A comparison of organizational climate and nurses� intention to leave among excellence awarded hospitals and other hospitals in 2013
Background: Human resource is the most important factor of performance, success and better revelation of excellence goals of each organization. By performing excellence plan, healthcare organizations improve their organizational climate and play a valuable role in retaining nurses and improving the quality of their services to patients. Objectives: The aim of this study was to compare hospital organizational climate and intention to leave among working nurses in hospitals performing the excellence plan and other hospitals of Tehran University of Medical Sciences. Patients and Methods: This was a cross-sectional descriptive comparison study. Its population included 248 nurses of the hospitals performing the excellence plan and other hospitals of Tehran University of Medical Sciences in Iran selected by random sampling. The used instrument had three parts: the first part was related to personal characteristics, the second part was the Munn�s organizational climate questionnaire and the third part was Hinshaw�s questionnaire of �anticipated turnover scale�. Data was analyzed using SPSS software, version 17 and indices of descriptive statistics and inferential statistics. Results: The results of the mean and standard deviation for organizational climate and intention to leave in both performing and non-performing hospitals of the organizational excellence plan were respectively (65.28 ± 19.31 and 56.42 ± 21.36) and (33.64 ± 5.58 and 35.59 ± 4.94). Independent T test revealed a significant difference between the mean scores for organizational climate in both performing and non-performing hospitals, and also a significant difference between the mean scores for intention to leave in both performing and non-performing hospitals (P = 0.004). Moreover, Pearson Correlation test showed a reverse significant correlation between organizational climate and intention to leave in performing hospitals of the organizational excellence plan (r = -0.337) and non-performing hospitals (r = -0.282) (P = 0.001). Conclusions: Performing quality improvement pattern such as organization�s excellence plan improves organizational climate of healthcare sectors, it can reduce nurses� intentions to leave and retain human resources. © 2015, Iranian Red Crescent Medical Journal
Enamel matrix derivative Emdogain® as an adjuvant for a laterally-positioned flap in the treatment of gingival recession: an electron microscopic appraisal
Enamel matrix derivative (EMD), such as Emdogain®, has been suggested for the improvement of wound healing in periodontal surgical therapy. The present qualitative study seeks to illustrate the ultrastructural changes associated with a human gingival wound at 10 days after the application of EMD as an adjunct to a laterally-positioned flap in a patient with gingival recession. An otherwise healthy patient, who had been suffering from bilateral gingival recession defects on teeth #23 and #26, was studied. One defect was treated with a laterally-positioned flap, while the other was treated with a combination of EMD and a laterally-positioned flap. Ten days after the operation gingival biopsy specimens were obtained from the dentogingival region and examined using a transmission electron microscope. A considerable difference was found in both the cellular and extracellular phases of EMD and non-EMD sites. The fibroblasts of EMD site were more rounded with plump cytoplasms and euchromatic nuclei. A well-developed rough endoplasmic reticulum and numerous mitochondria could be detected. In contrast, the fibroblasts of non-EMD site were of flattened spindle-like morphology. While the signs of apoptosis could rarely be detected at EMD site, apoptotic bodies and ultra-structural evidence of apoptosis (crescent-like heterochromatic nuclei and dilated nuclear envelopes) were consistent
features at non-EMD site. The extracellular matrix at EMD site mainly consisted of well-organised collagen fibres, while non-EMD site contained sparse and incompletely-formed collagen fibres. Coccoid bacteria were noted within the extracellular matrix and neutrophils at non-EMD site. It seems that EMD may enhance certain features of gingival wound healing, which may be attributable to its anti-apoptotic, anti-bacterial or anti-inflammatory properties
A rare variation of the vertebral artery
Variations of the vertebrobasilar arterial complex are important with regard to
their potential clinical impact. We present an unusual case of the vertebral
artery, in which the left vertebral artery in its ascent in the neck through the
transverse foramina passed posteriorly between the transverse processes of
C3 and C4 and supplied the posterior muscles of the neck without continuing
intracranially. Albeit speculatively, we hypothesise that the variation of the vertebral
