Consensus Statement for the Management of Patients with Chest Pain
According to mortality statistics, cardiovascular disease continues to be the major cause of death in Argentina,followed by tumors and infections.The analysis of cardiovascular mortality, shows that coronary heart disease (CHD), together with heart failure(HF) are the most frequent causes of death in our country.Chest pain is the most common form of presentation of patients with CHD and its proper assessment dependson the diagnosis of acute coronary syndrome (ACS) that without hospitalization and appropriate treatmentis associated with high mortality. On the other hand, the unnecessary hospitalization of a patient withnoncardiac chest pain is risky for the patient and generates high costs to the health system.We consider a Consensus statement is necessary because:- Chest pain is the second most common cause of care in emergency departments.- The prompt classification of patients with chest pain aims mainly to differentiate an ACS from othercauses.- Although 50% of these patients present with a clinical condition suggestive of ACS, this diagnosis is reachedonly in half of the cases.- It represents a medical challenge and an important issue for health systems from an economic point of view
Las mujeres con síndromes coronarios agudos reciben menos intervenciones en la fase aguda que los hombres en una población argentina
Introducción
Existen antecedentes en la bibliografía científica que indican que las mujeres con enfermedad
coronaria tienen peor pronóstico que los hombres y se observa asimismo una utilización menor
de métodos diagnósticos e intervenciones en ellas, por lo que resulta de interés establecer si
esta tendencia existe en nuestro país y las posibles asociaciones de valor pronóstico.
Objetivos
Investigar en una cohorte argentina de síndromes coronarios agudos las características clínicas,
los tratamientos y la evolución a 2 años de seguimiento.
Material y métodos
Componente argentino del estudio cooperativo GRACE de 4.708 hombres y 2.027 mujeres
con síndromes coronarios agudos sin supradesnivel persistente del segmento ST. Se analizaron
antecedentes, tipo de presentación, diagnóstico definitivo, tratamientos farmacológicos,
aplicación de revascularización e incidencia acumulativa de muerte e infarto recurrente en
la etapa hospitalaria y a los 6 meses y a los 2 años de seguimiento.
Resultados
Las mujeres fueron mayores (69,4 ± 12,3 vs. 63,1 ± 11,9 años; p < 0,01), con mayor prevalencia
de insuficiencia cardíaca e hipertensión. La proporción de signos de isquemia miocárdica
en el electrocardiograma, así como la de enzimas miocárdicas en rango anormal fue similar
para ambos géneros, con un uso significativamente menor de aspirina, clopidogrel y betabloqueantes
en las mujeres, quienes tuvieron la mitad de chance de angioplastia (OR: 0,55, IC
95% 0,48-0,62) y de cirugía de revascularización (OR: 0,49, IC 95% 0,36-0,67), con mayores
incidencias crudas de muerte e infarto en agudo y a los 2 años, pero sin diferencia en el riesgo
relativo de eventos mayores una vez ajustado por edad y otras covariables.
Conclusiones
A pesar de un riesgo clínico similar en mujeres y hombres, las primeras recibieron menos
intervenciones durante la fase hospitalaria. Nuestros resultados llaman a un adecuado alerta
e intervenciones ajustadas al riesgo para el sexo femenino
Presencia de angiogénesis en placas vulnerables ateroscleróticas en corazones humanos aparentemente sanos
Background
Angiogenesis or neovascularization involves the formation of new blood vessels adjacent to preexisting vessels. This vascular proliferation is prevalent in various clinical conditions, such as atherosclerosis. Microvessels in coronary artery atherosclerotic plaques may contribute to plaque instability.
Objectives
The aim of this study was to correlate the presence of angiogenesis in atherosclerotic plaques with the criteria of plaque vulnerability used by the American Heart Association (AHA).
Methods
One hundred and twenty one hearts from non-diabetic and apparently healthy transplant donors older than 40 years were selected. The coronary arteries were examined and all areas of cross-sectional luminal narrowing underwent histological, immunohistochemical and morphometric studies. A semi-quantitative score (scale 0-3) was used to identify of angiogenesis. Univariate and multivariate logistic regression analysis was performed to identify angiogenesis-related risk factors.
Results
On hundred and forty three high-risk lesions (AHA type IV, V and VI) in the left anterior descending coronary artery (46.3%), the circumflex coronary artery (28.9%) and the right coronary artery (43%) were identified. Angiogenesis had a statistically significant association with the severity of vascular occlusion, inflammatory cell infiltration, presence of a lipid core, fibrosis and periarteritis. A history of hypertension (HT) was associated with angiogenesis only in lesions of the left anterior descending coronary artery (LAD). According to the AHA classification angiogenesis was detected in 1 Type II, 5 Type III, 21 Type IV, 22 Type V, and 7 Type VI plaques.
