    Изучение терапевтических эффектов излучения с длиной волны 670 нм при различных типах диабетического макулярного отека

    The purpose of this study was to investigate the therapeutic effects of 670 nm irradiation in patients with diabetic macular edema. In several studies, positive effects of red/near-infrared irradiation showed in a range of ocular diseases such as macular degeneration, macular edema, and retinitis pigmentosa. This study was conducted on forty five eyes of 26 diabetic patients with macular edema between the ages of 51 and 80.Measurement of visual acuity and slit lamp examination, funduscopy, and optical coherence tomography were performed in all subjects. None of the patients had proliferative retinopathy. We used a portable LED device (Warp 10, Quantum Devices) for treatment. Patients held this device at a distance of 3 cm from their eyes for 240 seconds for three months. Full ophthalmic examinations were repeated 1, 2, and 3 months after treatment.After 3 months, the mean visual acuity improved from 0.44 ± 0.38 log MAR to 0.27 ± 0.24 log MAR and vision increased by 1.52 ± 1.16 lines post treatment (р<0.001). The mean central macula thickness decreased from 381.49 ± 144.40 μm to 359.72 ± 128.84 μm (р=0.050). In patients with mild and moderate nonproliferative diabetic retinopathy, the mean central retinal thickness decreased 52.06 ± 67.78 μm and 39.27 ± 44.69 μm, respectively, but patients with severe type showed an increase of 34.93 ± 65.65 μm in the mean central retinal thickness (р<0.001). Also, the severity of macular edema had no effect on final outcomes (р>0.05). Photobiomodulation can positively affect diabetic macular edema, especially in patients with mild to moderate diabetic retinopathy.Целью данной исследовательской работы являлось изучение терапевтических эффектов излучения с длиной волны 670 нм у пациентов с диабетическим макулярным отеком. Ряд предыдущих исследований свидетельствует о положительном эффекте красного инфракрасного излучения при некоторых заболеваниях глаз, таких как макулодистрофия (дегенерация желтого пятна), макулярный отек и пигментный ретинит. Наше исследование было проведено на 45 глазах у 26 больных сахарным диабетом в возрасте от 51 до 80 лет с макулярным отеком. Всем пациентам были проведены определение остроты зрения, осмотр глаз щелевой лампой, фундоскопия и оптическая когерентная томография. Ни у одного из пациентов не было пролиферативной ретинопатии. Для лечения нами был применен портативный светодиодный прибор (Warp 10, Quantum devices). Пациенты держали светодиод на расстоянии 3 см от глаза в течение 240 сек в течение 3 мес. Все офтальмологические исследования были повторены через 1, 2 и 3 мес после проведения лечебной процедуры. Через 3 мес средняя острота зрения улучшилась с показателем логарифма минимального угла разрешения 0,44 ± 0,38 до 0,27 ± 0,24, что показало увеличение показателя остроты зрения на 1,52 ± 1,16 после лечения (р<0,001). Средняя центральная толщина сетчатки в области макулы уменьшилась с 381,49 ± 144,40 мкм до 359,72 ± 128,84 мкм (р=0,050). У пациентов с легкой и умеренной непролиферативной диабетической ретинопатией средняя толщина сетчатки уменьшилась до 52,06 ± 67,78 и 52,06 ± 67,78 мкм, соответственно, а у пациентов с тяжелой ретинопатией наблюдалось увеличение на 34,93 ± 65,65 мкм (р<0,001). Помимо того, степень макулярного отека не повлияла на окончательный результат лечения (р>0,05). Фотобиомодуляция была эффективной при диабетическом макулярном отеке, в частности, у пациентов с легкой и умеренной диабетической ретинопатией

    Improving the Adoptability of a Mobile Service: A Case Study

    Mobile technologies are penetrating into everyday lives of people and are providing miscellaneous services on the mobile device for users. Most of researches in this area have been done after the introduction of the service to the market. In this tough market, service providers need to know exactly which elements of their services or technologies can be improved before final stage of development and surely before launching the application or service. This article studies intentions to use a service, through a mobile application, for possible improvements in adoptability of the service. The Nysveen et al’s integrated model (2005) has been utilized for conducting the research and data was collected over a survey study. Such insights allow mobile service providers and mobile marketers to create more customized services. This paper concludes with both theoretical and practical implications and limitations of the study results

