The prognostic significance of thrombocytosis in ovarian cancer
Ovarian cancer makes up 25–30% of all cases of cancers of the female genital tract. It has the highest mortality rate of any condition in oncological gynaecology. Early diagnosis is associated with a favourable 5-year survival prognosis. Many solid tumours have been detected with concomitant thrombocytosis. The tumour cell-induced platelet aggregation is a result of a direct integration of tumour cells with blood platelets. The aim of the present paper is an evaluation of platelet count as a prognostic parameter for ovarian cancer. Between 2000 – 2005, 349 patients with ovarian tumour (aged 12 – 88-years-old) underwent primary surgical treatment at a clinic. Ninety-seven patients with ovarian carcinomas underwent chemotherapy in the Oncology Centre. The control group comprised 252 women diagnosed with a histopathological lesion of mild intensity, whereas the cancer group constituted 97 women with a histopathologically-diagnosed malignant neoplasm. Thrombocytopaenia was assumed with a platelet count below 150G/L and thrombocythaemia at 350G/L and higher.
Thrombocytosis often coincides with ascites and the cytoreduction decreases platelet count. There is a positive correlation between platelet count and tumour grading. Thrombocytosis was more frequently found in high grade tumours. There is also a positive correlation between platelet count and tumour stage according the International Federation of Gynaecology and Obstetrics (FIGO). Thrombocytosis was more frequently found in stage III and IV cancers. Patients with co-occurring thrombocytosis were found to have shorter survival periods and shorter time free from disease. This seems to give grounds for measuring platelet count before the primary surgical intervention, and suggests that the platelet count should be included in the panel of prognostic factors for patients with ovarian tumours
Warning symptoms that oblige women to visit gynaecologist in postmenopausal period - preliminary communication
Wstęp. W okresie pomenopauzalnym wzrasta zapadalność na raka trzonu macicy, raka jajnika i nowotwory
sromu. Częściej obserwuje się zaburzenia statyki narządu rodnego i/lub nietrzymanie moczu. Celem pracy była
odpowiedź na pytanie, który z niepokojących objawów po zaprzestaniu miesiączkowania zmusza kobietę do
konsultacji z lekarzem ginekologiem?
Materiał i metody. Przebadano 31 kobiet po menopauzie, będących słuchaczkami Uniwersytetu Trzeciego
Wieku w Warszawie. Średnia wieku badanych wyniosła 64,3 roku. Wszystkie kobiety mieszkały w mieście liczącym
powyżej 300 tysięcy mieszkańców. Zamężnych kobiet było 6 (19,4%), wdów - 15 (48,4%), stanu wolnego
- 9 (29%), rozwiedziona - 1 (3,2%). Wyższe wykształcenie miało 17 kobiet (54,8%), średnie - 14 (45,2%).
Narzędziem badawczym była anonimowa ankieta. Pytania dotyczyły wieku, stanu cywilnego, wykształcenia,
miejsca zamieszkania i objawów zmuszających do wizyty u ginekologa.
Wyniki. Wyniki były zróżnicowane: 1. Krwawienie z dróg rodnych w okresie pomenopauzalnym zawsze zmuszało
kobiety do odbycia konsultacji u lekarza ginekologa; 2. Inne objawy, takie jak: upławy, świąd krocza, ból
w dole brzucha i nietrzymanie moczu były często bagatelizowane; 3. Co druga kobieta cierpiąca na nietrzymanie
moczu zgłaszała ten problem lekarzowi ginekologowi.Background. In postmenopausal period incidence of endometrial carcinoma, ovarian carcinoma, vulvae carcinoma
increases. Also urinary incontinence and pelvic organs mobility are more frequently observed in this
period. The aim of this study was to answer the question: which of many warning symptoms obliges women to
visit gynaecologist in postmenopausal period.
Material and methods. Thirty-one women, students of University of Third Age with the mean age of 64.3
were examined. All of them live in a city of more than 300,000 citizens. Six women were married, fifteen women
were widows, nine women were single and one woman was divorced. Seventeen women had higher education,
fourteen women had secondary education. A questionnaire was used as a research tool. Questiones concerned
age, education, residence and warning symptoms.
Results. The results were diverse: 1. Uterine bleeding in postmenopausal period always obliged women to visit
gynaecologist; 2. Symptoms such as: leukorrhea, pruritus of the vulva, pain in the lower abdomen and urinary
incontinence were often ignored by women; 3. Every second woman suffering from urinary incontinence indicated
this problem to gynaecologist
Methods of family planning in group of married woman - choice, estimate, satisfaction
Wstęp. Planowanie rodziny jest zagadnieniem istotnym w praktyce zawodowej położnej. Położna pracująca w konkretnej społeczności
lokalnej powinna znać strukturę, oczekiwania i poziom wiedzy swoich podopiecznych/klientek. To rozpoznanie stanowi podstawę do
planowania jej pracy w wymiarze edukacyjno-zdrowotnym.
