    The prevalence and genotype distribution of human papillomavirus in the genital tract of males in Iran

    Background: Human papillomavirus (HPV) is the most common viral sexually-transmitted infection. Despite HPV infection is associated with several malignant disorders including penile and anal cancers, little is known about the epidemiology of HPV infection in males, particularly in developing countries. Objectives: The aim of this study was to determine the prevalence of HPV infection and its genotype distribution among Iranian males. Patients and Methods: Between March 2009 and April 2014, a total number of 483 males, referred to Iran University of Medical Sciences-affiliated sexually transmitted infections (STI) clinics, were enrolled in this study. Following DNA extraction, HPV detection and genotyping were performed using INNO-LiPA HPV Genotyping Extra assay. To analyze the association of HPV infection and age, the logistic regression was employed. Results: No statistical association between HPV infection and age was observed (P = 0.469). Furthermore, there was no statistically significant correlation between HR HPV infection and age (P = 0.330). Conclusions: In this investigation, the prevalence of HPV infection was relatively substantial. Totally, 17 different HPV genotypes were detected and the most frequently detected genotypes were HPV6, HPV11, HPV16, HPV18 and HPV52, respectively. The data from this study is essential for planning future public health strategies including HPV vaccination programs. © 2015, Ahvaz Jundishapur University of Medical Sciences

    Detection of hepatitis B virus covalently closed circular DNA in the plasma of Iranian HBeAg-negative patients with chronic hepatitis B

    Background: Covalently closed circular DNA (cccDNA) of hepatitis B virus (HBV) is a marker of HBV replication in the liver of patients infected with HBV. Objectives: This study aimed to investigate the association between the presence of cccDNA in the plasma samples of Iranian treatment-naive patients with chronic hepatitis B infection and HBV viral load and HBsAg levels. Patients and Methods: From April 2012 to May 2015, 106 treatment-naive patients with chronic hepatitis B infection were enrolled in this cross-sectional study. The HBsAg titer was measured by the Roche HBsAg II assay on the Cobas e411 system, and HBV DNA quantitation was performed using the COBAS TaqMan 48 kit. Real-time polymerase chain reaction was performed for the detection of HBV cccDNA. Results: The mean (SD) age of the patients was 41.1 ± 12.4 years (range, 20 - 62 years). From a total of 106 study participants, 67 (63.2) were males. The HBV cccDNA was detected in plasma specimens in 19 (17.9) out of the total 106 patients, and a significant relationship was found between the presence of cccDNA in plasma sample of males (23.9) and females (7.7) (P = 0.039). Also, a significant correlation was found between the presence of cccDNA in plasma sample of the patients and HBV viral load level (P < 0.0001) and HBsAg titer (P = 0.0043). Conclusions: This study showed that cccDNA can be detected in the plasma specimen of 17.9 of Iranian treatment-naive patients with chronic hepatitis B infection. Therefore, designing prospective studies focusing on the detection of cccDNA in these patients would provide more information. © 2015, Kowsar Corp

    A field study for sars-cov-2 evaluation by two air sampling strategy during spread in tehran, iran

    Outbreak of COVID-19 in different countries is an emergency in global public health recently. According to high spread of the newborn virus, airborne transmission potency of SARS-CoV-2 is possible. Until now, there are contradictory results for air evaluation of newborn virus in the contaminated area. We studied SARS-CoV-2 in the patient room of a hospital by two air sampling strategies. Filtration method and liquid impaction sampling were used simultaneously to assess SARS-CoV-2 in air. Indoor air of seven stations in three hospital wards was evaluated according to glass midget impinger and polytetrafluoroethylene filter. RNA of SARS-CoV-2 was evaluated by real time reverse transcription-PCR. Studied rooms were contaminated by eleven confirmed patients and four suspected cases. Our results show polytetrafluoroethylene filtration and DMEM impaction could not determine the viral concentration in the air samples. The concentration of SARS-CoV-2 in air samples is a trace in the studied stations. In conclusion, sampling strategy is a challenge for newborn virus due to the level of SARS-CoV-2 virus in the air. A trace level of SARS-CoV-2 in the contaminated area could be highlighted higher pathogenicity properties, not disapproval of airborne properties. However, more studies should be performed to characterize new properties of SARS-CoV-2 in the contaminated air. © 2021, University of Sindh. All rights reserved

    Prevalence of intestinal parasitic infection in cancer, organ transplant and primary immunodeficiency patients in Tehran, Iran

