Neuroendocrine regulation and ovarian reserve in HIV-infected women with reproductive disorders
Background: Among the subjects of the Russian Federation, Irkutsk region is an area with an unfavorable epidemiological situation for HIV-infection. Women living with HIV in Russia and visiting AIDS-centers represent a socially and economically active part of the population are at a young reproductive age and plan to have children. Aims: to study the state of the neuroendocrine system and the ovarian reserve in HIV-infected women with reproductive disorders. Materials and methods. We examined in 17 HIV-infected women with reproductive disorders and 33 women with HIV-infection and reproductive health at the age of 18-45 years (mean age 31,1 Β± 4,4 years). The determination of hormone levels was carried out by a competitive solid-phase enzyme-linked immunosorbent assay. Results. The obtained results testify to neuroendocrine disorders in the form of a decrease in the level of total testosterone, 17-OH progesterone, dehydroepiandrosterone sulfate, as a manifestation of androgen deficiency of the body, as a result of ovarian dysfunction. Decrease in the level of Anti-Mullerian Hormone and increase the content of follicle-stimulating hormone, as a manifestation of the beginning of early menopause or a factor leading to infertility. Conclusions. The results of this study show that a woman's body on the background of HIV-infection reveals disorders of the neuroendocrine system dysfunction and decreased ovarian ovarian reserve, and as a result, reproductive disorders

Background. Highly active antiretroviral therapy (ART) has increased the life expectancy of people living with the humanΒ immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS). Advances in scientific knowledge and, in particular, in the management of an HIV-infected pregnant woman have also reduced the risk of mother-to-child transmission ofΒ HIV. Aims. Consider the current problems of gynecological and reproductive health disorders and the pathophysiologicalΒ aspects of these disorders in HIV-infected women. Methods. Review of Russian and international scientific publications relatedΒ to gynecology, reproductive health and HIV infection from indexed online journals and relevant websites using Pubmed andΒ Β Google search from 1990 to March 2019. Results. In recent years, the proportion of HIV-infected women has increased andΒ the role of sexual transmission of infection has increased, as well as the active involvement of women of reproductive age in theΒ HIV epidemic. In HIV-infected pregnant women who did not receive HAART, the frequency of obstetric complications, laborΒ and adverse outcomes in children is increased. HIV infection can reduce the fertility of women at any stage of the disease,Β marked violations of the menstrual cycle and ovulation, reduced ovarian reserve and the quality of oocytes. In HIV-infectedΒ patients, weight loss and dystrophy, impaired thyroid function, adrenal glands and hypogonadism are recorded. Violations ofΒ the menstrual, ovulatory function can occur for a variety of reasons that are not directly related to their illness, but to multipleΒ stressors. HIV-positive women suffer from increased frequency and severity of pelvic infections and precancerous and malignantΒ lesions of the cervix. Conclusions. Gynecological, reproductive needs, as well as the specifics of monitoring an HIV-positiveΒ woman are not sufficiently assessed, are not optimal and are largely ignored, which can negatively affect their quality of life andΒ efforts to combat the HIV epidemic.