artery reported here was caused by degeneration of the proximal portion
of the left postcostal longitudinal anastomosis (i.e. C1 and C2 intersegmental
arteries) in the context of a persistent third cervical intersegmental artery. Our case
is unique in that the left vertebral artery terminated extracranially. Knowledge of
the variations of the vertebrobasilar arterial complex is important for surgeons
operating at the skull base, craniocervical junction, and cervical region, and for
clinicians interpreting the imaging of this region
In-vitro prostate cancer biomarker detection by directed conjugation of anti-PSCA antibody to super paramagnetic iron oxide nanoparticless
Background: The main property of a successful conjugation of antibodies to nanoparticles is keeping the potency of antibody for binding the antigen, and an oriented conjugation can do that. Under such ground, this study was carried out to explore the efficiency of two conjugation methods in binding iron nanoparticles to an antibody produced against PSCA (prostate stem cell antigen) using in vitro labeling of PC3 cells. Methods: In this experimental study, we conjugated dextran-superparamagnetic iron oxide nanoparticles (dexSPIONs) to anti-PSCA antibody by two different methods, including targeting carbohydrate moieties in FC domain and the free amine group of amino acid side chains. Ultimately, Iron staining was done by anti-PSCA antibody-dexSPIONs in PC3 cells to detect antibody binding to the cells. Results: A strong blue dye was induced by iron staining in conjugated dexSPIONs on the membrane of PC3 cells by the former method than the second one. Moreover, cells treated with 20 nm diameters of dexSPIONs showed higher resolution of blue color than those treated with 100 nm nanoparticles. Conclusion: This oriented conjugation method promoted the efficiency of targeting tumor antigens, and the presence of iron particles might enhance MRI image intensity in vivo by targeting PSCA-overexpressing cells in future studies. © Iran University of Medical Sciences
The effects of an electromagnetic field on the boundary tissue of the seminiferous tubules of the rat: a light and transmission electron microscope study
Human beings are unavoidably exposed to ambient electromagnetic fields (EMF)
generated from various electrical devices and from power transmission lines.
Controversy exists about the effects of EMF on various organs. One of the critical
issues is that EMF may adversely affect the reproductive system. In order to
examine this 30 rat pups were exposed to 50 Hz EMF (non-ionising radiation)
during in utero development (approximately 3 weeks) and postnatal life (5 weeks).
Groups of exposed rats were subsequently left in an environment free of EMF in
order to observe recovery, if any, from the changes induced by EMF on the
boundary tissue of the seminiferous tubules. The materials were processed and
observed under a light and a transmission electron microscope. In the experimental
rats boundary tissue was found disrupted at various layers. This tissue
showed infoldings, which were perhaps due to the loss of collagen and reticular
fibrils from the inner and outer non-cellular layers. The outer non-cellular layer,
which was thinner than that of the control, was stripped away from the myoid
cell layer in multiple regions, giving a “blister-like” appearance. The myoid cells
showed fewer polyribosomes, pinocytotic vesicles and glycogen granules. Most
mitochondria were found to lack cristae. The connections between individual
myoid cells were apparently lost. There were signs of recovery in the boundary
tissue following withdrawal from EMF exposure. These results suggest that EMF
exposure may cause profound changes in the boundary tissue of the seminiferous
tubules. Therefore exposure to EMF may result in pathological changes that
lead to subfertility and infertility

The non-syndromic familial thoracic aortic aneurysms and dissections maps to 15q21 locus
Background: Thoracic aortic aneurysms and dissections (TAAD) is a critical condition that often goes undiagnosed with fatal consequences. While majority of the cases are sporadic, more than 20 are inherited as a single gene disorder. The most common familial TAA is Marfan syndrome (MFS), which is primarily caused by mutations in fibrillin-1 (FBN1) gene. Patients with FBN1 mutations are at higher risk for dissection compared to other patients with similar size aneurysms. Methods: Fifteen family members were genotyped using Affymetrix-10K genechips. A genome-wide association study was carried out using an autosomal dominant model of inheritance with incomplete penetrance. Mutation screening of all exons and exon-intron boundaries of FBN1 gene which reside near the peak Lod score was carried out by direct sequencing.Results: The index case presented with agonizing substernal pain and was found to have TAAD