    Case report: A rare case of synchronous mucinous neoplasms of the renal pelvis and the appendix

    BackgroundMucinous neoplasms are tumors arising in the epithelial tissue, characterized by excessive mucin secretion. They mainly emerge in the digestive system and rarely in the urinary system. They also seldom develop in the renal pelvis and the appendix asynchronously or simultaneously. The concurrence of this disease in these two regions has not yet been reported. In this case report, we discuss the diagnosis and treatment of synchronous mucinous neoplasms of the right renal pelvis and the appendix. The mucinous neoplasm of the renal pelvis was preoperatively misdiagnosed as pyonephrosis caused by renal stones, and the patient underwent laparoscopic nephrectomy. Herein, we summarize our experience with this rare case in combination with related literature.Case presentationIn this case, A 64‐year‐old female was admitted to our hospital with persistent pain in the right lower back for over a year. Computer tomography urography (CTU) showed that the patient was confirmed as right kidney stone with large hydronephrosis or pyonephrosis, and appendiceal mucinous neoplasm (AMN). Subsequently, the patient was transferred to the gastrointestinal surgery department. Simultaneously, electronic colonoscopy with biopsy suggested AMN. Open appendectomy plus abdominal exploration was performed after obtaining informed consent. Postoperative pathology indicated low-grade AMN (LAMN) and the incisal margin of the appendix was negative. The patient was re‐admitted to the urology department, and underwent laparoscopic right nephrectomy because she was misdiagnosed with calculi and pyonephrosis of the right kidney according to the indistinctive clinical symptoms, standard examination of the gelatinous material, and imaging findings. Postoperative pathology suggested a high‐grade mucinous neoplasm of the renal pelvis and mucin residing partly in the interstitium of the cyst walls. Good follow-up results were obtained for 14 months.ConclusionSynchronous mucinous neoplasms of the renal pelvis and the appendix are indeed uncommon and have not yet been reported. Primary renal mucinous adenocarcinoma is very rare, metastasis from other organs should be first considered, especially in patients with long-term chronic inflammation, hydronephrosis, pyonephrosis, and renal stones, otherwise, misdiagnosis and treatment delay may occur. Hence, for patients with rare diseases, strict adherence to treatment principles and close follow‐up are necessary to achieve favorable outcomes

    Case Report: A MiT family translocation renal cell carcinoma in the renal pelvis, calyces and upper ureter misdiagnosed as upper tract urothelial carcinoma

    BackgroundUpper tract urothelial carcinoma (UTUC) is the most common urothelial malignancy in the renal pelvis or ureter. Renal pelvic carcinoma accounts for 90% of all tumours in the renal pelvis, so the mass in the renal pelvis is usually considered a UTUC. Renal cell carcinoma (RCC) in the renal pelvis, calyces and upper ureter is extremely rare, especially MiT family translocation RCC, which makes this case even more uncommon.Case presentationWe report the case of a 54-year-old man had intermittent painless gross haematuria with occasional blood clots and urodynia for 2 years. Contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) scan showed an enlarged left kidney, and a soft tissue mass was seen in the renal pelvis, calyces and upper ureter. The patient’s urine-based cytology was positive three times. Due to the severity of the upper ureteral lumen stenosis, we did not perform pathological biopsy during ureteroscopy. In the current case, clinical symptoms, imaging examinations, urine-based cytology, and ureteroscopy were combined to obtain a preoperative diagnosis of UTUC. Therefore, robot-assisted laparoscopic left radical nephroureterectomy and retroperitoneal lymphadenectomy were performed. Unexpectedly, the patient was pathologically diagnosed with MiT family translocation RCC after surgery. The surgery was uneventful. There was no intestinal tube injury or other complications perioperatively. The postoperative follow-up was satisfactory.ConclusionMiT family translocation RCC in the renal pelvis, calyces and upper ureter is extremely rare, and can be easily confused with UTUC, resulting in the expansion of surgical scope. Preoperative ureteroscopy and biopsy or tumour punch biopsy should be used to obtain accurate pathology as far as possible, and the selection of correct surgical method is conducive to a good prognosis for patients

    Local Government Debt and Corporate Investment Behavior in China: Real versus Financial Investment

