Automated Classification of Breast Cancer Stroma Maturity from Histological Images
OBJECTIVE: The tumour microenvironment plays a crucial role in regulating tumour progression by a number of different mechanisms, in particular the remodelling of collagen fibres in tumour-associated stroma, which has been reported to be related to patient survival. The underlying motivation of this work is that remodelling of collagen fibres gives rise to observable patterns in Hematoxylin and Eosin (H&E) stained slides from clinical cases of invasive breast carcinoma that the pathologist can label as mature or immature stroma. The aim of this paper is to categorise and automatically classify stromal regions according to their maturity and show that this classification agrees with that of skilled observers, hence providing a repeatable and quantitative measure for prognostic studies. METHODS: We use multi-scale Basic Image Features (BIF) and Local Binary Patterns (LBP), in combination with a random decision trees classifier for classification of breast cancer stroma regions-ofinterest (ROI). RESULTS: We present results from a cohort of 55 patients with analysis of 169 ROI. Our multi-scale approach achieved a classification accuracy of 84%. CONCLUSION: This work demonstrates the ability of texture-based image analysis to differentiate breast cancer stroma maturity in clinically acquired H&E stained slides at least as well as skilled observers
Microstructural models for diffusion MRI in breast cancer and surrounding stroma: an ex vivo study
The diffusion signal in breast tissue has primarily been modelled using apparent diffusion coefficient (ADC), intravoxel incoherent motion (IVIM) and diffusion tensor (DT) models, which may be too simplistic to describe the underlying tissue microstructure. Formalin-fixed breast cancer samples were scanned using a wide range of gradient strengths, durations, separations and orientations. A variety of one- and two-compartment models were tested to determine which best described the data. Models with restricted diffusion components and anisotropy were selected in most cancerous regions and there were no regions in which conventional ADC or DT models were selected. Maps of ADC generally related to cellularity on histology, but maps of parameters from more complex models suggest that both overall cell volume fraction and individual cell size can contribute to the diffusion signal, affecting the specificity of ADC to the tissue microstructure. The areas of coherence in diffusion anisotropy images were small, approximately 1Â mm, but the orientation corresponded to stromal orientation patterns on histology
Infinite square-well, trigonometric P\"oschl-Teller and other potential wells with a moving barrier
Using mainly two techniques, a point transformation and a time dependent
supersymmetry, we construct in sequence several quantum infinite potential
wells with a moving barrier. We depart from the well known system of a
one-dimensional particle in a box. With a point transformation, an infinite
square-well potential with a moving barrier is generated. Using time dependent
supersymmetry, the latter leads to a trigonometric P\"oschl-Teller potential
with a moving barrier. Finally, a confluent time dependent supersymmetry
transformation is implemented to generate new infinite potential wells, all of
them with a moving barrier. For all systems, solutions of the corresponding
time dependent Schr\"odinger equation fulfilling boundary conditions are
presented in a closed form
How is childhood development of immunity to Plasmodium falciparum enhanced by certain antimalarial interventions?
The development of acquired protective immunity to Plasmodium falciparum infection in young African children is considered in the context of three current strategies for malaria prevention: insecticide-impregnated bed nets or curtains, anti-sporozoite vaccines and intermittent preventive therapy. Evidence is presented that each of these measures may permit attenuated P. falciparum blood-stage infections, which do not cause clinical malaria but can act as an effective blood-stage "vaccine". It is proposed that the extended serum half-life, and rarely considered liver-stage prophylaxis provided by the anti-folate combination sulphadoxine-pyrimethamine frequently lead to such attenuated infections in high transmission areas, and thus contribute to the sustained protection from malaria observed among children receiving the combination as intermittent preventative therapy or for parasite clearance in vaccine trials
HERALD (Health Economics using Routine Anonymised Linked Data)
<b>Background</b>
Health economic analysis traditionally relies on patient derived questionnaire data, routine datasets, and outcomes data from experimental randomised control trials and other clinical studies, which are generally used as stand-alone datasets. Herein, we outline the potential implications of linking these datasets to give one single joined up data-resource for health economic analysis.<p></p>
<b>Method</b>
The linkage of individual level data from questionnaires with routinely-captured health care data allows the entire patient journey to be mapped both retrospectively and prospectively. We illustrate this with examples from an Ankylosing Spondylitis (AS) cohort by linking patient reported study dataset with the routinely collected general practitioner (GP) data, inpatient (IP) and outpatient (OP) datasets, and Accident and Emergency department data in Wales. The linked data system allows: (1) retrospective and prospective tracking of patient pathways through multiple healthcare facilities; (2) validation and clarification of patient-reported recall data, complementing the questionnaire/routine data information; (3) obtaining objective measure of the costs of chronic conditions for a longer time horizon, and during the pre-diagnosis period; (4) assessment of health service usage, referral histories, prescribed drugs and co-morbidities; and (5) profiling and stratification of patients relating to disease manifestation, lifestyles, co-morbidities, and associated costs.<p></p>
