    南極中央高地は水蒸気量が極めて少なく低温で,かつ気流の乱れが少ないため,地球上で最も優れた天体観測が行える場所である可能性が高い.その調査の一環として,ドームふじ基地においてSODARによる接地境界乱流の測定を,第48次南極地域観測隊に委託して行った.2006年12月21日~2007年1月14日の連続25日間の有効な測定データ(乱流強度,3次元風速)が得られたので報告する.太陽高度に対応した乱流層高度の明瞭な日変化が見られた.また装置輸送時に測定した加速度計の記録も合わせて報告する.The Antarctic plateau can be the best astronomical observatory site on the earth because of its very dry air and low temperature.     Association of obesity paradox with prognosis of veno‐venous‐extracorporeal membrane oxygenation in patients with coronavirus disease 2019

    Abstract Aim Although the obesity paradox is known for various diseases, including cancer and acute respiratory distress syndrome, little is known about veno‐venous extracorporeal membrane oxygenation (VV‐ECMO) in patients with coronavirus disease 2019 (COVID‐19). In this study, we aimed to investigate the association between body mass index (BMI) and prognosis in critical patients with COVID‐19 requiring VV‐ECMO. Methods We conducted a retrospective observational single‐center study at Yokohama City University Civic General Medical Center between March 2020 and October 2021. Participants were patients with COVID‐19 who required VV‐ECMO. They were classified into two groups: BMI ≤30 kg/m2 and >30 kg/m2. Results In total, 23 patients were included in the analysis, with a median BMI of 28.7 kg/m2. Overall, 22 patients were successfully weaned from the ECMO. When comparing the two groups, there was a trend toward fewer days from onset to ECMO induction in the BMI >30 kg/m2 group. Moreover, the two groups had a similar prognosis. There were no statistically significant differences in the number of days from onset to hospitalization or the duration of ECMO induction between the groups. Conclusion VV‐ECMO induction for patients with COVID‐19 may lead to earlier indications in patients with BMI >30 kg/m2 than in those with BMI ≤30 kg/m2

    Effectiveness of a 3-Week Rehabilitation Program Combining Muscle Strengthening and Endurance Exercises Prior to Total Knee Arthroplasty: A Non-Randomized Controlled Trial

    We evaluated the effectiveness of a high-intensity preoperative resistance and endurance training program in improving physical function among patients scheduled for total knee arthroplasty. This non-randomized controlled trial included 33 knee osteoarthritis patients scheduled to undergo total knee arthroplasty at a tertiary public medical university hospital. Fourteen and nineteen patients were non-randomly assigned to intervention and control groups, respectively. All patients underwent total knee arthroplasty and a postoperative rehabilitation program. The intervention group participated in a preoperative rehabilitation program comprising high-intensity resistance and endurance training exercises to increase lower limb muscle strength and endurance capacity. The control group received only exercise instruction. The primary outcome was the 6-min walking distance, which was significantly higher in the intervention group (399 ± 59.8 m) than in the control group (348 ± 75.1 m) 3 months post-surgery. There were no significant differences between the groups 3 months post-surgery in muscle strength, visual analog scale, WOMAC-Pain, range of motion of knee flexion, and extension. A 3-week preoperative rehabilitation program combining muscle strengthening and endurance training improved endurance 3 months after total knee arthroplasty. Thus, preoperative rehabilitation is important for improving postoperative activity
