    Prohexadiona cálcio no crescimento de ramos de pereiras 'Starkrimson'

    The objective of this work was to evaluate the fruiting and vegetative growth of 'Starkrimson' pear in response to the application of prohexadione calcium (P-Ca). P-Ca (250 mg L-1) was sprayed to scaffold limbs and to whole trees in commercial orchards. A randomized complete block design was used to minimize declivity effects. Vegetative growth (length and number of shoots, increment in limb and trunk cross-sectional area, number of nodes, and internode length), production (fruit number, yield, yield efficiency, and fruit size), and return bloom were evaluated from 2010 to 2013. P-Ca efficiently controlled shoot growth in all growing seasons, when applied early in the season, due to the reduction in internode length. Production was not negatively affected by the application of P-Ca in all years and at both sites, and fruit size and return bloom were not reduced. Fruit quality attributes were also little affected by P-Ca, except for total soluble solids content, which was lower than that of the control. This shows that P-Ca is efficient to manage tree vigor in high-density 'Starkrimson' pear orchards.O objetivo deste trabalho foi avaliar a produção e o crescimento vegetativo de peras 'Starkrimson' em resposta à aplicação de prohexadiona cálcio (P-Ca). Aplicou-se P-Ca (250 mg L-1) em ramificações laterais e em plantas inteiras, em pomares comerciais. Utilizou-se o delineamento experimental de blocos ao acaso, para minimizar os efeitos da declividade do terreno. O crescimento vegetativo (comprimento e número de ramos, incremento da área da secção transversal dos ramos laterais e do tronco, número de nós e comprimento de entrenós), a produção (número de frutos, produtividade, eficiência produtiva e tamanho de fruto) e o retorno da floração foram avaliados de 2010 a 2013. P-Ca foi eficiente no controle do crescimento de ramos em todas as safras, quando aplicada no início do ciclo de crescimento, devido à redução no comprimento dos entrenós. A produção não foi afetada negativamente pela aplicação de P-Ca em todos os anos e em ambos os locais, e o tamanho de fruto e o retorno da floração não foram reduzidos. Os atributos de qualidade dos frutos também foram pouco afetados por P-Ca, exceto o teor de sólidos solúveis totais, que foi menor que o do controle. Assim, P-Ca é eficiente no manejo do vigor em pomares de alta densidade de peras 'Starkrimson'

    Perceptions of Precision Agriculture Technologies in the U.S. Fresh Apple Industry

    Advances in precision agriculture technologies provide opportunities to improve the efficiency of agricultural production systems, especially for high-value specialty crops such as fresh apples (Malus domestica). We distributed an online survey to apple growers in Washington, New York, and Michigan to elicit stakeholder perceptions of precision agriculture technologies. Findings from this study demonstrated that growers are willing to adopt precision agriculture technologies when they receive results from applied research projects and are engaged with active extension programs. The availability of customized services and purchasing and rental options may minimize the effects of the economies of size that create barriers to adopting increasing access to technologies. Finally, respondents deemed collaborative efforts between industry and academic institutions crucial for adapting the innovation to better address the needs of growers

