Right ventricle outflow tract stenting as a method of palliative treatment of severe tetralogy of Fallot
We present the case of a 3-month-old infant with tetralogy of Fallot (ToF) with pulmonary
artery hypoplasia, critical right ventricle outflow tract (RVOT) obstruction and the presence of
major aortopulmonary collateral arteries (MAPCA) and CATCH 22 syndrome. Due to anatomical
conditions (severe pulmonary artery hypoplasia), the patient was not qualified for
palliative operative treatment Blalock-Taussig shunt. We conducted catheterization with an
attempt of balloon plasty and stent implantation into the right ventricle outflow tract and main
pulmonary artery. Successful stent implantation into the right ventricle outflow tract was
performed. The stent created a 4.1 mm diameter channel and allowed for unrestricted blood
flow from the right ventricle to the pulmonary arteries. After the procedure we observed an
increase in blood saturation of up to 89%.
Control echocardiography revealed blood flow through the stent to the pulmonary arteries with
a pressure gradient of 45 mm Hg. There were neither rhythm nor conduction disturbances in
the control ECG after the procedure.
After 6 days of observation the patient was discharged from our department.
We conclude that successful stent implantation into the RVOT in patients with ToF and
hypoplastic pulmonary arteries improves their clinical condition, increases pulmonary blood
flow by physiological means and leads to an improvement of pulmonary artery development
before surgical treatment

Postępowanie lecznicze u pacjentów z zespołem hipoplazji lewej części serca (HLHS) jest oparte na wieloetapowym leczeniu operacyjnym, niejednokrotnie uzupełnionym dodatkowymi zabiegami interwencyjnymi. W tej grupie pacjentów szczególnie znaczenie ma prawidłowy rozwój łożyska płucnego, który należy do podstawowych czynników obniżających ryzyko powikłań w trakcie kolejnych etapów leczenia. Zabiegi przezskórnego poszerzenia tętnic płucnych pozwalają na optymalizację przepływu płucnego krwi, jednak wykonywane u najmłodszych pacjentów stanowią duże wyzwanie i wymagają znacznej precyzji. Angiografia rotacyjna z trójwymiarową rekonstrukcją (3DRA) to nowa metoda obrazowania umożliwiająca szczegółowe, przestrzenne uwidocznienie anatomii tętnic płucnych nieosiągalne w standardowej angiografii. W prezentowanym przypadku pacjenta z czynnościowo pojedynczą komorą oraz hipoplazją tętnic płucnych 3DRA okazała się pomocna przy kwalifikacji, monitorowaniu oraz końcowej ocenie implantacji stentu wewnątrznaczyniowego.Patients with hypoplastic left heart syndrome require multistage surgical treatment, often supported with additional percutaneous interventions. In this population normal development of pulmonary vasculature is crucial, as it belongs to key factors reducing complication rate at all stages of palliation. Transcatheter interventions allow for significant improvement of pulmonary blood flow but they can be very challenging in the youngest patients. Three-dimensional rotational angiography (3DRA) is an emerging imaging modality that enables detailed visualization of pulmonary arteries, not achievable in standard angiography. In presented patient with univentricular heart and pulmonary artery hypoplasia 3DRA proved helpful in qualification, monitoring and final evaluation of stent implantation
One center experience in pulmonary artery stenting without long vascular sheath
Background: Pulmonary artery stenting without a long vascular sheath has a special significance,
particularly for children with low body weight. Using only a short sheath often permits
the implantion of a stent of the correct size; moreover, it improves access to peripherally located
stenoses. The aim of this paper is to present the results of a balloon expandable stent implantation
into pulmonary arteries without using a long vascular sheath.
Methods: The subjects were divided into two groups. The first group (28 patients, mean age
3.2 years) comprised patients with a single-ventricle heart after bi-directional Glenn procedure
(Fontan procedure). The second group (22 patients, mean age 8.3 years) consisted of patients
with a two-ventricle heart. Patients were retrospectively analyzed with regard to stenosis size
widening and change in trans-stenotic pressure gradient after stenting.
Results: In our data, no statistically significant differences between the two groups in terms
of the number of complications and incorrect stent position following implantation were noted
(3% and 4%). Good treatment results, with a decrease in trans-stenotic pressure gradient in
the first group of 3.2 mm and in the second group of 13.4 mm of mercury, and a widening of
the stenosis, were obtained in most cases in both groups (97% and 96%). The average change
of the vessel’s diameter was in the first group 4.2 mm and in the second 5.4 mm.
Conclusions: The obtained results suggest that pulmonary artery stenting with a short vascular
sheath has numerous advantages and can be successfully performed in children. In the case
of single-ventricle hearts after a Glenn procedure, it may indeed be the method of choice.

