    Continuing education beyond the compulsory years of schooling is one of the most important choices an adolescent has to make. Higher education is associated with a host of social and economic benefits both for the person and its community. Today, there is ample evidence that educational aspirations are an important determinant of said choice. We implement a multilevel, networked experiment in 45 Mexican high schools and provide evidence of the malleability of educational aspirations. We also show there exists an interdependence of students' choices and the effect of our intervention with peer networks. We find that a video intervention, which combines role models and information about returns to education is successful in updating students' beliefs and consequently educational aspirations

    Application of Otolith Chemical Signatures to Estimate Population Connectivity of Red Snapper In the Western Gulf of Mexico

    Otolith chemical signatures of Red Snapper Lutjanus campechanus from six nursery regions were used to estimate the sources of recruits to four sampling regions in the western Gulf of Mexico (Gulf) and to estimate whether postsettlement mixing of Red Snapper occurs between the U.S. and Mexican portions of the western Gulf. In a previous study, region-specific otolith signatures (element : Ca ratios: Ba:Ca, Mg:Ca, Mn:Ca, Sr:Ca, and Li:Ca; stable isotope delta values: δ13C and δ18O) were developed based on age-0 Red Snapper (2005–2007 year-classes) sampled from the six nursery areas. In the present study, subadult and adult Red Snapper (ages 1–3) belonging to those same year-classes were collected from four sampling regions within the western Gulf (two regions in U.S. waters; two regions along the Mexican continental shelf) during summer in 2006–2008. Left sagittal otoliths were used to age subadults and adults to the corresponding nursery year-classes, and right sagittal otoliths were cored for chemical analysis. Off the southwestern U.S. coast, the sampled age-1–3 Red Snapper included locally derived recruits as well as recruits from the northwestern Gulf nursery region. However, analytical results were inconclusive with respect to estimating the connectivity between Red Snapper populations in U.S. and Mexican waters of the western Gulf

    Medical Biochemistry Without Rote Memorization: Multi-Institution Implementation And Student Perceptions Of A Nationally Standardized Metabolic Map For Learning And Assessment

    Despite the growing number of patients worldwide with metabolism-related chronic diseases, medical biochemistry education is commonly perceived as focusing on recall of facts irrelevant for patient care. The authors suggest that this focus on rote memorization of pathways creates excessive cognitive load that may interfere with learners’ development of an integrated understanding of metabolic regulation and dysregulation. This cognitive load can be minimized by providing appropriate references during learning and assessment. Biochemistry educators collaborated to develop a medically relevant Pathways of Human Metabolism map (MetMap) that is now being used at many medical schools as a nationally standardized resource during learning and assessments. To assess impact, students from three medical schools were surveyed about its benefits and disadvantages. Responses were obtained from 481 students (84%) and were examined using thematic analysis. Five main themes emerged as perceived benefits of using the MetMap: 1) aids visual and mental organization, 2) promotes deep learning and applied understanding, 3) decreases emphasis on memorization, 4) reduces anxiety on exams, and 5) aids recall. Perceived disadvantages were: 1) fear of under-preparation for licensing exams, 2) overwhelming nature of the map, and 3) reduced motivation for and time spent studying. Results affirm that students perceive use of the MetMap promotes focus on broader metabolic concepts and deep versus surface learning, supporting a shift in cognitive load toward desired goals. Although the long-term impact on learning needs to be further studied, the use of the MetMap represents a step toward open-reference exams that reflect “real world” practice

    'Simulation-based learning in psychiatry for undergraduates at the University of Zimbabwe medical school'.

