Transcatheter aortic valves produce unphysiological flows which may contribute to thromboembolic events: An in-vitro study.
PURPOSE: Transcatheter aortic valve implantation (TAVI) has been associated with large incidence of ischemic events, whose sources are still unclear. In fact, sub-acute complications cannot be directly related to the severity of the calcification in the host tissues, nor with catheter manipulation during the implant. A potential cause could be local flow perturbations introduced by the implantation approach, resulting in thrombo-embolic consequences. In particular, contrary to the surgical approach, TAVI preserves the presence of the native leaflets, which are expanded in the paravalvular space inside the Valsalva sinuses. The purpose of this study is to verify if this configuration can determine hemodynamic variations which may promote blood cell aggregation and thrombus formation. METHODS: The study was performed in vitro, on idealized models of the patient anatomy before and after TAVI, reproducing a range of physiological operating conditions on a pulse duplicator. The fluid dynamics in the Valsalva sinuses was analyzed and characterized using phase resolved Particle Image Velocimetry. RESULTS: Comparison of the flow downstream the valve clearly indicated major alterations in the fluid mechanics after TAVI, characterized by unphysiological conditions associated with extended stagnation zones at the base of the sinuses. CONCLUSION: The prolonged stasis observed in the Valsalva sinuses for the configuration modelling the presence of transcatheter aortic valves provides a fluid dynamic environment favourable for red blood cell aggregation and thrombus formation, which may justify some of the recently reported thromboembolic and ischemic events. This suggests the adoption of anticoagulation therapies following TAVI, and some caution in the patients׳ selection
On-chip III-V monolithic integration of heralded single photon sources and beamsplitters
We demonstrate a monolithic III-V photonic circuit combining a heralded
single photon source with a beamsplitter, at room temperature and telecom
wavelength. Pulsed parametric down-conversion in an AlGaAs waveguide generates
counterpropagating photons, one of which is used to herald the injection of its
twin into the beamsplitter. We use this configuration to implement an
integrated Hanbury-Brown and Twiss experiment, yielding a heralded second-order
correlation that confirms single-photon
operation. The demonstrated generation and manipulation of quantum states on a
single III-V semiconductor chip opens promising avenues towards real-world
applications in quantum information

Supergiant fast X-ray transients as an under-luminous class of supergiant X-ray binaries
The usage of cumulative luminosity distributions, constructed thanks to the
long-term observations available through wide field hard X-ray imagers, has
been recently exploited to study the averaged high energy emission (>17 keV)
from Supergiant Fast X-ray Transients (SFXTs) and classical Supergiant High
Mass X-ray Binaries (SgXBs). Here, we take advantage of the long term
monitorings now available with Swift/XRT to construct for the first time the
cumulative luminosity distributions of a number of SFXTs and the classical SgXB
IGR J18027-2016 in the soft X-ray domain with a high sensitivity focusing X-ray
telescope (0.3-10 keV). By complementing previous results obtained in the hard
X-rays, we found that classical SgXBs are characterized by cumulative
distributions with a single knee around 10-10 erg/s, while
SFXTs are found to be systematically sub-luminous and their distributions are
shifted at significantly lower luminosities (a factor of 10-100). As the
luminosity states in which these sources spend most of their time are typically
below the sensitivity limit of large field of view hard X-ray imagers, we
conclude that soft X-ray monitorings carried out with high sensitivity
telescopes are particularly crucial to reconstruct the complete profile of the
SFXT cumulative luminosity distributions. The difference between the cumulative
luminosity distributions of classical SgXBs and SFXTs is interpreted in terms
of accretion from a structured wind in the former sources and the presence of
magnetic/centrifugal gates or a quasi-spherical settling accretion regime in
