    The utility of fetal heart rate deceleration's descending slope in searching for a non-National Institute of Child Health and Human Development parameter for the detection of fetal acidosis

    AbstractObjectiveTo identify new parameters predicting fetal acidemia.MethodsA retrospective case–control study in a cohort of deliveries from a tertiary referral hospital‐based cohort deliveries in Zaragoza, Spain between 2018 and 2021 was performed. To predict fetal acidemia, the NICHD categorizations and non‐NICHD parameters were analyzed in the electronic fetal monitoring (EFM). Those included total reperfusion time, total deceleration area and the slope of the descending limb of the fetal heart rate of the last deceleration curve. The accuracy of the parameters was evaluated using the specificity for (80%, 85%, 90%, 95%) sensitivity and the area under the receiver operating characteristic curve (AUC).ResultsA total of 10 362 deliveries were reviewed, with 224 cases and 278 controls included in the study. The NICHD categorizations showed reasonable discriminatory ability (AUC = 0.727). The non‐NICHD parameters measured during the 30‐min fetal monitoring, total deceleration area (AUC = 0.807, 95% CI: 0.770, 0.845) and total reperfusion time (AUC = 0.750, 95% CI: 0.707, 0.792), exhibited higher discriminatory ability. The slope of the descending limb of the fetal heart rate of the last deceleration curve had the best AUC value (0.853, 95% CI: 0.816, 0.889). The combination of total deceleration area or total reperfusion time with the slope demonstrated high discriminatory ability (AUC = 0.908, 95% CI: 0.882, 0.933; specificities of 71.6% and 72.7% for a sensitivity of 90%).ConclusionsThe slope of the descending limb of the fetal heart rate of the last deceleration curve is the strongest predictor of fetal acidosis, but its combination with the total reperfusion time shows better clinical utility

    The Marboré Symphony: music for the deglaciation and Holocene in the central Pyrenees

    20th Congress of the International Union for Quaternary Research (INQUA), Dublín, 2019Geological sequences have been used as the main basis for musical pieces both as inspiration for musicians and as raw materials (geochemical data, stratigraphic logs, cycles and frequencies) for compositions produced by computer programs. Telling our stories of Quaternary changes with music presents an opportunity to reach a wider audience and to integrate art and science. Here we present an example of how to compose music from Quaternary lake sequences and illustrate in a new way the main changes in the Pyrenean landscapes since deglaciation. The creation of this music was undertaken within the framework of the REPLIM project, an INTERREG- POCTEFA – project aimed to create a network of climate change observatories on lakes and wetlands in the Pyrenees. Based on the sediment sequence from Marboré Lake (42°41′44.27″N, 0° 2′24.07″E, 2612 m a.s.l), we have selected lithological, compositional and pollen data (Leunda et al, 2017; Oliva et al., 2018) to represent the main changes in the lake and the region during the last 15000 years. To transform the geological data into musical notes, we have used a similar approach than in previous experiences (Simon et al, 2015) but in this case, notes were assigned to compositional range intervals and the tempos were defined using sediment accumulation rates. Different melodies and instruments were assigned at each data set: TOC and Br/Ti as lake bioproductivity, selected pollen data for vegetation dynamics in the valleys, Si/Ti as sediment influx and Pb/Ti as anthropogenic impact. An electronic version of the Marbore Symphony was created by computer software based on the raw data. The music group O’Carolan (http://www.ocarolanfolk.com) transformed the electronic version into a six minutes long acoustic version. The Symphony premiered in the town of Bielsa on December 14th, 2018.     Las relaciones geométricas y cinemáticas entre las unidades de basamento de los Pirineos Suroccidentales, específicamente entre la terminación de la Zona Axial y el afloramiento de los Macizos Vascos, son desconocidas y potencialmente muy complejas (Oliva-Urcia, 2018). La gravimetría se ha aplicado con éxito en otras regiones pirenaicas para resolver la geometría de las unidades de cobertera y basamento debido al contraste de densidad existente en las rocas involucradas. Con el objetivo de resolver dichas relaciones y sus implicaciones en la comprensión 4D de esta parte de la cadena, hemos llevado a cabo un exhaustivo programa de adquisición de datos gravimétricos (2018-2020) entre los valles de Salazar (al oeste) y del Aragón Subordán (hacia el este) en el marco del proyecto 3DGeoEU (GeoERA-H2020). Más de 1500 nuevas estaciones (varias de ellas en alta montaña) junto con > 1700 puntos de la base de datos SITOPO, ¿ 1200 de campañas mineras, y > 500 de anteriores proyectos del IGME hacen un total de > 5000 estaciones gravimétricas armonizadas. Además, se han recopilado > 500 datos de densidad de bases de datos o muestreados en el campo.
