    Changes in functional properties and 5-HT modulation above and below a spinal transection in lamprey.

    In addition to the disruption of neural function below spinal cord injuries (SCI), there also can be changes in neuronal properties above and below the lesion site. The relevance of these changes is generally unclear, but they must be understood if we are to provide rational interventions. Pharmacological approaches to improving locomotor function have been studied extensively, but it is still unclear what constitutes an optimal approach. Here, we have used the lamprey to compare the modulatory effects of 5-HT and lesion-induced changes in cellular and synaptic properties in unlesioned and lesioned animals. While analyses typically focus on the sub-lesion spinal cord, we have also examined effects above the lesion to see if there are changes here that could potentially contribute to the functional recovery. Cellular and synaptic properties differed in unlesioned and lesioned spinal cords and above and below the lesion site. The cellular and synaptic modulatory effects of 5-HT also differed in lesioned and unlesioned animals, again in region-specific ways above and below the lesion site. A role for 5-HT in promoting recovery was suggested by the potential for improvement in locomotor activity when 5-HT was applied to poorly recovered animals, and by the consistent failure of animals to recover when they were incubated in PCPA to deplete 5-HT. However, PCPA did not affect swimming in animals that had already recovered, suggesting a difference in 5-HT effects after lesioning. These results show changes in 5-HT modulation and cellular and synaptic properties after recovery from a spinal cord transection. Importantly, effects are not confined to the sub-lesion spinal cord but also occur above the lesion site. This suggests that the changes may not simply reflect compensatory responses to the loss of descending inputs, but reflect the need for co-ordinated changes above and below the lesion site. The changes in modulatory effects should be considered in pharmacological approaches to functional recovery, as assumptions based on effects in the unlesioned spinal cord may not be justified.This is the final version of the article.     Bayesian Sensitivity Analysis of Flight Parameters in a Hard-Landing Analysis Process

    A flight parameter sensor simulation model was developed to assess the conservatism of the landing gear component loads calculated using a typical hard-landing analysis process. Conservatism exists due to factors of safety that are incorporated into any hard-landing analysis process to account for uncertainty in the measurement of certain flight parameters. The flight parameter sensor simulation model consists of 1) an aircraft and landing gear dynamic model to determine the “actual” landing gear loads during a hard landing; 2) an aircraft sensor and data acquisition model to represent the aircraft sensors and flight data recorder systems to investigate the effect of signal processing on the flight parameters; and 3) an automated hard-landing analysis process, representative of that used by airframe and equipment manufacturers, to determine the “simulated” landing gear loads. Using a technique of Bayesian sensitivity analysis, a number of flight parameters are varied in the flight parameter sensor simulation model to gain an understanding of the sensitivity of the difference between actual and simulated loads to the individual flight parameters in symmetric and asymmetric two-point landings. This study shows that the error can be reduced by learning the true value of the following flight parameters: longitudinal tire–runway friction coefficient, aircraft vertical acceleration (related to vertical descent velocity), lateral acceleration (related to lateral velocity), Euler roll angle, mass, center of gravity position, and main landing gear tire type. It was also shown that, due to the modeling techniques used, shock absorber servicing state and tire pressure do not contribute significantly to the error

    Cost-Effectiveness of Adding Bed Net Distribution for Malaria Prevention to Antenatal Services in Kinshasa, Democratic Republic of the Congo

    We evaluated the cost-effectiveness of distributing insecticide-treated bed nets (ITNs) for malaria prevention at antenatal clinics in Kinshasa, Democratic Republic of the Congo. A decision tree model was used to estimate costs, outcomes, and incremental cost-effectiveness for 17,893 pregnant women attending 28 antenatal clinics who received long-lasting ITNs free of charge. Costs including purchase, transportation, storage, and distribution of ITNs were derived from program records. The ITN efficacy and other parameters were derived from peer-reviewed literature. Outcomes modeled included low birth weight (LBW) deliveries, infant deaths averted, life-years saved (LYs), and disability-adjusted life-years (DALYs) averted. Deterministic and probabilistic sensitivity analyses were conducted. For the 17,893 women in our program, ITN distribution would be expected to avert 587 LBW deliveries and 414 infant deaths. The incremental cost-effectiveness was US 17.22perDALYaverted(9517.22 per DALY averted (95% confidence interval [CI] = US 8.54-30.90),US30.90), US 15.70 per LY saved (95% CI = US 7.65−7.65-27.68), and US 411.13perinfantdeathaverted(95411.13 per infant death averted (95% CI = US 353.95-$1,085.89). If resources were constrained, the greatest benefit would be among women in their first through fourth pregnancies. Thus, ITN distribution is a cost-effective addition to antenatal services

