    Arsenic Poisoning in Rural Bangladesh-An intersectional analysis of impact in women

    Based on a qualitative case study in six arsenic acute villages in southwest Bangladesh, this paper presents the intersectional impact of the arsenic poisoning crisis in rural Bangladesh. Findings indicate that the arsenic poisoning crisis is aggravating existing gender inequalities as well as gender roles and responsibilities. The gender inequalities related to arsenicosis are manifested in the access to health care and in the degree of social stigma: women are the biggest victims, unmarried women in particular. The study shows that multiple axis of oppression as class, disability and age are crucial in determining the magnitude of the arsenic poisoning impacts in rural Bangladesh. Understanding the gendered dynamics in the arsenic poisoning crisis and, in particular its intersectional impact on women informs the debate on disaster management. Consequently, by expanding current knowledge, this case study lays the ground for more comprehensive and gender inclusive policy making in the context of hazardous waterscapes

    Welfare state context, female labour-market attachment and childbearing in Germany and Denmark

    This study investigates the role of female labour-market attachment and earnings in childbearing progressions in two very different European contexts. By applying event-history techniques to German and Danish register data during 1981–2001, we demonstrate how female earnings relate to first, second and third birth propensities. Our study shows that female earnings are positively associated with first birth fertility in Denmark, while this is not the case in West Germany. We interpret our findings based on the fact that Danish social context and policy encourage women to establish themselves in the labour market before becoming mothers, while the German institutional context during the 1980s and 1990s was not geared towards encouraging maternal employment. For higher-order births, the results are less clearcut. For Denmark we find a slightly positive correlation between female earnings and second-birth fertility, while the association is somewhat negative for third-order births. In Germany, women tend to leave the labour market when becoming mothers. Non-employed mothers have elevated second and, in particular, third-birth rates. For the group of mothers who are employed, we find only a weak association between their earnings and higher-order fertility

    Middle atmospheric ozone, nitrogen dioxide and nitrogen trioxide in 2002-2011: SD-WACCM simulations compared to GOMOS observations

    Most of our understanding of the atmosphere is based on observations and their comparison with model simulations. In middle atmosphere studies it is common practice to use an approach, where the model dynamics are at least partly based on temperature and wind fields from an external meteorological model. In this work we test how closely satellite measurements of a few central trace gases agree with this kind of model simulation. We use collocated vertical profiles where each satellite measurement is compared to the closest model data. We compare profiles and distributions of O3, NO2 and NO3 from the Global Ozone Monitoring by Occultation of Stars instrument (GOMOS) on the Envisat satellite with simulations by the Whole Atmosphere Community Climate Model (WACCM). GOMOS measurements are from nighttime. Our comparisons show that in the stratosphere outside the polar regions differences in ozone between WACCM and GOMOS are small, between 0 and 6%. The correlation of 5-day time series show a very high 0.9-0.95. In the tropical region 10° S-10° N below 10hPa WACCM values are up to 20% larger than GOMOS. In the Arctic below 6 hPa WACCM ozone values are up to 20% larger than GOMOS. In the mesosphere between 0.04 and 1hPa the WACCM is at most 20% smaller than GOMOS. Above the ozone minimum at 0.01hPa (or 80km) large differences are found between WACCM and GOMOS. The correlation can still be high, but at the second ozone peak the correlation falls strongly and the ozone abundance from WACCM is about 60% smaller than that from GOMOS. The total ozone columns (above 50hPa) of GOMOS and WACCM agree within ±2% except in the Arctic where WACCM is 10% larger than GOMOS. Outside the polar areas and in the validity region of GOMOS NO2 measurements (0.3-37 hPa) WACCM and GOMOS NO2 agree within -5 to +25% and the correlation is high (0.7-0.95) except in the upper stratosphere at the southern latitudes. In the polar areas, where solar particle precipitation and downward transport from the thermosphere enhance NO2 abundance, large differences up to -90% are found between WACCM and GOMOS NO2 and the correlation varies between 0.3 and 0.9. For NO3, we find that the WACCM and GOMOS difference is between -20 and 5% with a very high correlation of 0.7-0.95. We show that NO3 values strongly depend on temperature and the dependency can be fitted by the exponential function of temperature. The ratio of NO3 to O3 from WACCM and GOMOS closely follow the prediction from the equilibrium chemical theory.     Status Quo der Ökologischen Rinderhaltung in Deutschland

