    The social cognition of medical knowledge, with special reference to childhood epilepsy

    This paper arose out of an engagement in medical communication courses at a Gulf university. It deploys a theoretical framework derived from a (critical) sociocognitive approach to discourse analysis in order to investigate three aspects of medical discourse relating to childhood epilepsy: the cognitive processes that are entailed in relating different types of medical knowledge to their communicative context; the types of medical knowledge that are constituted in the three different text types analysed; and the relationship between these different types of medical knowledge and the discursive features of each text type. The paper argues that there is a cognitive dimension to the human experience of understanding and talking about one specialized from of medical knowledge. It recommends that texts be studied in medical communication courses not just in terms of their discrete formal features but also critically, in terms of the knowledge which they produce, transmit and reproduce

    Anomalous Fisher-like zeros for the canonical partition function of noninteracting fermions

    Noninteracting fermions, placed in a system with a continuous density of states, may have zeros in the NN-fermion canonical partition function on the positive real β\beta axis (or very close to it), even for a small number of particles. This results in a singular free energy, and instability in other thermal properties of the system. In the context of trapped fermions in a harmonic oscillator, these zeros are shown to be unphysical.     Hyperacusis in tinnitus patients relates to enlarged subcortical and cortical responses to sound except at the tinnitus frequency

    Hyperacusis, a hypersensitivity to sounds of mild to moderate intensity, has been related to increased neural gain along the auditory pathway. To date, there is still uncertainty on the neural correlates of hyperacusis. Since hyperacusis often co-occurs with hearing loss and tinnitus, the effects of the three conditions on cortical and subcortical structures are often hard to separate. In this fMRI study, two groups of hearing loss and tinnitus participants, with and without hyperacusis, were compared to specifically investigate the effect of the latter in a group that often reports hyperacusis. In 35 participants with hearing loss and tinnitus, with and without hyperacusis as indicated by a cut-off score of 22 on the Hyperacusis Questionnaire (HQ), subcortical and cortical responses to sound stimulation were investigated. In addition, the frequency tuning of cortical voxels was investigated in the primary auditory cortex. In cortical and subcortical auditory structures, sound-evoked activity was higher in the group with hyperacusis. This effect was not restricted to frequencies affected by hearing loss but extended to intact frequencies. The higher subcortical and cortical activity in response to sound thus appears to be a marker of hyperacusis. In contrast, the response to the tinnitus frequency was reduced in the group with hyperacusis. This increase in subcortical and cortical activity in hyperacusis can be related to an increase in neural gain along the auditory pathway, and the reduced response to the tinnitus frequency to differences in attentional resources allocated to the tinnitus sound.     Position-dependent shear-induced austenite-martensite transformation in double-notched TRIP and dual-phase steel samples

    While earlier studies on transformation-induced-plasticity (TRIP) steels focused on the determination of the austenite-to-martensite decomposition in uniform deformation or thermal fields, the current research focuses on the determination of the local retained austenite-to-martensite transformation behaviour in an inhomogeneous yet carefully controlled shear-loaded region of double-notched TRIP and dual-phase (DP) steel samples. A detailed powder analysis has been performed to simultaneously monitor the evolution of the phase fraction and the changes in average carbon concentration of metastable austenite together with the local strain components in the constituent phases as a function of the macroscopic stress and location with respect to the shear band. The metastable retained austenite shows a mechanically induced martensitic transformation in the localized shear zone, which is accompanied by an increase in average carbon concentration of the remaining austenite due to a preferred transformation of the austenite grains with the lowest carbon concentration. At the later deformation stages the geometry of the shear test samples results in the development of an additional tensile component. The experimental strain field within the probed sample area is in good agreement with finite element calculations. The strain development observed in the low-alloyed TRIP steel with metastable austenite is compared with that of steels with the same chemical composition containing either no austenite (a DP grade) or stable retained austenite (a TRIP grade produced at a long bainitic holding time).     Atomic quasi-Bragg diffraction in a magnetic field

