The relation of globalization and localization concepts with the design industry has been analyzed. As a domestic product the tea and coffee making instruments had been explained. Moreover, the product samples designed to make tea and coffee were examined. Today the Turkish identity in design field has not been constituted yet. This thesis has been aimed to be considered as an example for Turkish design. As conclusion, the local product reflects the identity of the culture where it belongs to. The product should protect its own cultural values in order to take part in international circulation. The product should have the harmony with that culture to be recognized by the other cultures

    Dyadic death due to family custom in marriage

In this study, the effectiveness of the customs onthe marriages is discussed with a related case of dyadicdeath.A 27 years old male and a 19 years old female were founddead in a car out of a city center away from hometown. Inthe external examination, there was an entrance woundon the right temporal region and an exit wound on theleft temporal region of the man, and the contrary for thewoman. Both of the exit wounds were contact wound.Both of the deceased were originally from the east partof Turkey and stated to be single; and the woman wasreported to the security units as missing two days beforethe incident. As the motivation of the suicide, a note nearto the man was explaining that the deceased couple lovedeach other for one year yet the family of the women hadnot let them to get married.Being an obstacle for the marriage even for adults and acause of suicide because of family pressure is indicativeof strict application of the family customs, in Turkey.     Next-generation screening of a panel of genes associated with periodic fever syndromes in patients with Familial Mediterranean Fever and their clinical characteristics

    Familial Mediterranean Fever (FMF) is a hereditary fever syndrome that primarily affects Mediterranean populations. For the study, total number of 182 patients with FMF disease were enrolled and screening of a panel of genes, called “fever panel” which comprises 17 genes, was performed. The most common mutations in MEFV gene were homozygous M694V missense mutation (4.3%) and R202Q missense mutation (4.9%). The most common heterozygous mutations were R202Q (26.5%), M694V (25.9%) and E148Q (11.9%). Compound heterozygous and homozygous mutations were also detected. Also, different types of mutations were identified in NOD2, CARD14, NLRP12, NLRP3, NLRP7, IL1RN, LPIN2, TNFRSF1A, MVK and PSTPIP1 genes. Two novel missense variations in the MEFV gene, Gln34Pro and Ile247Val, which have not been previously reported in the databases, were identified. Also, Thr91Ile missense variation in the NOD2 gene, Gly461Cys missense variation in NLRP3 and Tyr732Stop nonsense variation in LPIN2 were firstly identified. The results of the current study suggest that in addition to the MEFV gene which has an important roles in FMF, molecular screening of other genes related to other autoinflammatory diseases might provide support in suspected cases and provide detailed information about the course of the disease. © 2020 Elsevier Inc

    Minimal life “structures”: A studio experience

    The necessity of the space to respond correctly to the user's wishes and needs is an unchanging phenomenon. Today, especially with the pandemic, differences in housing meaning are sought. With this current problem, minimal living spaces have been determined as the design studio subject of Ostim Technical University 2022-2023 Fall Semester. In this study, in the context of the suggestions developed by the students, the main motivation is to examine the meaning of the project designs and the residence; at the same time, it is aimed to discuss this studio experience. Within the scope of this study, the method, process, and end products of the studio are discussed. In this study, the changing meaning of the house and minimal living spaces were questioned by using the "document review" method, one of the qualitative research methods. The information obtained was analyzed by evaluating the results of the studio course. With this study, in which studio products with different user types and different functions are examined, it is concluded that the design of living spaces in small houses is very important for healthier solutions in comfort conditions, and it has been a very developing experience for the studio participants and executives

    Emphysematous pyelonephritis with left renal vein thrombosis-case report successfully treated by conservative methods

    Presently emphysematous pyelonephritis is a rare but life-threatening disease, mostly seen in patients with urinary tract obstr uc- tion and diabetes mellitus. It is characterized by the production of gas and necrosis in the renal parenchyma, collecting systems, and perinephritic tissue. Radiologically, the Huang and Tseng classification is used to categorize the severity of the disease. Our case was 49 years old female pre- senting with unconsciousness, fever and bilateral Grade 3 lower extremity eodema one week after left percutaneous nephrolithotomy procedure because of urinary tract stone. Abdominal computerized tomography scan showed renal parenchymal and perinephritic tissue necrosis with the production of gas and renal vein thrombosis which was in accordance with Huang and Tseng classification Grade 3A. We treated our patient with the appropriate antibiotic, enoxaparin sodium, strict glycemic control, and supportive treatment without any surgical intervention or nephrectomy

