    Fogak morfológiai és metrikus jellegzetességeinek vizsgálata marosvásárhelyi középkori leleteken és a jelenkori helyi populáció körében = Dental morphological and metric characteristics in contemporary and historical population of Târgu Mureș

    Absztrakt: Bevezetés: A fogazat a szervezet egységes részeként nem képez kivételt az embernek a fejlődéstani okokból és életformából következő fizikai antropológiai változásai alól. A különböző káros stresszhatások mellett a természetes kiválasztódás is hozzájárul bizonyos fogmorfológiai jellemzők hangsúlyosabbá válásához. Célkitűzés: Középkori embertani leletek és mai páciensek gipszmintáin végzett odontometriai elemzés eredményeinek, valamint a járulékos csücskök jelenlétének összehasonlítása. Anyag és módszer: A 19 embertani leletből származó 132, valamint 30 páciens modelljén meglévő 694 fog morfológiai jellegzetességeit figyeltük meg. A koronák mesiodistalis, incisivogingivalis és vestibulooralis átmérőinek kétdimenziós képanalízise során nyert értékeit összehasonlítottuk, és megvizsgáltuk az antropológiai szempontból kiemelt fontosságú Carabelli- és taloncsücsköket. Eredmények: A XIV. századi és a jelenkori fogak méretei között statisztikai szempontból jelentős változást találtunk néhány fogcsoport esetében. A középkori leleteken az oldalsó fogak kevésbé széles koronával rendelkeztek, mint a mai populációban mért fogak. A történeti leleteken talált Carabelli-csücskök a Dahlberg-skála szerinti 1. és 3. fokozathoz tartoztak (23,07%-os előfordulás), míg a mai eseteken megfigyelt Carabelli-csücskök a 3., 4., 5., 6. és 7. fokozathoz tartoztak (50,90%-os előfordulás). Taloncsücsökkel a jelenkori csoportban találkoztunk, mindössze egy esetben. Következtetés: A vizsgált jelenkori fogakon a Carabelli-csücskök hangsúlyosabb morfológiai formában és nagyobb gyakorisággal jelentek meg, ami a mai populáció europid jellegét hangsúlyozza. Orv Hetil. 2018; 159(30): 1235–1240. | Abstract: Introduction: Teeth, as an integral part of human organism, are not exceptions from the physical anthropology changes caused by biological evolution and the way of living. Different stress factors and the natural selection are contributing to the emphasis of certain morphological characteristics. Aim: Comparison of dental metric characteristics and the presence of accessory tooth cusps between human remains from the Middle Ages and dental study models of today’s patients. Material and method: Morphological characteristics of 132 teeth from 19 skulls and 694 teeth on dental models of 30 patients were examined. The mesiodistal, incisivocervical and buccolingual diameters of crowns were measured by a validated 2D image analysis method. Carabelli and talon cusps were also examined using a magnifying glass. Results: Statistically significant differences between the size of the 14th-century and the present-day teeth were found in some of the teeth groups. In medieval artifacts, lateral teeth had smaller crown width than teeth measured in the contemporary population. The Carabelli cusps found in the archeological human remains belonged to grades 1 and 3 on Dahlberg scale (23.07%), while the Carabelli cusps observed in the contemporary group belonged to grades 3, 4, 5, 6 and 7 (50.90%). Talon cusp occurred only once in the contemporary group. Conclusion: In case of the contemporary teeth, the Carabelli cusps appeared in a more pronounced morphological form and with a higher frequency, which emphasizes the European origin of today’s population.     Bile acid- and ethanol-mediated activation of Orai1 damages pancreatic ductal secretion in acute pancreatitis

    Sustained intracellular Ca2+ overload in pancreatic acinar and ductal cells is a hallmark of biliary and alcohol-induced acute pancreatitis, which leads to impaired ductal ion and fluid secretion. Orai1 is a plasma membrane Ca2+ channel that mediates extracellular Ca2+ influx upon endoplasmic reticulum Ca2+ depletion. Our results showed that Orai1 is expressed on the luminal plasma membrane of the ductal cells and selective Orai1 inhibition impaired Stim1-dependent extracellular Ca2+ influx evoked by bile acids or ethanol combined with non-oxidative ethanol metabolites. The prevention of sustained extracellular Ca2+ influx protected ductal cell secretory functions in in vitro models and maintained exocrine pancreatic secretion in in vivo AP models. Orai1 inhibition prevents the bile acid-, and alcohol-induced damage of the pancreatic ductal secretion and holds the potential of improving the outcome of acute pancreatitis.Regardless of its etiology, sustained intracellular Ca2+ overload is a well-known hallmark of acute pancreatitis (AP). Toxic Ca2+ elevation induces pancreatic ductal cell damage characterized by impaired ion- and fluid secretion -essential to wash out the protein-rich fluid secreted by acinar cells while maintaining the alkaline intra-ductal pH under physiological conditions- and mitochondrial dysfunction. While prevention of ductal cell injury decreases the severity of AP, no specific drug target has yet been identified in the ductal cells. Although Orai1 -a store operated Ca2+ influx channel- is known to contribute to sustained Ca2+ overload in acinar cells, details concerning its expression and function in ductal cells are currently lacking. In this study, we demonstrate that functionally active Orai1 channels reside dominantly in the apical plasma membrane of pancreatic ductal cells. Selective CM5480-mediated Orai1 inhibition impairs Stim1-dependent extracellular Ca2+ influx evoked by bile acids or ethanol combined with non-oxidative ethanol metabolites. Furthermore, prevention of sustained extracellular Ca2+ influx protects ductal cell secretory function in vitro and decrease pancreatic ductal cell death. Finally, Orai1-inhibition partially restores and maintains proper exocrine pancreatic secretion in in vivo AP models. In conclusion, our results indicate that Orai1 inhibition prevents AP-related ductal cell function impairment and holds the potential of improving disease outcome.     Áttétes vesedaganatos betegek kabozantinibterápiájának multicentrikus magyarországi eredményei

    The aim of our analysis was to evaluate the efficacy of cabozantinib in patients with metastatic renal cell carcinoma. Cabozantinib therapy initiated between 01/01/2019 and 31/12/2022 was evaluated based on a retrospective review of data from 14 renal centers in Hungary. The starting dose was 60 or 40 mg. Physical examinations and laboratory tests were performed every 4 weeks and imaging studies 3-monthly. Tumor response was assessed according to RECIST 1.1, and toxicity according to NCI CTCAE 4.0. A total of 230 patient records were evaluated, 201 (87.4%) of them had clear cell RCC. Cabozantinib was administered as third, second and first-line treatment in 48.7%, 38.3% and <5% of cases, respectively. Dose reductions occurred in 62.6% and treatment interruption in 6.5%. Duration of therapy was 10.03 months, which was independent of dose reduction. Overall tumor response rate was 39.2% and clinical benefit was 82.8%. The duration of first-, second-, third- and fourth-line treatment was 11.47, 8.03, 11.57 and 10.13 months, respectively. Overall survival from the start of therapy was 22.0 months. Cabozantinib therapy in daily practice was more beneficial than according to registry study results. Dose reduction did not affect efficacy