Conclusions
Angiogenesis in vulnerable plaques was associated with the severity of vascular occlusion, inflammatory cell infiltration, fibrosis and presence of a lipid core, and with a history of HT in LAD lesions. There was no association between angiogenesis and plaque hemorrhage or calcification, suggesting that angiogenesis may anticipate plaque rupture.Introducción
La angiogénesis o neovascularización involucra la formación de nuevos conductos en las adyacencias
de vasos preexistentes. Esta proliferación vascular es frecuente en varias circunstancias
clínicas, como es el caso de la aterosclerosis. Los microvasos de las placas ateroscleróticas
coronarias pueden estar vinculados a la inestabilidad de la lesión.
Objetivo
Correlacionar la presencia de angiogénesis en placas ateroscleróticas con los criterios de
vulnerabilidad de la clasificación de la American Heart Association (AHA).
Material y métodos
En 121 corazones de donantes no diabéticos aparentemente sanos y mayores de 40 años
destinados para homoinjertos se examinaron las arterias coronarias y todas las áreas de
estrechamiento luminal se sometieron a estudios histológicos, inmunohistoquímicos y morfométricos.
Para el análisis de la angiogénesis se empleó un puntaje semicuantitativo (escala
0-3). Se realizó un análisis de regresión logística univariado y multivariado para identificar
factores de riesgo relacionados con la angiogénesis.
Resultados
Se hallaron 143 lesiones de riesgo alto (AHA tipos IV, V y VI) en las arterias descendente
anterior (46,3%), circunfleja (28,9%) y coronaria derecha (43%). La angiogénesis se asoció
en forma estadísticamente significativa con el grado de oclusión vascular, la infiltración de
células inflamatorias, la presencia de centro lipídico, la fibrosis, la periarteritis y, sólo en la
descendente anterior, con el antecedente de hipertensión arterial (p < 0,006). Se detectó angiogénesis
en 1 placa tipo II, en 5 tipo III, en 21 tipo IV, en 22 tipo V y en 7 placas tipo VI (AHA).
Conclusiones
La angiogénesis de placas vulnerables se asoció con el grado de oclusión vascular, la infiltración
de células inflamatorias, la fibrosis, la presencia de núcleo lipídico y, sólo en la descendente
anterior, con el antecedente de hipertensión arterial. No se encontró asociación con la hemorragia
intraplaca o la calcificación, lo cual sugiere que la angiogénesis puede anticipar la
rotura de las placas
Clinical Outcomes of Electrical Disorders after Transcatheter Aortic Valve Implantation
IntroductionThe indication of transcatheter aortic valve implantation inpatients considered not suitable candidates for surgery isincreasing. Despite acute disorders of the conduction systemare common complications, their clinical and electrocardiographicsignificance is not completely clear.ObjectivesTo determine whether acute disorders of the conduction system after transcatheter aortic valve implantation hasprognostic implications.ResultsBetween March 2009 and February 2012, 47 patients were included in the analysis. Thirty patients (63%) had acutedisorders of the conduction system: 19 patients present edisolated complete left bundle branch block (LBBB), 9 patients isolated complete atrioventricular block (CAVB), and 2 patients both conduction disorders, with a total of 21 LBBBsand 11 CAVBs. A definite pacemaker was implanted in 12 patients (25%) before discharge: in 11 due to CAVB and in1 due to acute LBBB plus atrial fibrillation. Complete AV Breverted in the catheterization laboratory in only one patient. At one month, average ventricular pacing was 90% in patients with persistent CAVB after the intervention, only 3% in the only patient in whom CAVB reverted at the catheterization laboratory and < 10% in the patient with LBB plus atrial fibrillation.The incidence of postoperative heart failure and hospital stay was greater in patients with acute disorders of the conduction system (p = 0.007 and p = 0.045, respectively). Therewere no differences in new hospitalizations and mortalityduring follow-up.ConclusionsIn this study, the development of acute disorders of theconduction system was associated with increased incidenceof heart failure and hospital stay but not with the incidenceof major events. A definite pacemaker could be implantedimmediately after CAVB develops as the conduction disorderis generally irreversible.Introducción: El implante valvular aórtico percutáneo (IVAP) es una opción terapéutica cada vez mas utilizada en los pacientes descartados de cirugía. A pesar de que las alteraciones agudas de la conducción (AAC) son una de las complicaciones más frecuentes, su significado clínico y electrocardiográfico no es del todo claro. Objetivo: Determinar la evolución de los pacientes (P) que desarrollan AAC luego del IVAP. Resultados: De los 47 pacientes incluidos (Marzo 2009-Febrero 2012), 30P (63%) presentaron AAC: 21 bloqueo completo de rama izquierda (BCRI), y 11 bloqueo AV completo (BAVC); 2P presentaron ambos. A 12 (25%) se les implantó un MCPd previo al alta. 11 por BAVC y 1 por BCRI agudo más fibrilación auricular. En 1 sólo P el BAVC revirtió en hemodinamia Los P que persistieron con BAVC post intervención presentaron una media de estimulación ventricular en la intervalometría al mes del 90%, mientras que el P con BAVC que revirtió en hemodinamia solo del 3% y aquel con BCRI más fibrilación auricular <10%. La incidencia de insuficiencia cardíaca (IC) en el postoperatorio fue mayor en los P con AAC (p= 0.007), al igual que la estadía hospitalaria (p=0.045). En el seguimiento no hubo diferencias en la tasa de reinternación ni en la mortalidad. Conclusión: En nuestro estudio, el desarrollo de AAC aumentó la incidencia de IC y los días de internación sin aumentar la tasa de otros eventos mayores. La colocación de MCPd luego del BAVC podría realizarse en forma inmediata, ya que el trastorno es generalmente irreversible