Cel pracy. Oceniono metody planowania rodziny stosowane przez kobiety w związkach małżeńskich. Analizowano źródła informacji,
kryteria wyboru, wpływ na pożycie płciowe oraz ogólną ocenę i satysfakcję badanych.
Materiał i metody. Za pomocą metody sondażu diagnostycznego przebadano 100 kobiet w wieku 20-45 lat będących w związkach
małżeńskich. Narzędziem badawczym był samodzielnie skonstruowany dla celów pracy kwestionariusz ankiety.
Wyniki i wnioski. Najczęściej stosowanym źródłem informacji na temat metod planowania rodziny były czasopisma, poradniki i książki
(59%). Następnie wskazano lekarza (56%), a tylko 13% szukało porady u położnej. Najczęściej stosowano następujące metody: prezerwatywę
(36%), hormonalne tabletki doustne (35%), stosunek przerywany (26%), metody naturalne (28%). Kobiety kierowały się takimi czynnikami
w wyborze, jak: skuteczność (49%), bezpieczeństwo zdrowotne (38%), wygoda w stosowaniu (31%) oraz cena (8%). Stosowaną
metodę (lub metody) oceniło dobrze 57% respondentek, 32% wybrało ocenę bardzo dobrą, 68% badanych kobiet uznało, że wybrana
metoda nie zaburza pożycia seksualnego, 30% zadeklarowało, że tylko w niewielkim stopniu. Zdecydowaną poprawę pożycia małżeńskiego
zadeklarowało 20% respondentek, 26% odpowiedziało, że raczej tak. Według 18% dana metoda raczej nie poprawiła jakości pożycia,
a 34% nie posiada zdania na ten temat. Większość par małżeńskich (54%) nie planuje powiększenia rodziny, 22% - planuje macierzyństwo,
a 22% jeszcze nie wie.

should know structure, expectations and level of knowledge of client. This reconnaissance presents base for planning her work in dimension
educationally - healthy.
Aim of the study. Estimate of method of planning of family applicable by women in conjugal associations. Analysis of source of information,
criteria of choices, influence on genital life, general estimates and satisfaction.
Material and methods. Method of diagnostic survey, 100 woman in age from 20 for 45 year old in conjugal associations. Questionnaire was
constructed independently for purposes of work.
Results and conclusions. Source of information about methods of planning of families: books and popular literature (59%), physician
(56%), midwife only 13%. Methods it most often applicable: contraceptive sheath (36%), hormonal oral tablets (35%), attitude intermittent
(26%). Natural methods it uses 28% respondents. Criteria of choices of methods: efficiency (49%), sanitary safety (38%), convenience in
application (31%) and price (8%). 57% respondents it has evaluated applicable method well, 32% very good. 68% researched women has
regard that it does not disturb chosen method in sexual life, it has declared 30%, that in only small degree. 20% respondents it has declared determined correction of conjugal life, it has answered 26%, that rather so. It has not corrected quality of life according to 18% give method
rather, but it lacks opinion (sentence) on this theme 34%. Majority conjugal couple (54%) it does not plan increase of family, 22% plans
motherhood, 22% it does not know.

Evaluation of TATI marker assays – the pancreatic trypsin inhibitor – in patients with endometrial cancer with unfavorable predictors
TATI marker (tumor-associated trypsin inhibitor) is most often assessed in patients with ovarian cancer. This paper presents the role and the usefulness of the marker in the evaluation of the treatment results in patients with endometrial cancer. The study was based on the evaluation of TATI marker level in 305 patients suffering from endometrial cancer treated at the Cancer Center between 1994 and1995 in Warsaw. In each patient the TATI marker level was measured from 3 to 7 times in the blood serum after each stage of the adjuvant treatment and in the initial part of the follow-up period. In order to assess the staging a postoperative protocol in force in 1988 (FIGO) was used and in accordance with this protocol, the patients were qualified for the adjuvant therapy, which consisted of the teletherapy, brachytherapy and hormonal therapy. All patients were observed on an outpatient basis after the treatment. The analysis of TATI marker levels in correlation with the features of cancer were carried out after a 17-year-long follow-up period. TATI marker levels have been evaluated in patients with unfavorable predictors identified in the operation protocol and in the histopathological protocol of the removed tumor. It was found that TATI level in the fourth sampling after the treatment was the most important value. TATI level increase in this assay was significantly correlated with the occurrence of local relapse or distant metastasis. The comparison of the results of patients with and without surgical removal of the lymph nodes has shown a significantly longer disease-free period in patients whose