Treatment of patients with lymphoma and coexisting infection with HCV, HBV or HIV
Chorzy z chłoniakami nieziarniczymi (NHL) i towarzyszącą infekcją wirusem zapalenia wątroby
typu B (HBV), typu C (HCV) lub zakażeniem ludzkim wirusem niedoboru odporności
(HIV) stanowią dla klinicysty problem polegający na wzajemnych zależnościach infekcji wirusowej,
chłoniaka i leczenia przeciwnowotworowego. Chorych z infekcją HCV i NHL należy
przed rozpoczęciem terapii ocenić kompleksowo wirusologicznie. Jeśli nie stwierdza się wskazań
do leczenia przeciwwirusowego, leczenie przeciwchłoniakowe należy przeprowadzić tak,
jak u osób bez tej infekcji. Ryzyko konsekwencji groźnych dla życia reaktywacji HCV i zapalenia
wątroby jest w tych przypadkach na tyle znikome, że nie powinno wpływać na decyzje
o leczeniu przeciwnowotworowym. W przypadku indolentnych NHL warto podjąć próbę terapii
interferonem z ribawiryną, ponieważ skuteczność eradykacji HCV koreluje z szansą uzyskania
remisji chłoniaka. W przeciwieństwie do HCV przebyta lub przewlekła infekcja HBV
stanowi czynnik ryzyka reaktywacji i związanej z nią śmiertelności u chorych leczonych
z powodu NHL. Profilaktyka przeciwwirusowa lamiwudyną powinna być rozważana rutynowo;
trwa ona od 7 dni przed rozpoczęciem chemioterapii do 6 miesięcy po jej zakończeniu.
U chorych bez wcześniejszego kontaktu z HBV należy rutynowo zalecać czynną immunizację
przed rozpoczęciem leczenia, o ile wcześniej nie zostali poddani szczepieniom. Chorych z infekcją
HIV i NHL powinno się leczyć, nie przerywając wysoce aktywnej terapii antyretrowirusowej
(HAART), według odpowiednich schematów leczenia, w tym z zastosowaniem rytuksymabu.
Schematy o zmniejszonej intensywności należy pozostawić dla chorych z liczbą komórek
CD4+ poniżej 100/mm3 krwi obwodowej. Szczególną uwagę należy przypisać profilaktyce
i leczeniu dokanałowemu tych chorych oraz terapii wspomagającej w trakcie chemioterapii.

diagnostics should be performed before lymphoma treatment is administered. If there are no
indications to initiate antiviral therapy, the lymphoma treatment should be administered like
in a subject without HCV infection. The risk of life-threatening complications of HCV reactivation
is minimal in this cases and should not be taken into consideration when decisions
related to lymphoma treatment are to be made. In the case of indolent NHL as the efficacy of
HCV eradication is associated with remission achievement, it is worth to start therapy with
antiviral schedules of interferon with ribavirin. On the contrary, previous or chronic HBV
infection is a risk factor for HBV reactivation and for reactivation-related mortality. Antiviral
prophylaxis with lamivudine is to be considered routinely with its duration of 7 days before
antineoplastic therapy initiation and ending 6 months after its completion. In patients not
previously exposed to HBV active immunization is recommended, unless the subject was earlier
vaccinated. Finally, patients with HIV infection and NHL should be treated with concomitant
highly active antiretroviral therapy (HAART) and adequate chemotherapy schedules (including
rituximab). Schedules with reduced intensity should be restricted to patients with CD4+ cells
count less than 100 cells/mm3. Special attention should be paid to intrathecal prophylaxis and
therapy as well as supportive treatment during chemotherapy.

This paper presents the most interesting, according to the author’s opinion, trial results presented at the American Society of Clinical Oncology Annual Meeting in 2016. Many study results were presented, most of them important for diagnostics, treatment and prognosis of patients with diffuse large B-cell lymphoma, follicular lymphoma, mantle cell lymphoma and Hodgkin lymphoma and others.W pracy przedstawiono najciekawsze, zdaniem autorki, doniesienia z dorocznej konferencji Amerykańskiego Towarzystwa Onkologii Klinicznej w 2016 roku. Podczas konferencji były prezentowane wyniki badań ważnych dla diagnozowania, leczenia i prognozowania dotyczące między innymi chorych na chłoniaka rozlanego z dużych komórek B, chłoniaka grudkowego, chłoniaka z komórek płaszcza i chłoniaka Hodgkina

Evolving therapy options for B-cell low grade lymphoma
W pracy przedstawiono aktualne poglądy na temat leczenia chłoniaków o małym stopniu złośliwości linii B.
Omówiono metody diagnostyczne mające na celu precyzyjne rozpoznanie podtypu chłoniaka, a także
czynniki prognostyczne dla ich przebiegu i predykcyjne dla odpowiedzi na różne metody terapeutyczne.