by transthoracic echocardiogram. The family history was significant for 3 first degree relatives with TAA. Nine additional family members were diagnosed with TAA by echocardiography examinations. The affected individuals had no syndromic features. A genome-wide analysis of linkage mapped the disease gene to a single locus on chromosome 15q21 with a peak Lod score of 3.6 at fibrillin-1 (FBN1) gene locus (odds ratio > 4000:1 in favour of linkage), strongly suggesting that FBN1 is the causative gene. No mutation was identified within the exons and exon-intron boundaries of FBN1 gene that segregated with the disease. Haplotype analysis identified additional mutation carriers who had previously unknown status due to borderline dilation of the ascending aorta.Conclusions: A familial non-syndromic TAAD is strongly associated with the FBN1 gene locus and has a malignant disease course often seen in MFS patients. This finding indicates the importance of obtaining detailed family history and echocardiographic screening of extended relatives of patients with non-syndromic TAAD to improve the outcome. In addition, association of non-syndromic TAAD with the Marfan disease gene locus poses the question whether secondary prevention strategies employed for Marfan syndrome patients should be applied to all patients with familial TAAD. © 2010 Keramati et al; licensee BioMed Central Ltd
Epistatic interaction between adiponectin and survivin gene polymorphisms in endometrial carcinoma
Adiponectin appears to play an important role in the development and progression of several obesity-related malignancies. Also, overexpression of survivin, an inhibitor of apoptosis protein, is associated with increased risk of cancers. The aim of this study was to investigate the association between two polymorphisms in the adiponectin gene and endometrial cancer (EC) risk. We also investigated whether epistasis between surviving and adiponectin gene polymorphisms are associated with EC risk in an Iranian population.The samples comprised formalin-fixed, paraffin-embedded tissue sections obtained from the archive of the pathology department, Imam-Khomeini Hospital and Firouzgar hospital. After DNA extraction the genotyping was performed using PCR-RFLP technique.Single nucleotide polymorphisms (SNPs) in adiponectin (rs1063539, rs2241766) and survivin (rs9904341) gene were evaluated in the study. The increased frequency of ADIPOQ rs1063539C allele (CC. +. CG genotype) was associated with decreased EC risk OR: 0.39(0.17-0.90). Survivin rs9904341C allele (CC. +. CG genotype) was associated with increased EC risk crude OR: 2.75(1.27-5.95), adjusted OR: 2.93(1.27-6.76). We observed an epistatic interaction between survivin rs9904341 CC. +. CG genotype and ADIPOQ rs1063539 GG genotype increasing the risk of EC compared to those with other genotypes OR: 4.86(1.88-12.54), P=0.001.Our findings indicate that adiponectin might have a modulatory effect on survivin role and function in EC, which requires further investigation. © 2014 Elsevier GmbH
Prevalence of mechanical dyssynchrony in heart failure patients with different QRS durations
Background: Cardiac resynchronization therapy (CRT) has emerged as an established therapy for congestive heart failure. However, up to 30 of patients fail to respond to CRT despite prolonged QRS. Objectives: This study aimed at defining the prevalence of interventricular and intraventricular dyssynchrony in heart failure patients with different QRS durations. Methods: A total of 123 consecutive patients with severe heart failure (LVEF < 35 and NYHA class III-IV) were prospectively evaluated using 12-lead electrocardiogram and complete echocardiographic examination including tissue Doppler imaging. Results: According to the QRS duration, 56 patients had a QRS duration �120 ms (Group 1), 33 patients had a QRS duration between 120 and 150 ms (Group 2), and 34 patients had a QRS duration �150 ms (Group 3). Intraventricular dyssynchrony was present in 36 of Group 1 patients, in 58 of Group 2 patients, and in 79 of Group 3 patients (P < 0.000). Linear regression demonstrated a weak relation between QRS and intraventricular dyssynchrony. A greater proportion of patients with interventricular dyssynchrony was observed in Group 3 or Group 2 compared to patients with normal QRS duration (32 in Group 1 vs. 51.5 in Group 2 vs. 76.5 in Group 3, P < 0.000). Linear regression demonstrated a significant relation between QRS duration and interventricular mechanical delay. Conclusions: Although both interventricular and intraventricular dyssynchrony increased with the increasing QRS duration, the correlation between intraventricular mechanical and electrical dyssynchrony was weak. The lack of intraventricular dyssynchrony in a fraction of patients with standard CRT indication by QRS duration may provide us insight into the nonresponders rates. © 2007, The Authors