    Brief comments on Jackiw-Teitelboim gravity coupled to Liouville theory

    Jackiw-Teitelboim gravity with non-vanishing cosmological constant coupled to Liouville theory is considered as a non-critical string on dd dimensional flat spacetime. It is discussed how the presence of cosmological constant yields additional constraints on the parameter space of the theory, even when the conformal anomaly is independent of the cosmological constant. Such constraints agree with the necessary conditions for the tachyon field to be a primary --prelogarithmic-- operator of the worldsheet conformal field theory. Thus, the linearized tachyon field equation allows to impose the diagonal condition for the interaction term. We analyze the neutralization of the Liouville mode induced by the coupling to the Jackiw-Teitelboim Lagrangian. The free field prescription leads to obtain explicit expressions for three-point correlation functions for the case of vanishing cosmological constant in terms of a product of Shapiro-Virasoro integrals.     The FIRST Bright Quasar Survey. II. 60 Nights and 1200 Spectra Later

    We have used the VLA FIRST survey and the APM catalog of the POSS-I plates as the basis for constructing a new radio-selected sample of optically bright quasars. This is the first radio-selected sample that is competitive in size with current optically selected quasar surveys. Using only two basic criteria, radio-optical positional coincidence and optical morphology, quasars and BL Lacs can be identified with 60% selection efficiency; the efficiency increases to 70% for objects fainter than magnitude 17. We show that a more sophisticated selection scheme can predict with better than 85% reliability which candidates will turn out to be quasars. This paper presents the second installment of the FIRST Bright Quasar Survey with a catalog of 636 quasars distributed over 2682 square degrees. The quasar sample is characterized and all spectra are displayed. The FBQS detects both radio-loud and radio-quiet quasars out to a redshift z>3. We find a large population of objects of intermediate radio-loudness; there is no evidence in our sample for a bimodal distribution of radio characteristics. The sample includes ~29 broad absorption line quasars, both high and low ionization, and a number of new objects with remarkable optical spectra.Comment: 41 pages plus 39 gifs which contain all quasar spectra.     GraphCombEx: A Software Tool for Exploration of Combinatorial Optimisation Properties of Large Graphs

    We present a prototype of a software tool for exploration of multiple combinatorial optimisation problems in large real-world and synthetic complex networks. Our tool, called GraphCombEx (an acronym of Graph Combinatorial Explorer), provides a unified framework for scalable computation and presentation of high-quality suboptimal solutions and bounds for a number of widely studied combinatorial optimisation problems. Efficient representation and applicability to large-scale graphs and complex networks are particularly considered in its design. The problems currently supported include maximum clique, graph colouring, maximum independent set, minimum vertex clique covering, minimum dominating set, as well as the longest simple cycle problem. Suboptimal solutions and intervals for optimal objective values are estimated using scalable heuristics. The tool is designed with extensibility in mind, with the view of further problems and both new fast and high-performance heuristics to be added in the future. GraphCombEx has already been successfully used as a support tool in a number of recent research studies using combinatorial optimisation to analyse complex networks, indicating its promise as a research software tool

    Streptococcus pneumoniae outbreaks and implications for transmission and control: a systematic review

    Abstract Background Streptococcus pneumoniae is capable of causing multiple infectious syndromes and occasionally causes outbreaks. The objective of this review is to update prior outbreak reviews, identify control measures, and comment on transmission. Methods We conducted a review of published S. pneumoniae outbreaks, defined as at least two linked cases of S. pneumoniae. Results A total of 98 articles (86 respiratory; 8 conjunctivitis; 2 otitis media; 1 surgical site; 1 multiple), detailing 94 unique outbreaks occurring between 1916 to 2017 were identified. Reported serotypes included 1, 2, 3, 4, 5, 7F, 8, 12F, 14, 20, and 23F, and serogroups 6, 9, 15, 19, 22. The median attack rate for pneumococcal outbreaks was 7.0% (Interquartile range: 2.4%, 13%). The median case-fatality ratio was 12.9% (interquartile range: 0%, 29.2%). Age groups most affected by outbreaks were older adults (60.3%) and young adults (34.2%). Outbreaks occurred in crowded settings, such as universities/schools/daycares, military barracks, hospital wards, and long-term care facilities. Of outbreaks that assessed vaccination coverage, low initial vaccination or revaccination coverage was common. Most (73.1%) of reported outbreaks reported non-susceptibility to at least one antibiotic, with non-susceptibility to penicillin (56.0%) and erythromycin (52.6%) being common. Evidence suggests transmission in outbreaks can occur through multiple modes, including carriers, infected individuals, or medical devices. Several cases developed disease shortly after exposure (< 72 h). Respiratory outbreaks used infection prevention (55.6%), prophylactic vaccination (63.5%), and prophylactic antibiotics (50.5%) to prevent future cases. PPSV23 covered all reported outbreak serotypes. PCV13 covered 10 of 16 serotypes. For conjunctival outbreaks, only infection prevention strategies were used. Conclusions To prevent the initial occurrence of respiratory outbreaks, vaccination and revaccination is likely the best preventive measure. Once an outbreak occurs, vaccination and infection-prevention strategies should be utilized. Antibiotic prophylaxis may be considered for high-risk exposed individuals, but development of antibiotic resistance during outbreaks has been reported. The short period between initial exposure and development of disease indicates that pneumococcal colonization is not a prerequisite for pneumococcal respiratory infection
