    Enhancing Credibility of Chemical Safety Studies: Emerging Consensus on Key Assessment Criteria

    Objectives: We examined the extent to which consensus exists on the criteria that should be used for assessing the credibility of a scientific work, regardless of its funding source, and explored how these criteria might be implemented. Data sources: Three publications, all presented at a session of the 2009 annual meeting of the Society for Risk Analysis, have proposed a range of criteria for evaluating the credibility of scientific studies. At least two other similar sets of criteria have recently been proposed elsewhere. Data extraction/synthesis: In this article we review these criteria, highlight the commonalities among them, and integrate them into a list of 10 criteria. We also discuss issues inherent in any attempt to implement the criteria systematically. Con c l u s i o n s: Recommendations by many scientists and policy experts converge on a finite list of criteria for assessing the credibility of a scientific study without regard to funding source. These criteria should be formalized through a consensus process or a governmental initiative that includes discussion and pilot application of a system for reproducibly implementing them. Formal establishment of such a system should enable the debate regarding chemical studies to move beyond funding issues and focus on scientific merit

    Household out-of-pocket medical expenditures and national health insurance in Taiwan: income and regional inequality

    BACKGROUND: Unequal geographical distribution of medical care resources and insufficient healthcare coverage have been two long-standing problems with Taiwan's public health system. The implementation of National Health Insurance (NHI) attempted to mitigate the inequality in health care use. This study examines the degree to which Taiwan's National Health Insurance (NHI) has reduced out-of-pocket medical expenditures in households in different regions and varying levels of income. METHODS: Data used in this study were drawn from the 1994 and 1996 Surveys of Family Income and Expenditure. We pooled the data from 1994 and 1996 and included a year dummy variable (NHI), equal to 1 if the household data came from 1996 in order to assess the impact of NHI on household out-of-pocket medical care expenditures shortly after its implementation in 1995. RESULTS: An individual who was older, female, married, unemployed, better educated, richer, head of a larger family household, or living in the central and eastern areas was more likely to have greater household out-of-pocket medical expenditures. NHI was found to have effectively reduced household out-of-pocket medical expenditures by 23.08%, particularly for more affluent households. With the implementation of NHI, lower and middle income quintiles had smaller decreases in out-of-pocket medical expenditure. NHI was also found to have reduced household out-of-pocket medical expenditures more for households in eastern Taiwan. CONCLUSION: Although NHI was established to create free medical care for all, further effort is needed to reduce the medical costs for certain disadvantaged groups, particularly the poor and aborigines, if equality is to be achieved

    The Boundaries of Justice: The Challenges of Environmental Justice Assessments for Transportation Projects

    Over recent decades, federal guidelines for transportation projects have required increasing attention to impacts on communities. Executive Order 12898 requires federal agencies to conduct environmental justice (EJ) assessments to determine if negative effects from projects will fall disproportionately on minority or low-income populations. Yet transportation agencies have not given specific guidance on the method for conducting such assessments. Therefore practitioners and researchers apply a variety of analytical techniques. This paper uses a case study of a planned road widening project in Daytona Beach, Florida, to compare the various methods currently used in EJ assessments. The choice of reference area and of method for determining the decision threshold for a finding of disproportionality are shown to have important implications for the outcome of an assessment. Because the spatial distribution of racial/ethnic and low-income groups will vary widely from place to place, practitioners and transportation agencies should not decide on the precise method, but carefully consider the characteristics and distribution of the data being used and select the method that most fairly represents the data distribution. Conducting genuine EJ assessments is not only required by federal regulations, but can head off conflicts, better reveal the true costs of projects, and allow for more equitable distribution of costs and benefits by better targeting mitigation efforts.     Defamilisation/familisation measures and pensions in Hong Kong and Taiwan

    This article discusses the link between familisation measures (to lower the negative consequences of participating in the family as a care-provider) and defamilisation measures (to reduce individual responsibility for providing care in the family), and pensions for women. To enhance women’s chance of having a secure retirement life, it makes two suggestions: government should provide defamilisation measures to assist women to accumulate pension income through work-based pension measures; and government should provide familisation measures extensively as an alternative to these measures. It also demonstrates how the case examples of Hong Kong and Taiwan provide support to these two suggestions
