    Spontaneous splenic rupture in Waldenstrom's macroglobulinemia: a case report

    <p>Abstract</p> <p>Introduction</p> <p>We report the case of a patient with Waldenstrom's macroglobulinemia complicated by spontaneous splenic rupture.</p> <p>Case presentation</p> <p>A 49-year-old Caucasian woman was referred to our emergency department by her general practitioner following a three-week history of malaise, night sweats, six kilograms of weight loss, intermittent nausea and vomiting, progressive upper abdominal pain and easy bruising. On the fourth day following her admission, she had a rapid clinical deterioration, with subsequent radiological investigations revealing a splenic rupture. Her morphology, biochemistry, flow cytometry and histology were strongly suggestive of Waldenstrom's macroglobulinemia.</p> <p>Conclusions</p> <p>Spontaneous splenic rupture is not an expected complication of low-grade lymphoplasmacytic lymphomas, such as Waldenstrom's macroglobulinemia. To the best of our knowledge, this is the only reported case of early spontaneous splenic rupture due to Waldenstrom's macroglobulinemia. Our case highlights that despite the typical disease course of low-grade hematological malignancies, signs and symptoms of imminent splenic rupture should be considered when formulating a clinical assessment.</p

    Etiological factors in primary hepatic B-cell lymphoma

    Sixty-four cases of malignant lymphoma involving the liver were examined. Of these, 20 cases were histologically confirmed to be primary hepatic B-cell lymphoma. Twelve of these 20 cases were diffuse large B-cell lymphoma (DLBCL) and eight cases were mucosa-associated lymphoid tissue (MALT) lymphoma. Of the 12 cases of DLBCL, six were immunohistologically positive for CD10 and/or Bcl6 (indicating a germinal center phenotype), six were positive for Bcl2, and five were positive for CD25. Eight of the 12 DLBCL cases (66.7%) and two of the eight MALT lymphoma cases (25%) had serum anti-hepatitis C virus (HCV) antibodies and HCV RNA. The incidence of HCV infection was significantly higher in the hepatic DLBCL cases than in systemic intravascular large B-cell cases with liver involvement (one of 11 cases, 9.1%) and T/NK-cell lymphoma cases (one of 19 cases, 5.3%) (p < 0.01 for both). Two hepatic DLBCL cases (16.7%) had rheumatoid arthritis treated with methotrexate, and four MALT lymphoma cases (50%) had Sjögren’s syndrome, primary biliary cirrhosis, or autoimmune hepatitis; one case in each of these two groups was complicated by chronic HCV-seropositive hepatitis. Although primary hepatic lymphoma is rare, persistent inflammatory processes associated with HCV infection or autoimmune disease may play independent roles in the lymphomagenesis of hepatic B cells

    The effect of deficit irrigation with treated wastewater on sweet corn: experimental and modelling study using SALTMED model

    This study investigated the impact of using treated wastewater and deficit irrigation on yield, water productivity, dry matter and soil moisture availability. The experiment included six treatments of deficit irrigation with treated wastewater during the 2010 and 2011 seasons and two deficit irrigation treatments combined with 3 organic amendment levels during the 2012 season. The experimental and SALTMED modelling results indicated that regulated deficit irrigation when applied during vegetative growth stage could stimulate root development, increase water and nutrient uptake and subsequently increase the yield. The organic amendment has slightly improved yield under full irrigation but had relatively small effect under stress conditions. The SALTMED model results supported and matched the experimental results and showed similar differences among the different treatments. The model proved its ability to predict soil moisture availability, yield, water productivity and total dry matter for three growing seasons under several deficit irrigation strategies using treated wastewater. The high values of the coefficient of determination R 2 reflected a very good agreement between the model and observed values. The SALTMED model results generally confirm the model’s ability to predict sweet corn growth and productivity under deficit irrigation strategies in the semi-arid region
