    Dubai's Potential As An Integrated Logistics Hub

    The objective of this study is to examine the potential of Dubai as an integrated logistics hub, identify gaps and recommend polices accordingly. The key features of an integrated logistic hub are identified. The logistics performance of Dubai is compared to that of the top ranked logistics performing country, Singapore. Gaps that need to be looked into for Dubai to attain a similar status as Singapore are identified. The views of company managers in Dubai with regards to logistics facilities in Dubai are elicited using a survey method and analyzed. The study reveals that Dubai requires further development and investment to match Singapore’s performance as a logistics hub. The survey results show that high rents and costs of operation will adversely impact on Dubai’s status as a logistics hub. There also appears to be a logistics skill gap amongst the workforce in Dubai and this must be addressed and e-commerce must be encouraged. The regional competitors in Saudi Arabia, Egypt and Kuwait pose a challenge to Dubai’s standing as a logistics hub for the region. The financial sector should be developed along with public institutions to facilitate a seamlessly integrated business environment. Much of the emphasis in developing Dubai as a logistics hub is on the physical infrastructure where the developments have been impressive. However, emphasis also needs to be given to containing inflation, developing professionals’ skills, expanding the financial sector and ensuring that government regulation is conducive to business

    Caesarean scar defect: a histopathological comparative study

    Background: We have evaluated the validity of this syndrome in Indian patients and analysed the gynaecological indications for hysterectomy in women with history of caesarean sections. We have studied pathological changes in the scar area and compared the findings with matched cases without previous caesarean scar.Methods: A prospective observational case control study was done at tertiary care hospital (Seth GS Medical College and King Edward Memorial Hospital) over two years (December 2018 to December 2020) with universal sampling and enrolled all consenting eligible patients. After hysterectomy histopathological study of the specimens was done. Total cases: 16 hysterectomy samples with history of previous caesarean section. Total controls: 40 hysterectomy samples with history of no previous caesarean section. The difference between the two proportions was analysed using Chi square or Fisher exact test. All analysis was 2 tailed and the significance level was set at 0.05.Results: Women with history of previous caesarean scar had gynaecological symptoms related to the caesarean scar defect such as abnormal uterine bleeding, dysmenorrhea and chronic pelvic pain, post-menopausal bleeding and the most frequent clinical symptom related to the scar defect was abnormal uterine bleeding. The clinical symptoms were found to be associated with histopathological changes at scar site.Conclusions: This study compared caesarean cases and no caesarean controls and sheds light on the role of histopathology in detection of caesarean scar site changes. It helped in comparison of various factors affected due to the presence of caesarean scar and its long-term complications, leading to hysterectomy

    An analysis of adult non-neoplastic nephrectomies at a tertiary care centre in Mumbai

    Introduction: Nephrectomies are an integral part of urological practice and a wide range of renal diseases are seen on histopathological examination of non-neoplastic nephrectomy specimens. Objective: The objective of this study was to assess the incidence, clinical presentation and the histological spectrum of various non-neoplastic renal diseases detected in nephrectomy specimens at our institute and to compare our observations with the conventional patterns seen. Material and Methods: This is a retrospective 6 year study of adult non-neoplastic nephrectomies (ANNN) conducted in a tertiary care centre in Mumbai and includes 359 cases from January 2009 to December 2015. The paraffin blocks and slides were retrieved from our archives and analysed along with clinical features, radiological findings. Results: 359 ANNN specimens received over a 6 year period were studied. Most of the patients were in the sixth decade with a slight female preponderance. Flank pain was the commonest presenting symptom seen in 79.94% of cases. The most frequent surgical indication for ANNN was obstructive uropathy (47.08%) which includes calculus disease and PUJ obstruction followed by non-functioning kidney (42.06%). On histopathological examination, the most common gross pathology was acalculus hydronephrosis (40.67%) and the most frequent histopathological diagnosis was chronic pyelonephritis seen in 79.38% cases. Calculus obstructive uropathy amounted to 39.27% cases and acalculus obstructive uropathy amounted to 25.07% cases of chronic pyelonephritis. Conclusion: The most frequent surgical indication was obstructive uropathy and the most common histopathological diagnosis was chronic pyelonephritis. Calculus and acalculus obstructive uropathy amounted to almost 65% of the cases of chronic pyelonephritis

    Renal cortical necrosis at autopsy: a 12 year experience at a tertiary care centre in Mumbai