    Epidemiology of Fracture Nonunion in 18 Human Bones

    Importance Failure of bone fracture healing occurs in 5% to 10% of all patients. Nonunion risk is associated with the severity of injury and with the surgical treatment technique, yet progression to nonunion is not fully explained by these risk factors. Objective To test a hypothesis that fracture characteristics and patient-related risk factors assessable by the clinician at patient presentation can indicate the probability of fracture nonunion. Design, Setting, and Participants An inception cohort study in a large payer database of patients with fracture in the United States was conducted using patient-level health claims for medical and drug expenses compiled for approximately 90.1 million patients in calendar year 2011.The final database collated demographic descriptors, treatment procedures as per Current Procedural Terminology codes; comorbidities as per International Classification of Diseases, Ninth Revision codes; and drug prescriptions as per National Drug Code Directory codes. Logistic regression was used to calculate odds ratios (ORs) for variables associated with nonunion. Data analysis was performed from January 1, 2011, to December 31, 2012, Exposures Continuous enrollment in the database was required for 12 months after fracture to allow sufficient time to capture a nonunion diagnosis. Results The final analysis of 309 330 fractures in 18 bones included 178 952 women (57.9%); mean (SD) age was 44.48 (13.68) years. The nonunion rate was 4.9%. Elevated nonunion risk was associated with severe fracture (eg, open fracture, multiple fractures), high body mass index, smoking, and alcoholism. Women experienced more fractures, but men were more prone to nonunion. The nonunion rate also varied with fracture location: scaphoid, tibia plus fibula, and femur were most likely to be nonunion. The ORs for nonunion fractures were significantly increased for risk factors, including number of fractures (OR, 2.65; 95% CI, 2.34-2.99), use of nonsteroidal anti-inflammatory drugs plus opioids (OR, 1.84; 95% CI, 1.73-1.95), operative treatment (OR, 1.78; 95% CI, 1.69-1.86), open fracture (OR, 1.66; 95% CI, 1.55-1.77), anticoagulant use (OR, 1.58; 95% CI, 1.51-1.66), osteoarthritis with rheumatoid arthritis (OR, 1.58; 95% CI, 1.38-1.82), anticonvulsant use with benzodiazepines (OR, 1.49; 95% CI, 1.36-1.62), opioid use (OR, 1.43; 95% CI, 1.34-1.52), diabetes (OR, 1.40; 95% CI, 1.21-1.61), high-energy injury (OR, 1.38; 95% CI, 1.27-1.49), anticonvulsant use (OR, 1.37; 95% CI, 1.31-1.43), osteoporosis (OR, 1.24; 95% CI, 1.14-1.34), male gender (OR, 1.21; 95% CI, 1.16-1.25), insulin use (OR, 1.21; 95% CI, 1.10-1.31), smoking (OR, 1.20; 95% CI, 1.14-1.26), benzodiazepine use (OR, 1.20; 95% CI, 1.10-1.31), obesity (OR, 1.19; 95% CI, 1.12-1.25), antibiotic use (OR, 1.17; 95% CI, 1.13-1.21), osteoporosis medication use (OR, 1.17; 95% CI, 1.08-1.26), vitamin D deficiency (OR, 1.14; 95% CI, 1.05-1.22), diuretic use (OR, 1.13; 95% CI, 1.07-1.18), and renal insufficiency (OR, 1.11; 95% CI, 1.04-1.17) (multivariate P < .001 for all). Conclusions and Relevance The probability of fracture nonunion can be based on patient-specific risk factors at presentation. Risk of nonunion is a function of fracture severity, fracture location, disease comorbidity, and medication use

    Clinical benefit of a precision medicine based approach for guiding treatment of refractory cancers

    Patients and methods: Patients with metastatic solid tumors who had progressed on at least one line of standard of care therapy were referred to the Indiana University Health Precision Genomics Program. Tumor samples were submitted for DNA & RNA next-generation sequencing, fluorescence in situ hybridization, and immunohistochemistry for actionable targets. A multi-disciplinary tumor board reviewed all results. For each patient, the ratio of progression-free survival (PFS) of the genomically guided line of therapy divided by the PFS of their prior line was calculated. Patients whose PFS ratio was ≥ 1.3 were deemed to have a meaningful improvement in PFS. Results: From April 2014-October 2015, 168 patients were evaluated and 101 patients achieved adequate clinical follow-up for analysis. 19 of 44 (43.2%) patients treated with genomically guided therapy attained a PFS ratio ≥ 1.3 vs. 3 of 57 (5.3%) treated with non-genomically guided therapy (p < 0.0001). Similarly, overall PFS ratios (irrespective of cutoff) were higher for patients with genomically guided therapy vs non-genomically guided therapy (p = 0.05). Further, patients treated with genomically guided therapy had a superior median PFS compared to those treated with non-genomically guided therapy (86 days vs. 49 days, p = 0.005, H.R. = 0.55, 95% C.I.:0.37-0.84). Conclusion: Patients with refractory metastatic cancer who receive genomically guided therapy have improved PFS ratios and longer median PFS compared to patients who do not receive genomically guided therapy