lymph nodes were not removed.Marker TATI (tumor-associated trypsin inhibitor) najczęściej oceniany jest u chorych na raka jajnika. W pracy przedstawiono jego rolę i przydatność w ocenie wyników leczenia u chorych na raka trzonu macicy. Badanie przeprowadzono na podstawie oceny poziomu markera TATI wśród 305 chorych na raka trzonu macicy leczonych w Centrum Onkologii w latach 1994–1995 w Warszawie. U każdej chorej wykonano od 3 do 7 oznaczeń markera z surowicy krwi po każdym z etapów leczenia uzupełniającego i w początkowym okresie obserwacji po zakończeniu leczenia. W ocenie zaawansowania posługiwano się wówczas obowiązującym protokołem pooperacyjnym FIGO z 1988 roku i zgodnie z nim chore kwalifikowano do leczenia uzupełniającego, na które składały się teleterapia, brachyterapia lub hormonoterapia. Wszystkie chore po leczeniu były obserwowane ambulatoryjnie. Analizę poziomów markera TATI w korelacji z cechami raka przeprowadzono po 17-letnim okresie obserwacji. Oceniono poziomy markera TATI w grupie chorych z niekorzystnymi czynnikami rokowniczymi stwierdzonymi na podstawie protokołu operacyjnego i histopatologicznego usuniętego guza nowotworowego. Stwierdzono, że poziom TATI w czwartym pobraniu, po ukończeniu leczenia jest najistotniejszą wartością. Wzrost poziomu TATI w tym oznaczeniu znamiennie koreluje z wystąpieniem wznowy lub przerzutów odległych. Porównując wyniki leczenia operacyjnego bez usunięcia węzłów chłonnych i z ich usunięciem, stwierdzono znamiennie dłuższy czas wolny od choroby u chorych, u których nie były usuwane węzły chłonne
Gravida's knowledge concerning harmful effect of smoking on the fetus and the awareness of HPV infection impact on the development of cervical cancer
Aim of the paper: The aim of the paper was to evaluate gravida's knowledge concerning the harmful effect of smoking on the fetus and the awareness concerning the impact of nicotine on the development of cervical cancer in women infected with HPV. Material and method: The study covered 127 women – healthy gravidas provided with a pecialist care of K clinic, inhabitants of cities and villages. The study was conducted from March to June 2015 with the utilization of a diagnostic survey. To that end, an author's questionnaire was drafted, which consisted of 23 questions concerning smoking tobacco and respondent's knowledge in terms of the HPV infection. Results: Responses of 127 women were evaluated, out of whom 76% had never smoked tobacco, 8% stopped smoking due to pregnancy and 16% limited smoking in that time or moved to "light" cigarettes, or used nicotine patches and limited smoking simultaneously. The majority of gravidas (94%) were aware that smoking is harmful to the child and toxic substances penetrate through the placenta; 76% knew that toxic substances also penetrate to breast milk. The largest group of women (36%) was exposed to passive smoking in own houses, among the closest family. A problem with motivation may be observed in the group of smoking gravidas – it may be necessary to consider obligatory consultations with a psychologist related to motivation and addiction for all the smokers being pregnant or planning their pregnancy. Moreover, the survey proved that young women lack basic knowledge in the field of HPV infection

Purpose of work is presentation of role and meanings of suitable communications in process of preparation of patient for gynecological
operative procedure with taking into consideration role of midwife. In article it discuss theoretical bases in turn before operative, characteristic

Cel pracy: próba zdiagnozowania rodzaju i częstotliwości
problemów zdrowotnych pojawiających się u matki i dziecka
w okresie połogu oraz określenie roli położnej środowiskowo
-rodzinnej w ich rozpoznawaniu i rozwiązywaniu.
Materiał i metoda: Badania metodą sondażu diagnostycznego,
z wykorzystaniem autorskiego kwestionariusza ankiety,
przeprowadzono w grupie 100 kobiet, podopiecznych
położnej środowiskowo-rodzinnej. Metodą uzupełniającą
była obserwacja i rozmowa prowadzona w czasie wizyt
patronażowych.
Wyniki: Większość badanych wykazywała prawidłowe
parametry stanu ogólnego (temperatura, tętno, ciśnienie
krwi) i położniczego. Najwięcej rozpoznanych problemów
dotyczyło procesu laktacji. Prawie połowa (41%) oceniła swoją
laktację jako niewystarczającą, tylko jedna trzecia uznała ją
za prawidłową. Tylko 21% respondentek twierdzi, że nie ma
żadnych problemów z brodawkami. Pozostałe zgłaszają ich
tkliwość (39%) i poranienie (35%). Tylko 41% karmi piersią
na żądanie. W przypadku 61% noworodków obserwuje się
nieprawidłowy mechanizm ssania piersi. Analiza aktywności
fizycznej kobiet pozwoliła stwierdzić, że ponad połowa
respondentek (61%) odczuwa ograniczenia w poruszaniu
się. Podobny odsetek przyznaje, że nie ma czasu dla siebie.
