    Sex Differences in Mechanisms and Outcome of Neonatal Hypoxia-Ischemia in Rodent Models: Implications for Sex-Specific Neuroprotection in Clinical Neonatal Practice

    Clinical findings show that male infants with hypoxic-ischemic injury (HI) fare more poorly than matched females on cognitive outcomes. Rodent models of neonatal hypoxia-ischemia support this difference, with data showing that perinatal brain injury leads to long-term behavioral deficits primarily in male rodents and in female rodents treated with early androgens. Results support the idea that sex-specific gonadal hormones may modulate developmental response to injury and dovetail with overwhelming evidence of developmental androgen effects on typical brain morphology and behavior. However, mechanisms underlying sex differences in response to early brain injury may be more complicated. Specifically, activation of cell death pathways in response to HI may also differ by sex. In females, the preferential activation of the caspase-dependent apoptotic pathway may actually afford greater protection, potentially due to the actions of X-linked inhibitor of apoptosis (XIAP) within this pathway. This contrasts the pattern of preferential activation of the caspase-independent pathway in males. While an integrated model of sex-specific hormonal and genetic modulation of response to early injury remains to be fully elucidated, these findings suggest that infants might benefit from sex-specific neuroprotection following HI injury

    Differentiating the EdD and the PhD in Higher Education: A Survey of Characteristics and Trends

    Purpose: Higher education, as a field of study, is one of the few programmatic areas that offer two doctoral degrees: The Doctor of Education (EdD) and the Doctor of Philosophy (PhD). In the United States, the two degrees are often conflated. Conversations, to this point, have done more to contribute to the theoretical debate than to operationally distinguish between the two degree paths. Method: The current study analyzed data collected from a review of the 188 doctoral programs at 145 institutions listed with the Association for the Study of Higher Education (ASHE). Results: Results suggest that while there is a shift toward operational differences between the EdD and the PhD in higher education, the similarities in entrance requirements, formats, and research expectations, to list a few, suggest that the EdD and PhD still require further refinement to reach the theoretical clarity common in many conversations about the education doctorate

    Will "Combined Prevention" Eliminate Racial/Ethnic Disparities in HIV Infection among Persons Who Inject Drugs in New York City?

    It has not been determined whether implementation of combined prevention programming for persons who inject drugs reduce racial/ethnic disparities in HIV infection. We examine racial/ethnic disparities in New York City among persons who inject drugs after implementation of the New York City Condom Social Marketing Program in 2007. Quantitative interviews and HIV testing were conducted among persons who inject drugs entering Mount Sinai Beth Israel drug treatment (2007–2014). 703 persons who inject drugs who began injecting after implementation of large-scale syringe exchange were included in the analyses. Factors independently associated with being HIV seropositive were identified and a published model was used to estimate HIV infections due to sexual transmission. Overall HIV prevalence was 4%; Whites 1%, African-Americans 17%, and Hispanics 4%. Adjusted odds ratios were 21.0 (95% CI 5.7, 77.5) for African-Americans to Whites and 4.5 (95% CI 1.3, 16.3) for Hispanics to Whites. There was an overall significant trend towards reduced HIV prevalence over time (adjusted odd ratio = 0.7 per year, 95% confidence interval (0.6–0.8). An estimated 75% or more of the HIV infections were due to sexual transmission. Racial/ethnic disparities among persons who inject drugs were not significantly different from previous disparities. Reducing these persistent disparities may require new interventions (treatment as prevention, pre-exposure prophylaxis) for all racial/ethnic groups

    Engaging Rural Nevadans in Participatory Research to Explore and Explain the Community Food and Physical Activity Context

    The purpose of this study was to engage residents of four rural Nevada communities to explain local resources and readiness to address environmental challenges to weight healthy lifestyles. Residents engaged in HEAL MAPPS™, a participatory research approach using photomapping and community conversations to document lived experiences of place-based resources as supports or barriers. Data were triangulated to scale community readiness-to-change. This study focuses on a description of methods and qualitative findings. Healthy food unavailability emerged consistently among communities as a barrier; produce options were limited and many residents relied on convenience foods. Physical activity opportunities were available, yet access was a barrier. Transportation-related issues emerged as barriers to healthy eating and physical activity. Communities ranged between “vague awareness” and “preplanning” on readiness-to-change. Local data and shared knowledge of the obesogenic context can inform community policy and environmental improvements that promote health and enhance quality of life for rural populations

    Affective response as a mediator of the association between the physical and social environment and physical activity behavior

