    Incorporating multiparametric MRI staging and the new histological Grade Group system improves risk-stratified detection of bone metastasis in prostate cancer

    BACKGROUND\textbf{BACKGROUND}: There remains uncertainty on the need for bone staging in men with intermediate-risk prostate cancer. Current guidelines do not use mpMRI-staging information and rely on historic pathology grading. METHODS\textbf{METHODS}: We investigated the ability of mpMRI and the new Grade Group system to better predict bone metastasis status in a retrospective cohort study of 438 men with prostate cancer undergoing baseline mpMRI and isotope bone scintigraphy (BS). RESULTS\textbf{RESULTS}: Including mpMRI-staging information significantly increased the specificity of bone metastasis detection from 3.0% to 24.2% (P<0.01) and sensitivity from 89.2% to 97.3%. The new Grade Group score demonstrated progressive increase in bone metastasis rates (P<0.001). A novel risk-stratification model combining Grade Groups, PSA and mpMRI staging shows promise in predicting bone metastasis and could potentially reduce BS usage by 22.4%-34.7%.     Perceived Stress, Thinking Style and Paranormal Belief

    Paranormal beliefs often become stronger in times of stress. Such beliefs have also been found to vary in accordance with thinking style, whereby stronger beliefs are often observed in experiential thinkers. Little research, however, has explored the interaction between perceived stress and thinking style. 82 males and females aged 18 to 62 years (mean = 29.96 ± 12.53 years) completed measures of perceived stress, thinking style (rational and experiential) and paranormal belief. The results revealed stronger beliefs in experiential thinkers, compared with those with a rational thinking style. Perceived stress alone, was not a prominent predictor of belief but the combination of stress and thinking style, specifically high perceived stress with a rational thinking style, significantly predicted greater global paranormal belief, belief in superstition, traditional religious belief, and belief in psi. High perceived stress appeared to facilitate belief in rational thinkers as conversely, belief was lowest in rational thinkers under conditions of low-perceived stress. These findings suggest that stress may lower the propensity for rational thinking and consequently, encourage belief in scientifically unsubstantiated phenomena. This interaction may have implications for coping during stressful situations

    On the algorithmic construction of classifying spaces and the isomorphism problem for biautomatic groups

    We show that the isomorphism problem is solvable in the class of central extensions of word-hyperbolic groups, and that the isomorphism problem for biautomatic groups reduces to that for biautomatic groups with finite centre. We describe an algorithm that, given an arbitrary finite presentation of an automatic group Γ\Gamma, will construct explicit finite models for the skeleta of K(Γ,1)K(\Gamma,1) and hence compute the integral homology and cohomology of Γ\Gamma.Comment: 21 pages, 4 figure

    Failing boys and moral panics: perspectives on the underachievement debate

    The paper re-examines the underachievement debate from the perspective of the ‘discourse of derision’ that surrounds much writing in this area. It considers the contradictions and inconsistencies which underpin much of the discourse – from a reinterpretation of examination scores, to the conflation of the concepts of ‘under’ and ‘low’ achievement and finally to the lack of consensus on a means of defining and measuring the term underachievement. In doing so, this paper suggests a more innovative approach for understanding, re-evaluating and perhaps rejecting the notion of underachievement

    This paper reports on a workshop hosted by the University of Minnesota, March 23-26, 1997. It addressed fundamental dynamical issues associated with the evolution of shell supernova remnants and the relationships between supernova remnants and their environments. The workshop considered, in addition to classical shell SNRs, dynamical issues involving X-ray filled composite remnants and pulsar driven shells, such as that in the Crab Nebula. Approximately 75 participants with wide ranging interests attended the workshop. An even larger community helped through extensive on-line debates prior to the meeting. Each of the several sessions, organized mostly around chronological labels, also addressed some underlying, general physical themes: How are SNR dynamics and structures modified by the character of the CSM and the ISM and vice versa? How are magnetic fields generated in SNRs and how do magnetic fields influence SNRs? Where and how are cosmic-rays (electrons and ions) produced in SNRs and how does their presence influence or reveal SNR dynamics? How does SNR blast energy partition into various components over time and what controls conversion between components? In lieu of a proceedings volume, we present here a synopsis of the workshop in the form of brief summaries of the workshop sessions. The sharpest impressions from the workshop were the crucial and under-appreciated roles that environments have on SNR appearance and dynamics and the critical need for broad-based studies to understand these beautiful, but enigmatic objects. \\Comment: 54 pages text, no figures, Latex (aasms4.sty). submitted to the PAS

    Changing presentation of prostate cancer in a UK population--10 year trends in prostate cancer risk profiles in the East of England.

    BACKGROUND: Prostate cancer incidence is rising in the United Kingdom but there is little data on whether the disease profile is changing. To address this, we interrogated a regional cancer registry for temporal changes in presenting disease characteristics. METHODS: Prostate cancers diagnosed from 2000 to 2010 in the Anglian Cancer Network (n=21,044) were analysed. Risk groups (localised disease) were assigned based on NICE criteria. Age standardised incidence rates (IRs) were compared between 2000-2005 and 2006-2010 and plotted for yearly trends. RESULTS: Over the decade, overall IR increased significantly (P<0.00001), whereas metastasis rates fell (P<0.0007). For localised disease, IR across all risk groups also increased but at different rates (P<0.00001). The most striking change was a three-fold increase in intermediate-risk cancers. Increased IR was evident across all PSA and stage ranges but with no upward PSA or stage shift. In contrast, IR of histological diagnosis of low-grade cancers fell over the decade, whereas intermediate and high-grade diagnosis increased significantly (P<0.00001). CONCLUSION: This study suggests evidence of a significant upward migration in intermediate and high-grade histological diagnosis over the decade. This is most likely to be due to a change in histological reporting of diagnostic prostate biopsies. On the basis of this data, increasing proportions of newly diagnosed cancers will be considered eligible for radical treatment, which will have an impact on health resource planning and provision

    Impact Factor: outdated artefact or stepping-stone to journal certification?

    A review of Garfield's journal impact factor and its specific implementation as the Thomson Reuters Impact Factor reveals several weaknesses in this commonly-used indicator of journal standing. Key limitations include the mismatch between citing and cited documents, the deceptive display of three decimals that belies the real precision, and the absence of confidence intervals. These are minor issues that are easily amended and should be corrected, but more substantive improvements are needed. There are indications that the scientific community seeks and needs better certification of journal procedures to improve the quality of published science. Comprehensive certification of editorial and review procedures could help ensure adequate procedures to detect duplicate and fraudulent submissions.Comment: 25 pages, 12 figures, 6 table

    Preoperative idoxuridine and radiation for large soft tissue sarcomas: Clinical results with five-year follow-up

    Background: Local control remains an important issue in the management of large soft tissue sarcomas. Radiation is the main adjuvant to surgery for local therapy of sarcomas, but it requires relatively high doses, hitherto considered prohibitive in areas such as the retroperitoneum.