Ze względu na niejednorodność omawianej grupy chorób przedstawiono odrębności ich przebiegu, konieczne
do właściwego zaplanowania terapii. Dla chłoniaków grudkowych optymalnym postępowaniem
jest obecnie chemioimmunoterapia z zastosowaniem rituksymabu. Dotychczas w toku badań randomizowanych
nie ustalono najskuteczniejszego schematu polichemioterapii, lecz wyniki tych badań mają
być opublikowane w najbliższych latach. Jednak niezależnie od sposobu leczenia indukującego chorzy
powinni otrzymywać leczenie podtrzymujące rituksymabem. W chłoniaku śledzionowym strefy brzeżnej
leczeniem pierwszej linii pozostaje splenektomia. W chłoniakach typu MALT należy oprócz antybiotykoterapii
rozważyć chemioterapię (szczególnie z analogami puryn), radioterapię (w niskich stopniach zaawansowania
klinicznego) i immunoterapię. W chłoniakach węzłowych strefy brzeżnej zaleca się zasady
postępowania takie jak dla zaawansowanych chłoniaków grudkowych. W chłoniaku limfoplazmocytowym
oprócz chemioterapii zaleca się w przypadkach przebiegających z nadlepkością plazmaferezę,
a w przypadku krioglobulinemii szczególnie uzasadnione jest leczenie z zastosowaniem rituksymabu
i/lub cyklofosfamidu.This article presents actual treatment opinions of low grade B-cell lymphomas. The precise diagnosis of
lymphoma subtype is necessary for adequate treatment planning and identification of prognostic factors
for adverse clinical outcome, as well as predictive factors for particular treatment response. Taking into
consideration that low grade lymphomas constitute a heterogenic group of entities, the differences
in clinical course have been described. The most adequate induction treatment for follicular lymphoma is
chemoimmunotherapy with rituximab. The optimal chemotherapy schedule has not been established yet
in clinical trials but their results are expected during the years to come. Independently of induction therapy,
patients with follicular lymphoma beneficiate from maintenance treatment with rituximab. In splenic
marginal zone lymphoma splenectomy remains the first line treatment of choice. In MALT lymphoma
chemotherapy, especially based on purine analogs is efficient as well as immunotherapy in chosen patients
but antibiotics need to be incorporated in first line therapy. Radiotherapy is a potential treatment
option for only local involvement. In nodal marginal zone lymphoma, the same approach as in follicular
lymphoma is advised. In lymphoplasmacytic lymphoma chemotherapy is efficient but in cases with blood orhyperviscosity
plasmapheresis is advised, while patients with cryoglobulinemia beneficiate from rituximab
or/and cyclophosphamide
Leczenie podtrzymujące rytuksymabem u chorych na chłoniaka grudkowego w pierwszej i kolejnych remisjach choroby
The treatment of patients with follicular lymphoma (FL) requires that the first line therapy isprecisely planned and individualised. Immunochemotherapy based on rituximab (R) remains thestandard of the first line treatment. Despite of the choice of the first line therapy (includingmonotherapy with R and chemotherapy), responding patient benefit from R maintenance treatment.When patients with relapsed FL require treatment, there is a broad number of options, rangingfrom R alone to combination chemotherapy plus R, radioimmunotherapy and for selectedpatients hematopoietic stem cell transplantation. A 2-year maintenance therapy with R, eitheras four weekly infusions every 6 months or as a single infusion every 2–3 months, should beadded to standard therapy for patients with relapsed or refractory FL after successfulre-induction therapy.Leczenie chorych na chłoniaka grudkowego (FL) wymaga starannego i indywidualnego zaplanowaniamomentu rozpoczęcia leczenia. Obecnie w leczeniu pierwszej linii standardem pozostajeimmunochemioterapia z rytuksymabem (R). Niezależnie od metody indukcji pierwszejremisji (także monoterapia R i chemioterapia) chorzy odpowiadający na to leczenie odnosząkorzyść z leczenia podtrzymującego R. Postępy w diagnostyce i leczeniu chorych na chłoniaki nie-Hodgkina
This paper discusses the vital advances made for patient diagnosis and treatment of non-Hodgkin lymphomas, which in particular includes follicular lymphoma, Waldenström macroglobulinaemia, mantle cell lymphoma, diffuse large B-cell lymphoma, primary mediastinal large B-cell lymphoma and peripheral T-cell lymphoma. Recent studies demonstrating key improvements in early and long-lasting therapy were specially considered.W pracy przedstawiono najważniejsze osiągnięcia związane z diagnostyką i leczeniem chorych na chłoniaki nie-Hodgkina, w szczególności chłoniaka grudkowego, makroglobulinemię Waldenströma, chłoniaka z komórek płaszcza, chłoniaki rozlane z dużych komórek B, pierwotnego chłoniaka śródpiersia z dużych komórek B oraz chłoniaki z obwodowych komórek T. Hodgkin lymphoma in a pregnant patient
