    Differential and Joint Effects of Metformin and Statins on Overall Survival of Elderly Patients with Pancreatic Adenocarcinoma: A Large Population-Based Study.

    Background: Published evidence indicates that individual use of metformin and statin is associated with reduced cancer mortality. However, their differential and joint effects on pancreatic cancer survival are inconclusive.Methods: We identified a large population-based cohort of 12,572 patients ages 65 years or older with primary pancreatic ductal adenocarcinoma (PDAC) diagnosed between 2008 and 2011 from the Surveillance, Epidemiology, and End Results (SEER)-Medicare-linked database. Exposure to metformin and statins was ascertained from Medicare Prescription Drug Event files. Cox proportional hazards models with time-varying covariates adjusted for propensity scores were used to assess the association while controlling for potential confounders.Results: Of 12,572 PDAC patients, 950 (7.56%) had used metformin alone, 4,506 (35.84%) had used statin alone, and 2,445 (19.45%) were dual users. Statin use was significantly associated with improved overall survival [HR, 0.94; 95% confidence interval (CI), 0.90-0.98], and survival was more pronounced in postdiagnosis statin users (HR, 0.69; 95% CI, 0.56-0.86). Metformin use was not significantly associated with overall survival (HR, 1.01; 95% CI, 0.94-1.09). No beneficial effect was observed for dual users (HR, 1.00; 95% CI, 0.95-1.05).Conclusions: Our findings suggest potential benefits of statins on improving survival among elderly PDAC patients; further prospective studies are warranted to corroborate the putative benefit of statin therapy in pancreatic cancer.Impact: Although more studies are needed to confirm our findings, our data add to the body of evidence on potential anticancer effects of statins.     Effects of technology driven pedagogy applications on the comprehension of complex and abstract concepts of chemical equilibrium

    Chemistry students often regarded abstract and complex Chemistry concepts as difficult to learn (1; 2) and this prevents many of them from continuing studies in pure chemistry courses. This study investigated how students develop their comprehension of complex topics of chemistry with the aid of Technology Driven Pedagogy (TDP) by using animation, simulation and video integrated with student-centered learning in visualizing complex and abstract concepts of chemistry. TDP technique in the chemistry classroom was used to promote effective learning. A chemist can view a chemical reaction at the macroscopic level, what the reaction will look like to the student’s eye, and at the particulate level, what changes are taking place among the particles. TDP is used to create mental images and displays on the macroscopic level. It also enables students to present information in a more dynamic, compelling and interactive way, engaging the environment prepared for learning by doing. To apply TDP in supporting student- centered learning activities, one week training on how to operate the computer with the experimental software was carried out on the experimental students of this study. This was followed by four weeks teaching and learning of chemical equilibrium topics for first-year undergraduate students with art technology flash and micro media player for 15 male and 15 female students who were conveniently grouped into control and experimental groups. A particular focus group of this investigation was used to determine to what extent the use of TDP can influence learning in Chemistry. This study employed sequential embedded mixed case study design. Pre and Post tests were administrated to the target groups. The quality of the learning strategy was evaluated by administrating open and closed ended questions containing schedule, focused group discussion, and observations. The results obtained from the tests, participants` responses to the questionnaire schedule, focused group discussion and observation showed that TDP greatly improves the comprehension ability and performance of students in the subject. Therefore, TDP technique is a promising tool for simplifying and clarifying complex and abstract concepts of Chemistry and hence it is conclusively recommended by the researchers for a wider use.Institute for Science and Technology Education (ISTE

    Antiretroviral Therapy outcomes among adolescents and youth in rural Zimbabwe

    Around 2 million adolescents and 3 million youth are estimated to be living with HIV worldwide. Antiretroviral outcomes for this group appear to be worse compared to adults. We report antiretroviral therapy outcomes from a rural setting in Zimbabwe among patients aged 10-30 years who were initiated on ART between 2005 and 2008. The cohort was stratified into four age groups: 10-15 (young adolescents) 15.1-19 years (adolescents), 19.1-24 years (young adults) and 24.1-29.9 years (older adults). Survival analysis was used to estimate rates of deaths and loss to follow-up stratified by age group. Endpoints were time from ART initiation to death or loss to follow-up. Follow-up of patients on continuous therapy was censored at date of transfer, or study end (31 December 2008). Sex-adjusted Cox proportional hazards models were used to estimate hazard ratios for different age groups. 898 patients were included in the analysis; median duration on ART was 468 days. The risk of death were highest in adults compared to young adolescents (aHR 2.25, 95%CI 1.17-4.35). Young adults and adolescents had a 2-3 times higher risk of loss to follow-up compared to young adolescents. When estimating the risk of attrition combining loss to follow-up and death, young adults had the highest risk (aHR 2.70, 95%CI 1.62-4.52). This study highlights the need for adapted adherence support and service delivery models for both adolescents and young adults

    The associations between QCT-based vertebral bone measurements and prevalent vertebral fractures depend on the spinal locations of both bone measurement and fracture

