Perinatal outcome in women with gestational diabetes mellitus treated with Gensulin or regular human insulin during pregnancy
WSTĘP. Cukrzyca ciążowa (GDM, gestational diabetes
mellitus) to każdy rodzaj zaburzeń gospodarki
węglowodanowej, który wystąpił lub został po raz
pierwszy rozpoznany podczas ciąży. Zaburzenia te
rozwijają się u części kobiet w ciąży na skutek nieadekwatnej
odpowiedzi trzustki na fizjologiczną insulinooporność
towarzyszącą każdej ciąży i prowadzą
do hiperglikemii u ciężarnej oraz zaburzeń
metabolicznych u rozwijającego się płodu. Warunkiem
urodzenia zdrowego dziecka jest odpowiednio
wczesne zdiagnozowanie GDM, a następnie
wdrożenie odpowiedniego postępowania terapeutycznego.
Celem pracy była ocena wyrównania metabolicznego
oraz wyników badań położniczych
u kobiet ciężarnych leczonych z powodu GDM insulinami
firmy Bioton (Gensulin).
MATERIAŁ I METODY. Badaniem objęto 60 ciężarnych
z GDM, które wymagały stosowania insuliny. Trzydzieści
z nich leczono insuliną Gensulin, a pozostałe
otrzymywały inne insuliny ludzkie dostępne na polskim
rynku. Podawanie insuliny rozpoczynano wówczas,
gdy mimo wprowadzenia leczenia dietetycznego,
wartości glikemii na czczo przekraczały poziomy
5,3 mmol/l na czczo, a 2 godziny po głównych posiłkach - 6,7 mmol/l. W obu grupach kobiet
w ciąży przeanalizowano wiek w chwili zajścia
w ciążę, przedciążową masę ciała oraz przyrost masy
ciała w ciąży, a także wartości glikemii w momencie
rozpoczęcia leczenia oraz przed porodem. Przeanalizowano
również zapotrzebowanie na insulinę u badanych
ciężarnych oraz stan noworodków.
WYNIKI. Analizowane podgrupy ciężarnych nie różniły
się znamiennie ani pod względem wieku, ani
pod względem współczynników masy ciała w momencie
włączenia terapii. W obu grupach uzyskano
bardzo dobre wyrównanie metaboliczne, przy zastosowaniu
bardzo zbliżonych dawek insuliny w przeliczeniu
na kilogram masy ciała. Średnia masa urodzeniowa
noworodków nie różniła się znamiennie
między grupami. Zanotowano jedynie istotne częstsze
urodzenie dzieci z nadmierną (powyżej 90 percentyla)
masą ciała przez matki leczone innymi niż
Gensulin insulinami.
WNIOSKI. Leczenie insulinami firmy Bioton oraz pozostałymi
preparatami insulin ludzkich pozwoliło uzyskać
dobre wyrównanie metaboliczne u badanych ciężarnych
oraz dobre wyniki badań położniczych.INTRODUCTION. Gestational diabetes mellitus (GDM)
is defined as a carbohydrate intolerance with variable
severity with onset or first recognition during
pregnancy. It develops during pregnancy as a consequence
of physiological insulin resistence, that is caused by increased production of several diabetogenic
hormones and is followed by maternal hyperglycemia
that reflects a continuum of risk for adverse
outcomes including neonatal morbidities such as
macrosomia, birth trauma and neonatal hypoglycemia.
Untreated GDM is associated with increased adverse
outcome in pregnancy and the treatment results
in perinatal outcome comparable to the general
population. Aim of the study was to evaluate metabolic
control and obstetrical results in the group
of pregnant women with GDM treated with Polish
human insulin (Gensulin).