    The ongoing expansion of local government debt (LGD) in China constitutes a significant impediment to economic development, while the existing literature predominantly concentrates on macro-level investigations, neglecting the repercussions of government debt expansion on firms. Firms serve as fundamental constituents of the real economy, and the suitability of their investment structure is a pivotal determinant of their robust development. Therefore, it is of great significance to investigate whether the investment structure of non-financial firms will undergo deviations attributable to the expansion of local government debt. This paper uses a two-way fixed-effects model to examine the causal effect of local government debt on firms’ investment structures. The quasi-natural experiment using the DID model with “Document 43” issued by China on local government debt governance as a policy shock can be a good endogeneity test. It is found that local government debt exacerbates the trend of “exit from real to virtual” of enterprises, leading to a bias towards financial investment in the investment structure of enterprises, and this result is still robust after a series of robustness tests. A heterogeneity analysis shows that the impact of LGD on the investment structure of firms mainly exists in non-state-owned firms, small-scale firms, and firms with high financing constraints. Overall, this study provides new evidence on how the government influences the investment structure of Chinese firms through the perspective of LGD, which helps firms to prevent and cope with the risks associated with LGD. Furthermore, it offers practical references and policy insights for government initiatives in the realm of local debt governance

    Irrigation and nitrogen (N) are two crucial factors affecting perennial grass seed production. To investigate the effects of irrigation and N rate on seed yield (SY), yield components, and water use efficiency (WUE) of Cleistogenes songorica (Roshevitz) Ohwi, an ecologically significant perennial grass, a four-year (2016–2019) field trial was conducted in an arid region of northwestern China. Two irrigation regimes (I1 treatment: irrigation at tillering stage; I2 treatment: irrigation at tillering, spikelet initiation, and early flowering stages) and four N rates (0, 60, 120, 180 kg ha−1) were arranged. Increasing amounts of both irrigation and N improved SY, evapotranspiration, WUE, and related yield components like fertile tillers m−2 (FTSM) and seeds spikelet−1. Meanwhile, no significant difference was observed between 120 and 180 kg N ha−1 treatments for most variables. The highest SY and WUE was obtained with treatment combination of I2 plus 120 kg N ha−1 with four-year average values of 507.3 kg ha−1 and 1.8 kg ha−1 mm−1, respectively. Path coefficient and contribution analysis indicated that FTSM was the most important yield component for SY, with direct path coefficient and contribution coefficient of 0.626 and 0.592. Overall, we recommend I2 treatment (three irrigations) together with 120 kg N ha−1 to both increase SY and WUE, especially in arid regions. Future agronomic managements and breeding programs for seed should mainly focus on FTSM. This study will enable grass seed producers, plant breeders, and government program directors to more effectively target higher SY of C. songorica

    Immediate analgesia effect of contralateral needling at Tiaokou (ST38) in patients with chronic shoulder pain: A randomized controlled exploratory trial

    Objective: To compare the immediate effect of acupuncture at the contralateral versus ipsilateral Tiaokou (ST38) in patients with Chronic shoulder pain (CSP). Method: Altogether, 38 patients with unilateral CSP were randomly assigned to a contralateral ST38 group (Contra group) or ipsilateral ST38 group (Ipsi group). Each underwent 20 min of manual acupuncture: Contra group at the contralateral ST38, Ipsi group at the ipsilateral ST38. Primary outcome measure was the visual analog scale (VAS) score. Secondary outcome measure for assessing shoulder mobility was the Constant-Murley (CM) score, which included pain level, activities of daily living (ADL), and shoulder range of motion (ROM). These factors were assessed before and immediately after acupuncture. A value of P < .05 indicated significance. Results: VAS scores decreased immediately after treatment in both groups, but the effect was significantly greater in the Contra group (P = .043). CM scores also improved in both groups, but the Contra group improved significantly more (P = .002). The CM's pain and ADL scores were similar in the two groups (P = .055 and P = .193, respectively), but ROM and strength scores were better in the Contra group than in the Ipsi group (P = .003 and P = .037, respectively). No adverse effects were reported. Conclusion: The immediate effect of acupuncture at the contralateral ST38 may be superior to that at the ipsilateral ST38 in CSP patients. They are trained using an evolutionary strategy for performing large-scale molecular dynamics (MD) simulations. A descriptor of the atomic environment is constructed based on Chebyshev and Legendre polynomials. The method is implemented in graphic processing units within the open-source gpumd package, which can attain a computational speed over atom-step per second using one Nvidia Tesla V100. Furthermore, per-atom heat current is available in NEP, which paves the way for efficient and accurate MD simulations of heat transport in materials with strong phonon anharmonicity or spatial disorder, which usually cannot be accurately treated either with traditional empirical potentials or with perturbative methods.Peer reviewe