    BACKGROUND: The use of simulated patients to teach in psychiatry has not been reported from low-income countries. This is the first study using simulation teaching in psychiatry in Africa. The aim of this study was to introduce a novel method of psychiatric teaching to medical students at the University of Zimbabwe and assess its feasibility and preliminary effectiveness. We selected depression to simulate because students in Zimbabwe are most likely to see cases of psychoses during their ward-based clinical exposure. METHODS: Zimbabwean psychiatrists adapted scenarios on depression and suicide based on ones used in London. Zimbabwean post-graduate trainee psychiatrists were invited to carry out the teaching and psychiatric nursing staff were recruited and trained in one hour to play the simulated patients (SPs). All students undertaking their psychiatry placement (n = 30) were allocated into groups for a short didactic lecture on assessing for clinical depression and then rotated around 3 scenarios in groups of 4-5 and asked to interview a simulated patient with signs of depression. Students received feedback from peers, SPs and facilitators. Students completed the Confidence in Assessing and Managing Depression (CAM-D) questionnaire before and after the simulation session and provided written free-text feedback. RESULTS: Post-graduate trainers, together with one consultant, facilitated the simulated teaching after three hours training. Student confidence scores increased from mean 15.90 to 20.05 (95% CI = 2.58- 5.71) t (20) = 5.52, (p > 0.0001) following the simulation teaching session. Free-text feedback was positive overall with students commenting that it was "helpful", "enjoyable" and "boosted confidence". CONCLUSIONS: In Zimbabwe, simulation teaching was acceptable and could be adapted with minimal effort by local psychiatrists and implemented by post-graduate trainees and one consultant, Students found it helpful and enjoyable and their confidence increased after the teaching. It offers students a broader exposure to psychiatric conditions than they receive during clinical attachment to the inpatient wards. Involving psychiatry trainees and nursing staff may be a sustainable approach in a setting with small number of consultants and limited funds to pay for professional actors

    Impact of the COVID-19 pandemic on the provision and uptake of services for the prevention of mother-to-child transmission of HIV in Zimbabwe

    Zimbabwe is targeting elimination of mother-to-child transmission of HIV by December 2025, however the COVID-19 pandemic challenged health service delivery globally. Monthly aggregated data were extracted from DHIS-2 for all facilities delivering antenatal care (ANC). ZIMSTAT and Spectrum demographic estimates were used for population-level denominators. Programme indicators are among those in HIV care and population indicators reflect the total population. The mean estimated proportion of pregnant women booking for ANC per month did not change (91% pre-pandemic vs 91% during pandemic, p = 0.95), despite dropping to 47% in April 2020. At a programme-level, the estimated proportion of women who received at least one HIV test fell in April 2020 (3.6% relative reduction vs March (95% CI 2.2–5.1), p<0.001) with gradual recovery towards pre-pandemic levels. The estimated proportion of women who were retested among those initially negative in pregnancy fell markedly in April 2020 (39% reduction (32–45%), p<0.001) and the subsequent increase was much slower, only reaching 39% by September 2021 compared to average 53% pre-pandemic. The mean estimated proportion of pregnant women with HIV on ART was unchanged at programme-level (98% vs 98%, p = 0.26), but decreased at population-level (86% vs 80%, p = 0.049). Antiretroviral prophylaxis coverage decreased among HIV-exposed infants, at programme- (94% vs 87%, p = 0.001) and population-levels (76% vs 68%, p<0.001). There was no significant change in HIV-exposed infants receiving EID (programme: 107% vs 103%, p = 0.52; population: 87% vs 79%, p = 0.081). The estimated proportion of infants with HIV diagnosed fell from 27% to 18%, (p<0.001), while the estimated proportion on ART was stable at a programme (88% vs 90%, p = 0.82) but not population (22% vs 16%, p = 0.004) level. Despite a drop at the start of the pandemic most programme indicators rapidly recovered. At a population-level indicators were slower to return, suggesting less women with HIV identified in care