MATERIAL AND METHODS. The study group consisted
of 60 GDM pregnant women who required insulin
treatment. Thirty of them were treated with
Gensulin, and remaining 30 were treated with other
human insulins. Insulin was introduced when despite
the restricted diet, fasting glucose levels exceeded
5.3 mmol/l and 2-h postprandial levels exceeded
6.7 mmol/l. In both groups maternal age and prepregnancy
body weight were analyzed. Weight gain during
pregnancy, glucose metabolism and insulin requirement
at the beginning of treatment and before delivery
were also studied. At the last step of the study
neonatal weight and condition were analyzed.
RESULTS. No difference in maternal age as well as in
pre pregnancy weight was noticed between analyzed
groups. In both groups sufficient metabolic control
was achieved, by using similar insulin doses.
Mean neonatal weight didn’t differ between both
analyzed groups, however percentage of newborns
with LGA was significantly lower in the group treated
with lower Gensulin.
CONCLUSION. Treatment with Gensulin as well as
with other human insulins enables proper management
during pregnancy complicated with GDM and
gives the chance for successful fetal outcome
Czy warto leczyć cukrzycę ciążową?
The aim of the paper was to present data that enable us to determine whether treatment of hyperglycemia diagnosed during pregnancy, in the era of a steadily growing number of women with gestational diabetes mellitus (GDM) and an ongoing debate about new criteria and diagnostic strategies, is a valid option. GDM is the main metabolic disorder developing during pregnancy, complicating around 6% of all pregnancies. Mistreatment of hyperglycemia during pregnancy may cause several fetal complications, especially neonatal overgrowth (macrosomia or LGA). The risk of macrosomia is directly related to maternal hyperglycemia (twice as high as in the control group with glucose levels exceeding 130 mg/dl). Apart from maternal hyperglycemia and fetal hyperinsulinemia, insulin-like growth factors and selected adipocytokines produced by adipose tissue and placenta are among the factors contributing to the development of diabetic fetopathy, whose risk increases by 2-fold with glucose levels exceeding 130 mg/dl. The role of hyperglycemia as a factor inducing several perinatal complications was confirmed by the HAPO study, but it is not the sole reason of adverse effects. Also, maternal obesity, weight gain during pregnancy and maternal hyperlipidemia seem to be involved in the pathogenesis of feto-maternal complications. Changes in fetal growth, disturbances in the perinatal period, there just some of the negative consequences of maternal GDM. Disturbance of carbohydrate metabolism during pregnancy causes long-lasting consequences for both, the mother and the child, including increased risk of overt diabetes, metabolic syndrome and cardiovascular complications. Children born to GDM mothers are at a significant risk of psycho-motoric disability and a higher risk of schizophrenia. ACHOIS and MFMU studies have confirmed that even mild hyperglycemia, detected and treated in a timely manner, significantly improves maternal and fetal outcome. Various meta-analyses have revealed a positive effect of GDM treatment. To the best of our knowledge, no clinical study has revealed negative effects of such treatment, with the exception of an increased number of perinatal visits.Celem pracy jest przedstawienie danych, które umożliwią udzielanie odpowiedzi na pytanie, czy w dobie szybko narastającej liczby kobiet z cukrzycą ciążową i toczącej się dyskusji nad upowszechnieniem nowych kryteriów i strategii diagnostycznych, leczenie hiperglikemii wykrytej w czasie ciąży ma znaczenie dla rozwijającego się płodu. Cukrzyca ciążowa (GDM – Gestational Diabetes Mellitus) to najczęstsze zaburzenie metaboliczne wikłające około 6% wszystkich ciąż. Niekontrolowana