    Background: Intestinal parasitic infection in immunodeficient patients especially those with impaired cellular immunity, like neoplasia, renal or heart transplant needs careful consideration. The objective of this study is to evaluate the prevalence of intestinal parasites in different group of patients including cancer patients; organ transplants recipients, and primary immunodeficiency patients. Methods: Stool samples from 190 patients including 80 patients with Primary Immunodeficiency, 85 cancer patients and 25 organ transplant recipients were collected; a direct examination with Phosphate buffered saline (PBS) and formalin ether concentration was performed. The DNA was extracted from parasitologically confirmed patients and nested PCR and sequencing was performed and new obtained sequences of Cryptosporidium parvum and Enterocytozoon bieneusi were compared with deposited ones. Results: In general, the prevalence of parasites was 26/80 (32.5) in primary immunodeficiency, 22/85(25.9) in cancer group, and 7/25 (28) in organ transplant. The prevalence of intestinal parasitic infections in primary immunodeficiency patients were Blastocystis hominis 13 (16.2), Giardia lamblia 10 (12.5), Cryptosporidium 1(1.2), Chilomastix mesnilii 1 (1.2), Dientamoeba fragilis 1(1.2). Of 25 organ transplants, 6 (24) Cryptosporidium sp were found, all of which were confirmed as Cryptosporidium parvum and one case of Microspora in a heart transplant recipient was confirmed as Enterocytozoon bieneusi by PCR sequencing. The predominant intestinal parasitic infection in cancer patients was 19 (22.3) Blastocystis hominis followed by two (2.3) Giardia lamblia and one Dientamoeba fragilis 1 (1.1). Conclusion: The high rate of infection with Blastocystis hominis was found in cancer patients especially colorectal cancer patients, so careful consideration should be given by physicians. Cryptosporidium sp was found to be the major cause of parasitic intestinal infection in patients with organ transplant compared to primary immunodeficiency patients; so transplant recipients undergoing immunosuppressive therapy should be considered as a risk group for acquiring microsporidiosis and Cryptosporidium infection. © 2019, Asian Pacific Organization for Cancer Prevention

    Distribution of hepatitis C virus genotypes among Azerbaijani patients in capital city of Iran-Tehran

    Background: Determination of the Hepatitis C virus (HCV) genotype distributed in a particular area has an important role on public health throughout the world. Objectives: The aim of this study was to determine the frequency of HCV genotypes in Azerbaijani patients. Patients and Methods: From March 2010 until March 2012, 235 Azerbaijani patients with established chronic hepatitis C, referred to Hospitals related to Iran University of Medical Sciences and Tehran Hepatitis Center, Clinical department of Baqiyatallah Research Center for Gastroeneterology and Liver Disease, were enrolled in this cross sectional study. About 5 mL of peripheral blood was collected from patients and after separation of plasma, viral RNA extracted. HCV-RNA were amplified by RT-nested PCR using primers from the 5�-UTR and genotyped by RFLP assay, and then HCV genotypes were confirmed using sequencing of cloned PCR products into pJET1.2/blunt cloning vector. Results: HCV genotyping of positive plasma samples demonstrated that predominant HCV subtype was noted for 1b (71.1) followed by subtype 3a (17.0), genotype 2 (6.8), 1a (1.7), and mixed infection (3.4). The mean ± SD age of patients was 37.3 ± 11.8 (range: 2-63) years. Out of 235 patients, 139 (59.1) were male. The frequency of HCV subtype 3a was higher in patients under 40 years old (3a: 18.1 vs. 15.0), and subtype 3a was higher in male patients (3a: 18.7 vs. 14.6). Conclusions: The current study shows that the predominant HCV genotype among Azerbaijani patients with established chronic hepatitis C is subtype 1b (71.1) followed by subtype 3a (17.0). © 2013, Kowsar Corp

    Molecular epidemiology of Kaposi�s sarcoma-associated herpes virus, and risk factors in HIV-infected patients in Tehran, 2014

    Background: Kaposi�s sarcoma (KS) remains themostcommonmalignancyamongHIV-infected patients. Humanherpesvirus type- 8 (HHV-8) is regarded as the infectious etiological agent of Kaposi�s sarcoma (KSHV). Diagnostic procedures associated with KSHV are not routinely performed in HIV-infected subjects. Objectives: The main objective of this study is to obtain information on KSHV epidemiology in Iranian HIV-infected individuals. Patients and Methods: In the present cross-sectional study, 109 patients with established HIV infection, who visited a governmental and referral center for HIV screening in Tehran (Tehran west health center (TWHC)) between May 2014 and July 2015 were enrolled according to the convenience sample strategy. After peripheral blood collection, isolation of plasma and peripheral blood mononuclear cell (PBMC) compartments, DNA extraction was performed. KSHV DNA was analyzed by nested polymerase chain reaction (nested PCR) using primers from ORF-26 (virus minor capsid). Results: Among all 109 HIV-infected patients, 67 (61.5) were male, with an age range of 2 - 64 years (mean±standard deviation 35.8 ±13.3). KSHV DNA was found in PBMC and plasma samples of six (5.5) and four (3.6) patients, respectively. Conclusions: This study revealed a considerable prevalence of KSHV DNA, during latent and lytic phases, among HIV-infected patients. Risk factors for KSHV infection acquisition and concurrent. 0+infection with HIV were also evaluated. Diagnosis of KSHV in the group could be helpful for prognosis of Kaposi�s sarcoma and clinical management. © 2016, Iranian Red Crescent Medical Journal

