Exploring expert and patient opinions and recommendations regarding anti-retroviral treatment compliance

Faculty of HumanitiesThe recent introduction of antiretroviral treatment (ART) to the public health sector has
meant that for millions of Human Immunodeficiency Virus (HIV)-positive patients this
deadly disease has been transformed into a chronic condition. There has been much
research done internationally on adherence to ART but in South Africa there has been
little investigation in this area. This study aimed to bridge this gap by exploring expert
and patient opinions and recommendatio ns regarding adherence to antiretroviral
medication. To attain this, four experts and seven patients were interviewed using a semistructured
interview schedule. The experts had worked within the HIV field for at least
two years while the patients had been chosen from public antiretroviral roll-out
programmes and had been on ART for at least six months. These interviews were audio
recorded and transcribed. The transcriptions were then explored for themes using
thematic content analysis. These themes were categorised and discussed under four broad
categories: patients’ perceptions of barriers to adherence, patients’ recommendations for
improving adherence, experts’ perceptions of barriers to adherence and experts’
recommendations for improving adherence

Effect of Dietary Composition of Weight Loss Diets on High Sensitivity C-Reactive Protein: The Randomized POUNDS LOST Trial
Overweight and obesity are associated with increased high sensitivity C-reactive protein (hsCRP) levels. The purpose of this study was to determine if weight loss diets differing in fat, protein, or carbohydrate composition differentially reduce hsCRP. POUNDS (Preventing Overweight Using Novel Dietary Strategies) LOST was a two-year trial of overweight and obese adults randomly allocated to one of four weight loss diets with targeted percentages of energy derived from fat, protein, and carbohydrates (20,15,65%;20,25,55%;40,15,45%;40,25,35%, respectively). hsCRP was measured at baseline, 6, and 24 months among 710 participants, and adiposity as measured by dual X-ray absorptiometry (N=340) or abdominal computed tomography (N=126) was correlated with hsCRP change. At 6 months, hsCRP was reduced in all trial participants by −24.7% (IQR +7%,−50%), weight by −6.7% (IQR −3%,−11%), and waist circumference by −6.0% (IQR −3%,−10%) (all P<.002), with no significant differences according to dietary composition. The percent change in hsCRP at 6 and 24 months correlated modestly with change in weight, waist circumference, fasting insulin, fasting glucose, HOMA, and most lipid levels. Reductions in hsCRP persisted despite an approximate 50% regain of weight by 24 months. The percent change in hsCRP at 24 months significantly correlated with changes in total body fat (r=0.42), total abdominal adiposity (r=0.52), subcutaneous abdominal adiposity (r=0.52), visceral adiposity (r=0.47), and hepatic tissue density (r=−0.34) (all P<0.0006). In conclusion, weight loss decreased hsCRP by similar magnitude, irrespective of dietary composition. Clinicians concerned about inflammation and cardiovascular risk should recommend weight loss diets most likely to succeed for their patients
Noncoronary Cardiac Abnormalities Are Associated With Coronary Artery Dilation and With Laboratory Inflammatory Markers in Acute Kawasaki Disease
ObjectivesWe explored the association of noncoronary cardiac abnormalities with coronary artery dilation and with laboratory inflammatory markers early after Kawasaki disease (KD) diagnosis.BackgroundLeft ventricular (LV) dysfunction, mitral regurgitation (MR), and aortic root dilation occur early after diagnosis; their associations with coronary artery dilation and inflammatory markers have not been well-described.MethodsCentrally interpreted echocardiograms were obtained at KD diagnosis and 1 and 5 weeks after diagnosis on 198 subjects in the National Institutes of Health-sponsored Pediatric Heart Network KD pulsed steroid trial. Regression models were constructed to investigate the relationships among early LV dysfunction, MR, and aortic root dilation with coronary artery dilation and laboratory inflammatory markers.ResultsAt diagnosis, LV systolic dysfunction was present in 20% of subjects and was associated with coronary artery dilation, seen in 29% (p = 0.004). Although LV dysfunction improved rapidly, LV dysfunction at diagnosis predicted greater odds of coronary artery dilation at 1 and 5 weeks after diagnosis (5-week odds ratio: 2.7, 95% confidence interval: 1.2 to 6.3). At diagnosis, MR was present in 27% of subjects and aortic root dilation was present in 8%; each was associated with larger coronary artery size at diagnosis. Left ventricular dysfunction was associated with higher erythrocyte sedimentation rate and, at diagnosis only, lower serum albumin; MR was associated with higher erythrocyte sedimentation rate and lower albumin at all times. Aortic root size had little association with inflammatory markers.ConclusionsNoncoronary cardiac abnormalities are associated with coronary artery dilation and laboratory evidence of inflammation in the first 5 weeks after KD, suggesting a shared inflammatory mechanism. The HIPASS Catalogue - II. Completeness, Reliability, and Parameter Accuracy
The HI Parkes All Sky Survey (HIPASS) is a blind extragalactic HI 21-cm
emission line survey covering the whole southern sky from declination -90 to
+25. The HIPASS catalogue (HICAT), containing 4315 HI-selected galaxies from
the region south of declination +2, is presented in Meyer et al. (2004a, Paper
I). This paper describes in detail the completeness and reliability of HICAT,
which are calculated from the recovery rate of synthetic sources and follow-up
observations, respectively. HICAT is found to be 99 per cent complete at a peak
flux of 84 mJy and an integrated flux of 9.4 Jy km/s. The overall reliability
is 95 per cent, but rises to 99 per cent for sources with peak fluxes >58 mJy
or integrated flux > 8.2 Jy km/s. Expressions are derived for the uncertainties
on the most important HICAT parameters: peak flux, integrated flux, velocity
width, and recessional velocity. The errors on HICAT parameters are dominated
by the noise in the HIPASS data, rather than by the parametrization procedure.Comment: Accepted for publication in MNRAS. 12 pages, 11 figures. Paper with

