Germinal tumors of mediastinum

INNOVATIONS - THE DETERMINING FACTOR OF ECONOMIC GROWTH
The study conducted on the basis of national, European and international reports on innovation activity has enabled authors to determine the trends and the importance of innovations for the development of countries or communities of states. For the elaboration of this study the authors applied different methods of research: monographic study, comparison, grouping, indexing method. Thus, the authors have found that countries with an accelerated pace of economic growth also allocate higher amounts of R&D funds relative to GDP. This once again demonstrates the direct link between the amount of spendings on innovation and the level of economic development.</p
Aspects of Accounting Cost for Development of Customized Software Products
In today's context, marked by digitization and the development of information technology, one of
the topics that needs to be addressed is the "costs of software development". This study investigates
the evolution of scientific production related to the research topic based on descriptive and
bibliometric analysis of selected articles on Web of Science and EBSCO, thus continuing existing
research in the field. The importance of this theme is due to the increasing interest of entities to use
customized software products that help them to achieve the desired results. The scientific approach
is based on information from national and international literature and practice and various
electronic sources. It has been shown that the development costs of custom software depend largely
on the number of technical resources, the number and complexity of software product features and
design patterns. Therefore, accurate estimation is impossible without prior analysis of the software
design, business objectives and cost factors
Evaluarea stării de sănătate și aprecierea calității vieții a pacienților incluși în registrul electronic COVID-19
Background. COVID-19 infection remains a global health emergency with clinical and management challenges
requiring comprehensive and relevant research. Objective
of the study. Complex assessment of confirmed cases of
COVID-19 infection treated in ten medical institutions in
Chisinau for the analysis of the impact of the infection on
health. Material and Methods. The software „Electronic
Register of Patients COVID-19” was developed, based on a
standardized form. In the register elaboration, the type, quality and scientific value of the information to be collected
and stored shall be taken into account. The included data
were selected from the files of COVID-19 patients hospitalized between 1.03.2020 - 30.06.2021. Results. The software
was designed with 2 interactive subsystems: the information collection component, implemented in hospital medical institutions and processing component, implemented in
Nicolae Testemițanu University. The registry included over
8000 patients with COVID-19, and their number continues
to grow. Epidemiological, socio-demographic, clinical, paraclinical data and information on the clinical management of
COVID-19 patients are stored securely with different access
levels. The created software allows the selection, visualization, and statistical analysis of information with the generation of different reports according to pre-established or free
criteria. Conclusion. The register facilitates research by rapidly processing data on hospitalized COVID-19 patients, is
a valuable support for scientific researchers, and allows the
life quality assessment of the hospitalized patients.Introducere. Infecția COVID-19 a rămas o urgență globală de sănătate cu provocări clinice și de management care
necesită o cercetare comprehensivă și relevantă. Scopul.
Evaluarea complexă a cazurilor confirmate cu infecția COVID-19 aflate la tratament în zece instituțiile medicale din
municipiul Chișinău pentru analiza impactului infecției
asupra stării de sănătate. Material și metode. S-a elaborat softul „Registrul electronic de evidență a pacienților
COVID-19”, format în baza unui formular standardizat. La
elaborarea registrului, s-a ținut cont de tipul, calitatea și valoarea științifică a informației care urmează a fi colectată și
stocată. Datele incluse au fost selectate din fișele pacienților
COVID-19 spitalizați în perioada 1.03.2020 – 30.06.2021.
Rezultate. Softul a fost proiectat pe subsisteme interactive: componenta de colectare a informației, implementată
în instituțiile medicale spitalicești și componenta de procesare a informației, implementată în USMF „N. Testemițanu”.