    Investigation of CTNNB1 gene mutations and expression in hepatocellular carcinoma and cirrhosis in association with hepatitis B virus infection

    Hepatitis B virus (HBV), along with Hepatitis C virus chronic infection, represents a major risk factor for hepatocellular carcinoma (HCC) development. However, molecular mechanisms involved in the development of HCC are not yet completely understood. Recent studies have indicated that mutations in CTNNB1 gene encoding for β-catenin protein lead to aberrant activation of the Wnt/ β-catenin pathway. The mutations in turn activate several downstream genes, including c-Myc, promoting the neoplastic process. The present study evaluated the mutational profile of the CTNNB1 gene and expression levels of CTNNB1 and c-Myc genes in HBV-related HCC, as well as in cirrhotic and control tissues. Mutational analysis of the β-catenin gene and HBV genotyping were conducted by direct sequencing. Expression of β-catenin and c-Myc genes was assessed using real-time PCR. Among the HCC cases, 18.1 showed missense point mutation in exon 3 of CTNNB1, more frequently in codons 32, 33, 38 and 45. The frequency of mutation in the hotspots of exon 3 was significantly higher in non-viral HCCs (29.4) rather than HBV-related cases (12.7, P = 0.021). The expression of β-catenin and c-Myc genes was found upregulated in cirrhotic tissues in association with HBV infection. Mutations at both phosphorylation and neighboring sites were associated with increased activity of the Wnt pathway. The results demonstrated that mutated β-catenin caused activation of the Wnt pathway, but the rate of CTNNB1 gene mutations was not related to HBV infection. HBV factors may deregulate the Wnt pathway by causing epigenetic alterations in the HBV-related HCC. © 2020 The Author(s)

    Exosomal miRNAs: Novel players in viral infection

    Exosomes are secreted nanovesicles that are able to transfer their cargo (such as miRNAs) between cells. To determine to what extent exosomes and exosomal miRNAs are involved in the pathogenesis, progression and diagnosis of viral infections. The scientific literature (PubMed and Google Scholar) was searched from 1970 to 2019. The complex biogenesis of exosomes and miRNAs was reviewed. Exosomes contain both viral and host miRNAs that can be used as diagnostic biomarkers for viral diseases. Viral proteins can alter miRNAs, and conversely miRNAs can alter the host response to viral infections in a positive or negative manner. It is expected that exosomal miRNAs will be increasingly used for diagnosis, monitoring and even treatment of viral infections. © 2020 Future Medicine Ltd

    The molecular epidemiology of respiratory viruses in military trainees in Iran

    Background: Military populations are more prone to respiratory infections worldwide. There is a dearth of research about the role of viral pathogens in the etiology of respiratory infections in military trainees in Iran. Hence, we aimed to investigate the molecular epidemiology and clinical symptoms of respiratory viruses among this population. Methods: This cross-sectional study was performed on 400 military trainees with symptoms of respiratory infection, referred to the military medical clinic center in the basic military training camp of the General Staff of the Armed Forces of the Islamic Republic of Iran. Nucleic acid extraction from the throat or nasopharyngeal swab samples was performed by an automated extraction system. The extracts were then analyzed by the CLART® PneumoVir array system for the detection of respiratory viruses. Results: All military trainees were male, aged between 18 and 57 years (mean: 21.69 years). Sore throat (75.5), rhinorrhea (63.2), cough (59.2), fever (59.2), and nasal congestion (50.5) were amongst the most common symptoms. Overall, viral pathogens were detected in a total count of 124 (31). The most commonly detected viruses were rhinovirus (7.2), respiratory syncytial virus A (7.2) and influenza B virus (6). Conclusion: This study was an important first step for understanding the etiological role of viral pathogens in respiratory infection among military trainees population in Iran. Our results indicated that rhinovirus, respiratory syncytial virus A and influenza B virus are important viral pathogens causing respiratory infection in military trainees, respectively. However, further multi-center studies with larger sample size are strongly recommended to confirm our findings. © Iran University of Medical Sciences