Interwencyjne zamknięcie drożnego przewodu tętniczego — kiedy, jak i czym?
Up to 10% of congenital heart defects is patent ductus arteriosus. Apart from newborns, in most of symptomatic patients, interventional closure of the ductus arteriosus is the preferred method of treatment. For this purpose, different types of endovascular coils or dedicated duct occluders are successfully applied. In rare situations devices for interatrial or interventricular defect closure or covered stents are used. In the current paper the indications, most commonly used devices and interventional techniques for closure of the ductus arteriosus have been discussed.Drożny przewód tętniczy stanowi do 10% wrodzonych wad serca. U większości objawowych pacjentów, poza okresem noworodkowym, interwencyjne zamknięcie przewodu tętniczego stanowi preferowaną metodę leczenia. Do tego celu z powodzeniem wykorzystywane są różnego rodzaju sprężynki wewnątrznaczyniowe lub dedykowane zestawy dyskowe. W szczególnych sytuacjach zastosowanie znajdują również zestawy do zamykania ubytków międzyprzedsionkowych lub międzykomorowych, a sporadycznie stenty pokryte łatą. W artykule omówiono wskazania, scharakteryzowano najczęściej wykorzystywany sprzęt oraz opisano typowe techniki interwencyjnego zamknięcia drożnego przewodu tętniczego
Wykorzystanie stentu w leczeniu wtórnego zwężenia łuku aorty w okresie niemowlęcym
We present the case of a 12 month-old infant with critical coarctaion of aorta, arch hypoplasia, atrial and ventricular septal
defects who underwent interventional treatment with stent implantation due to recoarctation of aorta. In neonatal period the
patient went through complete surgical correction. At the age of four months balloon angioplasty of recurrent coractation was
conducted. Eight months later the patient was hospitalised with heart failure (HF) symptoms. Clinical and echographic examination
confirmed critical stenosis of aortic isthmus. We performed heart catheterisation with stent implantation to transverse
and descending arch with immediate reduction of gradient from 45 mm Hg to 0 mm Hg, widening of the isthmus from 2 to
8 mm and gradual regression of HF symptoms. At 3 years of observation the patient has not presented with any signs of
recurrence of the stenosis. The child remains free of HF symptoms with arterial hypertension controlled with pharmacotherapy
and with good left ventricular function on echocardiographic examination.

Late emergency arterial duct stenting in a patient with tetralogy of Fallot and occluded Blalock-Taussig shunt
Over the last decade or so, stent implantation to the arterial duct has achieved acceptance as an
alternative to aortopulmonary shunt procedure. We present the case of a patient with tetralogy of
Fallot with diminished pulmonary flow and complete proximal occlusion of a right-sided Blalock-Taussig shunt. Surgical repair was not possible because of relative contraindications, and the
interventional shunt recanalisation attempt was unsuccessful. The coronary stent was implanted
into arterial duct. This resulted in oxygen saturation increase with normal contrast
flow to the left pulmonary artery and right pulmonary artery (prior to the procedure we
suspected non-confluent pulmonary artery). This brief report evaluates the feasibility of this
new therapeutical option in such special patients with duct-dependent pulmonary blood flow.