hiperglikemia może powodować szereg zaburzeń u rozwijającego się płodu, spośród których najbardziej typowym jest nadmierne wzrastanie (makrosomia, LGA). W rozwoju fetopatii cukrzycowej, której ryzyko wzrasta aż 2-krotnie przy średniej glikemii dobowej powyżej 130mg/dl, istotną rolę oprócz matczynej hiperglikemii i płodowej hiperinsulinemii, odgrywają insulinopodobne czynniki wzrostu, czy też adipocytokiny wydzielane przez tkankę tłuszczową i łożysko. Dobrze udokumentowana rola hiperglikemii w rozwoju powikłań okresu ciąży, potwierdzona liniową zależnością w badaniu HAPO, nie jest jednak jedyną przyczyną niekorzystnych zmian. Otyłość matki i nadmierny przyrost masy ciała w czasie ciąży czy też hiperlipidemia ciężarnej, wydają się mieć również istotny wpływ na wyniki położnicze. Typowe zaburzenia rozwoju i wzrastania płodu, wiążące się z nimi zaburzenia okresu okołoporodowego i noworodkowego oraz charakterystyczne powikłania matczyne okresu ciąży, to nie jedyne następstwa cukrzycy ciążowej. Zaburzenia gospodarki węglowodanowej pojawiające się w okresie ciąży mają również wpływ na dalsze losy matki i jej potomstwa. Zwiększają istotnie ryzyko rozwoju cukrzycy, zespołu metabolicznego i chorób układu sercowo-naczyniowego u obojga. U dzieci matek z GDM obserwuje się również zaburzenia rozwoju psycho-motorycznego i zwiększoną zapadalność na schizofrenię. Badania ACHOIS i MFMU udowodniły, iż leczenie nawet niewielkiego stopnia zaburzeń gospodarki węglowodanowej wykrytych w czasie ciąży istotnie poprawia losy matek i ich dzieci. Przeprowadzone kolejne duże metaanalizy badań potwierdzają te korzystne rezultaty, a wyniki żadnej z prób klinicznych nie wykazały negatywnych następstw leczenia GDM, poza zwiększeniem częstości wizyt prenatalnych

Objectives: We investigated how maternal endothelial function is affected by pregestational (Type 1) diabetes mellitus (PGDM) or gestational diabetes mellitus (GDM) and/or chronic hypertension (chHT) or gestational hypertension (PIH).
Methods: We conducted a prospective, observational study involving 78 participants with GDM, PGDM and/or hypertension (PIH-16, GDM + PIH-14, PGDM + chHT-8, PGDM-20, GDM-20) in the third trimester of a singleton viable pregnancy. Twenty healthy women with uncomplicated pregnancies matched for gestational age served as controls. We analysed maternal data, disease history and serum concentrations of E-selectin and Vascular cell
adhesion molecule 1 (sVCAM-1).
Results: Only the maternal serum concentration of sVCAM-1 differed significantly among the subgroups (p< 0.0001), with the highest levels evident in women with PIH or GDM + PIH and the lowest in women with PGDM alone or PGDM + chHT.
Conclusions: Pregestational or pregnancy associated disorders, although sharing similar clinical symptoms, have a different impact on endothelial function in pregnant women.Wstęp: Śródbłonek naczyniowy jest uważany obecnie za narząd docelowy w rozwoju powikłań towarzyszących cukrzycy, jak również nadciśnieniu tętniczemu. Celem badania była analiza wpływu cukrzycy ciążowej lub przedciążowej typu 1(GDM, PGDM) oraz nadciśnienia tętniczego przewlekłego lub indukowanego ciążą (chHT, PIH) na markery funkcji śródbłonka naczyniowego.
Materiał i metoda: Prospektywne badanie obserwacyjne na grupie 78 ciężarnych w III trymestrze pojedynczej ciąży (PIH-16, GDM+PIH-14, PGDM+chHT-8, PGDM-20, GDM-20). Grupę kontrolną stanowiło 20 zdrowych ciężarnych w pojedynczej, niepowikłanej ciąży dobranych pod względem wieku ciążowego. W grupie badanej analizowano dane antropometryczne i biochemiczne oraz stężenia rozpuszczalnych frakcji E-Selektyny (sE-Sel) i VCAM-1 (sVCAM-1) w surowicy krwi.
Wyniki: Wykazano znamienną różnicę w stężeniach sVCAM-1 między analizowanymi podgrupami (p < 0,0001) przy czym najwyższe stężenia zaobserwowano w podgrupach PIH oraz GDM+PIH, a najniższe stężenia w podgrupach z PGDM z lub bez chHT.