    Perceptions of the physical and social environment have been shown to be predictive of physical activity (PA) behavior. However, the mechanisms of this association have not been examined. Affective response to PA was examined as a putative mediator of the association between perceptions of the PA environment and subsequent PA behavior. As part of a PA promotion study, 59 low-active overweight or obese but otherwise healthy adults completed real-time assessments of the perceived physical and social PA environment, affective response to PA, and PA behavior over a 6-month period. As hypothesized, decreased latency to and greater duration of subsequent PA was predicted by engaging in PA with a partner (b = 17.24, SE = .45, p \u3c .01), engaging in PA outdoors versus indoors (b = 3.70, SE = 0.67, p \u3c .01), and perceived pleasantness of the physical (b = 0.59, SE = .17, p \u3c .01) and social settings (b = 0.68, SE = .16, p \u3c .01). Affective response to PA (a shift toward feeling good versus bad during PA) mediated the association between engaging in PA with a partner (a path: 0.53(.11), p \u3c .01, b path: 0.42(.12), p \u3c .01, ab path: 0.22(.08), 95% CI .09–.41) and perceived pleasantness of the physical (a path: .38(.02), p \u3c .01; b path: .65(.23), p = .01; ab path: .25(.09), 95% CI .08–.43) and social setting (apath: .35(.02), p \u3c .01; b path: .57(.23), p = .01; ab path: .20(.08), 95% CI .03–.37) and PA behavior, but not the association between engaging in PA outdoors versus indoors and PA behavior. These findings suggest that perceived environmental variables may have their effects on PA through the process of psychological hedonism

    Gidakiimanaanawigamig’s Circle of Learning: A Model for Partnership between Tribal Community and Research University

    Since 2002, the National Center for Earth-Surface dynamics has collaborated with the Fond du Lac Band of Lake Superior Chippewa, the Fond du Lac Tribal and Community College, the University of Minnesota, and other partner institutions to develop programs aimed at supporting Native American participation in science, technology, engineering, and mathematics (STEM) fields, and especially in the Earth and Environmental Sciences. These include the gidakiimanaaniwigamig math and science camps for students in kindergarten through 12th grade, the Research Experience for Undergraduates on Sustainable Land and Water Resources, which takes place on two native reservations, and support for new majors at tribal colleges. All of these programs have a common focus on collaboration with communities, place-based education, community-inspired research projects, a focus on traditional culture and language, and resource management on reservations. Strong partnerships between university, tribal college, and Native American reservation were a foundation for success, but took time and effort to develop. This paper explores steps towards effective partnerships that support student success in STEM via environmental education

    2019 Pediatric Initiative Network: Progress, proceedings, and plans

    Impairment of fertility and sexual/reproductive health are common after oncologic therapy, and are known to have negative impacts on romantic relationships and psychosocial well-being among childhood cancer survivors. The Pediatric Initiative Network (PIN) is an international, multidisciplinary group of providers within the Oncofertility Consortium dedicated to preserving and protecting the fertility of children and adolescents at risk for infertility due to medical conditions or treatments. The PIN and its Best Practices and Research committees meet virtually throughout the year, with one annual in-person meeting. The purpose of this “proceedings” is to highlight key discussion points from the annual PIN meeting which took place on November 11, 2019, to 1) provide a context for pediatric groups across the country on what oncofertility programs are currently doing and why, and 2) inform stakeholders of past, present and future initiatives that may be of value to them and the patient populations they serve

    The RAND/PPMD Patient-Centeredness Method: a novel online approach to engaging patients and their representatives in guideline development

    Although clinical practice guidelines (CPGs) provide recommendations for how best to treat a typical patient with a given condition, patients and their representatives are not always engaged in CPG development. Despite the agreement that patient participation may improve the quality and utility of CPGs, there is no systematic, scalable method for engaging patients and their representatives, as well as no consensus on what exactly patients and their representatives should be asked to do during CPG development. To address these gaps, an interdisciplinary team of researchers, patient representatives, and clinicians developed the RAND/PPMD Patient-Centeredness Method (RPM) - a novel online approach to engaging patients and their representatives in CPG development. The RPM is an iterative approach that allows patients and their representatives to provide input by (1) generating ideas; (2) rating draft recommendations on two criteria (importance and acceptability); (3) explaining and discussing their ratings with other participants using online, asynchronous, anonymous, moderated discussion boards, and (4) revising their responses if needed. The RPM was designed to be consistent with the RAND/UCLA Appropriateness Method used by clinicians and researchers to develop CPG, while helping patients and their representative rate outcome importance and recommendation acceptability - two key components of the GRADE Evidence to Decision (EtD) framework. With slight modifications, the RPM has the potential to explore consensus among key stakeholders on other dimensions of the EtD, including feasibility, equity, and resource use
