    A study on conservative management on paediatric bone forearm fractures in tertiary care hospital in Cuddalore district

    Background: Diaphyseal fractures of the radius and ulna are common in the pediatric population. The standard management for pediatric forearm fractures remains conservative management with closed reduction and immobilization with an above elbow plaster cast. Though the fracture unites readily, malunion is very common. Stiffness of joints, compartment syndrome is other complications of conservative management with plaster cast Forearm fractures are common in the pediatric population. These fractures are mostly managed conservatively by means of closed manual reduction and casting. The aim of this study was to reiterate the importance of conservative management in both bone forearm fractures in the pediatric population.Methods: This was a prospective study from a tertiary level trauma center on 70 children up to 14 years of age with both bone forearm fractures from June 2018 to June 2019, cases were obtained from medical records. Data were collected and confirmed by plain X-ray films and medical records.Results: All fractures achieved union at the final visit. More than 90 of diaphyseal forearm fractures achieved exceptional radiological and clinical outcomes and almost no case had a significantly poor outcome. There were significantly reduced angulation deformities before and after treatment (p<0.05).Conclusions: Nonsurgical treatment as a model of management by means of closed reduction and casting is a very well accepted method of treating pediatric diaphyseal forearm fractures.

    Individual-Based Modeling: Comparing Model Outputs to Telemetry Data with Application to the Florida Panther

    Mean distance of the locations of an animal from the boundaries of its home range was presented as a measure of its space-use preference. Methods for evaluating the predictive ability of an individual-based model were also presented. These methods were applied to data on the Florida panther and some interesting results were obtained. A strong negative correlation was observed between age and home range size of the panther, indicating constrained mobility of the panther during its old age. Space-use preference was also highly dependent on age of the panther. A general trend was found for panthers, which indicates they stay away from the boundaries of their home range during old age. It was also observed that sex of the panther and season of the year do not have any effect on the space-use preference. A random movement model was used to simulate panther movement; applicability of this model to panther data was evaluated based upon its ability to depict the animal’s space-use preference and shift of activity center over time. It was concluded that comparison of modeled and observed movement data accumulated over a long period of time might give misleading results. Data should be subdivided into different age periods and the model should be tested in each period

    Processing parameters and chamber length impact on detached die and attached die resin injection pultrusion

    Injection pultrusion is an efficient and highly automated continuous process for high-quality, low-cost, high-volume manufacturing of composites. The main objective of this study is to analyze attached die configuration and detached die configuration for the better injection pultrusion process. In this work the impact of various processing parameters on complete wet out of composite parts is investigated in the attached die and detached die injection pultrusion with various chamber length considerations. The various processing parameters considered are pull speed, fiber volume fraction, resin viscosity, injection port location and compression ratio. 3-D finite volume technique is used to simulate the liquid resin flow through the fiber reinforcement in the injection pultrusion process. The purpose of the present work is to investigate the resin injection pressure needed to achieve complete wet-out, the corresponding maximum pressure inside the resin injection chamber and to predict the resin flow front by varying the length of injection chamber for different processing parameters

    A study of atenolol and nebivolol in prehypertension

    Background: Patients with prehypertension have an increased risk of cardiovascular disease. The coexistence of prehypertension with risk factors increases cardiovascular morbidity and mortality. That’s why it is important to treat pre-hypertensive patients having risk factors. The objective was to evaluate the effect of atenolol and nebivolol in pre-hypertensive patients.Methods: Pre-hypertensive patients having risk factors were selected, and non-pharmacological therapy was advised to all patients. Those patients who were not able to follow strictly non-pharmacological guidelines and remained pre-hypertensive were included in this study. Pre-hypertensive patients were divided into three groups. One group received atenolol 50 mg orally, once daily. Second group received nebivolol 5 mg orally, once daily. Third group received placebo orally, once daily. All groups received treatment for 1 month.Results: In the nebivolol group after 1 month of study, the mean reduction in systolic blood pressure (SBP) was 134.2±3.07-118.26±4.66 and mean reduction in diastolic BP (DBP) was 87.13±1.87-80.73±1.99. Reduction in SBP and DBP in the nebivolol group was significant (p≤0.0001). In the placebo, and atenolol group results were not significant.Conclusion: Nebivolol produces a significant reduction in SBP and DBP in pre-hypertensive patients. Atenolol and placebo did not show beneficial results

    Classification Based Analysis on Cancer Datasets Using Predictor Measures

    Cancer is a life-threatening disease. Probably the most effective way to reduce cancer deaths is to detect it earlier. Diagnosing the disease earlier needs an accurate and reliable procedure which could be used by physicians to distinguish between cancer from malignant ones without leaving for surgical biopsy. Data mining offers solution for such types of the problems where a large quantity of information about patients and their conditions are stored in clinical database. This paper focuses on prediction of some such diseases like Leukemia and Breast cancers. Naïve Bayes and SVM prediction models are built for the prediction and classification. The performance of the proposed models produced significant results of above 96% while compared with other models in terms of accuracy, computational time and convergence.     Primary arthroplasty as an option for surgical treatment of unstable intertrochanteric fracture femur in elderly patients: a retrospective study

    Background: An unstable intertrochanteric fracture in osteoporotic elderly patients presents a very challenging problem to the surgeons. This category of patients if kept in bed for long tends to have many complications like deep vein thrombosis, pneumonia, bed sores, etc. Surgeon’s worldwide face difficulty in getting a good anatomical reduction and do not allow early mobility to these patients because of risk of loss of reduction and implant cut out. The purpose of our study is to study the usefulness of arthroplasty in these patients in terms of better functional outcomes and no increase in complication rates.Methods: It was a retrospective study involving 25 patients (17 females, 8 males) with mean age of 76±2.3 years having intertrochanteric femur fractures (AO type A 2.2 and above) with osteoporosis operated upon with bipolar hemiarthroplasty at IQ City Medical College and NM Hospital between January 2015 and January 2017.Results: All the patients were followed up for a period of 1 year. We had a mean operative time of 75 min (range 55-125 min) with an average blood loss of 450 ml. 4 of our patients required postoperative blood transfusion. All patients walked on 2nd postoperative day. We had a mean Harris hip score of 82 and mean VAS of 1 at the end of 1 year.Conclusions: Hemiarthroplasty done in cases of unstable intertrochanteric femur fractures in elderly patients with osteoporotic bones allows early weight bearing thus improves the final functional outcomes. Further randomized trials are required before deriving any conclusions

    Understanding of effects of potassium on cardiac tissue by medical students: a critical appraisal

    Potassium (K+) is one of the most important ion present in the human body and involved in numerous physiological activities. It mainly affects heart and skeletal muscle but the effects are not confined to theses organs only. The article precisely focuses on the explaining the physiological as well as pathological aspects of potassium on cardiac tissue. This article tends to explain: The cause of difference in extra cellular and Intra cellular concentration of potassium when potassium channels are open in resting conditions, why are Purkinje fibers and ectopic tissue are more sensitive to effect of potassium, mechanisms responsible for increased action potential duration by hypokalemia and decreased action potential duration by hyperkalemia. Hypokalemia generates ectopic activities and hyperkalemia inhibits them, therapeutic effects of potassium administration without causing hyperkalemia. These issues will be discussed and try to be explained with the help of Ohm’s law , Nernst equation and Nernst potential to sort out the complicated actions of potassium on cardiac tissue in a simplified manner. The primary aim of article is to improve understanding of potassium physiology by medical graduates, secondarily, convey message regarding improvement in teaching methodology in Pharmacology for the benefit of new generations