    A survey was carried out to get key features of the organic cattle production in Germany. Questionnaires were evaluated from 750 farms with dairy or beef-suckler cows. Furthermore, 101 farms were visited to get more detailed information in the fields of housing, feeding, breeding, health, economy. Dairy farms were more specialized than beef farms. 30 dairy cows respective 15 beef cows were kept on average per farm. Most dairy farmers hold the typical regional breeds. About 10 % kept old, endangered breeds. One third of dairy farmers had still stanchion barns, but less than 10 % of beef farmers. Main health problems on dairy farms were mastitis, claw or fertility disorders. Milk production averaged on 6.150 kg per cow and year. In many dairy farms, improvements are possible with regard to animal welfare, animal health or productivity. About one fifth of the milk and one third of the beef produced still had to be sold conventionally. Beef producers are dependent on subsidies

    D-region ion–neutral coupled chemistry (Sodankylä Ion Chemistry, SIC) within the Whole Atmosphere Community Climate Model (WACCM 4) – WACCM-SIC and WACCM-rSIC

    This study presents a new ion–neutral chemical model coupled into the Whole Atmosphere Community Climate Model (WACCM). The ionospheric D-region (altitudes ∼  50–90 km) chemistry is based on the Sodankylä Ion Chemistry (SIC) model, a one-dimensional model containing 307 ion–neutral and ion recombination, 16 photodissociation and 7 photoionization reactions of neutral species, positive and negative ions, and electrons. The SIC mechanism was reduced using the simulation error minimization connectivity method (SEM-CM) to produce a reaction scheme of 181 ion–molecule reactions of 181 ion–molecule reactions of 27 positive and 18 negative ions. This scheme describes the concentration profiles at altitudes between 20 km and 120 km of a set of major neutral species (HNO3, O3, H2O2, NO, NO2, HO2, OH, N2O5) and ions (O2+, O4+, NO+, NO+(H2O), O2+(H2O), H+(H2O), H+(H2O)2, H+(H2O)3, H+(H2O)4, O3−, NO2−, O−, O2, OH−, O2−(H2O), O2−(H2O)2, O4−, CO3−, CO3−(H2O), CO4−, HCO3−, NO2−, NO3−, NO3−(H2O), NO3−(H2O)2, NO3−(HNO3), NO3−(HNO3)2, Cl−, ClO−), which agree with the full SIC mechanism within a 5 % tolerance. Four 3-D model simulations were then performed, using the impact of the January 2005 solar proton event (SPE) on D-region HOx and NOx chemistry as a test case of four different model versions: the standard WACCM (no negative ions and a very limited set of positive ions); WACCM-SIC (standard WACCM with the full SIC chemistry of positive and negative ions); WACCM-D (standard WACCM with a heuristic reduction of the SIC chemistry, recently used to examine HNO3 formation following an SPE); and WACCM-rSIC (standard WACCM with a reduction of SIC chemistry using the SEM-CM method). The standard WACCM misses the HNO3 enhancement during the SPE, while the full and reduced model versions predict significant NOx, HOx and HNO3 enhancements in the mesosphere during solar proton events. The SEM-CM reduction also identifies the important ion–molecule reactions that affect the partitioning of odd nitrogen (NOx), odd hydrogen (HOx) and O3 in the stratosphere and mesosphere

    Optimizing imaging in suspected appendicitis (OPTIMAP-study): A multicenter diagnostic accuracy study of MRI in patients with suspected acute appendicitis. Study Protocol