    We report on a new technique to split an atomic beam coherently with an easily adjustable splitting angle. In our experiment metastable helium atoms in the |{1s2s}^3S_1 M=1> state diffract from a polarization gradient light field formed by counterpropagating \sigma^+ and \sigma^- polarized laser beams in the presence of a homogeneous magnetic field. In the near-adiabatic regime, energy conservation allows the resonant exchange between magnetic energy and kinetic energy. As a consequence, symmetric diffraction of |M=0> or |M=-1> atoms in a single order is achieved, where the order can be chosen freely by tuning the magnetic field. We present experimental results up to 6th order diffraction (24 \hbar k momentum splitting, i.e., 2.21 m/s in transverse velocity) and present a simple theoretical model that stresses the similarity with conventional Bragg scattering. The resulting device constitutes a flexible, adjustable, large-angle, three-way coherent atomic beam splitter with many potential applications in atom optics and atom interferometry.Comment: 4 pages, 5 figure

    The discovery of a paving slab with a number of prints suggestive of footprints of tetrapods led to a reinvestigation of a trace fossil, known from three localities, of which only two photographs had been published, each with only four prints. The slab was traced to a previously unrecorded site, De Puts, near Calvinia, located in the Tierberg Formation of the Ecca Group of the Karoo Supergroup.     “I’m not here to push you:” Raising adolescents’ treatment engagement via autonomy support

    More than 45% of adolescents with aggressive behavior problems drop out of treatment prematurely. Building on insights from self-determination theory, we examined in three studies whether clinicians can raise adolescents' treatment engagement by supporting their autonomy. In an interview study (Study 1), clinicians (N = 16; 43.8% female; ages 30–57) spontaneously described 12 times more autonomy-supportive than controlling strategies to engage adolescents. In a preregistered experiment (Study 2), clinicians (N = 68; 88.2% female; ages 23–65) were confronted with videos of adolescents displaying resistance. We manipulated the DSM diagnosis of adolescents to indicate either aggressive behavior problems or other problems. We found that, regardless of diagnosis, clinicians used both autonomy-supportive strategies (57.7% of responses) and controlling strategies (39.3%), suggesting that applying autonomy support can be challenging with any adolescent displaying resistance. In an experimental study (Study 3), adolescents (N = 252; 50.0% female; ages 12–17) reported higher therapeutic alliance (d = 0.95, 95% CI [0.80, 1.10]) and treatment engagement (d = 0.77, 95% CI [0.63, 0.91]) after listening to audio-recorded autonomy-supportive versus controlling responses from clinicians, regardless of whether these adolescents had aggressive behavior problems. Overall, this research suggests that clinicians can raise adolescents’ treatment engagement through autonomy support

    Sampson (this issue) argues for a concept of “realistic grammatical description” in which the distinction between grammatical and ungrammatical sentences is irrelevant. In this article I also argue for a concept of “realistic grammatical description” but one in which a binary distinction between grammatical and ungrammatical sentences is maintained. In distinguishing between the grammatical and ungrammatical, this kind of grammar differs from that proposed by Sampson, but it does share the important property that invented sentences have no role to play, either as positive or negative evidence

    Evaluation and bias correction of satellite rainfall data for drought monitoring in Indonesia

    The accuracy of three satellite rainfall products (TMPA 3B42RT, CMORPH and PERSIANN) was investigated through comparison with grid cell average ground station rainfall data in Indonesia, with a focus on their ability to detect patterns of low rainfall that may lead to drought conditions. Each of the three products underestimated rainfall in dry season months. The CMORPH and PERSIANN data differed most from ground station data and were also very different from the TMPA 3B42RT data. It proved possible to improve TMPA 3B42RT estimates by applying a single empirical bias correction equation that was uniform in space and time. For the six regions investigated, this reduced the root mean square error for estimates of dry season rainfall totals by a mean 9% (from 44 to 40 mm) and for annual totals by 14% (from 77 to 66 mm). The resulting errors represent 10% and 3% of mean dry season and annual rainfall, respectively. The accuracy of these bias corrected TMPA 3B42RT data is considered adequate for use in real-time drought monitoring in Indonesia. Compared to drought monitoring with only ground stations, this use of satellite-based rainfall estimates offers important advantages in terms of accuracy, spatial coverage, timeliness and cost efficiency
