukru w surowicy krwi. Zarówno pierwsza metoda (tzw. steadystate plasma glucose), jak i kolejna (tzw. klamra metaboliczna) są jednak pracochłonne i uciążliwe dla
pacjenta. Wśród nowych metod, oceniających stopień
wrażliwości na działanie insuliny, zastosowano jądrowy rezonans magnetyczny oraz podawanie znakowanych: glukozy, insuliny, aminokwasów i tłuszczy. Złotym standardem pozostaje jednak metoda
klempu insulinowego. Wysoka korelacja pomiędzy
wiązaniem insuliny do receptorów insulinowych

zmierzonej metodą klamry metabolicznej nasuwa
możliwość szybkiej i dokładnej oceny wrażliwości

Mechanisms leading to prereceptor, receptor and
postreceptor insulin resistance with special auention paid to
those changes which may occur in essential hypertension have
been discussed in this study The methods used to diagnose
insulin resistance are based on simultaneous measuring of
insulin and glucose concentration in blood serum. The most
exact methods of estimating insulin resistance consist in
measuring glucose and insulin concentration in serum during steady-state, quantitatively defined glucose or insulin infusion at such a speed that the physiological glucose concentration in blood serum can be maintained. Both methods
however- i. e. steady-state plasma glucose as well as metabolic clamp are laboratorious and tiresome for the patient.
Nuclear magnetic resonance and administrating marked glucose, insulin, amino acids and fats have been used to estimate the degree of insulin sensitivity. The insulin clamp
method, however, still remains the golden standard.
