Living with Uncertainty: Acting in the Best Interests of Women
A recent multi-country study on hormonal contraceptives (HC) and HIV acquisition and transmission among African HIV-serodiscordant couples reported a statistically significant doubling of risk for HIV acquisition among women as well as transmission from women to men for injectable contraceptives. Together with a prior cohort study on African women seeking health services, these data are the strongest yet to appear on the HC-HIV risk. This paper will briefly review the Heffron study strengths and relevant biological and epidemiologic evidence; address the futility of further trials; and propose instead an alternative framework for next steps. The weight of the evidence calls for a discontinuation of progestin-dominant methods. We propose here five types of productive activities: (1) scaling injectable hormones down and out of the contraceptive mix; (2) strengthening and introducing public health strategies with proven potential to reduce HIV spread; (3) providing maximal choice to reduce unplanned pregnancy, starting with quality sexuality education through to safe abortion access; (4) expanding provider training, end-user counseling and access to male and female barriers, with a special renewed focus on female condom; (5) initiating a serious research agenda to determine anti-STI/HIV potential of the contraceptive cervical cap. Trusting women to make informed choices is critical to achieve real progress in dual protection
New Day Films: Collective Aesthetics and the Collection
New Day Films was founded in 1972 - on the cusp of sweeping changes in documentary and feminist filmmaking practice - by four documentarists who were unable to gain distribution in the bottleneck of the New York nonfiction filmmaking scene (Figure 1). Despite these challenges, their films abot1t the everyday texture of women\u27s lives were in demand with audiences as the women\u27s movement grew across the country.\u27 Today, New Day is one of the most financially stable nontheatrical distribution collectives in North America, boasting more than 165 members and $1rn in yearly revenues.2 Films distributed by the collective have been screened, broadcast and awarded around the world, studied in media journals, discussed at organising events, showcased in museums, and collected by libraries. Continued Use of IT: An Emotional Choice
Information technology (IT) is ubiquitous in modern workplaces. Achieving the business benefits of an IT system is intimately tied up with the continued incorporation of the system into the work practices it is intended to support. What makes people incorporate IT into their practices and how do they do it? While much is known about different use behaviors, the social, cognitive and technical factors that influence use, less is known about non-use behaviors as well as the role of emotional factors in users’ choices on how to continue using a new technology post adoption. Through a longitudinal field study and a survey conducted in two universities, we examine how and why specific use patterns emerge and what the role of emotions is in this process. We find that based on emotional experiences around IT, people develop personal valuations towards an IT artifact, which are expressed in various (non-)use patterns
What determines adult cognitive skills?: Impacts of preschooling, schooling, and post-schooling experiences in Guatemala
"Most investigations into the importance and determinants of adult cognitive skills assume that (1) they are produced primarily by schooling, and (2) schooling is statistically predetermined or exogenous. Predictors of Comorbid Eating Disorders and Association with Other Obsessive-Compulsive Spectrum Disorders in Trichotillomania
Trichotillomania (TTM) and eating disorders (ED) share many phenomenological similarities, including ritualized compulsive behaviors. Given this, and that comorbid EDs may represent additional functional burden to hair pullers, we sought to identify factors that predict diagnosis of an ED in a TTM population. Subjects included 555 adult females (age range 18–65) with DSM-IV-TR TTM or chronic hair pullers recruited from multiple sites. 7.2% (N = 40) of our TTM subjects met criteria for an ED in their lifetime. In univariable regression analysis, obsessive-compulsive disorder (OCD), Yale-Brown Obsessive Compulsive Scale (Y-BOCS) worst-ever compulsion and total scores, certain obsessive-compulsive spectrum disorders, anxiety disorder, attention-deficit/hyperactivity disorder (ADHD), and substance disorder all met the pre-specified criteria for inclusion in the multivariable analysis. In the final multivariable model, diagnosis of OCD (OR: 5.68, 95% CI: 2.2–15.0) and diagnosis of an additional body-focused repetitive behavior disorder (BFRB) (OR: 2.69, 95% CI: 1.1–6.8) were both associated with increased risk of ED in TTM. Overall, our results provide further support of the relatedness between ED and TTM. This finding highlights the importance of assessing for comorbid OCD and additional BFRBs in those with TTM. Future research is needed to identify additional predictors of comorbid disorders and to better understand the complex relationships between BFRBs, OCD and EDs
Datification and the pursuit of meaningfulness in work
Proliferation of digital means of tracking worker activities has contributed to the rise of data-driven approaches to managing people, with employees often required to record their activities for accountability purposes. Increased requirement for such datification work occurs at a juncture where meaningfulness is one of the most sought-after work features. Datification work could both facilitate and hinder the pursuit of meaningfulness, yet literature provides little guidance into the nature of the connection and how it transpires. Our inductive study of academic professionals using an accountability system suggests that datification work characteristics link to meaningful work experiences in complex ways. We advance current theory on work meaningfulness by theorizing the role of a new work condition – datification – in meaningfulness experiences of professionals, outlining how system design and the institutional context become important elements influencing meaningful work experiences, and explaining how meaningfulness experiences are constructed through system appropriations