Występowanie choroby nowotworowej u chorej w ciąży jest rzadkim problemem w onkologii i hematologii. Przedstawiony opis przebiegu klinicznego dotyczy 29-letniej pacjentki w 28. tygodniu ciąży, u której rozpoznano chłoniaka Hodgkina (nodular sclerosis [NS] II). Zaawansowanie określono jako stopień IIA z masą „bulky” według klasyfikacji Ann Arbor. Ze względu na narastającą duszność, spowodowaną limfadenopatią śródpiersia, pierwszy cykl leczenia według schematu EVA (etopozyd, winblastyna, doksorubicyna) podano w 32. tygodniu ciąży. Pacjentka w 35. tygodniu ciąży urodziła zdrową córkę. Po zakończeniu ciąży rozpoczęto leczenie właściwe dla chłoniaka Hodgkina. W badaniach obrazowych po zakończonej terapii stwierdzono całkowitą remisję choroby. Przedstawiony opis przypadku ukazuje liczne problemy diagnostyczne, terapeutyczne oraz etyczne, jakie stawia przed lekarzem współwystępowanie u pacjentki aktywnej choroby nowotworowej i ciąży.The occurrence of cancer in a pregnant patient is a rare problem in oncology and hematology. We present a case report about the 29 years old female pregnant patient with newly diagnosed Hodgkin lymphoma (nodular sclerosis [NS] II). We assess the clinical stage as IIA with bulky mass according to Ann Arbor classification. The first cycle of treatment according to the EVA (etoposide, vinblastine, doxorubicin) schedule was administrated at 32 weeks of gestation, due to the progressive dyspnoea caused by growing mediastinal lymphadenopathy. At 35 weeks of pregnancy the patient gave birth to a healthy daughter. After the end of pregnancy we started standard Hodgkin lymphoma treatment. At the end of treatement the radiological examination showed complete metabolic remission of the disease. The featured case report shows the number of diagnostic, therapeutic and ethical problems related with coexistence of pregnancy and active malignant disease
Multicentre, prospective observational study of pegfilgrastim primary prophylaxis in patients at high risk of febrile neutropenia in Poland : PROFIL study
AIM OF THE STUDY: PROFIL was a prospective observational study conducted to investigate physicians’ evaluation of febrile neutropenia (FN) risk and reasons for giving pegfilgrastim primary prophylaxis (PP) in routine clinical practice in Poland. MATERIAL AND METHODS: Adult cancer patients treated with chemotherapy (CT), assessed by investigators as having high overall FN risk, and who received pegfilgrastim in cycle 1 were enrolled between 03/2009 and 09/2010. Investigators assessed FN risk of the CT regimen, individual risk factors, and overall FN risk, and were asked to provide the most important reasons for providing pegfilgrastim PP. Investigator-assessed CT FN risk was compared with guideline classification. RESULTS: Data were analysed from 1006 breast, ovarian, and lung cancer, and non-Hodgkin (NHL) and Hodgkin lymphoma (HL) patients. The most important reasons for using pegfilgrastim PP were high CT FN risk and advanced disease; these were consistent across tumour types and treatment intent. The investigators generally assessed high CT FN risk in agreement with guideline classification. Febrile neutropenia occurred in 4% of patients, most commonly in HL, NHL, and patients with advanced disease. CONCLUSIONS: High CT FN risk and advanced stage of disease were found to be the most important reasons for providing pegfilgrastim PP by physicians in Poland
Nasal T/NK-cell lymphoma - case report
W pracy przedstawiono opis przebiegu klinicznego chłoniaka nosowego T/NK-komórkowego,
rozpoznanego na podstawie badania histopatologicznego zmiany guzowatej u nasady nosa
(IIA wg klasyfikacji Ann Arbor). Chorego leczono według schematów CN3OP (cyklofosfamid,
mitoksantron, winkrystyna, prednizon) oraz IVAC (etopozyd, ifosfamid, cytarabina), ale mimo
początkowej dobrej odpowiedzi na chemioterapię szybko doszło do progresji choroby w postaci
uogólnionych zmian skórnych i zajęcia płuc. Chory zmarł po 7 miesiącach od rozpoznania
z objawami niewydolności oddechowo-krążeniowej. Przypadek ilustruje trudności diagnostyczne
i terapeutyczne w bardzo rzadko występującym chłoniaku nosowym z komórek T/NK.In the present paper we describe clinical course of nasal T/NK cell lymphoma diagnosed on the
basis of histopathologic examination of tumor lesion within the base of nose (Ann Arbor IIA).
The patient received chemotherapy according to CN3OP (cyclophosphamide, mitoxantrone,
vincristine, prednisone) and IVAC (etoposide, ifosfamide, cytarabine) protocols. His initial
response to chemotherapy was good, but rapid disease progression within skin and lungs
subsequently occured. The patient died due to cardio-pulmonary insufficiency 7 months after
lymphoma diagnosis. This report reveals diagnostic and therapeutic difficulties of the very rare
