Complications de l'obésité pendant la grossesse (revue de le littérature)
La grossesse est un moment critique où deux vies dépendent l une de l autre, où les comportements à risque de la mère ont des répercussions parfois irréversibles sur le développement du fœtus. L obésité devenant un problème majeur de santé publique, l objet de cette thèse est de faire le point sur les complications de la grossesse liées à l obésité et sur les voies de recherche développées actuellement. De plus en plus de femmes enceintes sont obèses et les conséquences pour la mère et l enfant sont parfois dramatiques. La littérature médicale montre que l obésité maternelle entraîne entre autre pour la mère un accroissement du risque d hypertension gravidique, d éclampsie, de diabète gestationnel, de césarienne et de complications infectieuses post-opératoires ; et pour l enfant de plus grand risque de macrosomie, de malformations du tube neural et de morbidité néonatale. La prévention de ces complications devra passer par une prise de conscience globale de la nécessité d une meilleure nutrition dès le plus jeune âge ainsi que par une meilleure information des patientes et un complément de formation des médecins.LIMOGES-BU Médecine pharmacie (870852108) / SudocPARIS-BIUM (751062103) / SudocSudocFranceF

Mortalité maternelle périnatale en Limousin (étude rétrospective de 1990 à 2005)
La mortalité maternelle périnatale est un indicateur de la qualité des soins obstétricaux. L'exhaustivité du recueil des décès est un préalable indispensable à sa surveillance. Dans ce travail rétrospectif, nous avons recensé les décès maternels sur une période allant de 1990 à 2005 en Limousin. Ce recensement a été comparé à celui des organismes de statistiques officielles. Nous avons également détaillé les causes précises du décès de chacune de ces femmes. Nous avons mis en évidence une sous-estimation de la mortalité maternelle dans les données statistiques officielles. Cette étude a également montré qu'une part non négligeable des décès maternels recensés aurait pu être évitée. Afin d'améliorer la qualité du recensement de la mortalité maternelle en France, il est indispensable d'affiner la méthodologie d'identification des décès. Obesity and cervical ripening failure risk.
International audienceOBJECTIVE: To estimate the impact of obesity on cervical ripening with prostaglandin E2 (PGE2). MATERIALS AND METHOD: A retrospective study compared the cervical ripening efficiency of PGE2 on patients with BMI above 30 kg/m(2) and normal weight patients with BMI between 20 kg/m(2) and 25 kg/m(2). In case of a Bishop score (≤3), a dinoprostone tampon was used over 12 h. In case of a Bishop score (4;6), a dinoprostone gel was used over 6 h. Failure of first cervical ripening attempt was defined, as the persistence of a Bishop score <6, 12 h after dinoprostone tampon or 6 h after dinoprostone gel. RESULTS: Ninety-five obese patients were matched according to parity to 190 non-obese patients. The failure rate of the first attempt at cervical ripening was significantly higher (P = 0.0016) among obese patients (53.7%) as compared to patients with a BMI in between 20 and 25 (34.2%). After multivariate analysis BMI ≥ 30 (OR = 2.32 (1.47-4.00), P = 0.0019), parity ≤2 (OR = 2.50 (1.20-5.26) P = 0.0137), and the Bishop score ≤3 (OR = 2.62 (1.45-4.72), P = 0.0014) were significantly and independently associated to prostaglandin ripening failure. CONCLUSION: Obesity seems to be associated to lower E2 prostaglandin sensitivity
Olivier Aumage Madeleine II: a Portable and Efficient Communication Library for High-Performance Cluster Computing
This paper introduces Madeleine II, a new adaptive and portable multi-protocol implementation of the Madeleine communication library. Madeleine II has the ability to control multiple network interfaces (BIP, SISCI, VIA) and multiple network adapters (Ethernet, Myrinet, SCI) within the same application session. We report on performance measurements obtained using BIP/Myrinet and SISCI/SCI and we present preliminary results about our MPICH/Madeleine II and Nexus/Madeleine II ports. We also discuss an extension of Madeleine II for clusters of clusters which is able to handle heterogeneous networks. In particular, we present the fast internal data-forwarding mechanism that is used on gateway nodes to speed up intercluster transmissions. Preliminary experiments show that the resulting inter-cluster bandwidth is close to the one delivered by the hardware.
Machine learning analysis of pregnancy data enables early identification of a subpopulation of newborns with ASD
International audienceTo identify newborns at risk of developing ASD and to detect ASD biomarkers early after birth, we compared retrospectively ultrasound and biological measurements of babies diagnosed later with ASD or neurotypical (NT) that are collected routinely during pregnancy and birth. We used a supervised machine learning algorithm with a cross-validation technique to classify NT and ASD babies and performed various statistical tests. With a minimization of the false positive rate, 96% of NT and 41% of ASD babies were identified with a positive predictive value of 77%. We identified the following biomarkers related to ASD: sex, maternal familial history of auto-immune diseases, maternal immunization to CMV, IgG CMV level, timing of fetal rotation on head, femur length in the 3rd trimester, white blood cell count in the 3rd trimester, fetal heart rate during labor, newborn feeding and temperature difference between birth and one day after. Furthermore, statistical models revealed that a subpopulation of 38% of babies at risk of ASD had significantly larger fetal head circumference than age-matched NT ones, suggesting an in utero origin of the reported bigger brains of toddlers with ASD. Our results suggest that pregnancy follow-up measurements might provide an early prognosis of ASD enabling pre-symptomatic behavioral interventions to attenuate efficiently ASD developmental sequels