Wnioski: W ciążach powikłanych hiperglikemią i/lub nadciśnieniem tętniczym zróżnicowany wpływ chorób matczynych na śródbłonek naczyniowy ciężarnej zależy od przewlekłego lub indukowanego ciążą charakteru zaburzeń
Ciąża heterotopowa u pacjentki bez czynników ryzyka – trudności diagnostyczne
Heterotopic pregnancy (HP) is a rare entity occurring in 1:30000 pregnancies but its incidence raises as a result of assisted reproductive techniques (ART) to 1:100-1:500. The aim of this report was to present a rare case of simultaneous intrauterine and extrauterine tubal pregnancy in a natural conception cycle. The ectopic pregnancy was not diagnosed in the first trimester scan and at 14 and 16 weeks of gestation led to severe hemoperitoneum and two exploratory laparotomies. Diagnostic problems were discussed and literature was reviewed.Ciąża heterotopowa występuje z częstością 1:30000 ciąż, ale częstość jej wzrasta, w konsekwencji stosowania technik wspomaganego rozrodu, do 1:100-1:500. Celem tej pracy jest opis rzadkiego przypadku ciąży jajowodowej współistniejącej z ciążą wewnątrzmaciczną. Ciąża ektopowa nie była zdiagnozowana w pierwszym trymestrze ciąży a w 14 i 16 tygodniu ciąży dwukrotnie doprowadziła do laparotomii z powodu obfitego krwawienia do jamy brzusznej. Insulinooporność w ciąży powikłanej cukrzycą typu 1. Czy wiemy na jej temat dostatecznie dużo?
Insulin resistance (IR) is defined clinically as the inability of a known quantity of exogenous or endogenous insulin
to increase glucose uptake and utilization. In recent years the increasing role of IR in the pathogenesis of type 1
diabetes mellitus (T1DM) related complications has been taken into account.
The aim of this article is to discuss the possible role of IR in pregnancy complicated by T1DM.
At the moment, there is no doubt that IR is not only frequently observed in T1DM patients, but also is a separate risk
factor of several complications in non- pregnant patients. The role of IR in pregnancy complicated by T1DM has not
been widely studied yet. However, data from the studies on different populations showed that IR may predispose
to such conditions as miscarriage, preeclampsia and macrosomia. Interestingly, all of these are more frequently
diagnosed in women with T1DM in comparison to healthy subjects.
The literature on the role of IR in human pregnancy is relatively rich. However, despite its significance in pathophysiology
of T1DM and its complications in general population, there is a lack of understanding of how it affects maternal
and fetal health in pregnancy complicated by this disease. Nonetheless, based on the available literature, IR
may be proposed as an additional factor modifying pregnancy outcome in woman with T1DM. Therefore, measures
that might reduce IR such as good glycemic control and control of weight gain should be recommended for every
woman with T1DM, optimally when planning but also throughout the pregnancy.Insulinooporność definiuje się jako zmniejszenie wrażliwości tkanek obwodowych na insulinę zarówno endogenną
jak i egzogenną. W ostatnich latach podnosi się rolę insulinooporności w patogenezie powikłań cukrzycy typu 1.
