    Test of an empirical method for ozone detection in the stratosphere using two filtered broadband UV-meters

    We describe a simple method to detect significant changes of the total ozone column from global (diffuse and direct) zenith sky measurements taken at the Earth’s surface. The calculation of the total ozone column relies on measured irradiance at two wavelengths in the ultra violet part of the solar spectrum. One of these (i.e. 306 nm) are appreciable absorbed by ozone whereas the other (i.e. 360 nm) is not. The method provides measurements for clear as well as for cloudy sky conditions. The natural logarithm of the irradiance ratio at the two wavelengths, corrected for solar elevation dependence, is assumed to be proportional to the amount of ozone in the atmospheric column. It is assumed that the two wavelengths have same properties in the atmosphere excluding the impact of ozone. Therefore variations in atmospheric conditions should cancel out in the ratio. We found a strong correlation between our calculated quantity and ozone measurements at The Danish Meteorological Institute, DMI, Copenhagen, Denmark, which is approximately 30 km away from the measuring site. The correlation coefficient, R, from linear regression had the value 0.90, and the standard deviation, Sres, for the residuals were 10.6 DU (Dobson Units), and the mean value was 322 DU, obtained from every day point measurements during the Swedish summer, total 67 days

    Production of tumor necrosis factor-α and interleukin-6 in whole blood stimulated by live Gram-negative and Gram-positive bacteria

    ObjectiveTo investigate the production of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) induced by live Gram-negative and Gram-positive bacteria in whole blood in vitro.MethodsIn all, 49 different isolates were studied. Each of the 49 different isolates was incubated for 4 h with whole blood at a ratio of one monocyte per 1-5 bacteria. Plasma was then separated and frozen, and the concentrations of TNF-α and IL-6 were measured by enzyme immunoassays.ResultsThere was a positive correlation between TNF-α and IL-6 values, r= 0.9. Gram-negative bacteria induced higher levels of both TNF-α and IL-6 than Gram-positive bacteria. Group G streptococci (GGS) induced higher levels of TNF-α than Streptococcus pneumoniae, Staphylococcus aureus, Staphylococcus epidermidis and group A streptococci (GAS). Klebsiella pneumoniae induced higher levels of TNF-α than Haemophilus influenzae, Escherichia coli and Neisseria meningitidis. GGS induced higher levels of IL-6 than Staphylococcus epidermidis, Staphylococcus aureus and GAS. When the relative amounts of cytokine induced by the strains were compared to serum concentrations measured on admission in patients with bacteremia caused by the same bacterial isolates there was no significant correlation.ConclusionSpecies- and strain-related differences in cytokine-inducing properties were found which may have significance in clinical infections

    Construction of two large-size four-plane micromegas detectors

    We report on the construction and initial performance studies of two micromegas detector quadruplets with an area of 0.5 m2^2. They serve as prototypes for the planned upgrade project of the ATLAS muon system. Their design is based on the resistive-strip technology and thus renders the detectors spark tolerant. Each quadruplet comprises four detection layers with 1024 readout strips and a strip pitch of 415 ÎŒ\mum. In two out of the four layers the strips are inclined by ±\pm1.5∘^{\circ} to allow for the measurement of a second coordinate. We present the detector concept and report on the experience gained during the detector construction. In addition an evaluation of the detector performance with cosmic rays and test-beam data is given.Comment: 26 pages, 25 figure

    Contraception after pregnancy

    Whatever the outcome, pregnancy provides the opportunity to offer effective contraception to couples motivated to avoid another pregnancy. This narrative review summarizes the evidence for health providers, drawing attention to current guidelines on which contraceptive methods can be used, and when they should be started after pregnancy, whatever its outcome. Fertility returns within 1 month of the end of pregnancy unless breastfeeding occurs. Breastfeeding, which itself suppresses fertility after childbirth, influences both when contraception should start and what methods can be used. Without breastfeeding, effective contraception should be started as soon as possible if another pregnancy is to be avoided. Interpregnancy intervals of at least 6 months after miscarriage and 1‐2 years after childbirth have long been recommended by the World Health Organization in order to reduce the chance of adverse pregnancy outcome. Recent research suggests that this may not be necessary, at least for healthy women <35 years old. Most contraceptive methods can be used after pregnancy regardless of the outcome. Because of an increased risk of venous thromboembolism associated with estrogen‐containing contraceptives, initiation of these methods should be delayed until 6 weeks after childbirth. More research is required to settle the questions over the use of combined hormonal contraception during breastfeeding, the use of injectable progestin‐only contraceptives before 6 weeks after childbirth, and the use of both hormonal and intrauterine contraception after gestational trophoblastic disease. The potential impact on the risk of ectopic pregnancy of certain contraceptive methods often confuses healthcare providers. The challenges involved in providing effective, seamless service provision of contraception after pregnancy are numerous, even in industrialized countries. Nevertheless, the clear benefits demonstrate that it is worth the effort

    Clinical improvement and radiological progression in a girl with early onset scoliosis (EOS) treated conservatively – a case report