OUT-OF-OVEN CURING OF POLYMERIC COMPOSITES VIA RESISTIVE MICROHEATERS COMPRISED OF ALIGNED CARBON NANOTUBE NETWORKS
The broader adoption of composite materials in next-generation aerospace architectures is currently limited by the geometrical constraints and high energy costs of traditional manufacturing techniques of PMCs such as autoclave and vacuum-bag-only oven curing techniques. Here, an in situ curing technique for PMCs using a resistive heating film comprised of an aligned carbon nanotube (A-CNT) network is presented. A carbon fiber reinforced plastic (CFRP) prepreg system is effectively cured via a single-side CNT network heater incorporated on the outer surface of the laminate without using an autoclave. Evaluation of the curing efficacy shows that composites cured by A-CNT film heaters can achieve degrees of cure that are equivalent or better than composites cured by an autoclave. This manufacturing technique enables highly efficient curing of PMCs while adding multifunctionality to finished composites.United States. 976-14 Immediate Heart Rate Response to Orthostatic Stress During β-blocker Therapy for Vasodepressor Syncope
Although β-blockers are preferred agents for therapy of vasodepressor syncope (VDS), they are not uniformly effective and their mechanism of action is incompletely understood. Since we have previously shown a differential therapeutic response to β-blocker therapy between pts with isoproterenol-independent [iso(-)] and isoproterenol-dependent [iso(+)] VDS during tilt table testing we sought to determine whether this was due to a differential heart rate (HR) response to orthostasis during β-blockade. We therefore examined immediate HR and blood pressure responses to upright tilt before and after initiation of therapy with atenolol (12.5–50mg daily) in 62 pts with VDS and positive tilt tests. The protocol comprised upright tilt (60°) for up to 60min followed by repeat tilt for 15min during isoproterenol (iso) infusion. Supine HR, mean arterial pressure (MAP) and pulse pressure (PP) were determined as the mean of 3 consecutive 1-min samples during supine rest; orthostatic HR, MAP, and PP were the mean of the samples recorded in the first 3min after upright tilt (before infusion of iso). Response to atenolol required completion of tilt with and without infusion of iso. There were 15 iso(-) pts and 47 iso(+) pts. The groups did not differ significantly in blood pressure response (MAP, PP) to orthostasis. What Determines Adult Cognitive Skills? Impacts of Pre-Schooling, Schooling and Post-Schooling Experiences in Guatemala
Most investigations of the importance of and the determinants of adult cognitive skills assume that (a) they are produced primarily by schooling and (b) schooling is statistically predetermined. But these assumptions may lead to misleading inferences about impacts of schooling and of pre-schooling and post-schooling experiences on adult cognitive skills. This study uses an unusually rich longitudinal data set collected over 35 years in Guatemala to investigate production functions for adult (i) reading-comprehension and (ii) nonverbal cognitive skills as dependent on behaviorally-determined pre-schooling, schooling and post-schooling experiences. Major results are: (1) Schooling has significant and substantial impact on adult reading comprehension (but not on adult nonverbal cognitive skills)—but estimates of this impact are biased upwards substantially if there are no controls for behavioral determinants of schooling in the presence of persistent unobserved factors such as genetic endowments and/or if family background factors that appear to be correlated with genetic endowments are included among the first-stage instruments. (2) Both pre-schooling and post-schooling experiences have substantial significant impacts on one or both of the adult cognitive skill measures that tend to be underestimated if these pre- and post-schooling experiences are treated as statistically predetermined—in contrast to the upward bias for schooling, which suggests that the underlying physical and job-related components of genetic endowments are negatively correlated with those for cognitive skills. (3) The failure in most studies to incorporate pre- and post-schooling experiences in the analysis of adult cognitive skills or outcomes affected by adult cognitive skills is likely to lead to misleading over-emphasis on schooling relative to these pre-and post-schooling experiences. (4) Gender differences in the coefficients of the adult cognitive skills production functions are not significant, suggesting that most of the fairly substantial differences in adult cognitive skills favoring males on average originate from gender differences in schooling attainment and in experience in skilled jobs favoring males. These four sets of findings are of substantial interest in themselves. But they also have important implications for broader literatures, reinforcing the importance of early life investments in disadvantaged children in determining adult skills and options, pointing to limitations in the cross-country growth literature of using schooling of adults to represent human capital, supporting hypotheses about the importance of childhood nutrition and work complexity in explaining the “Flynn effect” of substantial increases in measured cognitive skills over time, and questioning the interpretation of studies that report productivity impacts of cognitive skills without controlling for the endogeneity of such skills.