    <p>Abstract</p> <p>Background</p> <p>In patients with clinically suspected appendicitis, imaging is needed to substantiate the clinical diagnosis. Imaging accuracy of ultrasonography (US) is suboptimal, while the most accurate technique (CT) is associated with cancer related deaths through exposure to ionizing radiation. MRI is a potential replacement, without associated ionizing radiation and no need for contrast medium administration. If MRI is proven to be sufficiently accurate, it could be introduced in the diagnostic pathway of patients with suspected appendicitis, increasing diagnostic accuracy and improving clinical outcomes, without the risk of radiation induced cancer or iodinated contrast medium-related drawbacks. The multicenter OPTIMAP study was designed to estimate the diagnostic accuracy of MRI in patients with suspected acute appendicitis in the general population.</p> <p>Methods/Design</p> <p>Eligible for this study are consecutive patients presenting with clinically suspected appendicitis at the emergency department in six centers. All patients will undergo imaging according to the Dutch guideline for acute appendicitis: initial ultrasonography in all and subsequent CT whenever US does not confirm acute appendicitis. Then MRI is performed in all patients, but the results are not used for patient management. A final diagnosis assigned by an expert panel, based on all available information including 3-months follow-up, except MRI findings, is used as the reference standard in estimating accuracy. We will calculate the sensitivity, specificity, predictive values and inter-observer agreement of MRI, and aim to include 230 patients. Patient acceptance and total imaging costs will also be evaluated.</p> <p>Discussion</p> <p>If MRI is found to be sufficiently accurate, it could replace CT in some or all patients. This will limit or obviate the ionizing radiation exposure associated risk of cancer induction and contrast medium induced nephropathy with CT, preventing the burden and the direct and indirect costs associated with treatment. Based on the high intrinsic contrast resolution of MRI, one might envision higher accuracy rates for MRI than for CT. If so, MRI could further decrease the number of unnecessary appendectomies and the number of missed appendicitis cases.</p> <p>Trial registration</p> <p><b>NTR2148</b></p

    Microbiome profiling by Illumina sequencing of combinatorial sequence-tagged PCR products

    We developed a low-cost, high-throughput microbiome profiling method that uses combinatorial sequence tags attached to PCR primers that amplify the rRNA V6 region. Amplified PCR products are sequenced using an Illumina paired-end protocol to generate millions of overlapping reads. Combinatorial sequence tagging can be used to examine hundreds of samples with far fewer primers than is required when sequence tags are incorporated at only a single end. The number of reads generated permitted saturating or near-saturating analysis of samples of the vaginal microbiome. The large number of reads al- lowed an in-depth analysis of errors, and we found that PCR-induced errors composed the vast majority of non-organism derived species variants, an ob- servation that has significant implications for sequence clustering of similar high-throughput data. We show that the short reads are sufficient to assign organisms to the genus or species level in most cases. We suggest that this method will be useful for the deep sequencing of any short nucleotide region that is taxonomically informative; these include the V3, V5 regions of the bac- terial 16S rRNA genes and the eukaryotic V9 region that is gaining popularity for sampling protist diversity.Comment: 28 pages, 13 figure

    Dose escalation improves therapeutic outcome: post hoc analysis of data from a 12-week, multicentre, double-blind, parallel-group trial of trospium chloride in patients with urinary urge incontinence

    <p>Abstract</p> <p>Background</p> <p>Flexible dosing of anticholinergics used for overactive bladder (OAB) treatment is a useful strategy in clinical practice for achieving a maximum effective and maximum tolerated level of therapeutic benefit. In this post hoc analysis we evaluated the efficacy and tolerability of trospium chloride treatment for urinary urge incontinence (UUI) with focus on flexible dosing.</p> <p>Methods</p> <p>The data came from a 12-week, randomised, double-blind, phase IIIb study in which 1658 patients with urinary frequency plus urge incontinence received trospium chloride 15 mg TID (n = 828) or 2.5 mg oxybutynin hydrochloride TID (n = 830). After four weeks, daily doses were doubled and not readjusted in 29.2% (242/828) of patients in the trospium group, and in 23.3% (193/830) in the oxybuytnin group, until the end of treatment. We assessed the absolute reduction in weekly UUI episodes and the change in intensity of dry mouth, recorded in patients' micturition diaries. Adverse events were also evaluated. Statistics were descriptive.</p> <p>Results</p> <p>Dose escalation of either trospium or oxybutynin increased reduction in UUI episodes in the population studied. At study end, there were no relevant differences between the "dose adjustment" subgroups and the respective "no dose adjustment" subgroups (trospium: <it>P </it>= 0.249; oxybutynin: <it>P </it>= 0.349). After dose escalation, worsening of dry mouth was higher in both dose adjusted subgroups compared to the respective "no dose adjustment" subgroups (<it>P </it>< 0.001). Worsening of dry mouth was lower in the trospium groups than in the oxybutynin groups (<it>P </it>< 0.001). Adverse events were increased in the dose adjusted subgroups.</p> <p>Conclusions</p> <p>Flexible dosing of trospium was proven to be as effective, but better tolerated as the officially approved adjusted dose of oxybutynin.</p> <p>Trial registration (parent study)</p> <p>The study was registered with the German Federal Institute for Drugs and Medical Devices (BfArM, Berlin, Germany), registration number 4022383, as required at the time point of planning this study.</p