    Phlomis Pungens’in fitokimya ve in vitro farmakolojik etkilerinin değerlendirilmesi

    Objective: This study aimed to investigate the in vitro wound healing, anti-inflammatory, antimicrobial and antioxidant activity of Phlomis pungens Willd. extract derived from the aerial parts. Material and Method: The phytochemical analysis was performed using GC-MS in order to identify the volatile components of the bioactive Hex extract. The wound healing activity of P. pungens extract was evaluated based on in vitro antimicrobial, antioxidant, anti-inflammatory, and, scratch activity was studied. In addition, the in vitro cytotoxicity of the extract was also evaluated. Result and Discussion: P. pungens methanol extract depicted a 5-LOX inhibitory activity at 78.2µg/mL (IC50), while the antioxidant activity by DPPH radical provided an IC50=2.41mg/mL, and the ABTS radical showed IC50=3.32mg/mL, respectively. The extract showed dose-dependently anti-inflammatory activity while L-NAME and P. pungens methanol extract significantly decreased LPS stimulated PGE2 production. According to the scratch assay results, all treatments led to an increase in cell migration rate with a dose-dependent effect. Our findings suggested that P. pungens methanol extract may have a role in wound healing according to the scratch test, and it is thought that its antioxidant and anti-inflammatory activity also contributed. Further evaluations are ongoing to confirm the in vitro activity under in vivo conditions.Amaç: Bu çalışmada, Phlomis pungens Willd. topraküstü kısımlarından elde edilen ekstrelerin in vitro yara iyileşmesi, antiinflamatuar, antimikrobiyal ve antioksidan aktivitesinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Biyoaktif hekzan ekstresinin uçucu bileşenlerini belirlemek için fitokimyasal analiz GC-MS kullanılarak yapılmıştır. P. pungens ekstresinin yara iyileştirme aktivitesi, in vitro antimikrobiyal, antioksidan, antiinflamatuar etkinlikleri değerlendirilmiş ve ek olarak ekstrenin in vitro sitotoksisitesi de değerlendirilmiştir. Sonuç ve Tartışma: P. pungens metanol ekstresi, 78,2 µg/mL'de (IC50) 5-LOX inhibe edici aktivite gösterirken, DPPH yöntemi ile antioksidan aktivitesi IC50=2.41mg/mL ve ABTS IC50=3.32 mg/mL olarak bulunmuştur. Ekstre, doza bağlı olarak anti-inflamatuar aktivite gösterirken, L-NAME ve P. pungens metanol ekstresi, LPS ile uyarılan PGE2 üretimini önemli ölçüde azaltmıştır. Strach metodu sonuçlarına göre doza bağlı etki ile hücre göç hızında bir artış gözlemlenmiştir. Bulgularımız, starch testine göre P. pungens metanol ekstresinin yara iyileşmesinde rol oynayabileceğini ve antioksidan ve antiinflamatuar aktivitesinin de katkıda bulunduğu düşündürmüştür. İn vivo koşullar altında in vitro aktiviteyi doğrulamak için başka değerlendirmeler devam etmektedir

    Profile differences of medical doctors from three different hospitals in Turkey concerning burnout, job satisfaction, and depression

    Objective: This study aimed to compare job satisfaction, burnout, and depression scores of medical doctors from different regions in Turkey. Method: A cross-sectional study was performed during October 2013 in three cities from Turkey. Participants were asked to self-administer a demographic information questionnaire, the Minnesota Satisfaction Questionnaire (Internal work satisfactionIWS and External work satisfaction-EWS), the Beck Depression Inventory (BDI), the Maslach Burnout Inventory (MBI), Maslach Emotional Exhaustion Score (MEES), and Maslach Personal Accomplishment Score (MPAS). Results: Mean (±SD) Total work satisfaction score was 66.2±12.6. 118 participants had BDI scores of 10 or less while 40 participants had 11 or higher scores. Proportions for poor depersonalization, emotional exhaustion, and personal accomplishment were 18.6% (n=29), 5.8% (n=9), and 81.9% (n=127) respectively. There were significant differences concerning some variables between the three cities. Most of the doctors in Şırnak and Hakkari were younger, single, and less experienced compared to the more developed Çanakkale (p<0.05). Discussion: In a hierarchical regression analysis, independent of the other factors, MPAS and MEES were predictors of the total job satisfaction score. MPAS and MEES are the most significant independent variables affecting job satisfaction. Decreasing stressors and assuring a motivating and supportive environment may prove to be helpful in the enhancement of job satisfaction

