    MONTELUCAST APPLICATION IN TREATMENT FOR BRONCHIAL ASTHMA OF VARIOUS SEVERITY

    Occurrence of bronchial asthma is growing. The experts have proved its genetic and clinical heterogeneity. This is what also calls forth incomplete response to the conventional anti inflammatory medications. During asthma, dozens of various mediators take an active part in the pathogenesis of inflammation in the respiratory passages, including a set of c4, d4 and е4 leukotrienes, which causes contractions of the bronchi plain muscles. Among all the anti leukotriene medications ever studied for the sales in the Russian federation, the federal agencies approved of and registered zafirlukast and montelucast only. Montelucast is recommended in treatment for bronchial asthma of all the severity levels, as well as both as a medication to enhance the basic therapy and a medication for monotherapy, which is thanks to the data from a handful of multicentered, double, blind, placebo, controlled studies. The application of the peroral medications, montelucast in particular, may be a good alternative to the inhalant medications used among the patients with poor discipline and inhalation techniques (children, aged patients and etc.). When we speak of allergic rhinitis, montelucast application is essential to exclude polypragmasy, as well as for the initial asthma prevention.Key words: bronchial asthma, children, treatment, anti leukotrienes

    SUBLINGUAL SPECIFIC IMMUNOTHERAPY IN THE TREATMENT OF HAY FEVER CAUSED BY TREE POLLEN IN CHILDREN

    The results of the investigation of effectiveness and safety of sublingual allergen specific immunotherapy (slit) with tree pollen mixture for hay fever treatment in children are presented in the article. 42 children 4 to 18 years old with hay fever (allergic rhinoconjunctivitis, pollen asthma) were included in the study. Birch, alder and hazel pollen sensitization was proved by skin prick test (positive in 5 patients and strongly positive in 37). 2 courses of slit out of flowering season were conducted in every child. Total dose of allergen was 43321 ± 73 PNU (3990 to 120 000 PNU) after 1st course of slit, and 93 850 ± 128 PNU (8750 to 249 000 PNU) after 2nd course. Effectiveness of the 1st course of slit was 3,5 points out of 4,0 and of the 2nd course — 3,8 points. Disease remission was registered in 59,5% patients after the 1st course of slit and in 83,3% patients after the 2nd course. Local side effects (tongue and throat itching, nausea and o edema of lips) and systemic side effects (rhinorrhea, eye itching, fatiguability and bronchospasm) of mild severity did not lead to treatment cessation. Slit is highly effective and safe method for hay fever treatment in children.Key words: pollen sensitization, sublingual allergen specific immunotherapy, children

Этим обусловлен, в том числе, и неполный ответ на традиционные противовоспалительные препараты. В патогенезе воспаления в дыхательных путях при астме участвуют десятки различных медиаторов, в том числе комплекс лейкотриенов C4, D4 и Е4 вызывающий сокращение гладких мышц бронхов. Среди всех исследованных антилейкотриеновых препаратов для продажи в РФ зарегистрированы только зафирлукаст и монтелукаст. Монтелукаст рекомендован для применения при всех степенях тяжести бронхиальной астмы и как средство усиления базисной терапии, и как препарат монотерапии, что основано на данных многочисленных многоцентровых двойных слепых плацебо контролируемых исследований. Применение пероральных препаратов, в частности, монтелукаста, может стать альтернативой ингаляционным средствам у больных с низкой дисциплиной и плохой техникой ингаляций (дети, пациенты пожилого возраста и др.). Применение монтелукаста при аллергическом рините важно для исключения полипрагмазии и первичной профилактики астмы. Данная статья посвящена обзору основных фармакодинамических и клинических эффектов активированного пиритиона цинка при атопическом дерматите (АтД). Активированный пиритион цинка обладает выраженным противовоспалительным, проапоптогенным, противогрибковым и антимикотическим действием, благодаря чему воздействует на все компоненты патогенеза АтД. Проведенные исследования свидетельствуют о высокой эффективности и безопасности этого препарата при АтД у детей, что позволяет отнести его к эффективным нестероидным средствам для наружной терапии АтД у детей.     Atopic dermatitis is one of the most widespread children’s diseases, it has a chronic recurrent clinical course, may be often complicated with the secondary infection.