Prospective Registry of Antithrombotic Therapy in Acute Coronary Syndromes (EPICOR)
Objectives: To present the two-year follow-up results of the EPICOR study Argentine cohort, a prospective, international,observational, multicenter registry designed to determine the use of antithrombotic therapy patterns in the routine clinicalpractice of patients with acute coronary syndrome (ACS).Methods: The study enrolled a total of 438 consecutive patients with ST-segment elevation myocardial infarction (STEMI,41%) or non-ST-segment elevation ACS (NSTE ACS, 59%) discharged alive from public, private, and community hospitals.Mean age was 62 years, 76% of patients were male, 71% hypertensive, 64% smokers, 19% diabetic and 40% had history ofprevious cardiovascular disease.Results: Overall mortality was 4.8% at 1 year and 7.3% at 2 years. Use of dual antiplatelet therapy was 80% at one year and53% at 2 years (p0.0001), sin diferencias entre aquellos con o sin supradesnivel del ST. La incidencia de eventos isquémicos y hemorrágicos mayores a los 2 años fue de 15,3% y 1,8% respectivamente. Conclusión: se observó un elevado porcentaje de persistencia de la doble antiagregación plaquetaria a los 2 años, más allá del año recomendado por las guías, con baja incidencia de hemorragias mayores, lo que sugiere una selección clínica de riesgo-beneficio
2010-2017 Evaluation and Accreditation of Cardiology Residencies
Introducción: El Ministerio de Salud y Desarrollo Social de la Nación implementa un Sistema Nacional de Acreditación de Residencias del Equipo de Salud. La Sociedad Argentina de Cardiología (SAC) participa como entidad evaluadora de los programasde residencias médicas en cardiología.Objetivo: El propósito del presente trabajo es presentar los resultados del proceso de evaluación y acreditación de residencias de Cardiología.Material y métodos: Revisión documental de los informes elevados por los pares evaluadores designados por la SAC y de los dictámenes del Ministerio publicados en el Boletín Oficial entre 2010 y 2017.Resultados: Se revisaron 37 informes elaborados por los pares evaluadores, se identificaron fortalezas y debilidades. Fortaleza destacada: supervisión continua recibida por los residentes. Debilidad: escaso tiempo dedicado a la atención de pacientes ambulatorios. Se revisaron 28 dictámenes del Ministerio, se analizaron las recomendaciones más frecuentes: incorporación de contenidos transversales, regulación de la cantidad de guardias, de su duración y de la organización del descanso posguardia.Conclusiones: Si bien a primera vista parece que ambas instituciones tienen distintos criterios a la hora de acreditar unprograma de formación en servicio, en realidad, se trata de miradas complementarias que coinciden en los siguientes aspectos:importancia de la supervisión, necesidad de sistematizar la evaluación del desempeño de los residentes y necesidad deincrementar la producción de trabajos científicos y la participación de los residentes en aquellos. También se encontró un alto grado de coincidencia entre la sugerencia de la SAC y el dictamen del Ministerio en relación con la categoría y los años de acreditación que merecía cada programa evaluado.Background: The National Ministry of Health and Social Development implements a National System of Accreditation ofHealth Team Residencies. The Argentine Society of Cardiology (SAC) participates as an evaluation entity of the medicalresidency programs in cardiology.Objective: The purpose of this study is to present the results of the evaluation process and accreditation of cardiology residencies.Methods: This was a documentary review of the reports submitted by the peer reviewers appointed by SAC and the opinionsof the Ministry of Health in the Official Gazette between 2010 and 2017.Results: Thirty-seven reports prepared by peer reviewers were analyzed and strengths and weaknesses were identified. Outstandingstrength: continuous supervision received by residents. Weakness: time spent on outpatient care. Twenty-eight Ministryof Health opinions were reviewed and the most frequent recommendations were analyzed: incorporation of transversalcontents; regulation of number of on-call duties, their duration and the organization of the rest period after on-call shifts.Conclusions: Although both institutions seem initially to have different criteria when accrediting a health service trainingprogram, in reality, they have complementary views which coincide in the following aspects: importance of supervision; needto systematize the evaluation of residents’ performance; and need to increase the number of scientific works with residentparticipation. A high degree of coincidence was also obtained between SAC’s suggestion and the opinion of the Ministry ofHealth, in relation to the category and years of accreditation that each evaluated program deserved
Maltrato en la formación médica: situación en las residencias de cardiología
Background: Mistreatment is defined as the behavior that makes another person feel hurt, undervalued or incompetent. A
certain level of intimidation and humiliation during training was considered necessary to prepare the doctor for a difficult
profession. The conditions in which medical residences and professional practice are developed today generate a high prevalence of burnout.
