Is the Death Instinct Silent or Clinically Relevant? From Freud’s Concept of a Silent Death Instinct to Understanding Its Clinical Manifestations

When Freud introduced his concept of the death instinct in Beyond the Pleasure Principle (1920) he solved three theoretical problems which could not be explained by the one drive theory: masochism, repetition compulsion and the negative therapeutic reaction. The concept of two inherently opposed instincts remained one of the most controversial parts of Freud’s theory. For Melanie Klein, Freud’s idea of the death instinct was a powerful instrument in solving her greatest problems of integrating her clinical evidence of an earlier, very harsh superego. In Freud’s account, the superego was the manifestation at birth of the death instinct operating in destructiveness towards the person, as he had argued. In this way, Klein put – as Hinshelwood claims – clinical “flesh on the bones of Freud’s theory of the death instinct.” I will describe the development of Freud’s theory and how this was elaborated by Klein and her followers Bion, Esther Bick, Segal and Rosenfeld. With three clinical vignettes--from an Infant Observation, a child analysis and an adult analysis--the clinical use of the concept will be illustrated

Aspects psychanalytiques de la grossesse en tant que transformation du monde intérieur des futurs parents

Ich beschäftige mich mit den psychoanalytischen Aspekten der Auswirkung der Schwangerschaft auf die innere Welt der Eltern - der Mutter und dem Vater als ganze Personen und nicht als Objekte der kindlichen Phantasie. In der psychoanalytischen Tradition nach S. Freud,M. Klein und W. Bion wird die innere Realität der Eltern und die Umgestaltung deren innerer Objekte als bewusste und unbewusste Vorbereitung auf die Elternschaft betrachtet. Es wird untersucht, wie die emotionale Besetzung des Körpers und die archaischen Bilder des mütterlichen Körpers das Bild des „imaginären Babys“ beeinflussen. In the tradition of S. Freud, M. Klein and W. Bion we look at the “inner reality" of the parents and the reorganisation of the inner objects as a conscious and unconscious preparation for parenthood. Their cathexis of the body and archaic images of their mothers' body will influence their image of the “imaginary baby". Dans la tradition psychanalytique établie par Freud, Melanie Klein et Wilfried Bion, on saisit la réalité intérieure des parents et la transformation de leurs objets psychiques comme une préparation consciente et inconsciente au statut de parents. Dans ce sens, la grossesse est considérée comme un tournant dans la vie psychique, comme un « point de nonretour», qui devient réalité même si le bébé ne vient pas au monde ou si la grossesse est interrompue par une fausse-couche ou suite à une décision des parents. Dans mon travail, j'examine comment l'investissement affectif du corps et les images archaïques du corps maternel influencent l'image que les parents se font de leur « bébé imaginaire ». Des peurs archaïques sont constellées, dont la forme dépend des fantasmes enfantins sur le corps maternel et sur la reproduction qu'avaient les futurs parents durant leur propre enfance. Je traite de l'identification de l'enfant avec les parents car c'est ce mécanisme qui produit le désir d'avoir des enfants. La grossesse est une « phase transitoire », de manière similaire à l'espace transitoire conceptualisé par Winnicott ; elle est décrite en tant que période de préparation à la transformation du monde intérieur, durant laquelle des fantasmes enfantins de type oral, anal et phallique peuvent être analysés par le biais d'images et de rêves. La gestion de ces processus intérieurs est une pierre de touche car elle est en rapport direct avec les capacités psychiques de l'homme et de la femme. Ceux qui craignent une malformation de l'embryon ou qui ont peur de perdre l'enfant et ceux qui ont peur d'être détruits ou « abîmés » par le bébé sont ceux qui entretiennent inconsciemment des fantasmes de punition. Lorsque les futurs parents ont entretenu des rapports affectueux avec leurs propres parents et qu'ils ont été suffisamment protégés et aimés dans leur enfance, ils disposent d'une base qui leur permet d'être confiants qu'ils seront capables d'assumer leurs responsabilités parentales. L'embryon et son développement en cours de grossesse est aussi porteur d'espoir pour les futurs parents, dans le sens où ils vont pouvoir réaliser leur désir d'immortalité.Des cas exemplaires sont présentés, concernant deux couples et leurs expériences durant la grossesse, ainsi qu'après la naissance de l'enfant. Ces récits sont analysés, tout comme celui (autobiographique) d'une mère ayant perdu son bébé. Ces données sont extraites d'une étude psychanalytique plus large concernant les effets de l'observation des nouveaux-nés, telle qu'elle fut pratiquée à la Tavistock Clinic de Londres (Esther Bick). Les parents de deux enfants racontent la manière en priorité positive dont ils ont vécu la grossesse et l'influence de cette dernière sur leur relation ; ils parlent aussi de leurs espoirs et de leurs craintes. Concernant deux exemples de grossesses sources de conflits, certaines émotions et tensions intérieures sont décrites; dans un cas, il y a eu fausse-couche, dans l'autre, homicide - les motifs ayant conduit à ce dernier sont décrits. Pour terminer, je reviens sur le fait qu'il est important que les médecins et les sages-femmes aient une attitude empathique par rapport à la future mère; ce faisant, ils lui offrent un «containment» qui peut représenter un soutien considérable
