    Design of MW-Class Coaxial Gyrotron Cavities With Mode-Converting Corrugation Operating at the Second Cyclotron Harmonic

    This article presents investigations on the design of coaxial gyrotron cavities with mode-converting corrugations, operating at the second harmonic of the electron cyclotron frequency with output power of the order of megawatts. The suppression of the competing modes interacting at the fundamental cyclotron frequency is achieved by the combination of a corrugated coaxial insert and mode-converting corrugation on the outer wall. The outer corrugation couples the key competing modes to lower order modes with reduced quality factor. The design steps, which form a generally applicable design procedure, are described in detail. As an illustrative example, the proposed procedure is used for the design of a cavity for a fusion-relevant, second-harmonic MW-class gyrotron, operating at 170 GHz with the TE 37,1837,18 mode. From the simulations, it is found that for the proposed design, this mode is excited with an output power of around/ ∼ 1.5 MW. Two additional paths for cavity optimization toward even higher output power are also presented

    Late Holocene Hydro-Climate Variability in the Eastern Mediterranean: A Spatial Multi-Proxy Approach

    A total of thirteen (13) paleoclimatic coastal and hinterland archives of the broader eastern Mediterranean region were collected and examined statistically in search of underlying trends for the period 2800 to 200 BP. For each archive, a proxy record representative of hydro-climatic changes was selected, normalized using z-factors to facilitate intercomparison, and analyzed statistically. Multivariate statistical analysis was performed using a clustering analysis (HCA) and dimension reduction (PCA), which led to groupings of similar records temporally, and allowed the identification of spatially underlying modes of variability. Two main modes of variability were identified, further supporting complex trajectories of paleoclimatic evolution in the region. The first mode was identified for sites presenting a trend from a wetter to an overall drier phase, with respective changes at major phase shifts at 1400 BP and 1100 BP. All sites were from the southern and northern Balkan region, as well as southwestern Turkey. A contrasting dry to wet trend was identified for a site in the Peloponnese (Greece) and the Levant, with a major phase shift at around 750 BP. The inclusion of different proxies from very different environmental settings and the 200-year window has complicated the connection of established short-term climatic events to the study’s findings

    In recent years, the importance of the quality in health sector is more and more acknowledged. In broader terms the literature on quality in health care contributed to this, and stressed the importance of meeting the needs of the service recipient. Safety and quality in health care constitute a multi-dimensional parameter and involve many factors and various resources. Safety is positively associated to the quality, as the existence of the one ensures the improvement of the other. Thus, there is a great effort to create a framework, through guidelines and instructions that could contribute to the protection and development of quality and safety. It is important that this framework includes many features that have been expressed as requests by the patients themselves and which can contribute to the development of realistic and effective recommendations for improvement. Greek reality reveals certain gaps in safety and quality of services delivered, so the main attention has to be focused on developing an integral national health policy; the development of guidelines and the appropriate evaluation of their implementation could be a first effective approach. Formulating an institutional framework about safety and quality in health sector should be incorporated in the culture of all health organizations. To this end, the involvement of health professionals is a vital and strategic point. Health care practitioners should incorporate safety and quality culture in their daily routine and health managers should enact efficient ways of evaluation and control mechanisms in order to achieve better outcomes. Motivation to this direction and active participation should be encouraged with positive approaches, away from any kind of sanction. Any mistakes, adverse effects and deviations should be identified, reported, analyzed and formulate the base of the corrective action. In conclusion, safety and quality in health sector are essential and strongly associated with health services users’ satisfaction and quality of life; thus stakeholders and health professionals should focus on these basic conditions in order to achieve the “Health for All” goals in a national, international and universal perspectiv

    Evaluation of Blood Biomarkers and Parameters for the Prediction of Stroke Survivors’ Functional Outcome upon Discharge Utilizing Explainable Machine Learning

    Despite therapeutic advancements, stroke remains a leading cause of death and long-term disability. The quality of current stroke prognostic models varies considerably, whereas prediction models of post-stroke disability and mortality are restricted by the sample size, the range of clinical and risk factors and the clinical applicability in general. Accurate prognostication can ease post-stroke discharge planning and help healthcare practitioners individualize aggressive treatment or palliative care, based on projected life expectancy and clinical course. In this study, we aimed to develop an explainable machine learning methodology to predict functional outcomes of stroke patients at discharge, using the Modified Rankin Scale (mRS) as a binary classification problem. We identified 35 parameters from the admission, the first 72 h, as well as the medical history of stroke patients, and used them to train the model. We divided the patients into two classes in two approaches: “Independent” vs. “Non-Independent” and “Non-Disability” vs. “Disability”. Using various classifiers, we found that the best models in both approaches had an upward trend, with respect to the selected biomarkers, and achieved a maximum accuracy of 88.57% and 89.29%, respectively. The common features in both approaches included: age, hemispheric stroke localization, stroke localization based on blood supply, development of respiratory infection, National Institutes of Health Stroke Scale (NIHSS) upon admission and systolic blood pressure levels upon admission. Intubation and C-reactive protein (CRP) levels upon admission are additional features for the first approach and Erythrocyte Sedimentation Rate (ESR) levels upon admission for the second. Our results suggest that the said factors may be important predictors of functional outcomes in stroke patients

    Cytotoxic effect of menadione and sodium orthovanadate in combination on human glioma cells

    Gliomas are the most common primary brain tumor, and their treatment is still a challenge. Here, we evaluated the antiproliferative effect of a novel combination of two potent oxidative stress enhancers: menadione (M) and sodium orthovanadate (SO). We observed both short-term and prolonged growth inhibitory effects of M or SO alone as well as in combination (M:SO) on DBTRG.05MG human glioma cells. A stronger antiproliferative effect was observed in the short-term proliferation assay with the M:SO combination compared to either investigated agent alone. In the long-term proliferation assay, a 10-day exposure to M:SO at concentrations of 10 mu M:17.5 mu M or 17.5 mu M:10 mu M was enough to kill 100% of the cells; no cell regrowth was observed after re-incubation in drug-free media. When used in combination, the single concentration of M and SO could be decreased by 2.5- to 5-fold of those used for each experimental drug alone and still obtain a similar antiproliferative effect. The underlying molecular mechanism was investigated by co-incubating M:SO with dithiothreitol (DTT) and genistein. Both substances partially neutralized the effects of the M:SO combination, showing additive effects. This observation suggests a role of oxidative stress and tyrosine kinase stimulation in the M:SO cytotoxic effect. Our results indicate that M:SO combination is an attractive alternative for glioma treatment that encourages further study. The neutralizing effects of genistein and DTT reveal a possibility for their use in the minimization of potential M:SO systemic toxicity