    Intrapartum and Postpartum Reactivation of Hepatitis B in Pregnant Patients: a multicentre study

    Introduction: Changes in the immune system during pregnancy have been associated with reactivation of the hepatitis B virus in women chronic hepatitis B infection not receiving antiviral therapy. The aim of this study is to examine the development of intrapartum and postpartum hepatitis B reactivation in pregnant patients not being treated for chronic hepatitis B. Material and Methods: Pregnant women diagnosed with chronic hepatitis B and not receiving treatment between 2017 and 2022 in five centres in the east and southeast Turkey included in this study. In order to evaluate biochemical and viral reactivation from intrapartum and postpartum periods, patients with data for at least two periods were included in the study. Results: Evaluations were made on 171 pregnant women diagnosed with chronic hepatitis B. Reactivation occurred in 43 (25.2%) patients, in the postpartum period in 14 (32.35%) and in the intrapartum period in 29 (67.44%). Reactivation occurred most often in the 3rd trimester (n: 13, 30.2%). A significant increase was observed in the alanine aminotransferase levels of the patients with reactivation in the first trimester compared to 6 months prepartum and in the second trimester compared to the first trimester (p=0.038, p=0.039, respectively). The prepartum HBV DNA level (cut-off point =192 IU/ml) of patients with HBeAg negativity was found to have diagnostic power for reactivation of 0.684 (95% CI: 0.575-0.792, p=0.002) with 65.9% sensitivity and 68.6% specificity. Viral reactivation was observed in the first trimester in one patient and hepatitis B surface antibody was seen in the postpartum period. Conclusions: Asymptomatic viral reactivation occurred at the high rate of 25.1% in this series. To be able to identify reactivation as early as possible, pregnant patients should be followed up closely in the intrapartum and postpartum periods

    The effect of high-fructose feeding on hemodynamic behavior and infarct size of isolated rat hearts subjected to low-flow ischemia.

    Objective: The aim of this study was to investigate the potentially deleterious effect of in vivo high-fructose feeding of rats on ex vivo hemodynamic recovery and infarct size of isolated rat hearts subjected to low-flow ischemia and reperfusion. Methods: After feeding Sprague-Dawley male rats with a high-fructose (n=9), high-glucose (n=9) or a standard diet (n=9) for four weeks, the hearts were extirpated and perfused ex vivo with a Krebs-Henseleit solution for 15 min; thereafter the hearts were subjected to low flow (0.3 ml/min) ischemia during 30 min followed by 60 min reperfusion. Left ventricular developed pressure (LVDP), first derivative of pressure development during the contraction and relaxation phase and heart rate were recorded prior to ischemia and after reperfusion. Infarct area was measured at the end of the reperfusion period. Results: In isolated hearts, subjected to ex vivo low-flow ischemia followed by 60 min reperfusion, the relative myocardial infarct size did not differ between the three groups. Post-ischemic cardiac contractile recovery appeared to be complete in both the high-fructose and high-glucose group at 60 min reperfusion. In contrast, in the control group LVDP and left ventricular relaxation rate were still depressed at the end of 60 minutes of reperfusion. Conclusion: Feeding with a high-fructose diet of rats appears to have a positive effect on the recovery of left ventricular contractile function after low-flow ischemia, in comparison to a standard diet, without a difference in relative myocardial infarct size. Similar results were obtained in the high-glucose fed rats

    Effects of GLP-1 Receptor Polymorphisms on Adolescent Obesity

    Obesity is becoming a concerning disease in developing countries. Like other multifactorial diseases, genetics plays a substantial role in the development of this disease. We tried to investigate genetic variations (mutation/polymorphism) of GLP-1R gene in children diagnosed with obesity and to identify their possible connections with obesity and other conditions. Genomic DNA was extracted from 162 overweight/obese patients and 100 controls. Later, full exon sequencing and association studies were carried out. Three polymorphisms and one mutation were detected in the fourth and fifth exons of the GLP-1R gene. Some variations were detected in three cases from which 1/3 had non-alcoholic fatty liver disease (NAFLD) but none showed insulin resistance (IR). There were also statistically meaningful results for ‘Odds Ratio’ among different genotypes and allele frequencies in groups with NAFLD and/or IR. In addition, there was an increase in risk for NAFLD and a decrease in risk for IR. In the homozygous group, also the prospect of IR was double declined. Patients with the A allele of this polymorphism showed a drop in risk for IR as well. GLP-1R polymorphisms could influence obesity and diabetes and thus the functional analysis of the GLP-1R polymorphisms is benevolent