Insulinooporność jest nie tylko częstym zjawiskiem u pacjentów z cukrzycą typu 1 ale również stanowi izolowany
czynnik ryzyka szeregu powikłań w populacji kobiet nie będących w ciąży oraz mężczyzn. Rola insulinooporności
w ciąży powikłanej cukrzycą typu 1 nie była dotychczas szeroko zbadana. Dane z badań przeprowadzonych
w innych populacjach ciężarnych pokazują iż insulinooporność zwiększa ryzyko takich powikłań jak poronienia,
stan przedrzucawkowy i makrosomia. Co istotne, wszystkie te powikłania spotyka się istotnie częściej u pacjentek
z cukrzycą typu 1. Mimo braku badań dotyczących roli insulinoopornośći w ciąży powikłanej cukrzycą typu 1, na
podstawie danych z innych populacji ciężarnych, insulinnoporność może być uznana za dodatkowy czynnik ryzyka
powikłań. Stąd też działania, które potencjalnie mogą doprowadzić do zmniejszenia insulinoopornośći takie jak kontrola
glikemii oraz kontrola przyrostu masy ciała powinny być zalecane pacjentkom zarówno w okresie planowania
ciąży jak i w trakcie jej trwania
Zapalenie wyrostka robaczkowego u ciężarnej chorującej na cukrzycę typu 1 – opis przypadku
We present a case report of a 22-year-old pregnant patient with type 1 diabetes mellitus diagnosed with an appendicitis at 21st week of gestation, who underwent laparotomy and appendectomy. In later pregnancy, she required treatment for recurrent urinary tract infections and nephrolithiasis. Despite having several risk factors for an unfavorable perinatal outcome, she had caesarean section performed at term and delivered a healthy, full-term newborn. In this patient, we also discuss clinical conundrum of pregnancy complicated with several conditions that may manifest with acute abdominal symptoms and perioperative care for a pregnant woman with type 1 diabetes..Przedstawiono opis przypadku 22-letniej pierworódki chorującej na cukrzycę typu 1, z ropnym zapaleniem wyrostka robaczkowego w 21. tygodniu ciąży, wymagającym pilnego leczenia operacyjnego. Dalszy przebieg ciąży był powikłany nawracającym infekcjami układu moczowego i kamicy nerkowej. Mimo licznych czynników ryzyka niekorzystnego wyniku położniczego (stan po przebyciu appendektomii, długotrwała cukrzyca, nawracająca kamica nerkowa), ciążę zakończono w terminie, cięciem cesarskim, porodem zdrowego, donoszonego noworodka. Na przykładzie omawianej sytuacji klinicznej przedstawiamy trudności diagnostyczne charakterystyczne dla ciąży powikłanej licznymi stanami mogącymi imitować objawy tzw. „ostrego brzucha” oraz specyfikę postępowania okołooperacyjnego w warunkach zagrożenia dekompensacją cukrzycy typu 1 u ciężarnej
Polyhydramnios — frequency of congenital anomalies in relation to the value of the amniotic fluid index
Objectives: The aims of our study were to assess the correlation between the amniotic fluid index (AFI) value and the frequency and type of fetal anomalies.
Material and methods: The material included 94 patients at the third trimester of pregnancy, 60 with mild polyhydramnios, 19 with moderate one, and 15 with severe one. Polyhydramnios was diagnosed if AFI was > 24 cm. All patients were divided into three groups based on the value of AFI: 1) mild polyhydramnios with AFI between 24.1 and 29.9 cm, 2) moderate polyhydramnios with AFI between 30–34.9 cm, and 3) severe polyhydramnios with AFI ≥ 35 cm.
Results: The incidence of fetal malformations correlated significantly with the degree of polyhydramnios and was the highest in patients with severe polyhydramnios (53.3%, p = 0.002). Congenital malformations of the gastrointestinal tract were the most frequent fetal anomalies in the whole group of patients (5.3%). Trisomy 18 was the most frequent aneuploidy found in women with polyhydramnios (2.1%).
Conclusions:
The incidence of fetal congenital anomalies significantly increases with the degree of polyhydramnios, being most frequent in severe one and rather rare in a mild one.