Registrul a inclus peste 8000 de pacienți cu COVID-19, iar
numărul lor continuă să crească. Datele epidemiologice, socio-demografice, clinice, paraclinice și informațiile cu privire la managementul clinic al pacienților cu COVID-19 sunt
stocate în maximă siguranță cu diferite nivele de acces. Softul creat permite selectarea, vizualizarea și analiza statistică a informației cu generarea diferitor rapoarte conform
criteriilor prestabilite sau libere. Concluzii. Registrul creat
facilitează cercetările prin procesarea rapidă a datelor despre pacienții spitalizați cu COVID-19, este un suport valoros
pentru cercetătorii științifici și permite evaluarea calității

About Workshop in the framework of NIMSPPS project
In the period of September 18-19, 2018 the Workshop in the framework of NIMSPPS project ``Network for informational methods in supporting persons predisposed to preventable strokes using common devices'', took place in Chisinau, Moldova.
The previous NIMSPPS Workshop was organized in Mainz, Germany on May 29-30, 2018 by Johannes Gutenberg University Mainz, where Cooperation Agreement was signed.
The September NIMSPPS Workshop was organized by the Institute of Mathematics and Computer Science. 22 scientists from 5 countries (Austria, Germany, Moldova, Romania and Ukraine) attended this workshop
Clinical-epidemiological characteristics of adults hospitalized with Covid–19 in the Republic of Moldova
Introduction: The COVID-19 pandemic has a major
negative impact on health and socio-economic well-being.
Understanding the characteristics of COVID-19 disease and
identifying the wide range of factors affecting health and
quality of life can be the key to providing viable solutions
to improve the management of patients and their physical
and psycho-emotional rehabilitation. The purpose of the
present study was to evaluate the influence of SARS CoV-2
infection on the health status of adults hospitalized with the
diagnosis of COVID-19 in the Republic of Moldova.
Material and methods: The presented study is a retrospective, cohort, consisting of a sample of 7441 patients
randomly selected, aged 18 y.o. and older, hospitalized in
10 public medical institutions in Chisinau, Moldova. Diagnosis of COVID-19 was confirmed by detection of CoV-2 SARS
RNA. The data in the patients’ medical records were processed and stored according to the unified, pre-established
form, prepared in accordance with the requirements of the
software „Electronic Patient Record COVID-19”. The severity of COVID-19 disease was assessed using two principles:
(1) according to the criteria of the National Clinical Protocol
PCN-371; (2) according to the 7-point graduated scale developed by the WHO Special Committee (V.3.0, 3 March 2020) in
randomized multicenter clinical trials.
Result: Only 30.07% patients mentioned the presence
of a close contact with a COVID-19 positive person. The average age of the patients in the study was 52.83 years. Mild
form was diagnosed in 5.00% of patients, medium - 66.15%,
severe –20.67%, critical-8.18%. The main complaints of patients were fever, fatigue or physical asthenia, cough, and
headache. More than 1/4 of those hospitalized have severe
or critical forms of COVID-19; more than 1/3 - require oxygen therapy, and every 6-th patient needs non-invasive
high-flow oxygen ventilation or mechanical ventilation. Old
age, male sex, chronic comorbidities increase statistically
significantly the probability of patients having an unfavorable prognosis in COVID-19. 7.93% of patients died, according to the age group: every 2-nd patient over 90 years, every 3-rd over 80 years, every 5-th over 70 years, and every
9-th over 60 years died.