Comparison of Weight-Loss Diets with Different Compositions of Fat, Protein, and Carbohydrates
BACKGROUND:
The possible advantage for weight loss of a diet that emphasizes protein, fat, or carbohydrates has not been established, and there are few studies that extend beyond 1 year.
METHODS:
We randomly assigned 811 overweight adults to one of four diets; the targeted percentages of energy derived from fat, protein, and carbohydrates in the four diets were 20, 15, and 65%; 20, 25, and 55%; 40, 15, and 45%; and 40, 25, and 35%. The diets consisted of similar foods and met guidelines for cardiovascular health. The participants were offered group and individual instructional sessions for 2 years. The primary outcome was the change in body weight after 2 years in two-by-two factorial comparisons of low fat versus high fat and average protein versus high protein and in the comparison of highest and lowest carbohydrate content.
RESULTS:
At 6 months, participants assigned to each diet had lost an average of 6 kg, which represented 7% of their initial weight; they began to regain weight after 12 months. By 2 years, weight loss remained similar in those who were assigned to a diet with 15% protein and those assigned to a diet with 25% protein (3.0 and 3.6 kg, respectively); in those assigned to a diet with 20% fat and those assigned to a diet with 40% fat (3.3 kg for both groups); and in those assigned to a diet with 65% carbohydrates and those assigned to a diet with 35% carbohydrates (2.9 and 3.4 kg, respectively) (P>0.20 for all comparisons). Among the 80% of participants who completed the trial, the average weight loss was 4 kg; 14 to 15% of the participants had a reduction of at least 10% of their initial body weight. Satiety, hunger, satisfaction with the diet, and attendance at group sessions were similar for all diets; attendance was strongly associated with weight loss (0.2 kg per session attended). The diets improved lipid-related risk factors and fasting insulin levels.
CONCLUSIONS:
Reduced-calorie diets result in clinically meaningful weight loss regardless of which macronutrients they emphasize
The Northern HIPASS catalogue - Data presentation, completeness and reliability measures
The Northern HIPASS catalogue (NHICAT) is the northern extension of the
HIPASS catalogue, HICAT (Meyer et al. 2004). This extension adds the sky area
between the declination range of +2 deg < dec. < +25.5 deg to HICAT's
declination range of -90 deg < dec. < +2 deg. HIPASS is a blind HI survey using
the Parkes Radio Telescope covering 71% of the sky (including this northern
extension) and a heliocentric velocity range of -1,280 km/s to 12,700 km/s .
The entire Virgo Cluster region has been observed in the Northern HIPASS. The
galaxy catalogue, NHICAT, contains 1002 sources with v_hel > 300 km/s . Sources
with -300 km/s < v_hel < 300 km/s were excluded to avoid contamination by
Galactic emission. In total, the entire HIPASS survey has found 5317 galaxies
identified purely by their HI content. The full galaxy catalogue is