Objective: The aim of this study was to investigate the perception that Cardiology residents have on the mistreatment received
and if they acknowledge having incurred in any aggravating behavior.
Methods: A survey was conducted in residents attending the Biannual Cardiology Course of the Argentine Society of Cardiology.
Results: A total of 183 residents responded the survey and all of them (100%) reported some type of mistreatment. Being
humiliated for making a mistake and shouted at were the most frequently mentioned forms of mistreatment from a senior
resident, a patient or their relatives. Almost half of the residents (46%) said they had been mistreated by the nursing staff.
while 33.5% acknowledged having incurred in aggravating situations directed against another resident, a staff doctor and/
or another specialty physician.
Discussion: The results are similar to other studies already published. For the safety of patients and the health of physicians, it
is recommended to include strategies for coping with stress and it is considered essential to promote cultural changes within
academic and care institutions aimed at creating more democratic and healthier working environments.Introducción: El maltrato se define como el comportamiento que hace que otra persona se sienta herida, desvalorizada o
incompetente. Un cierto nivel de intimidación y humillación durante la formación se consideraba necesario para preparar al
médico para una profesión difícil. Las condiciones en las que hoy se desarrollan las residencias médicas y la práctica profesional
generan una alta prevalencia de burnout (agotamiento).
Objetivo: Indagar la percepción que tienen los residentes de Cardiología sobre maltrato recibido y si reconocen haber incurrido
en alguna conducta agraviante.
Material y Métodos: Encuesta aplicada a residentes que asisten al Curso Bianual de Cardiología de la Sociedad Argentina
de Cardiología
Resultados:Respondieron 183 residentes. El 100% dijo haber recibido algún tipo de maltrato. Ser humillado por un error
cometido y los gritos, son las formas de maltrato mencionadas con mayor frecuencia; un residente superior, un paciente y/o
sus familiares fueron señalados como los responsables de las agresiones recibidas. Casi la mitad de residentes (46%) dijeron
haber sido maltratados por el personal de enfermería. El 33,5% reconoció haber incurrido en situaciones de maltrato y que
éste estuvo dirigido a otro residente, a un médico de planta y/o un médico de otra especialidad.