    BACKGROUND: ChĂȘneau-Brace treatment of a certain standard reduces the rate of surgery, prevents progression and in a certain patient population leads to marked improvement of Cobb angle and cosmetic appearance. During the last two years a patient refusing surgery with a double major curvature of initially 60° showed a clear cosmetic improvement and a clear radiological progression at the same time. The findings of this patient have been reviewed in order to find out how cosmetic appearance and Cobb angle can develop differently. METHODS: The patient entered conservative treatment at the age of 13 years, premenarchial with Tanner II and a Cobb angle of 60° thoracic and 59° lumbar. The angle of trunk rotation (ATR; Scoliometer) was 13° thoracic and 13° lumbar. We have documented the findings of this patient (Surface topography, ATR, Cobb angles and angles of vertebral rotation (according to Raimondi) during the treatment period (27 Month) until 2 years after the onset of menarche. RESULTS: After a treatment time of 27 Month the Cobb angle increased to 74° thoracic and 65° lumbar. The angles of vertebral rotation according to Raimondi increased slightly from 26° thoracic and 28° lumbar to 30° thoracic and 28° lumbar. The ATR improved to 12° thoracic and 5° lumbar while Lateral deviation improved from 22,4 mm to 4,6 mm and average surface rotation improved from 10,6° to 6°. In the X-rays a reduction of decompensation was visible. The patient felt comfortable with the cosmetic result. CONCLUSION: Conservative treatment may improve cosmetic appearance while the curve progresses radiologically. This could be explained by assuming that (1) the Rigo ChĂȘneau brace is able to improve cosmetic appearance by changing the shape of the thorax when the curve itself is too stiff to be corrected by a brace, that (2) reduction of decompensation leads to significant cosmetical improvements or (3) that the patient gained weight and therefore the deformation is masked. However, the weight the patient gained cannot explain the cosmetical improvement in this case. Conservative treatment with a certain standard of quality seems a viable alternative for patients with Cobb angles of > 60° when surgical treatment is refused. Specialists in scoliosis management should be aware of the fact that curve progression can occur even if the clinical measurements show an improvement

    (2+1)-dimensional Einstein-Kepler problem in the centre-of-mass frame

    We formulate and analyze the Hamiltonian dynamics of a pair of massive spinless point particles in (2+1)-dimensional Einstein gravity by anchoring the system to a conical infinity, isometric to the infinity generated by a single massive but possibly spinning particle. The reduced phase space \Gamma_{red} has dimension four and topology R^3 x S^1. \Gamma_{red} is analogous to the phase space of a Newtonian two-body system in the centre-of-mass frame, and we find on \Gamma_{red} a canonical chart that makes this analogue explicit and reduces to the Newtonian chart in the appropriate limit.     Assisting nurses in care documentation: from automated sentence classification to coherent document structures with subject headings

    Background: Up to 35% of nurses' working time is spent on care documentation. We describe the evaluation of a system aimed at assisting nurses in documenting patient care and potentially reducing the documentation workload. Our goal is to enable nurses to write or dictate nursing notes in a narrative manner without having to manually structure their text under subject headings. In the current care classification standard used in the targeted hospital, there are more than 500 subject headings to choose from, making it challenging and time consuming for nurses to use. Methods: The task of the presented system is to automatically group sentences into paragraphs and assign subject headings. For classification the system relies on a neural network-based text classification model. The nursing notes are initially classified on sentence level. Subsequently coherent paragraphs are constructed from related sentences. Results: Based on a manual evaluation conducted by a group of three domain experts, we find that in about 69% of the paragraphs formed by the system the topics of the sentences are coherent and the assigned paragraph headings correctly describe the topics. We also show that the use of a paragraph merging step reduces the number of paragraphs produced by 23% without affecting the performance of the system. Conclusions: The study shows that the presented system produces a coherent and logical structure for freely written nursing narratives and has the potential to reduce the time and effort nurses are currently spending on documenting care in hospitals. </div

    Brace related stress in scoliosis patients – Comparison of different concepts of bracing

    <p>Abstract</p> <p>Background</p> <p>The BSSQbrace questionnaire has been shown to be reliable with good internal consistency and reproducibility estimating the stress scoliosis patients have whilst wearing their brace. Eight questions are provided focussing on this topic. A max. score of 24 can be achieved (from 0 for most stress to 24 for no stress). The subdivision of the score values is: 0–8 (strong stress), 9–16 (medium stress) and 17–24 (little stress).</p> <p>Study design</p> <p>Two BSSQbrace questionnaires have been posted to 65 patients under brace treatment from our Cheneau light data base. All patients had another kind of brace prior to the Cheneau light. The patients have been asked to rate their stress level using one questionnaire for the current brace and the other for the previous one.</p> <p>Results</p> <p>63 Patients (59 girls and 4 boys) returned their fully completed questionnaires (average age 13,6 years, average Cobb angle 43,7 degrees). Stress level in the previous brace was 11,04 and in the Cheneau light(r) 13,87. The differences were highly significant in the t-test; t = -4,67; p < 0,001.</p> <p>Conclusion</p> <p>The use of the Cheneau light<sup>¼ </sup>brace leads to reduced stress and/or impairment for the patients under treatment compared to heavier brace models used so far.</p