    Summary We examined how spinal location affects the relationships between quantitative computed tomography (QCT)-based bone measurements and prevalent vertebral fractures. Upper spine (T4–T10) fractures appear to be more strongly related to bone measures than lower spine (T11–L4) fractures, while lower spine measurements are at least as strongly related to fractures as upper spine measurements. Introduction Vertebral fracture (VF), a common injury in older adults, is most prevalent in the mid-thoracic (T7–T8) and thoracolumbar (T12–L1) areas of the spine. However, measurements of bone mineral density (BMD) are typically made in the lumbar spine. It is not clear how the associations between bone measurements and VFs are affected by the spinal locations of both bone measurements and VF. Methods A community-based case–control study includes 40 cases with moderate or severe prevalent VF and 80 age- and sex-matched controls. Measures of vertebral BMD, strength (estimated by finite element analysis), and factor of risk (load:strength ratio) were determined based on QCT scans at the L3 and T10 vertebrae. Associations were determined between bone measures and prevalent VF occurring at any location, in the upper spine (T4–T10), or in the lower spine (T11–L4). Results Prevalent VF at any location was significantly associated with bone measures, with odds ratios (ORs) generally higher for measurements made at L3 (ORs = 1.9–3.9) than at T10 (ORs = 1.5–2.4). Upper spine fracture was associated with these measures at both T10 and L3 (ORs = 1.9–8.2), while lower spine fracture was less strongly associated (ORs = 1.0–2.4) and only reached significance for volumetric BMD measures at L3. Conclusions Closer proximity between the locations of bone measures and prevalent VF does not strengthen associations between bone measures and fracture.     Impact of Climate Change on the Hydrology of the Upper Awash River Basin, Ethiopia

    This study investigated the impacts of climate change on the hydrology of the Upper Awash Basin, Ethiopia. A soil and water assessment tool (SWAT) model was calibrated and validated against observed streamflow using SWAT CUP. The Mann–Kendall trend test (MK) was used to assess climate trends. Meteorological drought (SPEI) and hydrological drought (SDI) were also investigated. Based on the ensemble mean of five global climate models (GCMs), projected increases in mean annual maximum temperature over the period 2015–2100 (compared with a 1983–2014 baseline) range from 1.16 to 1.73 °C, while increases in minimum temperature range between 0.79 and 2.53 °C. Increases in mean annual precipitation range from 1.8% at Addis Ababa to 45.5% over the Hombole area. High streamflow (Q5) declines at all stations except Ginchi. Low flows (Q90) also decline with Q90 equaling 0 m3 s−1 (i.e., 100% reduction) at some gauging stations (Akaki and Hombole) for individual GCMs. The SPEI confirmed a significant drought trend in the past, while the frequency and severity of drought will increase in the future. The basin experienced conditions that varied from modest dry periods to a very severe hydrological drought between 1986 and 2005. The projected SDI ranges from modestly dry to modestly wet conditions. Climate change in the basin would enhance seasonal variations in hydrological conditions. Both precipitation and streamflow will decline in the wet seasons and increase in the dry seasons. These changes are likely to have an impact on agricultural activities and other human demands for water resources throughout the basin and will require the implementation of appropriate mitigation measures

    The Role of Environmental Factors in the Etiology of Nonsyndromic Orofacial Clefts

    Background: Nonsyndromic orofacial clefts (NSOFCs) represent the most common congenital anomalies in the head and neck region. Multiple factors contribute to the occurrence of this anomaly. The etiology of NSOFCs in the Ethiopian population has not been investigated prior to this study.Aims of the Study: To assess the role of maternal environmental factors in the occurrence of NSOFCs in the Ethiopian Population.Methods: The authors used unmatched case control study design and evaluated the role of environmental factors to the occurrence of NSOFCs in the Ethiopian population. The participants were recruited from the same institution (Yekatit 12 Hospital Medical College). The authors studied 760 mothers (359 mothers of children born with NSOFCs and 401 mothers of children born without any congenital anomalies). Univariate and multivariate logistic regression analyses were used to calculate relative risk by odds ratio and 95% confidence interval.Results: Mothers who gave history of bronchial asthma and mothers who were admitted for threatened abortion had a higher risk of delivering a child with NSOFCS P value=0.013; AOR=0.194, 95% CI [0.053-0.712], P value &lt;0.001; AOR= 0.179, 95% CI [0.091-0.352] respectively. Higher number of children with NSOFCs were born to mothers who were exposed to diagnostic X-ray investigation during early pregnancy than those who were not exposed P value 0.048; AOR=0.375, 95% CI [0.142-0.990].Conclusion: Maternal exposure to diagnostic x-ray, maternal chronic illness like bronchial asthma and threatened abortion were found to be associated with the occurrence of NSOFCS in the studied population.</p

    Relevance of B-Lines on Lung Ultrasound in Volume Overload and Pulmonary Congestion: Clinical Correlations and Outcomes in Patients on Hemodialysis.

    BACKGROUND: Volume overload in patients on hemodialysis (HD) is an independent risk factor for cardiovascular mortality. B-lines detected on lung ultrasound (BLUS) assess extravascular lung water. This raises interest in its utility for assessing volume status and cardiovascular outcomes. METHODS: End-stage renal disease patients on HD at the Island Rehab Center being older than 18 years were screened. Patients achieving their dry weight (DW) had a lung ultrasound in a supine position. Scores were classified as mild (0-14), moderate (15-30), and severe (\u3e30) for pulmonary congestion. Patients with more than 60 were further classified as very severe. Patients were followed for cardiac events and death. RESULTS: 81 patients were recruited. 58 were males, with a mean age of 59.7 years. 44 had New York Heart Association (NYHA) class 1, 24 had class 2, and 13 had class 3. In univariate analysis, NYHA class was associated with B-line classes ( CONCLUSION: At DW, BLUS is an independent risk factor for death and cardiovascular events in patients on HD