<b>Results</b>
Using the GP data system we tracked about 183 AS patients retrospectively and prospectively from the date of questionnaire completion to gather the following information: (a) number of GP events; (b) presence of a GP 'drug' read codes; and (c) the presence of a GP 'diagnostic' read codes. We tracked 236 and 296 AS patients through the OP and IP data systems respectively to count the number of OP visits; and IP admissions and duration. The results are presented under several patient stratification schemes based on disease severity, functions, age, sex, and the onset of disease symptoms.<p></p>
<b>Conclusion</b>
The linked data system offers unique opportunities for enhanced longitudinal health economic analysis not possible through the use of traditional isolated datasets. Additionally, this data linkage provides important information to improve diagnostic and referral pathways, and thus helps maximise clinical efficiency and efficiency in the use of resources
Operationalising the concept of ecosystem collapse for conservation practice
Concern is growing about ecosystem collapse, namely the abrupt decline or loss of an ecosystem resulting from human activities. While efforts to assess the risk of ecosystem collapse have developed at large spatial scales, less attention has been given to the local scales at which conservation management decisions are typically made. Development of appropriate management responses to ecosystem collapse has been limited by uncertainty regarding how collapse may best be identified, together with its underlying causes. Here we operationalise ecosystem collapse for conservation practice by providing a robust definition of collapse, in a form that is relevant to the scale of conservation decision-making. We provide an overview of different causes of collapse, and then explore the implications of this understanding for conservation practice, by examining potential management responses. This is achieved through development of a decision tree, which we illustrate through a series of case studies. We also explore the role of indicators for the early detection of collapse and for monitoring the effectiveness of management responses. Ecosystem collapse represents a significant challenge to conservation practice, as abrupt changes in ecosystem structure, function and composition can occur with little warning, leading to profound impacts on both biodiversity and human society. The risks of ecosystem collapse are likely to increase in future, as multiple forms of environmental change continue to intensify. We suggest that selection of management responses should be based on an understanding of the causal mechanisms responsible for collapse, which can be identified through appropriate monitoring and research activities
Columnar cell lesions of the canine mammary gland: pathological features and immunophenotypic analysis
<p>Abstract</p> <p>Background</p> <p>It has been suggested that columnar cell lesions indicate an alteration of the human mammary gland involved in the development of breast cancer. They have not previously been described in canine mammary gland. The aim of this paper is describe the morphologic spectrum of columnar cell lesions in canine mammary gland specimens and their association with other breast lesions.</p> <p>Methods</p> <p>A total of 126 lesions were subjected to a comprehensive morphological review based upon the human breast classification system for columnar cell lesions. The presence of preinvasive (epithelial hyperplasia and in situ carcinoma) and invasive lesions was determined and immunophenotypic analysis (estrogen receptor (ER), progesterone receptor (PgR), high molecular weight cytokeratin (34βE-12), E-cadherin, Ki-67, HER-2 and P53) was perfomed.</p> <p>Results</p> <p>Columnar cell lesions were identified in 67 (53.1%) of the 126 canine mammary glands with intraepithelial alterations. They were observed in the terminal duct lobular units and characterized at dilated acini may be lined by several layers of columnar epithelial cells with elongated nuclei. Of the columnar cell lesions identified, 41 (61.2%) were without and 26 (38.8%) with atypia. Association with ductal hyperplasia was observed in 45/67 (67.1%). Sixty (89.5%) of the columnar cell lesions coexisted with neoplastic lesions (20 in situ carcinomas, 19 invasive carcinomas and 21 benign tumors). The columnar cells were ER, PgR and E-cadherin positive but negative for cytokeratin 34βE-12, HER-2 and P53. The proliferation rate as measured by Ki-67 appeared higher in the lesions analyzed than in normal TDLUs.</p> <p>Conclusions</p> <p>Columnar cell lesions in canine mammary gland are pathologically and immunophenotypically similar to those in human breast. This may suggest that dogs are a suitable model for the comparative study of noninvasive breast lesions.</p
Complete In Vitro Life Cycle of Trypanosoma congolense: Development of Genetic Tools
Trypanosoma congolense is a parasite responsible for severe disease of African livestock. Its life cycle is complex and divided into two phases, one in the tsetse fly vector and one in the bloodstream of the mammalian host. Molecular tools for gene function analyses in parasitic organisms are essential. Previous studies described the possibility of completing the entire T. congolense life cycle in vitro. However, the model showed major flaws including the absence of stable long-term culture of the infectious bloodstream forms, a laborious time-consuming period to perform the cycle and a lack of genetic tools. We therefore aimed to develop a standardized model convenient for genetic engineering. We succeeded in producing long-term cultures of all the developmental stages on long-term, to define all the differentiation steps and to finally complete the whole cycle in vitro. This improved model offers the opportunity to conduct phenotype analyses of genetically modified strains throughout the in vitro cycle and also during experimental infections