Congenital malformations of the gastrointestinal tract were the most frequent anomalies in patients with polyhydramnios, especially in women with severe polyhydramnios
Influence of pregnancy planning on obstetrical results in women with pregestational diabetes mellitus
Abstract Objective: The aim of the study was to compare obstetric results in two groups of pregnant women (patients that had and had not planned their pregnancy) suffering from pregestational diabetes mellitus (PGDM). Material and methods: 400 women with PGDM were divided into two subgroups: planned pregnancy (P) (N=200) and unplanned pregnancy (NP) (N=200). The study was based on retrospective analysis of data collected at the Department of Obstetrics and Women Diseases, University of Medical Sciences, Poznan, from patients who delivered between 1995-2006. In both groups obstetrical complications and selected neonatal parameters were analyzed. Results: Statistically important differences in obstetric results between the two analyzed subgroups of patients were observed. P-group was metabolically better controlled, both upon admission to our department and in the third trimester. The mean daily insulin requirement among P-group patients was smaller than among pregnant NP-women upon admission to our department. P-patients suffered from miscarriages and stillbirths less frequently. Neonates born to P-mothers had higher gestational age at delivery and their Apgar score and cord blood results were significantly better. Conclusions: Our results confirmed that patients with PGDM who plan their pregnancies achieve significantly better obstetric results compared with unplanned pregnancy group. Pregnancy planning should become a standard of obstetric and diabetic care in this group of patients
Współczesne metody wczesnej diagnostyki stanu przedrzucawkowego i nadciśnienia indukowanego ciążą
Preeclampsia remains to be a serious perinatal complication and early screening for this disease to identify the high risk population before the first symptoms develop constitutes a considerable clinical challenge. Modern methods of screening for preeclampsia and pregnancy-induced hypertension include patients history, biochemical serum markers and foetal DNA and RNA in maternal serum. They aid the process of developing an optimal protocol to initiate treatment in early pregnancy and to reduce the rate of complications. Our review presents an overview of the novel methods and techniques used for early screening for preeclampsia and pregnancy-induced hypertension. Most of the research focuses on 11-13 weeks of gestation due to the fact that the first prenatal examination is performed at that time. The most important information seems to be: weight, mass, mean blood pressure, history of pregnancy-induced hypertension or preeclampsia at previous pregnancies as well as the ethnic origin. During an ultrasound scan, pulsatility index of the uterine arteries is measured. Blood samples are obtained during the last part of the examination. At the moment only a few markers seem to be strong predictors of hypertensive disorders during pregnancy: pregnancy-associated plasma protein-A (PAPP-A), placental growth factor (PlGF) and soluble fms-like tyrosine kinase-1 (sFlt-1). Also, fetal DNA and RNA in maternal plasma are helpful in the prediction of preeclampsia as they are markers of the trophoblast apoptosis. Researchers aim at identifying the population at high risk of pregnancy-induced hypertension and preeclampsia in order to offer appropriate antenatal care to these women. At the moment many drugs and diet supplements are investigated to reduce the prevalence of hypertensive disorders in pregnancy. These medications are usually administrated in early gestation (up to 16 week of gestation) before the first clinical symptoms present. Low doses of aspirin were found to decrease the risk of preeclampsia in high-risk groups. Moreover, according to some recent research, also essential Ω-3 fatty acids reduce the incidence of preeclampsia. None of the other investigated diet supplements or antioxidants were proven to successfully reduce incidents of hypertensive disorders. So far, there is available evidence on the lack of any effect for vitamins C,D or E. Further studies are necessary to define clinical useful markers of gestational hypertension.Stan przedrzucawkowy jest poważnym powikłaniem położniczym. Wyselekcjonowanie pacjentek wysokiego ryzyka wystąpienia zaburzeń nadciśnieniowych we wczesnej ciąży, przed pojawieniem się pierwszych objawów klinicznych, stanowi wyzwanie dla współczesnego położnictwa. Współcześni badacze analizują przeszłość położniczą ciężarnych, wyniki badania przedmiotowego, stężenie markerów biochemicznych w krwi a nawet obecność płodowego DNA i RNA w surowicy matki. Znalezienie markera stanu przedrzucawkowego pozwoli na wdrożenie wczesnego leczenia i zmniejszenie częstości występowania zaburzeń regulacji ciśnienia tętniczego krwi u ciężarnych.