    GJ 1252b: A Hot Terrestrial Super-Earth With No Atmosphere

    The increasing numbers of rocky, terrestrial exoplanets known to orbit nearby stars (especially M dwarfs) has drawn increased attention to the possibility of studying these planets' surface properties, and atmospheric compositions & escape histories. Here we report the detection of the secondary eclipse of the terrestrial exoplanet GJ1252b using the Spitzer Space Telescope's IRAC2 4.5 micron channel. We measure an eclipse depth of 149(+25/-32) ppm, corresponding to a day-side brightness temperature of 1410(+91/-125) K and consistent with the prediction for no atmosphere. Comparing our measurement to atmospheric models indicates that GJ1252b has a surface pressure of <10 bar, substantially less than Venus. Assuming energy-limited escape, even a 100 bar atmosphere would be lost in <1 Myr, far shorter than estimated age of 3.9+/-0.4 Gyr. The expected mass loss could be overcome by mantle outgassing, but only if the mantle's carbon content were >7% by mass - over two orders of magnitude greater than that found in Earth. We therefore conclude that GJ1252b has no significant atmosphere. Model spectra with granitoid or feldspathic surface composition, but with no atmosphere, are disfavored at >2 sigma. The eclipse occurs just +1.4(+2.8/-1.0) min after orbital phase 0.5, indicating e cos omega=+0.0025(+0.0049/-0.0018), consistent with a circular orbit. Tidal heating is therefore likely to be negligible to GJ1252b's global energy budget. Finally, we also analyze additional, unpublished TESS transit photometry of GJ1252b which improves the precision of the transit ephemeris by a factor of ten, provides a more precise planetary radius of 1.180+/-0.078 R_E, and rules out any transit timing variations with amplitudes <1 min.Comment: ApJL in press. 16 pages, 12 figures, 10 eclipses, 1 bandpass.     Promoting successful participation of people living with Alzheimer's disease and related dementias in pain-related neuroimaging research studies

    Recruitment and retention of participants for pain-related neuroimaging research is challenging and becomes increasingly so when research participants have a diagnosis of Alzheimer's disease and related dementias (ADRD). This article shares the authors' recommendations from several years of successful recruitment and completion of pain-related neuroimaging studies of people living with ADRD and includes supportive literature. While not an exhaustive list, this review covers several topics related to recruitment and retention of participants living with ADRD, including community engagement, capacity to consent, dementia diagnostic criteria, pain medication and other study exclusion criteria, participant and caregiver burden, communication concerns, and relationships with neuroimaging facilities. Threaded throughout the paper are important cultural considerations. Additionally, we discuss implications of the coronavirus (COVID-19) pandemic for recruitment. Once tailored to specific research study protocols, these proven strategies may assist researchers with successfully recruiting and retaining participants living with ADRD for pain-related neuroimaging research studies toward improving overall health outcomes

    PTHrP Induces Autocrine/Paracrine Proliferation of Bone Tumor Cells through Inhibition of Apoptosis

    Giant Cell Tumor of Bone (GCT) is an aggressive skeletal tumor characterized by local bone destruction, high recurrence rates and metastatic potential. Previous work in our lab has shown that the neoplastic cell of GCT is a proliferating pre-osteoblastic stromal cell in which the transcription factor Runx2 plays a role in regulating protein expression. One of the proteins expressed by these cells is parathryroid hormone-related protein (PTHrP). The objectives of this study were to determine the role played by PTHrP in GCT of bone with a focus on cell proliferation and apoptosis. Primary stromal cell cultures from 5 patients with GCT of bone and one lung metastsis were used for cell-based experiments. Control cell lines included a renal cell carcinoma (RCC) cell line and a human fetal osteoblast cell line. Cells were exposed to optimized concentrations of a PTHrP neutralizing antibody and were analyzed with the use of cell proliferation and apoptosis assays including mitochondrial dehydrogenase assays, crystal violet assays, APO-1 ELISAs, caspase activity assays, flow cytometry and immunofluorescent immunohistochemistry. Neutralization of PTHrP in the cell environment inhibited cell proliferation in a consistent manner and induced apoptosis in the GCT stromal cells, with the exception of those obtained from a lung metastasis. Cell cycle progression was not significantly affected by PTHrP neutralization. These findings indicate that PTHrP plays an autocrine/paracrine neoplastic role in GCT by allowing the proliferating stromal cells to evade apoptosis, possibly through non-traditional caspase-independent pathways. Thus PTHrP neutralizing immunotherapy is an intriguing potential therapeutic strategy for this tumor