    Identification of qanats in Rafsanjan, Sirjan and Ravar cities for fish culture

    Study and recognition of the Quanat water of Rafsanjan, Sirjan and Ravar cities has been beginning since the November of 2004. Among 228 Quanat chains were studied in these cities, 165 quanat chains had a discharge more than 10 liter per second (L/Se) which were sampled. In this research oxygen, pH, discharge, water temperature and air temperature were measured in two times and in the local place. Height from the sea level, longitude and latitude were measured (by GPS Machine) in one time and in the local place and other physical and chemical factors including No_2, No_3, Co_2, total hardness, NH_3 and So_4 were measured one time in the laboratory. In this manner, the condition of connection ways of considered Quanat with city was studied. With examination on the Quanat water in above said cities, all of Quant was within desired extents for aquaculture activities. Moreover in some of them, water temperature was higher than 20 degree centigrade that with some changes and in specific months can be used in aquaculture

    Coronary calcium scoring: are the results comparable to computed tomography coronary angiography for screening coronary artery disease in a South Asian population?

    Background: The need of having feasible screening tools like Coronary Calcium Scoring (CCS) and CT Coronary Artery (CTCA) for Coronary Artery Disease (CAD) has become paramount. We aimed to evaluate the accuracy of CCS in determining the degree of stenosis of coronary vessels as compared to that determined by CTCA in a South Asian population. Methods: A retrospective study was conducted at The Aga Khan University Hospital. A total of 539 patient records were reviewed who had undergone CCS and CTCA between 2008 and 2010. Patient records were reviewed by comparing their CCS and CTCA results. Results: About 268 out of 301 (89%) patients with a CCS of 0-9 were found to be free of stenosis on CTCA. On a CCS of 10-99, 110 out of 121 (91%) patients were either free of stenosis or had mild stenosis. About 66 out of 79 (84%) patients had moderate or severe stenosis with a calcium score of 100-400 while none of the patients were free of stenosis. Around 28 out of 38 (74%) patients with a CCS of more than 400 had severe stenosis. However, only 04 patients (11%) were found to have mild stenosis. Spearman\u27s rho revealed a correlation coefficient of 0.791 with a p-value of \u3c0.001. Conclusion: Our study reaffirms that in South Asian population, low CCS (\u3c100) is associated with no or minimal stenosis while high CCS warrants further investigation; hence, making it a reliable tool for screening patients with CAD

    Impaired aortic distensibility measured by computed tomography is associated with the severity of coronary artery disease.

    Impaired aortic distensibility index (ADI) is associated with cardiovascular risk factors. This study evaluates the relation of ADI measured by computed tomographic angiography (CTA) with the severity of coronary atherosclerosis in subjects with suspected coronary artery disease (CAD). Two hundred and twenty-nine subjects,age 63 ± 9 years, 42% female, underwent coronary artery calcium (CAC) scanning and CTA, and their ADI and Framingham risk score (FRS) were measured. End-systolic and end-diastolic (ED) cross-sectional-area(CSA) of ascending-aorta (AAo) was measured 15-mm above the left-main coronary ostium. ADI was defined as: [(Δlumen-CSA)/(lumen-CSA in ED × systemic-pulse-pressure) × 10(3)]. ADI measured by 2D-trans-thoracic echocardiography (TTE) was compared with CTA-measured ADI in 26 subjects without CAC. CAC was defined as 0, 1-100, 101-400 and 400+. CAD was defined as luminal stenosis 0, 1-49% and 50%+. There was an excellent correlation between CTA- and TTE-measured ADI (r(2)=0.94, P=0.0001). ADI decreased from CAC 0 to CAC 400+; similarly from FRS 1-9% to FRS 20% + (P<0.05). After adjustment for risk factors, the relative risk for each standard deviation decrease in ADI was 1.66 for CAC 1-100, 2.26 for CAC 101-400 and 2.32 for CAC 400+ as compared to CAC 0; similarly, 2.36 for non-obstructive CAD and 2.67 for obstructive CAD as compared to normal coronaries. The area under the ROC-curve to predict significant CAD was 0.68 for FRS, 0.75 for ADI, 0.81 for CAC and 0.86 for the combination (P<0.05). Impaired aortic distensibility strongly correlates with the severity of coronary atherosclerosis. Addition of ADI to CAC and traditional risk factors provides incremental value to predict at-risk individuals

    Identification of Kerman Fishes

During one year identification of fish in qanat basins of Markazi desert and Sirjan desert was carried out. In this study Electroshoker and salik net with o.5 and 1 cm network and padel net with 1cm were used to catch the fishes from 73 series of qanat.
