Endovascular repair of aortic dissection and intramural hematoma: indications and serial changes
Thoracic aortic dissection (AD) is one of the most common aortic emergencies. It can be fatal if not promptly diagnosed and treated. Intramural hematoma (IMH) of the aorta is recognized as distinct from classic (double-barreled) AD. IMH also frequently leads to aortic emergency, which can be fatal unless rapidly diagnosed and treated.Recently, thoracic endovascular aortic repair (TEVAR) has been used for the treatment of complications caused by AD. TEVAR is also a viable option for the treatment of complicated IHM. Endovascular treatment of thoracoabdominal aortic aneurysm: a case report
<p>Abstract</p> <p>Introduction</p> <p>Thoracoabdominal aortic aneurysms usually present in elderly patients with serious renal, pulmonary, cerebral, or cardiac comorbidities that pose a great challenge to the attending surgeon. Endovascular techniques for the treatment of thoracoabdominal aneurysms are not yet widely used due to limitations associated with them, such as spinal and visceral ischemia.</p> <p>Case presentation</p> <p>An 87-year-old Caucasian man with a symptomatic Crawford type I thoracoabdominal aortic aneurysm was treated successfully with a long tube stent graft using endovascular techniques and without any complication in follow-up examinations. The stent was placed distal to the left subclavian artery, and proximal to the celiac axis.</p> <p>Conclusion</p> <p>The use of endovascular stents for long segment thoracoabdominal aortic aneurysms needs to undergo clinical investigation to determine whether this procedure decreases morbidity and mortality rates.</p

Obesity and Brain Vulnerability in Normal and Abnormal Aging: A Multimodal MRI Study

How the relationship between obesity and MRI-defined neural properties varies across distinct stages of cognitive impairment due to Alzheimer’s disease is unclear.
Objective:
We used multimodal neuroimaging to clarify this relationship.
Methods:
Scans were acquired from 47 patients clinically diagnosed with mild Alzheimer’s disease dementia, 68 patients with mild cognitive impairment, and 57 cognitively healthy individuals. Voxel-wise associations were run between maps of gray matter volume, white matter integrity, and cerebral blood flow, and global/visceral obesity.
Results:
Negative associations were found in cognitively healthy individuals between obesity and white matter integrity and cerebral blood flow of temporo-parietal regions. In mild cognitive impairment, negative associations emerged in frontal, temporal, and brainstem regions. In mild dementia, a positive association was found between obesity and gray matter volume around the right temporoparietal junction.
Conclusion:
Obesity might contribute toward neural tissue vulnerability in cognitively healthy individuals and mild cognitive impairment, while a healthy weight in mild Alzheimer’s disease dementia could help preserve brain structure in the presence of age and disease-related weight loss.European Union Seventh Framework Programme (FP7/2007– 2013) under grant agreement no. 601055, VPH-DARE@IT; Neurocare; University of Sheffield, Faculty of Medicine, Dentistry and Health PhD scholarship

Rapid advances in endovascular technology and procedural breakthroughs have contributed to a dramatic transformation of the entire field of thoracic aortic surgery. TEVAR procedures can be challenging and, at times, extraordinarily difficult. They require seasoned endovascular experience and refined skills. Of all endovascular procedures, meticulous assessment of anatomy and preoperative procedure planning are absolutely paramount to produce optimal outcomes. These guidelines are intended for use in quality-improvement programs that assess the standard of care expected from all physicians who perform TEVAR procedures

The impact of tides on the capillary transition zone
The capillary transition zone, also known as the capillary fringe, is a zone where water saturations decrease with height above the water table/oil–water contact as a result of capillary action. In some oil reservoirs, this zone may contain a significant proportion of the oil in place. In groundwater assessments, the capillary fringe can profoundly affect contaminant transport. In this study, we investigated the influence of a tidally induced, semi-diurnal, change in water table depth on the water saturation distribution in the capillary fringe/transition zone. The investigation used a mixture of laboratory experiments, in which the change in saturation with depth was monitored over a period of 90 days, and numerical simulation. We show that tidal changes in water table depth can significantly alter the vertical water saturation profile from what would be predicted using capillary–gravity equilibrium and the drainage or imbibition capillary pressure curves
