Opportunities for primary and secondary prevention of excess gestational weight gain: General Practitioners' perspectives
BackgroundThe impact of excess gestational weight gain (GWG) on maternal and child health outcomes is well documented. Understanding how health care providers view and manage GWG may assist with influencing healthy gestational weight outcomes. This study aimed to assess General Practitioner\u27s (GPs) perspectives regarding the management and assessment of GWG and to understand how GPs can be best supported to provide healthy GWG advice to pregnant women.MethodsDescriptive qualitative research methods utilising semi - structured interview questions to assess GPs perspectives and management of GWG. GPs participating in shared antenatal care in Geelong, Victoria and Sydney, New South Wales were invited to participate in semi - structured, individual interviews via telephone or in person. Interviews were digitally recorded and transcribed verbatim. Data was analysed utilising thematic analysis for common emerging themes.ResultsTwenty eight GPs participated, 14 from each state. Common themes emerged relating to awareness of the implications of excess GWG, advice regarding weight gain, regularity of gestational weighing by GPs, options for GPs to seek support to provide healthy lifestyle behaviour advice and barriers to engaging pregnant women about their weight. GPs perspectives concerning excess GWG were varied. They frequently acknowledged maternal and child health complications resulting from excess GWG yet weighing practices and GWG advice appeared to be inconsistent. The preferred support option to promote healthy weight was referral to allied health practitioners yet GPs noted that cost and limited access were barriers to achieving this.ConclusionsGPs were aware of the importance of healthy GWG yet routine weighing was not standard practice for diverse reasons. Management of GWG and perspectives of the issue varied widely. Time efficient and cost effective interventions may assist GPs in ensuring women are supported in achieving healthy GWG to provide optimal maternal and infant health outcomes.<br /
Space Telescope and Optical Reverberation Mapping Project. V. Optical Spectroscopic Campaign and Emission-line Analysis for NGC 5548
We present the results of an optical spectroscopic monitoring program targeting NGC 5548 as part of a larger multiwavelength reverberation mapping campaign. The campaign spanned 6 months and achieved an almost daily cadence with observations from five ground-based telescopes. The Hβ and He ii Îť4686 broad emission-line light curves lag that of the 5100 +-optical continuum by 4.17+0.36-0.36 and 0.79+0.35-0.34 days, respectively. The Hβ lag relative to the 1158 ultraviolet continuum light curve measured by the Hubble Space Telescope is âź50% longer than that measured against the optical continuum, and the lag difference is consistent with the observed lag between the optical and ultraviolet continua. This suggests that the characteristic radius of the broad-line region is âź50% larger than the value inferred from optical data alone. We also measured velocity-resolved emission-line lags for Hβ and found a complex velocity-lag structure with shorter lags in the line wings, indicative of a broad-line region dominated by Keplerian motion. The responses of both the Hβ and He ii emission lines to the driving continuum changed significantly halfway through the campaign, a phenomenon also observed for C iv, LyÎą, He ii(+O iii]), and Si iv(+O iv]) during the same monitoring period. Finally, given the optical luminosity of NGC 5548 during our campaign, the measured Hβ lag is a factor of five shorter than the expected value implied by the R BLR-L AGN relation based on the past behavior of NGC 5548
Space Telescope and Optical Reverberation Mapping Project. VII. Understanding the Ultraviolet Anomaly in NGC 5548 with X-Ray Spectroscopy
During the Space Telescope and Optical Reverberation Mapping Project observations of NGC 5548, the continuum and emission-line variability became decorrelated during the second half of the six-month-long observing campaign. Here we present Swift and Chandra X-ray spectra of NGC 5548 obtained as part of the campaign. The Swift spectra show that excess flux (relative to a power-law continuum) in the soft X-ray band appears before the start of the anomalous emission-line behavior, peaks during the period of the anomaly, and then declines. This is a model-independent result suggesting that the soft excess is related to the anomaly. We divide the Swift data into on- and off-anomaly spectra to characterize the soft excess via spectral fitting. The cause of the spectral differences is likely due to a change in the intrinsic spectrum rather than to variable obscuration or partial covering. The Chandra spectra have lower signal-to-noise ratios, but are consistent with the Swift data. Our preferred model of the soft excess is emission from an optically thick, warm Comptonizing corona, the effective optical depth of which increases during the anomaly. This model simultaneously explains all three observations: the UV emission-line flux decrease, the soft-excess increase, and the emission-line anomaly
Space Telescope and Optical Reverberation Mapping Project. IX. VelocityâDelay Maps for Broad Emission Lines in NGC 5548
In this contribution, we achieve the primary goal of the active galactic nucleus (AGN) STORM campaign by recovering velocityâdelay maps for the prominent broad emission lines (LyÎą, C iv, He ii, and Hβ) in the spectrum of NGC 5548. These are the most detailed velocityâdelay maps ever obtained for an AGN, providing unprecedented information on the geometry, ionization structure, and kinematics of the broad-line region. Virial envelopes enclosing the emission-line responses show that the reverberating gas is bound to the black hole. A stratified ionization structure is evident. The He ii response inside 5â10 lt-day has a broad single-peaked velocity profile. The LyÎą, C iv, and Hβ responses extend from inside 2 to outside 20 lt-day, with double peaks at Âą2500 km sâ1 in the 10â20 lt-day delay range. An incomplete ellipse in the velocityâdelay plane is evident in Hβ. We interpret the maps in terms of a Keplerian disk with a well-defined outer rim at R = 20 lt-day. The far-side response is weaker than that from the near side. The line-center delay days gives the inclination i â 45°. The inferred black hole mass is MBH â 7 Ă 107 Mâ. In addition to reverberations, the fit residuals confirm that emission-line fluxes are depressed during the "BLR Holiday" identified in previous work. Moreover, a helical "Barber-Pole" pattern, with stripes moving from red to blue across the C iv and LyÎą line profiles, suggests azimuthal structure rotating with a 2 yr period that may represent precession or orbital motion of inner-disk structures casting shadows on the emission-line region farther out
Osmotic Release Oral System Methylphenidate is More Effective Than Immediate Release Methylphenidate: A Retrospective Chart Review in Turkish Children with Attention Deficit Hyperactivity Disorder
WOS: 000348334900006Objective: The aim of this study was to evaluate the efficacy and safety of osmotic release oral system methylphenidate (OROS-MPH) compared with immediate release methylphenidate (IR-MPH) in Turkish children with attention deficit hyperactivity disorder (ADHD). Method: The medical records of primary school-aged children, who were first-time referrals to the outpatient clinic, were reviewed; 67 children receiving OROS-MPH and 47 children receiving IR-MPH were recruited for the study. A total of 114 children receiving treatment for ADHD were evaluated over 8 weeks. Results: The total Turgay DSM-IV Based Child and Adolescent Behavior Disorders Screening and Rating Scale scores from both the parent and teacher forms decreased significantly in both groups over 8 weeks (p<0.001). OROS-MPH was found to be superior to IR-MPH when comparing baseline-to-8th-week- mean inattention score changes on both the teacher (p=0.007) and parent (p=0.015) forms. OROS-MPH and IR-MPH were both well tolerated, with similar side-effect profiles. Conclusion:OROS-MPH was found to be effective and safe in the treatment of ADHD symptoms in Turkish children
