    Terminology for Achilles tendon related disorders

    The terminology of Achilles tendon pathology has become inconsistent and confusing throughout the years. For proper research, assessment and treatment, a uniform and clear terminology is necessary. A new terminology is proposed; the definitions hereof encompass the anatomic location, symptoms, clinical findings and histopathology. It comprises the following definitions: Mid-portion Achilles tendinopathy: a clinical syndrome characterized by a combination of pain, swelling and impaired performance. It includes, but is not limited to, the histopathological diagnosis of tendinosis. Achilles paratendinopathy: an acute or chronic inflammation and/or degeneration of the thin membrane around the Achilles tendon. There are clear distinctions between acute paratendinopathy and chronic paratendinopathy, both in symptoms as in histopathology. Insertional Achilles tendinopathy: located at the insertion of the Achilles tendon onto the calcaneus, bone spurs and calcifications in the tendon proper at the insertion site may exist. Retrocalcaneal bursitis: an inflammation of the bursa in the recess between the anterior inferior side of the Achilles tendon and the posterosuperior aspect of the calcaneus (retrocalcaneal recess). Superficial calcaneal bursitis: inflammation of the bursa located between a calcaneal prominence or the Achilles tendon and the skin. Finally, it is suggested that previous terms as Haglund’s disease; Haglund’s syndrome; Haglund’s deformity; pump bump (calcaneus altus; high prow heels; knobbly heels; cucumber heel), are no longer used

    Repression of RNA polymerase by the archaeo-viral regulator ORF145/RIP

    Little is known about how archaeal viruses perturb the transcription machinery of their hosts. Here we provide the first example of an archaeo-viral transcription factor that directly targets the host RNA polymerase (RNAP) and efficiently represses its activity. ORF145 from the temperate Acidianus two-tailed virus (ATV) forms a high-affinity complex with RNAP by binding inside the DNA-binding channel where it locks the flexible RNAP clamp in one position. This counteracts the formation of transcription pre-initiation complexes in vitro and represses abortive and productive transcription initiation, as well as elongation. Both host and viral promoters are subjected to ORF145 repression. Thus, ORF145 has the properties of a global transcription repressor and its overexpression is toxic for Sulfolobus. Das liegt daran, dass der Begriff InformalitĂ€t hĂ€ufig stillschweigend durch den Begriff der Organisationskultur ersetzt wurde, ohne dass dafĂŒr eine prĂ€zise, abgrenzungsscharfe Definition vorgenommen worden wĂ€re. Unter RĂŒckgriff auf Überlegungen von Dario RodrĂ­guez argumentiert dieser Artikel, dass die beiden Begriffe Organisationskultur und InformalitĂ€t das gleiche PhĂ€nomen bezeichnen: die nichtentschiedenen EntscheidungsprĂ€missen einer Organisation. Dabei wird systematisch zwischen „unentscheidbaren EntscheidungsprĂ€missen“ und „prinzipiell entscheidbaren, aber nicht entschiedenen EntscheidungsprĂ€missen“ unterschieden. Es wird gezeigt, wie sich mit einer prĂ€zisen Bestimmung ĂŒber das Konzept der EntscheidungsprĂ€missen Ordnung in die „wilden Merkmallisten“ der Literatur sowohl ĂŒber InformalitĂ€t als auch Organisationskultur bringen lĂ€sst und empirische PhĂ€nomene genauer erfasst werden können

    The Achilles Tendon Insertion is Crescent-shaped: An In Vitro Anatomic Investigation

    Anatomic and operative textbooks and current literature do not clearly describe the Achilles tendon interface to the calcaneal tuberosity. We dissected 51 specimens to identify the detailed anatomy of the Achilles tendon insertion. Achilles tendon fascicles expanded from the anterior aspect of the distal Achilles tendon over the retrocalcaneal bursa to the anterior part of the Haglund’s tuberosity in nearly half of the specimens. The insertion of the transverse section of the Achilles tendon regularly had a crescent-shape corresponding to the posterior calcaneal prominence. In transverse sections, all specimens had a curved appearance with a radius of curvature ranging from 13.8 mm to 43.6 mm (mean, 20.4 mm) and Achilles tendon extensions to the lateral and medial calcaneal surfaces reached 1.0 mm (mean) and 3.5 mm (mean) anterior in relation to the most posterior point of the calcaneal tuberosity. Knowledge of the arcuate configuration and of the medial and lateral extensions of the plantaris and the Achilles tendon insertion with respect to the transverse plane is important to avoid iatrogenic complications during resection of Haglund’s tuberosity
