    Traitement par alcoolisation des kystes biliaires. Expérience personnelle sur 13 cases

    But de l’étude: Evaluer si l’alcoolisation percutanée, technique introduite depuis une dizaine d’années dans le traitement palliatif du carcinome hépatocellulaire inopérable, peut être utilisée à titre curatif dans les kystes biliaires avec de bons résultats, tout en évitant l’intervention chirurgicale. Matériel et méthodes: Pour l’étude, ont été observés 13 patientes (M 4; F 9 – age 38-71, moyenne 54 ans) qu’ils étaient symptomatiques. Tous les 13 malades ont été traités par alcoolisation par voie percutanée sous contrôle échographique. Résultats: L’amélioration de la technique et l’encadrement des protocoles permettent aujourd’hui de considérer l’alcoolisation percutanée comme un bon traitement du patient symptomatique seule indication à ce dernier. Conclusion: La simplicité d’exécution, le bas coût et le faible nombre de complications ont fait de l’alcoolisation percutanée le traitement de choix chez les patients porteurs de kystes biliaires

    Image analytics and machine learning for in-situ defects detection in Additive Manufacturing

    In the context of Industry 4.0, metal Additive Manufacturing (AM) is considered a promising technology for medical, aerospace and automotive fields. However, the lack of assurance of the quality of the printed parts can be an obstacle for a larger diffusion in industry. To this date, AM is most of the times a trial-and-error process, where the faulty artefacts are detected only after the end of part production. This impacts on the processing time and overall costs of the process. A possible solution to this problem is the in-situ monitoring and detection of defects, taking advantage of the layer-by-layer nature of the build. In this paper, we describe a system for in-situ defects monitoring and detection for metal Powder Bed Fusion (PBF), that leverages an off-axis camera mounted on top of the machine. A set of fully automated algorithms based on Computer Vision and Machine Learning allow the timely detection of a number of powder bed defects and the monitoring of the object's profile for the entire duration of the build

    Metal Additive Manufacturing (AM) is a pillar of the Industry 4.0, with many attractive advantages compared to traditional subtractive fabrication technologies. However, there are many quality issues that can be an obstacle for mass production. The in-situ camera-based monitoring and detection of defects, taking advantage of the layer-by-layer nature of the build, can be an effective solution to this problem. In this context, the use of Computer Vision and Machine Learning algorithms have a very important role. Nonetheless, they are up to this date limited by the scarcity of data for the training, as well as by the difficulty of accessing and integrating the AM process data throughout the fabrication. To tackle this problem, this paper proposes a system for in-situ monitoring that analyses images from an off-axis camera mounted on top of the machine to detect the arising defects in real-time, with automated generation of synthetic images based on Generative Adversarial Network (GAN) for dataset augmentation purposes. The computing functionalities are embedded into a holistic distributed AM platform allowing the collection, integration and storage of data at all stages of the AM pipeline

    Interleukin-10 polymorphisms in Spanish IgA deficiency patients: a case-control and family study

    BACKGROUND: IgA deficiency (IgAD) is the most common primary immunodeficiency in Caucasians. Genetic and environmental factors are suspected to be involved in the development of the disease. Interleukin-10 (IL-10) is a cytokine with stimulatory activity on immunoglobulin production and it may be an important regulator in IgAD pathogenesis. The IL-10 gene contains several single nucleotide polymorphisms (SNPs) and two polymorphic microsatellites located in the 5'-flanking region. Our aim was to ascertain if any of these polymorphic markers are associated or linked to IgAD in Spanish patients. METHODS: We genotyped 278 patients with IgAD and 573 ethnically matched controls for the microsatellites IL-10R and IL-10G and for three single nucleotide polymorphisms at positions -1082, -819 and -592 in the proximal promoter of the gene. We also included in this study the parents of 194 patients in order to study the IL-10 haplotypes transmitted and not transmitted to the affected offspring. RESULTS: The only allele where a significant difference was observed in the comparison between IgA deficiency patients and controls was the IL-10G12 allele (OR = 1.58 and p = 0.021). However, this p value could not withstand a Bonferroni correction. None of the IL-10R or promoter SNP alleles was found at a different frequency when patients were compared with controls. CONCLUSION: Our data do not show any significant difference in IL-10 polymorphism frequencies between control and IgAD patient samples. Their haplotype distribution among patients and controls was also equivalent and therefore these microsatellites and SNPs do not seem to influence IgAD susceptibility

    Choline PET/CT in recurrent prostate cancer

    PurposeBiochemical recurrence (BR) occurs in up to 40% of patients with prostate cancer (PCa) treated with primary radical prostatectomy (RP). Choline PET/CT may show, in a single-step examination, the site of tumor recurrence earlier than traditional imaging methods, particularly at low prostate-specific antigen (PSA) levels, thus influencing subsequent treatment. Methods/patientsPatients with recurrent and non-metastatic prostate cancer (nmPCa), who were assessed with choline PET/CT, were included in the analysis. Based on imaging results, the following therapeutic strategies were chosen: radiotherapy to the prostatic bed, androgen deprivation therapy (ADT), and chemotherapy or stereotactic body radiotherapy (SBRT) to either the pelvic lymph nodes or distant metastases. We assessed the impact of age, PSA levels, Gleason score (GS), and adjuvant therapy on oncological outcomes.ResultsData from 410 consecutive nmPCa patients with BR who underwent RP as primary treatment were analyzed. One hundred seventy-six (42.9%) patients had a negative choline PET/CT, and 234 (57.1%) patients resulted positive. In the multivariate analysis, only chemotherapy and PSA at recurrence were significant independent prognostic factors on overall survival (OS). In the PET-positive subgroup, the number of relapses, PSA post-prostatectomy, and chemotherapy impacted on OS. PSA (post-surgery and at recurrence) affected progression-free survival (PFS) in the univariate analysis. In the multivariate analysis, GS, the number of relapse sites, and PSA (post-surgery and at recurrence) were significant prognostic factors for disease-free survival (DFS).ConclusionCholine PET/CT provides better accuracy than conventional imaging for the assessment of nmPCa with BR after prostatectomy, thereby enabling salvage strategies and improving quality of life
