Skin microcirculation and echocardiographic and biochemical indices of left ventricular dysfunction in non-diabetic patients with heart failure
Background: We sought to noninvasively investigate skin microcirculation and to assess the
relation between parameters of microcirculation and echocardiographic and biochemical parameters
of left ventricular (LV) function in non-diabetic patients with heart failure (HF).
Methods and Results: We measured skin microcirculation with laser Doppler flowmetry
(LDF) at basal conditions (MFb), after warming to 44 degrees Centigrade (MF44) and after
occlusion (AUC, PF%). Blood was sampled for NT-proBNP. We obtained information on
patients’ medical history and medication status. The mean (SD) age of 100 patients (43 women)
was 68.2 ± 11.5 years. LV ejection fraction (LVEF) averaged 34.9 ± 13.3%, LV end-diastolic
diameter (LVEDD) 6.0 ± 0.9 cm, NT-proBNP 4,582.6 ± 6,339.7 pg/mL. The parameters of
microcirculation averaged: MFb 6.2 ± 4.7 perfusion units (PU), PF% 716.0 ± 437.8%, AUC
794 ± 706.1 PU/s, and MF44 77.9 ± 40.2 PU. NT-proBNP correlated negatively with LVEF
(p £ 0.0001) and positively with LVEDD (p = 0.003). MFb was positively correlated with
LVEF (r = 0.24, p = 0.03), and MF44 was negatively correlated with LVEDD (r = 0.22,
p = 0.02). The relations remained significant after adjustments for sex, age, and use of
medication. We observed no relation between NT-proBNP and microcirculatory derangement.
Conclusions: LDF-derived parameters of skin microcirculation are related to echocardiographic,

Short-term efficacy of telmisartan in antihypertensive treatment in 6985 patients

dotyczy głównie obniżania ciśnienia
tętniczego. Prezentowane badanie miało na
celu ocenę krótkoterminowej skuteczności przeciwnadciśnieniowej
telmisartanu - długodziałającego
leku blokującego receptory typu pierwszego dla angiotensyny
II. Badanie było otwartą, nieinterwencyjną,
9-tygodniową obserwacją pacjentów z nadciśnieniem
tętniczym, u których do leczenia włączono 40-80 mg
telmisartanu. Oceniano skuteczność leku w obniżaniu
ciśnienia tętniczego w pomiarze gabinetowym
oraz w pomiarach domowych, a także wpływ leczenia
na liczbę pomiarów domowych przekraczających
w ciągu tygodnia wartości progowe (130/85 mm Hg)
w godzinach porannych i wieczornych.
Materiał i metody Przeciętny wiek 6985 pacjentów
(mężczyźni - 49,5%) wyniósł 56,3 (± 12,1) roku.
Wyjściowe ciśnienie tętnicze w pomiarze gabinetowym
wynosiło 161/95 (± 13/8) mm Hg, a w pomiarze
domowym - 158/94 (± 14/8) mm Hg. Odsetek
pomiarów domowych przekraczających 130/85 mm Hg
wynosił 46% w godzinach porannych i 43% w godzinach
wieczornych. Przeciętna dawka telmisartanu na
końcu obserwacji wynosiła 50,3 (± 17,6) mg, zaś
23,7% chorych przyjmowało hydrochlorotiazyd.
Wyniki Ciśnienie tętnicze gabinetowe obniżyło się
o 23/11 mm Hg (skurczowe/rozkurczowe), a w pomiarze
domowym o 28/14 mm Hg (skurczowe/rozkurczowe)
(p < 0,0001). Odsetek pomiarów domowych
przekraczających próg odcięcia zmniejszył się do 13%
w godzinach porannych i do 9% w godzinach wieczornych.
Według analizy w podgrupach, płeć i wiek
nie wpływały na osiągnięty efekt. Efekt przeciwnadciśnieniowy
nie był mniejszy u chorych z nadciśnie niem tętniczym powikłanym, osób palących, chorych
na cukrzycę i osób otyłych. Efekt był tym większy, im
wyższe było wyjściowe ciśnienie tętnicze (p < 0,0001).
U dwóch osób stwierdzono przejściowy wzrost parametrów
nerkowych wymagający odstawienia leku.
Wnioski W 9-tygodniowej obserwacji telmisartan
okazał się skutecznym i bezpiecznym lekiem przeciwnadciśnieniowym.Background Antihypertensive treatment-associated cardiovascular
protection is to a large extend a function of blood pressure
reduction. Current study aimed at assessing a short-term
antihypertensive efficacy of telmisartan - an angiotensin-II
type 1 receptor blocker. The study was an open, nine-week
observation of hypertensive patients in whom treatment with
telmisartan 40 to 80 mg/d was introduced. We assessed an
impact of telmisartan on blood pressure measured conventionally
in the office and obtained by self-measurements at
patients’ home. We also checked the impact the therapy had
on one-week frequency of morning and evening self-measurements
above the 130/85 mm Hg cut-off.
Material and methods Average age of 6985 patients (49.5%
men) was 56.3 (± 12.1) years. Baseline office blood pressure
averaged 161/95 (± 13/8) mm Hg, and baseline self-measured
blood pressure averaged 158/94 (± 14/8) mm Hg.
Percentage of self-measurements exceeding 130/85 mm Hg
was 46 in the morning and 43 in the evening. Average dose
of telmisartan was 50.3 (± 17.6) mg, and 23.7% of patients
received hydrochlorothiazide.
Results Office blood pressure fell by 23/11 mm Hg
(systolic/diastolic), and self-measured by 28/14 mm Hg
(p < 0.0001). The percentage of morning and evening
self-measured blood pressures exceeding the threshold fell
to 13 and 9, respectively. In the stratified analyses, sex and
age did not differentiate the antihypertensive response. The
effect was not inferior in patients with complicated hypertension, smokers, diabetics, and obese subjects. The effect
was proportional to level of baseline blood pressure
(p < 0.0001). In two patients we observed transient elevation
of renal function parameters which required drug withdrawal.
Conclusion In conclusion, in nine-week observation,
telmisartan proved to be an effective and safe antihypertensive
medication

Arterial hypertension after age 65: from epidemiology and pathophysiology to therapy Do we know where we stand?
Arterial hypertension is a prevalent disease with great harming potential. After the age of 55 years the remaining lifetime risk of hypertension amounts to 90%. Despite the constant advances some important issues such as the cut-off blood pressure for the initiation of antihypertensive therapy or the therapeutic goal are debated.
In this review, we present — based on the available literature — the current concepts concerning the pathophysiology, epi-demiology and antihypertensive therapy in patients aged 65 years or older.
The pathophysiology of hypertension in older patients in principle rests on stiffening of large conduit arteries, which leads to greater systolic and lower diastolic blood pressure. This in most older patients results in isolated systolic hypertension. Additionally most of these patients have low-renin hypertension. Data from large-scale clinical trials indicate that therapy of such individuals with thiazide-like diuretics and long-acting dihydropiridine calcium channel blockers as first-line medications reduces risk of complications. Based on results of recently published trials, meta-analyses, and prospective observations, the optimal on-treatment blood pressure values for most older hypertensive patients should be set within the 130–139 mmHg range. At present, lower values of standard office blood pressure in this group of patients have not been shown to be associ-ated with additional benefits, and may be associated with a greater risk of adverse events.
In conclusion, we recommend that for most patients aged 65 years or more, standard office systolic blood pressure should be cautiously reduced to within 140 and 130 mmHg, preferably with a thiazide-like diuretic, long acting dihydropiridine calcium channel blocker or their combination
