    Professional occupation after cochlear implantation

    The aims of this study were to verify whether cochlear implants helped profoundly deaf adults to maintain or even to develop their professional occupations, and to identify other elements that may contribute to or, on the contrary, impede such patients' professional success. All adult patients received a questionnaire concerning their professional activities before and after implantation. Demographic data, health information, hearing performance and degree of satisfaction with the implant were also considered. Sixty-seven adults had been implanted, with three different devices, since 1985. At the time of implantation, 34 had been professionally active. After implantation, 29 had remained professionally active, four of whom reported positive developments in their careers. Five patients had become professionally inactive. Those patients who had previously been professionally inactive remained so. There had been no difference in performance, either between different types of cochlear implants or between professionally active or inactive patients. The implanted patients had kept their jobs and many of them had developed their professional skills. In spite of this, cochlear implants may still be perceived as proving insufficiently satisfactory hearing to enable professionally inactive patients to reintegrate and to facilitate further learning or career developments

    Follow-up of a cohort of tinnitus patients attending a medical-psychological joint consultation

    Objectifs: évaluation d'une consultation conjointe médico-psychologique (CCMP) recevant des patients souffrant d'acouphène. Principaux critères d'évaluation: amélioration de la qualité de vie des patients, arrêt de leur consommation médicale pour acouphène, présentation et évaluation de cette consultation. Matériel et méthodes: follow-up de 120 patients ayant consulté entre 1993 et 1996 à la Clinique et Policlinique d'ORL des Hôpitaux Universitaires de Genève, Suisse. Patients ayant consulté en première instance versus patients référés ou ayant déjà consulté un ou plusieurs spécialistes ORL. Patients souffrant d'acouphène uniquement versus patients ayant en plus d'autres troubles somatiques ou psychologiques. Evaluation actuelle et rétrospective. Enquête par questionnaire par téléphone. Traitement informatique des données (SPSS, version 6.1).Résultats: taux de réponse: 74%. Soixante et onze pour-cent des patients n'ont plus consulté après la CCMP: 83% des patients ayant consulté en première instance versus 63% des patients référés ou ayant déjà consulté un ou plusieurs spécialistes ORL. Vingt-deux pour-cent des patients souffrent d'un acouphène uniquement versus78% qui expriment d'autres plaintes somatiques et/ou psychologiques. Dix-neuf pour-cent vont mieux, 69% ne notent aucun changement et 12% vont moins bien depuis cette consultation. Cinquante-six pour-cent ont été satisfaits; 26% l'ont trouvée identique aux consultations ORL classiques et 13% ont été insatisfaits. Conclusions: Ce type de consultation semble particulièrement adapté à cette population de patients dont la majorité souffre d'autres affections somatiques et/ou psychologiques. Un objectif est atteint: une majorité d'entre eux (71%) n'a plus consulté pour acouphène après la CCMP. [Auteurs] Objective: Assessment of a joint medical-psychological consultation (JMPC) available to patients suffering from tinnitus. Main criteria for assessment: improvement of patients' quality of life, stopping their quest for medical attention, presentation and assessment of this consultation. Material and Methods: Follow-up of patients counseled at the Otolaryngology and Head and Neck Surgery Polyclinic between 1993 and 1996. Patients who had consulted for the first time were compared to those who had been referred by other specialists or their family doctors because of tinnitus. Patients who suffered solely from tinnitus were compared to patients with multi- ple somatic and/or psychological complaints. Present and retrospec- tive assessment. Questionnaires were delivered over the phone. Data were processed by computer (SPSS, version 6.1). Results: Rate of response: 74%. Seventy-one percent of patients had not consulted further after the JMPC: for 83% of the patients, the JMPC had been their first consultation, while 63% of the patients had either been referred by an ENT specialist or had already consulted one or more ENT specialists. For 22% of patients, the sole complaint was tinnitus, while 78% mentioned other somatic and/or psychological complaints. Since the consultation, 19% feel better, 69% have noticed no change and 12% feel worse; 56% were satisfied, 26% found the consultation no different from traditional ENT consultations, and 13% were dissatisfied. Conclusions: This type of consultation seems particularly suited to this patient population, of whom the majority also suffer from other somatic and/or psychological disorders. The majority of patients (71%) have not consulted elsewhere for tinnitus since this consultation; so the study was successful in meeting this objective.     Activité professionnelle des utilisateurs d'implant cochléaire

    This study verifies whether cochlear implants helps deaf adults to maintain or develop their professional occupations. Sixty-seven patients received a questionnaire concerning their professional activities before and after implantation. At the time of implantation 34 were professionally active. After the implantation 29 remained active, 4 of them reporting positive developments in their careers. Five patients became inactive. The previously inactive patients remained inactive. There was no difference in auditory performances between professionally active or inactive patients. Cochlear implants enable most implanted adults to maintain and even progress in their professions. However, deafness still represents an obstacle to social integration as inactive patients who searched for a job were rejected after the job interviews.Cette étude vérifie si l’implant cochléaire chez des adultes sourds contribue au maintien de l’activité professionnelle. Un questionnaire a été envoyé à 67 patients implantés. Lors de l’implantation, 34 étaient professionnellement actifs. Vingt-neuf le sont restés, quatre ayant progressé dans leur carrière. Cinq patients sont devenus inactifs. Tous les inactifs avant l’implantation le sont restés. Les performances auditives des patients professionnellement inactifs sont identiques à celles des actifs. En conclusion, l’implant cochléaire permet aux patients sourds de conserver leur emploi, voire de progresser professionnellement. Mais la surdité, même réhabilitée de façon satisfaisante, représente encore un obstacle à la réintégration professionnelle, tous les inactifs souhaitant retravailler après l’implantation ayant été rejetés

    La consultation conjointe médico-psychologique (CCMP) constitue un mode de prise en charge des patientes souffrant de paresthésies buccales médicalement inexpliquées. Une étude préliminaire de follow-up a été réalisée dans le but de déterminer les variables à mesurer pour évaluer l’efficacité de cette technique de prise en charge. L’analyse des résultats suggère de poursuivre le recueil des auto-évaluations et des hétéro-évaluations après les CCMP, et d’investiguer l’état somatique, l’état psychique, les acquisitions faites pendant les CCMP, l’utilité des CCMP et l’évolution de la consommation médicale des patientes. Mais aussi de donner plus de poids à la fonction de rassurer et d’expliquer. (Med Buccale Chir Buccale 2006 ; 12 : 31-38)

    First field trials with a portable CIS processor for the Ineraid multichannel cochlear implant

    The continuous interleaved sampling (CIS) strategy is a promising sound processing strategy for multichannel cochlear implants which provides immediate improvements in speech recognition when tested on Ineraid users: patients with only a few hours of experience (in laboratory testing) with the CIS strategy score better than with the Ineraid prosthesis they used since they, were implanted. The goal of this study was to evaluate the benefits that can be gained by the use of the new strategy in every day life. Two patients, implanted with the Ineraid multichannel cochlear implant, were equipped with a portable numerical processor programmed to implement a high rate CIS strategy. Their speech recognition was evaluated periodically with consonant and vowel identification tests for more than 6 months of use. Tests were also made with the Ineraid processor during the same experimental sessions and patients were regularly interviewed about their experience. Performance with the portable CIS processor was superior or equal to that obtained previously in the laboratory with the same strategy. Both patients achieved the best scores in 6 years of cochlear implant use. Qualitative reports from the patients suggest that the CIS strategy can improve "hearing" performance of cochlear implant users in many important situations of every day life.