Wnioski: 1. Najczęstsze problemy okresu połogowego dotyczą
procesu laktacji. 2. Pomimo edukacji przedporodowej
i wsparcia w czasie wizyt patronażowych, wciąż zbyt mało
kobiet karmi piersią w sposób ekologiczny. 3. Położna środowiskowo-
rodzinna jest pierwszą osobą, która jest w stanie, w czasie wizyt patronażowych, rozpoznać problemy zdrowotne
matki i dziecka, zaplanować, wdrożyć i ocenić sposoby
ich rozwiązania.The purpose of the work: an attempt to diagnose the nature
and frequency of emerging health problems in mothers
and children during labour or post-natal period and to
define the role of the family - midwife in their diagnosis and
troubleshooting.
Material and method: a survey of the Diagnostic Test
method using standardized questionnaire survey, carried
out in a group of 100 women. Supplementary methods were
observation and interview conducted during to visits.
Results: Most of the examined persons showed the correct
status in the general parameters (temperature, pulse, blood
pressure) and obstetric. Most of the identified problems were
in the process of lactation. Nearly half (41%) has assessed
its lactation as insufficient, only one-third considered it
correct. Only 21% argues that there are no problems with
breast. Others declare their pain (39%) and wounds (35%).
Only 41% of women were breastfeeding on demand. In 61%
of neonates was observed abnormal breast sucking. Analysis
of physical activity for women has revealed that more than
half (61%) suffers from limitations in operating. A similar
percentage of breastfeeding mothers saying that they do
not have time for themself.
Conclusions: 1. the most common problems concern the
process of lactation. 2. in spite of the prenatal education
and support during postnatal visits, still too few women
breastfeeding in an environmentally sound manner. 3. the
family-midwife is the first person who is able, at the time
of postnatal visits, to resolve the problems of health of the mother and child, plan, implement and assess the ways of
their solutio

Role of classic tumor markers in genital malignancies in the females
Since the 1970s we are witnessing a continuing search for new markers that would assist in the treatment and follow- up of patients with malignant tumors. First reports on benefits of assessment of tumor markers authored by Goldman and Freedman date back to 1965. Discovery of the first carcinoembryonic antigen (CEA) initiate an era of search for substances, still insufficiently sensitive and specific as to be used to screen tumors, but increasingly helpful in the assessment of treatment effects. This paper discusses the role of tumor markers, increasingly often referred to as “classic” in the monitoring of tumors. We present an update on markers with the longest history in oncology practice, e.g. CEA and on the recently introduced marker TATI. Selection of markers was made based on their role in three basic processes taking place in tumor cells, i.e. proliferation, differentiation and apoptosis. We highlight novel and expanding fields of research – genomics and proteomics, which appear to be the future of oncology. They are extremely useful in the evaluation of molecular prognostic factors, enabling implementationof individually tailored targeted therapies in cancer patients. We discuss classic markers and the few known cancer- specific substances. To sum-up, we state that understanding of the role of more sensitive and more specific markers in oncology may contribute to a more personalized treatment and thus may improve the outcome in cancer patients.Od lat siedemdziesiątych XX wieku nieustannie poszukuje się nowych markerów, które mogą być pomocne w leczeniu i kontroli po jego ukończeniu u chorych na nowotwory złośliwe. Po raz pierwszy przydatność określania markerów nowotworowych opisali Goldman i Freedman w 1965 roku. Oznaczyli pierwszy marker CEA (karcynoembrionalny) i tak zapoczątkowali erę odkryć substancji o wciąż niezadowalającej czułości i swoistości, aby mogły być używane do skriningu chorób nowotworowych, lecz coraz bardziej przydatnych w ocenie efektu leczenia. W pracy opisano zastosowanie markerów nowotworowych coraz częściej określanych mianem klasycznych w procesie śledzenia chorób nowotworowych. Zaprezentowano najnowsze informacje o markerach najdłużej stosowanych w praktyce onkologicznej, na przykład CEA, jak również o nowo wykorzystywanym markerze TATI. Doboru markerów dokonano na podstawie ich udziału w trzech procesach toczących się w komórkach nowotworowych: proliferacji, różnicowaniu i obumieraniu komórek. Zwrócono uwagę na rozwijające się nowe dyscypliny nauki – genomikę i proteomikę, stanowiące przyszłość onkologii. Są one niezwykle pomocne w określeniu molekularnych czynników predykcyjnych umożliwiających stosowanie leczenia celowanego u chorych na nowotwory złośliwe. Opisano markery klasyczne, a także nieliczne markery molekularne występujące w różnych nowotworach. W podsumowaniu stwierdzono, że poznanie natury markerów o znaczącej czułości i swoistości w chorobach nowotworowych może wpłynąć na personalizację leczenia i tą drogą na poprawę wyleczeń chorych na nowotwory złośliwe