    Effects of Anacetrapib in Patients with Atherosclerotic Vascular Disease

    BACKGROUND: Patients with atherosclerotic vascular disease remain at high risk for cardiovascular events despite effective statin-based treatment of low-density lipoprotein (LDL) cholesterol levels. The inhibition of cholesteryl ester transfer protein (CETP) by anacetrapib reduces LDL cholesterol levels and increases high-density lipoprotein (HDL) cholesterol levels. However, trials of other CETP inhibitors have shown neutral or adverse effects on cardiovascular outcomes. METHODS: We conducted a randomized, double-blind, placebo-controlled trial involving 30,449 adults with atherosclerotic vascular disease who were receiving intensive atorvastatin therapy and who had a mean LDL cholesterol level of 61 mg per deciliter (1.58 mmol per liter), a mean non-HDL cholesterol level of 92 mg per deciliter (2.38 mmol per liter), and a mean HDL cholesterol level of 40 mg per deciliter (1.03 mmol per liter). The patients were assigned to receive either 100 mg of anacetrapib once daily (15,225 patients) or matching placebo (15,224 patients). The primary outcome was the first major coronary event, a composite of coronary death, myocardial infarction, or coronary revascularization. RESULTS: During the median follow-up period of 4.1 years, the primary outcome occurred in significantly fewer patients in the anacetrapib group than in the placebo group (1640 of 15,225 patients [10.8%] vs. 1803 of 15,224 patients [11.8%]; rate ratio, 0.91; 95% confidence interval, 0.85 to 0.97; P=0.004). The relative difference in risk was similar across multiple prespecified subgroups. At the trial midpoint, the mean level of HDL cholesterol was higher by 43 mg per deciliter (1.12 mmol per liter) in the anacetrapib group than in the placebo group (a relative difference of 104%), and the mean level of non-HDL cholesterol was lower by 17 mg per deciliter (0.44 mmol per liter), a relative difference of -18%. There were no significant between-group differences in the risk of death, cancer, or other serious adverse events. CONCLUSIONS: Among patients with atherosclerotic vascular disease who were receiving intensive statin therapy, the use of anacetrapib resulted in a lower incidence of major coronary events than the use of placebo.     The profitability of modern apple orchard plantings depends largely on how rapidly the costs of planting are returned. Tree establishment and growth in the formative years are often can be limited by transplant shock associated with bare-root trees. In this experiment, we examined the effect of two planting systems, air-pruning containers, and field-grown liners, on above- and below-ground growth and development during the first year in the nursery. M9 &lsquo;Nic 29&rsquo; bench grafts for three apple cultivars of different vigor classes, &lsquo;Fuji&rsquo; (high vigor), &lsquo;Gala&rsquo; (moderate vigor), and &lsquo;Honeycrisp&rsquo; (low vigor). We hypothesized that air root-pruning containers would alter rooting distribution compared to field-grown liners by increasing fine root production, and this, in turn, would result in improved resource allocation and greater biomass partitioned to above-ground organs. Bench grafts were divided evenly between a field-grown liner bed (for bare root production) and an air-pruning container system. Air-pruning containers produced trees with significantly more root tips and greater total root length per tree than field-grown liners. Importantly, air-pruning containers resulted in a marked and significant increase in roots &lt; 0.4 mm in diameter; however, field-grown liners produced trees had significantly more roots with diameters &gt; 1.5 mm and, thus, significantly greater root dry matter content (DMC). Above-ground parameters (scion length, DMC of wood and leaves, and individual leaf area and DMC) were significantly increased for plants in air-pruning containers. Generally, the growth benefits from air-pruning containers were less pronounced in &lsquo;Gala&rsquo; compared to &lsquo;Honeycrisp&rsquo; or &lsquo;Fuji&rsquo;.
