Dysphagie
Zusammenfassung: Die Differenzialdiagnosen von Patienten mit Schluckbeschwerden sind vielfĂ€ltig. Die Ă€tiologische AbklĂ€rung betrifft Störungen der Passage flĂŒssiger und fester Nahrung von der Mundhöhle bis in den distalen Ăsophagus und erfordert daher die Anwendung von multidisziplinĂ€ren diagnostischen Algorithmen. Eine oropharyngeale Dysphagie ist gekennzeichnet durch eine Bolustransportstörung im Rachenbereich, oft kombiniert mit HustenanfĂ€llen, Aspiration oder nasaler Regurgitation. HĂ€ufig ist sie Ausdruck neurologischer oder muskulĂ€rer Störungen. Die ösophageale Dysphagie bezeichnet eine subjektiv empfundene Passagebehinderung fĂŒr feste und flĂŒssige Nahrung, hĂ€ufig distal des Jugulums, und tritt in Kombination mit thorakalen Schmerzen, Regurgitationen, evtl. mit Husten und Aspirationen auf und muss bis zum Ausschluss einer malignen Genese abgeklĂ€rt werden. In dieser Ăbersicht werden neben der klinischen Symptomatik die Differenzialdiagnosen und gegenwĂ€rtige sowie neue Verfahren der Funktionsdiagnostik wie hochauflösende Manometrie und kombinierte ösophageale Mehrkanalimpedanzmessung zur zielgerichteten und prĂ€zisen Diagnostik der ösophagealen Dysphagie vorgestell
Atemteste zur gastroenterologischen Funktionsdiagnostik: Grundlagen und klinische Anwendungen
Zusammenfassung: Eine Vielzahl gastroenterologischer Funktionsteste beinhaltet invasive Testverfahren oder die Verwendung ionisierender Strahlung. Atemteste bieten dazu patientenfreundliche und kostengĂŒnstige Alternativen. Mit dem Wasserstoffatemtest lassen sich Kohlenhydratmalabsorptionen oder bakterielle Fehlbesiedelungen nachweisen. Zur Untersuchung von vielfĂ€ltigen gastrointestinalen Organfunktionen können 13CO2-Atemteste mit stabilen 13C-Isotopen eingesetzt werden. Zur Diagnostik von MotilitĂ€tsstörungen des Magens und des DĂŒnndarms stehen diverse 13C-Markersubstanzen und Testverfahren zur VerfĂŒgung. Die Magenentleerungsmessung fĂŒr flĂŒssige bzw. feste Mahlzeiten ist in der klinischen Praxis zur Diagnostik der Gastroparese etabliert. Jedoch kann die Herleitung typischer physiologischer Parameter der Magenentleerung und des intestinalen Transits Schwierigkeiten bereiten. In dieser Arbeit werden die grundlegende Funktionsweise, gĂ€ngige Testmahlzeiten, die praktische DurchfĂŒhrung und Interpretation von Atemtesten in der gastrointestinalen Funktionsdiagnostik beschriebe
Frustrated spin- Heisenberg magnet on a square-lattice bilayer: High-order study of the quantum critical behavior of the ---- model
The zero-temperature phase diagram of the spin-
---- model on an -stacked square-lattice
bilayer is studied using the coupled cluster method implemented to very high
orders. Both nearest-neighbor (NN) and frustrating next-nearest-neighbor
Heisenberg exchange interactions, of strengths and , respectively, are included in each layer. The two layers are
coupled via a NN interlayer Heisenberg exchange interaction with a strength
. The magnetic order parameter (viz.,
the sublattice magnetization) is calculated directly in the thermodynamic
(infinite-lattice) limit for the two cases when both layers have
antiferromagnetic ordering of either the N\'{e}el or the striped kind, and with
the layers coupled so that NN spins between them are either parallel (when
) to one another. Calculations
are performed at th order in a well-defined sequence of approximations,
which exactly preserve both the Goldstone linked cluster theorem and the
Hellmann-Feynman theorem, with . The sole approximation made is to
extrapolate such sequences of th-order results for to the exact limit,
. By thus locating the points where vanishes, we calculate
the full phase boundaries of the two collinear AFM phases in the
-- half-plane with . In particular, we provide the
accurate estimate, (), for the
position of the quantum triple point (QTP) in the region . We also
show that there is no counterpart of such a QTP in the region ,
where the two quasiclassical phase boundaries show instead an ``avoided
crossing'' behavior, such that the entire region that contains the nonclassical
paramagnetic phases is singly connected
Structure, phase behavior and inhomogeneous fluid properties of binary dendrimer mixtures
The effective pair potentials between different kinds of dendrimers in
solution can be well approximated by appropriate Gaussian functions. We find
that in binary dendrimer mixtures the range and strength of the effective
interactions depend strongly upon the specific dendrimer architecture. We
consider two different types of dendrimer mixtures, employing the Gaussian
effective pair potentials, to determine the bulk fluid structure and phase
behavior. Using a simple mean field density functional theory (DFT) we find
good agreement between theory and simulation results for the bulk fluid
structure. Depending on the mixture, we find bulk fluid-fluid phase separation
(macro-phase separation) or micro-phase separation, i.e., a transition to a
state characterized by undamped periodic concentration fluctuations. We also
determine the inhomogeneous fluid structure for confinement in spherical
cavities. Again, we find good agreement between the DFT and simulation results.
For the dendrimer mixture exhibiting micro-phase separation, we observe rather
striking pattern formation under confinement.Comment: 8 pages, 10 figure
The magnetization process of the spin-one triangular-lattice Heisenberg antiferromagnet
We apply the coupled cluster method and exact diagonalzation to study the
uniform susceptibility and the ground-state magnetization curve of the
triangular-lattice spin-1 Heisenberg antiferromagnet. Comparing our theoretical
data for the magnetization curve with recent measurements on the s=1 triangular
lattice antiferromagnet Ba3NiSb2O9 we find a very good agreement.Comment: 2 pages, 3 figure