    Introduction: Renal cortical necrosis (RCN) is an unusual cause of acute renal failure but still causes significant mortality in developing countries. Objective: To review autopsies of histologically proven RCN reported in complete autopsies over the last 12 years. Methods: All autopsies performed from 2004-2016 with RCN were analyzed retrospectively. Results: A total of 13,896 autopsies were done at this centre over this twelve year period from 2004-2016. Of these, 26 cases were diagnosed as RCN, with an incidence of 0.19%. 978 of our total number of autopsies presented with Acute Kidney Injury (AKI) and the incidence of RCN in AKI was 2.7%. There were 844 maternal mortality deaths during this period of which 134 (15.9%) had AKI. RCN accounted for 2% of all maternal mortality deaths during this period. Obstetric cases amounted to 65.4% and non-obstetric cases to 34.6%. Pregnancy induced hypertension; intrauterine foetal death and postpartum hemorrhage were the leading causes of RCN amongst obstetric causes while septicemia, hemolytic uremic syndrome and poisoning were the leading non-obstetric causes. Microscopically, diffuse cortical necrosis was seen in 53.8% autopsies and patchy cortical necrosis was seen in 46.2% autopsies. Histopathology also showed intra-alveolar hemorrhage, bronchopneumonia, cerebral edema and acute respiratory distress syndrome. Conclusion: Our incidence of RCN was 0.19% at general autopsy, 2.7 % in autopsies with AKI and 2% of maternal mortality deaths at a tertiary care centre.RCN in AKI being 2.7% is significantly less than the 6-7% reported in developing countries. Diffuse pattern of cortical necrosis was more common than patchy cortical necrosis

    Giant adrenal myelolipoma

    Adrenal myelolipoma is a rare benign tumor composed of adipose and hematopoietic tissue. Most tumors are small in size and incidentally detected. We report a case of giant adrenal myelolipoma weighing 2200 gms which was diagnosed on radiology as a liposarcoma. This case is unusual in view of the large size and presence of bony spicules. To the best of our knowledge, not more than 10 giant adrenal myelolipoma cases have been reported in literature

    Tumors of the lateral ventricles: a clinico-pathological study

    Introduction: Brain tumors are rare compared to other fatal and life-diminishing diseases and its incidence is increasing. The objective is to study the lateral ventricular tumors with reference to age groups, gender, anatomical location, the histopathological features including the histological grades, and the clinico- radio- pathological correlation of the above cases. Methodology: Retrospective analysis of tumors of the lateral ventricle of the CNS diagnosed in the department of Neuropathology at a tertiary care and referral hospital, from January 2001 to June 2009. Results: Average Incidence of primary lateral ventricular tumor was 1.6%, that tumor involve the left lateral ventricle (46.3%) more commonly than the right lateral ventricle (29.9%). Commonest tumors in our study were the Neuronal and mixed glioneuronal tumors [31%]. 78.1% of the lateral ventricular tumors were correctly diagnosed on radiology. Conclusion: Commonest tumors were Neuronal and mixed glioneuronal tumors. Most of the tumors belonged to low WHO grade. Fairly good correlation was found between radiological and histopathological diagnosis

    Extent of globalisation in MENA countries: an empirical analysis

    This paper estimates the extent of globalisation for the Middle East and North African (MENA) countries using the Kearney/foreign policy approach for the period 1995–2005. This index is composed of four components: economic integration, technology, personal contact and political engagement and it is used to identify the extent of globalisation of MENA countries and to show how globalisation has evolved over time in these countries. UAE, Bahrain and Malta are ranked as the three most globalised countries in the MENA region while Algeria, Libya and Djibouti are among the lowest. As far as the sources of globalisation is concerned the technology and the economic integration play a key role in the globalisation process in the region followed by personal contacts while the political engagement exhibits negligible role. Our results also show a strong positive association between globalisation and economic growth for the MENA countries

    Adrenal cysts-a report of four cases including one with bilateral involvement

    Adrenal cystic lesions are rare entities. Four types of cystic lesions of the adrenal gland have been described in literature namely, epithelial cysts, endothelial cysts, pseudocysts and parasitic cysts. Tumors of the adrenal gland, both benign and malignant may also show cystic change and have to be considered in the differential diagnosis of cystic lesions of the adrenal gland. Lymphangioendotheliomas, though common at other sites, are unusual in the adrenal glands and bilateral involvement is extremely rare. We present four cases of adrenal cysts, two cases of lymphangioendothelial cysts and two cases of pseudocysts from 108 adrenalectomy specimens received over 5 years. Two of these had hypertension which resolved after surgery. All of our cases did well postoperatively. Prognosis of these benign cysts is excellent and the patients are disease-free after surgery

    Ovotesticular disorder of sex development (DSD) is an extremely rare condition and is defined as presence of both ovarian and testicular tissue in the same individual, regardless of the karyotype and the status of the external or internal genitalia. This disorder was formerly called True hermaphroditism and 400-500 cases have been reported in literature till date. Various chromosomal abnormalities have been described but 46XX is the commonest. Both Mullerian and Wolffian duct derivatives are present in the same individual and the external as well as internal genitalia display a spectrum of phenotypes. The external genitalia are ambiguous and a uterus and a phallus areoften present. The gonads may be ovotestis or a combination of ovary on one side and testes or ovotestis on the other side. These gonads are prone to develop both benign and malignant neoplasms. Tumors like dysgerminoms, seminomas, gonadoblastoma, and mucinous cystadenomas have been described. Serous cystadenomas however, have not been described