Conclusions: (1) The uncertainty of the source of infection lead to delay specific prophylactic public health measures; (2) In COVID-19, in a hospital-type medical management, the emphasis should be placed mainly on patients
over the age of 50; (3) There is no specific clinical manifestation in COVID-19, that would allow to distinguish the
disease from other pathologies; (4) Age over 60 y.o., male
sex, and chronic cardiovascular diseases, diabetes mellitus, chronic kidneys diseases and malignant tumors unfavorable influence the evolution of COVID-19; (5) Antibiotic administration remains at a high level in hospitalized patients
and is often unjustified and unnecessary

Durerea cronică prezentă la pacientă are localizare multiplă (cefalee, durere lombară, artralgii), etiologie multiplă (nociceptivă, neurogenă, mecanică) care se suprapune pe comorbidități (anxietate, depresie, tulburări de somn, artrită reumatoidă), particularități de comportament (strategie pasivă de confruntare a durerii, refuzul de administrare a medicamentelor specifice pentru artrita reumatoidă) și tratamente cu reacții adverse diferite (abuz medicamentos AINS) ceea ce determină potențarea reciprocă și gradul crescut de dizabilitate al pacientei. Doză redusă de Tocilizumab 200 mg, comparativ cu 400 mg, la pacienții cu forme severe de infecție COVID-19
Introduction. Excessive, deregulated pro-inflammatory cytokine secretion has a detrimental impact on the evolution of COVID-19, aggregating the tissue impairment, organ failure, and an increased risk of death. Several studies have demonstrated the beneficial effect of Tocilizumab (TCZ) in reducing hyperimmune response in severe forms of COVID-19. Material and methods. This is an experimental controlled clinical trial, consisting of 66 patients hospitalized with severe COVID-19. Results. On overage, the decision to administer TCZ was made on average on the 11.34±0.31 day of the disease, when the beginning of Cytokine Storm was suspected in the patients already on dexamethasone treatment. The clinical and paraclinical parameters, including fever, asthenia and dyspnea duration, SpO2 level, oxygen therapy need, improvement of the radiological picture, and duration of hospitalization were more favorable in patients treated with TCZ 400 mg compared to those treated with TCZ 200 mg (p<0.0001). The relative risk of rapid worsening after TCZ (RR=0.88), the relative risk of decreasing blood pressure (RR=0.29) and the relative risk of transfer to intensive care units for invasive or non-invasive ventilation (RR=0.8) was lower in patients treated with TCZ 200 mg compared to the 400 mg TCZ lot. Conclusions. The dose of TCZ had a significant impacton the duration of clinical manifestations, the duration of oxygen therapy and the duration of patient hospitalization, with better results for TCZ 400 mg compared to TCZ 200mg. Although the risk of worsening after TCZ and the risk of transfer to intensive care were lower in patients treated with TCZ 200 mg. So, the 200 mg fixed dose of TCZ can be a life-saving option for severely ill patients with COVID-19 in the context of IL-6 inhibitor supply shortages.Introducere. Eliberarea excesivă a citokinelor proinflamatorii are un impact negativ asupra evoluției infecției COVID-19, sporind afectarea tisulară, insuficiența organică și riscul de deces. Mai multe studii au demonstrat efectul benefic al preparatului Tocilizumab (TCZ) în reducerea răspunsului hiperimun în formele severe de infecție COVID-19. Material și metode. Este un studiu experimental, clinic controlat, care include 66 pacienți, internați cu forme severe de infecție COVID-19. Rezultate. Decizia privind administrarea de TCZ a fost luată în medie în ziua a 11,34±0,31 de boală, când a fost suspectat debutul furtunii de citokine la pacienții aflați deja în tratament cu dexametazonă. Parametri clinici și paraclinici, precum durata febrei, asteniei și a dispneei, nivelul SpO2, necesitatea în terapie cu oxigen, ameliora-rea tabloului imagistic radiologic, și durata spitalizării au fost mai favorabili la pacienții tratați cu TCZ 400 mg față de cei tratați cu TCZ 200 mg (p<0.0001). Riscul relativ de agravare rapidă după administrarea TCZ (RR=0,88), riscul relativ de diminuare a tensiunii arteriale (RR=0,29) și riscul relativ de transfer în secțiile de terapie intensivă pentru ventilație invazivă sau non-invazivă (RR=0,8) a fost mai scăzut la pacienții tratați cu TCZ 200 mg, comparativ cu lotul TCZ 400 mg. Concluzii. Doza de TCZ a influențat durata manifestărilor clinice, durata terapiei cu oxigen și durata spitalizării pacienților, cu rezultate mai bune pentru TCZ 400 mg față de TCZ 200 mg, deși riscul de agravare după TCZ și riscul de transfer în terapie intensivă au fost mai joase la pacienții tratați cu TCZ 200 mg. Deci, doza de TCZ de 200 mg poate fi o opțiune de salvare a vieții pacienților gravi cu infecție COVID-19, în contextul deficitului de aprovizionare cu inhibitori de IL-6