Discusión: Los resultados son similares a otros estudios ya publicados. Por la seguridad de los pacientes y por la salud de los
médicos se recomienda incluir en la formación profesional las estrategias para el afrontamiento del estrés y se considera
indispensable promover un cambio cultural dentro de las instituciones académicas y asistenciales orientado a crear espacios
de trabajo más democráticos y más saludables
Registro multicéntrico prospectivo de pacientes hospitalizados por síndrome coronario agudo sin elevación del segmento ST en centros de alta complejidad. Resultados intrahospitalarios y evolución a 6 meses (Buenos Aires I)
Background: Non-ST elevation acute coronary syndrome (NSTE-ACS) management has evolved over the past years, based on newpharmacological agents and progress in revascularization techniques. The aim of this study was to analyze the current managementof NSTE-ACS in high complexity centers of the city of Buenos Aires and the province of Buenos Aires.Methods: Patients hospitalized in 21 centers with coronary care unit, 24-hour catheterization lab availability and cardiovascularsurgery were prospectively enrolled in the study and followed up for 6 months after hospital discharge.Results: The registry included 1,100 consecutive patients: 61% corresponded to non-ST-segment elevation myocardial infarction and37.4% were unstable angina. Mean age was 65.4 ± 11.5 years and 77.2% were men; 27.6% had diabetes mellitus and 31.5% previousmyocardial infarction. An early invasive management was used in 86.7% of cases with a median time to coronary angiography of 18hours (IQR 7-27.7). During hospitalization, 5.2% of the patients presented reinfarction, 0.3% stroke and overall mortality was 2.7%.The rate of bleeding events ≥ BARC type 2 was 10.1%. At 6-month follow-up, the rates of reinfarction, ACS and overall mortalitywere 8.4%, 10.9% and 5.7%, respectively.Conclusions: The registry demonstrated a predominantly invasive therapeutic approach in patients with NSTE-ACS treated in highcomplexity centers with low rates of in-hospital complications and during follow-up.Introducción: El manejo de los síndromes coronarios agudos (SCA) sin elevación del segmento ST (SCASEST) ha sufrido cambios en los últimos años, basados en nuevos agentes farmacológicos y el avance de las técnicas de revascularización coronaria. El objetivo de este estudio fue determinar cómo es el manejo de los SCASEST en la actualidad en centros de alta complejidad de la ciudad y la provincia de Buenos Aires. Métodos: Se registraron en forma prospectiva pacientes hospitalizados en 21 centros con servicio de unidad coronaria, hemodinamia disponible las 24 horas y cirugía cardíaca. Se realizó seguimiento a 6 meses. Resultados: Se incluyeron 1100 pacientes consecutivos, un 62,6 % fue catalogado como infarto sin elevación del ST y 37,4 % como angina inestable. La edad media fue de 65,4 ± 11,5 años, con un 77,2 % de sexo masculino. Un 27,6 % presentaba diabetes mellitus y el 31,5 % infarto previo. El manejo inicial fue invasivo en el 86,7 %, con una mediana de tiempo a la cinecoronariografía de 18 horas (RIC 7-27,7). En la evolución intrahospitalaria, la incidencia de nuevo infarto fue del 5,2 %, el accidente cerebrovascular de 0,3 % y mortalidad total 2,7 %. La tasa de sangrado BARC ³ 2 fue del 10,1 %. En el seguimiento a 6 meses, la tasa de infarto fue de 8,4 %, SCA 10,9 % y la mortalidad total de 5,7 %. Conclusiones: El registro evidenció un abordaje terapéutico predominantemente invasivo de los pacientes con SCASEST en centros con alta complejidad, con baja prevalencia de complicaciones intrahospitalarias y en la evolució
Invasive vs non-invasive treatment in acute coronary syndromes and prior bypass surgery
BACKGROUND: We evaluated the association between invasive and non-invasive management and hospital and 6-month outcomes in patients with a prior coronary artery bypass graft (CABG) who experienced an acute coronary syndrome.
METHODS: Data were analysed from patients with a prior CABG who developed an acute coronary syndrome and were enrolled in the Global Registry of Acute Coronary Events. From 44,991 patients included in the study, 3853 fulfilled the inclusion criteria. Of these, 3356 received non-invasive treatment approaches while 497 underwent invasive treatment (percutaneous coronary intervention [PCI] within 48 h of admission).
RESULTS: The primary composite endpoint of death, non-fatal myocardial infarction, and recurrent ischaemia during hospitalization was similar in patients in the non-invasive and invasive groups (31% vs 30%, respectively; P=0.53). The rates of hospital mortality (non-invasive 3.4% vs invasive 3.2%) and non-fatal myocardial infarction (3.4% vs 5.1%, respectively) were similar. At 6-month follow-up, the mortality rate was 6.5% in the non-invasive group vs 3.4% in the invasive group (P\u3c0.02); the combined endpoint of death or myocardial infarction was lower in the invasive group (P\u3c0.01). Multivariable analysis showed that, at 6-month follow-up, the combined endpoint of death, non-fatal myocardial infarction, and rehospitalization for heart disease was similar (P=0.10). A greater proportion of patients in the invasive group required unscheduled diagnostic and therapeutic invasive procedures compared with those in the non-invasive group (angiography 15.4% vs 8.1%; PCI 10% vs 5.0%; both P\u3c0.001).
CONCLUSIONS: The results from this observational study show no statistically significant differences in hospital outcomes between acute coronary syndrome patients with a prior CABG who undergo invasive or non-invasive treatment. Invasively treated patients experienced higher rates of readmission and additional cardiac procedures than non-invasively treated patients but a lower incidence of cardiovascular complications at 6 months