Immediate and long-term outcomes of percutaneous transcatheter pulmonary valve implantation
Background: Transcutaneous pulmonary valve replacement (TPVR) has become an alternative to heart surgery for patients after previous right ventricular outflow tract (RVOT) or pulmonary artery (PA) surgical interventions. The objective was to present immediate and long-term outcomes of trans¬cutaneous pulmonary valve replacement.
Methods: Between 06/2009 and 06/2016, 46 patients underwent TPVR. Initial diagnoses included tetralogy of Fallot, common arterial trunk, transposition of great arteries post Rastelli correction, left ventricle outflow obstruction after Ross operation, pulmonary atresia, and isolated dysplastic pulmonary valve stenosis. Thirty eight (78%) patients had previously implanted conduits in the pulmonary position, the rest had either RVOT patch reconstruction (n = 6; 13%) or biological valve implantation (n = 2; 4%). They presented primarily with pulmonary stenosis (n = 18; 39%) or regurgitation (n = 28; 60%).
Results: All procedures were successful — 44 Melody and 2 Edwards-Sapien valves were implanted. Before each procedure exclusion of potential coronary compression and RVOT prestenting was performed. Significant RVOT systolic gradient reduction (from 35.3 ± 19.5 to 13.5 ± 7.1 mm Hg; p < 0.001) and decrease of right to left ventricle systolic pressure ratio from 0.58 ± 0.18 to mean 0.37 ± 0.1 (p < 0.001) was achieved. Also, in every patient PA-RVOT competence was restored, with minor in¬competence in only a few patients. Post procedure follow-up ranged from 2 to 86 (mean 35.2) months. Follow-up fluoroscopy or chest X-ray revealed 6 stent fractures (2 stent defragmentation — with only 1 significant valve stenosis).
Conclusions: Transcutaneous pulmonary valve replacement is a safe procedure with encouraging results, it also enables deferring surgical reintervention in the majority of patients
Walwuloplastyka balonowa w krytycznym zwężeniu zastawki aortalnej u noworodków
Wstęp: Krytyczne zastawkowe zwężenie aorty stanowi wadę wrodzoną serca powodującą
niewydolność serca już w najwcześniejszym okresie życia dziecka, stanowiąc bezpośrednie
zagrożenie życia. Leczenie wady polega na operacyjnym nacięciu spoideł zastawki aortalnej
lub na przezskórnej walwuloplastyce balonowej. Celem pracy jest przedstawienie własnych doświadczeń
w leczeniu noworodków z krytycznym, zastawkowym zwężeniem aorty za pomocą
balonowej walwuloplastyki.
Materiał i metody: Badaniami objęto 27 noworodków w wieku 1-27 dni (śr. 7 dni)
z krytycznym zwężeniem zastawki aortalnej, leczone interwencyjnie w Klinice Kardiologii
Instytutu CZMP w Łodzi w latach 1996-2003. Zastosowanie stałej podaży prostaglandyny E1
było konieczne u 18 pacjentów, a 16 dzieciom podano aminy katecholowe. U 8 noworodków
stosowano oddech zastępczy przed zabiegiem. U wszystkich pacjentów przed zabiegiem wykonano
pełne badanie echokardiograficzne, w którym oceniano: zastawkę aortalną (jej budowę
i średnicę pierścienia), gradient zwężenia lewa komora–aorta, wymiar rozkurczowy lewej
komory i frakcję wyrzutową, obecność i stopień niedomykalności zastawki dwudzielnej. Analizie
porównawczej poddano grupę dzieci, które przeżyły z grupą dzieci, które zmarły.
Wyniki: U wszystkich pacjentów zabieg wykonano z dostępu przez prawą tętnicę szyjną
wspólną. Tylko w jednym przypadku skuteczną walwuloplastykę wykonano przez tętnicę
udową. Uzyskano zmniejszenie średniego gradientu w miejscu zwężenia z 58 mm Hg do
22 mm Hg. Zabieg przeżyło 19 pacjentów (70,4%). Zgon w badanej grupie nastąpił u 8 osób
(29,6%). Podczas około 52-miesięcznej obserwacji u 4 pacjentów konieczne było zastosowanie
leczenia kardiochirurgicznego. U pozostałych 15 chorych resztkowy gradient lewa komora–aorta wynosił średnio 36 mm Hg przy prawidłowej kurczliwości lewej komory.
Wnioski: Balonowa walwuloplastyka zastawki aortalnej jest skuteczną metodą paliatywnego
leczenia noworodków z krytycznym jej zwężeniem. Wyniki terapii zależą od stanu dziecka
przed zabiegiem i stopnia uszkodzenia mięśnia lewej komory

Complex percutaneous intervention on pulmonary arteries in an adult patient with a corrected Taussig-Bing anomaly
W niniejszej pracy przedstawiono przypadek udanej złożonej interwencji przezskórnej na tętnicach płucnych u 23-letniego dorosłego pacjenta ze skorygowaną anomalią Taussiga–Binga. Wada skorygowana była zabiegiem typu arterial switch; ponadto w wywiadzie odnotowano przebytą operację Bentalla i wymianę zastawki mitralnej. Przy przyjęciu pacjent był bezobjawowy, jednak wykryto istotne zwężenie tętnic płucnych. Pacjent został zakwalifikowany do cewnikowania serca. Złożony zabieg wysokiego ryzyka z implantacją trzech stentów poprawił morfologię prawej tętnicy płucnej, a w konsekwencji funkcję prawej komory. Podsumowując, dzięki niezwykłemu rozwojowi technik przezskórnych, coraz więcej pacjentów otrzymuje optymalne, spersonalizowane leczenie.This study reports a case of a successful complex percutaneous intervention on pulmonary arteries in a 23-year-old adult patient with a corrected Taussig-Bing anomaly. The patient had a history of multiple surgeries, including an arterial switch operation, the Bentall procedure, and mitral valve replacement. On admission, the patient was asymptomatic, however significant stenosis of the pulmonary arteries was detected. The patient was qualified for cardiac catheterization. The complex, high-risk procedure with the implantation of three stents improved the morphology of the right pulmonary artery and consequently the function of the right ventricle. It is concluded that with the remarkable development of percutaneous techniques, more and more patients are receiving optimal, personalised treatment
